1.Efficacy Comparison of Misoprostol Administrated with Two Routes in the Prevention of Postpartum Hem-orrhage

Objective:To compare the effect of misoprostol respectively with sublingual administration and rectal administration in the treatment of postpartum hemorrhage. Methods:Totally 72 pregnant women were randomly divided into two groups with 36 cases in each. The sublingual group was given misoprostol 600 μg under the tongue immediately after the front shoulder of baby was delivered, while the rectal group was given misoprostol 600μg into rectum deeply. The amount of blood loss in 2 hours and 24 hours after the de-livery and the adverse reactions were compared between the two groups. Results: The postpartum hemorrhageon in 2 hours and 24 hours of the sublingual group was significantly less than that of the rectal group, and the incidence of postpartum hemorrhage in the sub-lingual group was significantly lower than that of the rectal group, and the difference was statistically significant (P<0. 05). After the treatment, the incidence of adverse reactions between the two groups had no significant difference (P>0. 05). However, the incidence of shiver in the sublingual group was significantly higher than that in the rectal group, and the difference was statistically significant ( P<0. 05). Conclusion:The preventive effect of misoprostol with sublingual administration on postpartum hemorrhage after natural vagi-nal delivery is better than that with rectal administration, which is safe and convenient, and worthy of promoted application in clinics.
2.Efficacy and Safety of Ornidazole in the Treatment of Trichomonal Vaginitis

Objective:To explore the efficacy and safety of ornidazole in the treatment of trichomonal vaginitis. Methods:Totally 112 cases of trichomonal vaginitis from February 2011 to December 2012 in our hospital were selected and randomly divided into the ob-servation group and the control group with 56 cases in each. The control group was treated with metronidazole tablets 200mg, po,tid, and the observation group was treated with ornidazole tablets 500mg, po,bid. The treatment course was 7 days. The clinical effect, score of clinical symptoms and incidence of adverse reactions of the two groups were compared. Results:The total effective rate of the observation group was 96. 4%, which was higher than that of the control group (82. 1%), and the difference was statistically signifi-cant (P<0. 05). The symptom scores of the two groups were significantly improved after the treatment (P<0. 05), however, the score between the two groups showed no significant difference (P>0. 05). The incidence rate of adverse reactions in the observation group was 10. 71%, which was significantly lower than that in the control group (37. 5%), and the difference was statistically signifi-cant (P<0. 05). Conclusion:The clinical curative effect of ornidazole in the treatment of trichomonal vaginitis is notable with faster onset, shorter treatment course, lower adverse reaction rate and better patient tolerance,which is valuable in the clinical promotion.

4.Antimicrobial Resistance of Bacteria Isolated from a Hospital 2007-2008

OBJECTIVE To investigate the resistance of clinical isolated strains to the commonly used antibacterials in our hospital 2007-2008.METHODS Clinical isolated strains and sensitivity of drugs were detected by ATB system.The result of drug sensitivity was judged by CLSI standard and analyzed with statistical software WHONET5.3.RESULTS Altogether 3150 strains bacteria were isolated,17.4% were Gram-positive strains and 82.6% were Gram-negative strains,and the top five isolates were Pseudomonas aeruginosa,Escherichia coli,Klebsiella pneumoniae,Acinetobacter baumannii,and Staphylococcus aureus.The reasistance rate of Gram-positive strains to minocycline was 15.4%.Five VRE strains were isolated.Various Enterobacteriaceae bacteria were sensitive to imipenem meropenem,cefoperazone-sulbactam and piperacillin-tazobactam,and their rate was 86.5% to 97.7%.Some of Acinetobacter and Stenotrophomonas maltophilia were multidrug resistant.CONCLUSIONS It is serous that multidrug resistance of isolated strains of the patients exists in our hospital.
5.STUDIES ON INDIRECT FLUORESCENT ANTIBODY TEST FOR DIAGNOSIS AND EPIDEMIOLOGICAL INVESTIGATION OF FILARIASIS

Indirect fluorescent antibody test (IFAT) using frozen sections of Brugia malayi adult worms as antigen was employed in the diagnosis and epidemiological investigation of human filariasis. Sera were collected from 704 cases with bancroftian or malayan microfilaremia. the positive rate was 92.8-99.1%. Of 150 healthy people from non-endemic areas, only one showed a positive reaction (false positive rate 0.7%) (Table 1). This technique proved to be highly sensitive and specific for the diagnosis of filariasis and the antigen is easy to prepare. It might be used in sero-epidemiological investigation for the assessment of filariasis control.
6.Effect of community-based rehabilitation training on cognitive disorders of cerebral apoplexy patients

Objective To study the effects of community-based rehabilitation on cerebral apoplexy patients with cognitive disorders.Methods 30 cases of cerebral apoplexy patients were enrolled according to relevant standards,who were given standard rehabihtation care in Community Health Service Center(training group) ,while another 30 similar cases from subordinate health service stations (control group) given only rehabilitation guidance once a week.Two groups were not do professional cognitive training.All patients were assessed with Mini Mental State Examination( MMSE), Fugl-Meyer Assessment(FMA) and modified Bathel index rating scale(MBI) to evaluate their cognition ,movement function and activity of daily living(ADL) before training as well as after 4-week and 8-week training.Results There was no signficant difference in cognition,movement functions and ADL between training group and control group before treatment(P>0.05).The differences in the assessment of Brounnstrom scale and MBI between two groups after 4-week training were not significant( P > 0.05 ).The scores of Fugl-Meyer scale, MMSE and Barthel index of the training group after 8-week training were significantly higher than those of control group( all P < 0.05 ).Conclusion Community-based rehabilitation training could improve the movement functions and ADL,and cognitive functions of cerebral apoplexy patients.
7.Evaluation of the diagnostic accuracy of modified Berlin questionnaire on predicting obstructive sleep apnea-hypopnea syndrome in adults.

OBJECTIVE:
To evaluate the diagnostic value of modified Berlin questionnaire on predicting obstructive sleep apnea-hypopnea syndrome (OSAHS)in Chinese adults.
METHOD:
Differ from the original version, BMI cut-off point was adjusted to 25.0 in modified Berlin questionnaire according to the Asia -Pacific obesity definition. A total number of 244 samples who experienced polysomnography (PSG) were included. After well informed, each patient finished questionnaire by an interview. The results of the original and modified questionnaires and polysomnography reports were compared with polysomnography reports to evaluate the diagnostic accuracy of the modified Berlin questionnaire.
RESULT:
In male population, the sensitivity and specificity of original Berlin questionnaire were 74.03% and 65.71%, respectively. The percentage of diagnostic consistency was 72.49%, and Kappa coefficient was 0.304 (P < 0.01). In aspect of modified version, the sensitivity and specificity were 92.21% and 48.57%. The percentage of diagnostic consistency was 84.13%, and Kappa coefficient was 0.437 (P < 0.01). In female population, the sensitivity and specificity of original version were 50.00% and 61.90%, respectively. The percentage of diagnostic consistency was 54.55%, and Kappa coefficient was 0.110 (P > 0.01). When comes to the modified Berlin questionnaire, the sensitivity and specificity were 76.47% and 47.62% in female subjects, and the percentage of consistency agreement was 65.45%, with a Kappa coefficient 0.248 (P > 0.01).
CONCLUSION
Compared to the original version, the modified Berlin questionnaire has a better diagnostic consistency with a considerable sensitivity and specificity in male population. However, in female subjects group, the consistency of the modified Berlin questionnaire was still not significant remains unsatisfactory, although P value has been improved. Modified Berlin questionnaire could be. used as a primary screening tool for male OSAHS patients. However, but the screening questionnaire for female should still be need to be further explored.

convalescents, and to screen for broad-spectrum neutralizing antibodies against the SARS-CoV-2 RBD. Methods Using biotinylated RBD as a molecular probe, flow cytometry was employed to perform single-cell sorting of B cells from peripheral blood mononuclear cells (PBMCs) of convalescents. The obtained B cells were lysed and subjected to reverse transcription, followed by nested PCR amplification of the heavy and light chains of antibodies was conducted using random primers. The amplified products were cloned into corresponding expression vectors, and the respective matched heavy-light chain plasmids were co-transfected into 293F cells for expression. Monoclonal antibodies were then purified using Protein A column chromatography. Neutralization experiments were conducted with the wild-type (WT) pseudovirus, and antibodies with IC50<0.1 μg/mL were selected for further testing of neutralizing breadth and potency against the wild-type (WT), Beta variant (B.1.351), Delta variant (B.1.617.2), and currently prevalent pseudovirus strains (XBB, BA.5, BF.7). Results A total of 21 RBD-specific monoclonal B cells were obtained from two recovered patients, resulting in the isolation of 13 pairs of antibody light/heavy chains. Nine antibodies were successfully expressed, with P1-A1, P1-B6, and P1-B9 exhibiting IC50 values below 0.1 μg/mL against the pseudovirus of the wild-type strain (WT). Specifically, P1-B6 effectively neutralized the wild-type strain (WT), Beta variant (B.1.351), and Delta variant (B.1.617.2), with IC50 values reaching 0.01 μg/mL. P1-B9 demonstrated effective neutralization against the wild-type strain (WT), Beta variant (B.1.351), Delta variant (B.1.617.2), and Gamma variant (P.1) pseudoviruses, with IC50 values of 0.42 μg/mL, 0.63 μg/mL, 0.28 μg/mL, and 2.50 μg/mL, respectively. Additionally, P1-B6 exhibited good neutralization against BA.5 and BF.7 pseudoviruses, with IC50 values of 0.06 μg/mL and 0.09 μg/mL, respectively. Conclusions Infection with the SARS-CoV-2 WT strain can induce the generation of neutralizing antibodies with broad-spectrum activity. Generating these broadly neutralizing antibodies does not require an excessively high somatic hypermutation. The obtained antibodies can be used as candidates for SARS-CoV-2 diagnosis and prevention.
9.Application of abdominal cupping along meridians in patients with central obesity

Objective:To explore effects of abdominal cupping along meridians on body weight, waist circumference and glucose and lipid metabolism in patients with central obesity.Methods:Using the convenient sampling method, a total of 76 patients who were treated in Traditional Chinese Medicine Treatment Clinic of Ningbo Hospital of Traditional Chinese Medicine from March to August 2019 were selected as the research objects. According to the simple random number table method, they were divided into the control group and the observation group, with 38 cases in each group. The control group was guided by health education of lifestyle and behavior based on traditional Chinese medicine theory. On this basis, the observation group took Shenque point as the center and performed flash cupping slowly in a clockwise and counterclockwise direction around the navel. Then cupping moving was performed on the abdomen following abdominal spleen meridian, stomach meridian, gallbladder meridian, kidney meridian, ren meridian and belt meridian from bottom to top and from top to bottom. Finally, cups were keptin Daheng acupoint, Fuai acupoint, Tianshu acupoint, Shuidao acupoint, Zhongwan, Guanyuan and belt meridian acupoint. Intervention was performed once a week, with 4 times as a course of treatment, and 3 courses of treatment were carried out for 12 weeks. The weight, body mass index (BMI) , waist circumference, blood glucose and blood lipid levels of patients of two groups after intervention were compared.Results:A total of 30 patients in the observation group and 34 patients in the control group completed the study. After 12 weeks of intervention, the weight, BMI, waist circumference, fasting blood glucose, total cholesterol and low-density lipoprotein (LDL) levels of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Abdominal cupping along meridians can effectively improve waist circumference, body weight, BMI and glucose and lipid metabolism in patients with central obesity. The method is simple and safe, which is worthy of further promotion.
10.Trajectory of postpartum post-traumatic stress disorder in women of advanced maternal age based on latent growth mixture model

Objective:To explore the trajectory of postpartum post-traumatic stress disorder (PP-PTSD) in women of advanced maternal age and its influencing factors.Methods:From September 2021 to March 2023, convenience sampling was used to select women of advanced maternal age in the Department of Gynecology and Obstetrics at Taizhou Hospital of Zhejiang Province as participants. Parturients were surveyed at one week, one month, six months, and 12 months after delivering using General Information Questionnaire, Post-traumatic Stress Disorder Check-list-civilian Version, Social Capital Scale, and 10-item Connor-Davidson Resilience Scale. The latent growth mixture model was used to analyze the trajectory of PP-PTSD.Results:A total of 467 questionnaires were distributed, and 456 valid questionnaires were collected, with a valid rate of 97.64%. There were four types of PP-PTSD trajectories in women of advanced maternal age, namely non PP-PTSD type (71.27%, 325/456), early onset-gradual recovery type (12.06%, 55/456), early onset-prolonged type (9.21%, 42/456), and late onset-risk type (7.46%, 34/456). Univariate analysis showed that delivery mode, pregnancy complications, social capital, and psychological resilience were potential categories of influencing factors for the trajectory of PP-PTSD in women of advanced maternal age ( P<0.05) . Conclusions:There is population heterogeneity in the trajectory of PP-PTSD among women of advanced maternal age. It is recommended that medical and nursing staff dynamically monitor the PP-PTSD of women of advanced maternal age, identify their trajectory types early, and develop precise intervention measures based on classification.
