1.Change of depression-like behavior in chronic alcoholism and withdrawal model, and co-mechanism of depression and chronic alcoholism in mice

AIM: To investigate the behavior of depression in chronic alcoholism and withdrawal model of mice, and to explore the co-mechanism of alcoholism and depression.METHODS: A novel model of chronic alcoholism was constructed in this study.The animals were divided into normal control group, and alcohol 7 d, 14 d, 21 d and 28 d groups.The mice were given alcohol preference test on the 6th, 13th, 20th and 27th days.After the test, alcohol were withdrawn for 1 d, then the next day the mice were given behavior test of depression.After the test, the mice were sacri-ficed.The contents of 5-hydroxytryptamine (5-HT) and norepinephrine (NE) were detected by HPLC.The expression of cAMP response element-binding protein ( CREB) and brain-derived neurotrophic factor ( BDNF) was detected by Western blot.RESULTS:The mice showed an obvious drinking phenomenon, and the immobility time of forced swimming test and tail suspension test was significantly increased, with increasing drinking days and withdrawal times.5-HT level in 7 d group mice only increased in frontal cortex (P<0.05).However, compared with control group, 5-HT levels in hippocampus and cortex were decreased on the 21th and 28th days (P<0.01).NE levels in 21 d and 28 d groups were decreased in hippo-campus and frontal cortex (P<0.05), and no significant change was observed in 7 d and 14 d groups.The protein levels of p-CREB and BDNF were significantly decreased in hippocampus and frontal cortex of 12 d and 28 d groups (P<0.05), and no significant change was observed in 7 d group and 14 d group.CONCLUSION:The co-mechanism of alcoholism, withdrawal and depression is related to 5-HT.5-HT-cAMP-CREB-BDNF signaling pathway may be a common mechanism for alcoholism and depression.
2.Association of single nucleotide polymorphism of interleukin 6 receptor gene and metabolic syndrome

The association of interleukin 6 receptor ( IL-6R ) gene - 183 A/G ( rs4845617 ) and Asp358 Ala (rs8192284 A/C) polymorphisms with metabolic syndrome was investigated in Chinese Han population.The result showed that the frequencies of AA genotype and A allele were higher in patients with metabolic syndrome ( MS ) than those in healthy subjects ( P<0.05 or P<0.01 ).The risk of MS in patients with A allele was 1.643 folds of that with allele C(95% CI 1.163-2.320,P<0.01 ).No differences were found in the genotype and allele frequencies of -183A/G between two groups ( P>0.05 ).The Asp358Ala polymorphism of IL-6R was significantly associated with MS in Chinese Han population.
3.The study on expression of TNF-α in acute lung injury caused by paraquat arid the protection effect of rhubarb

Objective To observe the expression of tumor necrosis factor-α(TNF-α)in acute lung injury caused by paraquat(PQ)in rats,and investigate the mechanism of the rhubarb in respect of pmteetive effects.Method PQ intragastrically poisoning at the dose of 50 mg/kg made a model of the acute lung injury in Sprague-Dawley(SD)rats.Totally 144 adult healthy SD rats(72 female,72male)were randomly divided into control group (group A,n=24),poisoned group(group B,n=48),rhubarb treated group(group C,n=48)and the shaln poisoning group(group D,n=24).Rats of group B and group C were poisoned intmgastrically with PQ(50 mg/kg).and rats of group C and group D were intervened intragastrieally with 300 mg/(kg·d)of rhubarb in 15 min-utes.The white blood cells and total cells in bronchoalveolar lavage fluid(BALF)were counted by using a blood cell counting plate and the protein content of BALF was measured by using the way of Lowry in order to calculate the neutmphiks pereentage and lung permeability index.A small portion of left lung was stained with HE to observe the pathological changes and the expression oftumor necrosis factor-α in the rest of the left lung was observed with immunohistochemistry.The data were handled by the analysis of variance and NK method using SPSS 14.0.Re-suits Compared with group A,the lungs of rats mainly showed congestion,edema and leukocytes infiltration in group B,and fibrosis was found onlyt in a few rats.And the rate neutrophils percentage,protein content and lung permeability index in BALF increased(P<0.01).The expression of TNF-α were obviously inereased at 12 hours after PQ poisoning,and immtmohistochemistry score (IHS)was higher,and peaked at 24 hours later(P<0.05),then remained on a high level for a while and sluggishly declined.Compared with group B,the changes of above mentioned were alleviated obviously,and the expression of TNF-α delayed with the less magnitude of increasing an an obvious tendency of less expression.Compared with group B,delayed,lower increasing extent,obviously re-ducing tendency in group C with statistical difference in IHS(P<0.05).Conclusions Rhubarb ameliorates a-cute lung injury caused by PQ poisoning in rats by means of inhibiting the expression of TNF-α in turn to alleviate inflammatory reaction.
4.Expression of homosapiens chromosome segregation gene in homogenous nasopharyngeal carcinoma tissue cells

OBJECTIVE To study the expression of CSE1L gene sifted from the whole genome expression profiling in the aRNA from homogenous human nasopharyngeal carcinoma (NPC) and non-NPC biopsy tissue samples using semi-quantitative RT-PCR (sqRT-PCR). METHODS CSE1L gene was sifted from the whole genome expression profiling from homogenous NPC and non-NPC biopsy tissue samples. RNA later RNA Stabilization Reagent was used to preserve the tissue samples harvested from the nasopharynx of NPC and non-NPC patients. The samples were microdissected to get the homogenous tissue cells and then the total RNA was isolated from them. The antisense RNA (aRNA) was amplified from the total RNA by “in vitro transcription” (IVT). CSE1L gene expression in the homogenous tumor cells of NPC and the pure epithelial pillar cells of normal nasopharyngeal tissue was investigated by sqRT-PCR. RESULTS The high quality total RNA could be harvested from the microdissected homogenous tissue cells, and then the sufficient aRNA was amplified from it. The expression of CSE1L gene was identified using these aRNA by sqRT-PCR. There were significantdifference between the average expression value of CSE1L in NPC tissue (1.740?1.105) and in non-NPC tissue (0.618?0.183; df=30, t=3.159, P=0.004). The expression of CSE1L gene in the whole expression profiling were 1.056?0.296 in NPC tissue and 0.465?0.835 in non-NPC tissue respectively (df=16, t=4.317, P=0.001) . CONCLUSION The whole genome expression profiling with sqRT-PCR could be used to sift the marker genes from biopsy tissue samples. CSE1L may be as a candidate oncogene in NPC.
5.Emergent reconstruction and repair of thumb and finger defect by toes and combined toes transplantation

Objective To explore the method and effect of thumb and finger reconstruction and repair by emergent primary toes transplantation. Methods Three hundred and fourteen fingers of 282 cases including finger thumb defect and complex injury with hyperthena palm radid soft tissue of forearm were reconstructed and repaired by the second toes, wrap-around flap and the second third toes wrap-around flap combined with distal flap of leg or other complex tissue flap transplantation. Results Three hundred and eight reconstructed fingers survived, the survival rate was 98%. After a follow up to 6 months to 6 years, the reconstructed thumbs and fingers have accepted nip, grasp, opponent function. The result was graded as excellent in 214 fingers, good in 63, fair in 26, poor in 5. The excellent and good rate was 88%according to the upper limb functional evaluation criteria issued by the Hand Surgery Society of the Chinese Medical Association. Conclusion Different type of toes and combined toes transplantation can obtain preferable clinical effect on emergent primary thumb and finger defect.
6.Practice of the curriculum ideological and political education in metabolism-related curriculum chain of clinical medicine: taking Tongji University School of Medicine as example

Guided by the development of new medical science proposed by Ministry of Education and the "Education and Training Plan for Excellent Doctors 2.0", shifted from treatment-oriented to whole life-health cycle, we have explored curriculum ideological and political education in metabolic-related curriculum chain. Firstly, we constructed a core teaching team and had the training of curriculum ideological and political education. The top-level design was made with the integration of moral education into medical education. Secondly, the syllabus was comprehensively revised, containing the connotation of "morality education". The elements relevant to curriculum ideological and political education hidden behind professional courses were excavated. Finally, the mixed teaching mode of online combining with offline was carried out. Metabolism-related curriculum chain, focused on "metabolism, diabetes, obesity and patient education", formed a progressive link from basic medical science to practice to clinical, strengthening the "prevention, treatment and health care" based "one health" philosophy and giving full play to the implicit curriculum ideological and political education hidden behind professional courses. Our practice shows that the implementation of curriculum ideological and political education in metabolism-related curriculum has been accepted by students, and curriculum ideological and political education has been become part of professional courses. The "gene chimera" mode for curriculum ideological and political education incorporation into professional courses needs to be infiltrated imperceptibly, and the effect will be visualized in the future.
7. Predictive value of single nucleotide polymorphisms of HLA-C and UBE2L3 in evaluating the effect of telbivudine antiviral therapy during pregnancy

Objective:
To investigate the association between single nucleotide polymorphisms (SNPs) of rs3130542 and rs4821116 in the HLA-C and UBE2L3 genes and the effect of telbivudine antiviral therapy during pregnancy in HBeAg-positive mothers through a large-sample control study, and to provide a basis for the development of individualized blocking strategies for pregnant women with a high viral load.
Methods:
The genotypes of rs3130542 and rs4821116 were determined for 312 pregnant women with a high viral load who received telbivudine antiviral therapy during the second or third trimester of pregnancy, and the dominant model, recessive model, and additive model were used to analyze the association between the genotypes of these two loci and the reduction in HBV DNA load. The Shapiro-Wilk test and the Levene test were used to evaluate data normality and homogeneity of variances, and the t-test or the non-parametric Mann-Whitney U test was selected based on data type and was used for the comparison of means between groups. The Hardy-Weinberg equilibrium was used to determine the genotype of SNPs, and the dominant model, recessive model, and additive model were used for analysis.
Results:
Mothers with an AA/AG genotype of rs3130542 in the HLA-C gene had a significantly higher probability of HBV DNA load ≥103 IU/ml at the time of delivery (

Nonalcoholic steatohepatitis (NASH) is a serious type of nonalcoholic fatty liver disease (NAFLD) and can develop into life-threatening liver cirrhosis and liver cancer. Toll-like receptor 4 (TLR4) is a type of pattern recognition receptor in innate immunity, and after being activated, it can trigger a cascade reaction and activate nuclear factor-κB (NF-κB) which mediates inflammatory response. The TLR4/NF-κB signaling pathway is a classic inflammatory pathway. In clinical practice, traditional Chinese medicine has achieved a good effect in the treatment of NASH, and the TLR4/NF-κB signaling pathway is one of the approaches for traditional Chinese medicine to treat NASH. By searching the relevant literature, this article summarizes the active components and compounds of traditional Chinese medicine that can regulate the TLR4/NF-κB signaling pathway to alleviate NASH, so as to provide ideas for future research and medication.

Objective:From the perspective of behavioral economics,it analyzes the reasons for the low utilization rate of palliative care and propose intervention measures,to promote the acceptance and application of palliative care by terminal cancer patients and their families.Methods:Using the theories and methods of behavioral economics to explore how cognitive and behavioral biases lead to irrational medical decisions in the choice of palliative care;based on the dual-process theory and nudge method,it aims to construct an intervention model for"palliative"care decision-making and propose intervention strategies.Results:Terminal cancer patients and their families with bounded rationality tend to prefer short-term aggressive treatment plans while making decisions,and use the extension of survival as the decision anchor point,weakening the influence of quality of life factors.The lack of sound mechanisms related to palliative care will further increase the cognitive bias of cancer patients and their families,putting them in a negative choice framework and risking losses,leading to the abandonment of more rational palliative care plans.Conclusion:By adopting the"perceptual-rational"thinking from the dual-process theory,transforming inter-temporal selection into the most cost-effective option,anchoring treatment expectations to patients'quality of life,increasing the value of palliative care benefits,and constructing a supportive selection framework as a target for intervention,it can help promote the use of palliative care.
