Objective To study the applicability of hysteroseopy for abnormal uterine bleeding of women and to analyze the etiology.Methods 679 cases of women with abnormal uterine bleeding were examined by hysteroscopy,whose situations were showed first under bimanual or trimanual examinations,then trams abdominal or traps vaginal B-ultrasonograpy.Results The postive rate of uterie abnormality was 98.6% detected by hysteroscopy and biopsy.Endometrial hyperplasia and endometrialpolyp were the main cause of abnormal uterine bleeding,which occupied 56.7%,then were myoma and endometritis.Women of child-bearing age were the largdy group that in volved.Conclusion Hysteorscopic examination was useful for abnormality uterine bleeding of women.We can underatand the relationship with abnormal uterine bleeding and intra-uterine disease,and the distribution of the different diseases in different age to enhance accurate diagnosis.

Objective To investigate the nursing intervention of pregnant women with antiphespholipid syndrome(APS). Methods In experimental group 40 cases with APS positive and control group 40 cases with APS ACA-negative for comparison. Results The abortion occurred in 6 cases(15. 0%) ,2 cases of stillbirth(5.0%) ,loss of pregnancy(abortion+stillbirth) 8 cases (20. 0%) in experimental group. The abortion occurred in 2 cases (5.0%), no stillbirth in control group. The loss of pregnancy in experimental group were significantly higher than the control group (X2=9. 54, P＜0. 005 ). The rate of positive in ACA-IgG (40. 0%) after treatment lower than before treatment (52. 5 %) (X2 = 3.85, P＜0. 05). The rate of positive in ACA IgM (37. 5%) after treatment lower than before treatment(40. 0)% (X2=3.81 ,P＜0. 05 ). Conclusion The APS positive rate and the loss of pregnancy were closely related. The overall nursing intervention conld improve efficacy and reduce the complications.

Objective To investigate the efficacy to pregnant outcoule with antieardiolipin antibody(ACA) of qualitative and quantitative before treatment and after treatment in women pregnant with antiphospho-lipid syndrome (APS).Methods The heparin combined with aspirin therapy APS 40 patients before and after treatment to momtor the change of ACAIgG,IgM.Results 40 patients were treated 37 eases of live births,success rate were92.5%,the rate of pregnancy loss were3 cases(7.5%);The lever of ACA-IgG and ACA-IgM were(67.63±2.53)u and (49.40±3.64)u beforetreatment andlever of ACA-IgG and ACA-IgM were(23.87±1.54)u and(21.60±2.59)u after treatment.The lever after treatment were significandy Iower than the before treatment(P＜0.01);The level of ACA were overall downward in 40 cases after treatment by dynamic observation;The rate of positive in ACAIgG (40.0)% after treatment lower than before treatment(52.5)%(X2=3.85,P＜0.05);The rate of positive in ACA IgM(37.5)% after treatment lower than before treatment(47.5)%(X2=3.81,P＜0.05).Conclusion Hepfinar treatment combined with aspirinmay be a safe and effective method for patients with APS.

Objective To discuss the influence of comprehensive rehabilitative nursing by traditional Chinese medicine(TCM)the neural function impairment, activity of daily living(ADL)and accompanying complications of patients in acute phase of cerebral apoplexy. Methods 238 patients from 3 cooperative hospitals from September in 2004 to September in 2006 were randomly chosen and divided into the test group(117 cases)and the control group(121 cases).The medication treatment in the two groups were basically the same. The comprehensive rehabilitative nursing with traditional Chinese medicine was adopted in the test group and the control group received routine nursing intervention. The clinical nerve function limitation score(CNS)and the ADL were evaluated within the first week and the 4th week after intervention. The incidence rate of complications was also determined. Results The CNS and ADL results in the observation group were better than those in the control group(P＜0.01).The incidence rate was lower than that in the observation group. Conclusion The comprehensive rehabilitative nursing by TCM could facilitate the functional recovery of limbs and improve activity of daily living.This intervention could be applied in clinic, family and community because of its characteristics such as simplicity, feasibility and little adverse effect.

Objective To explore the early coal pneumoconiosis patients with HRCT staging and pulmonary function abnormali-ties indicators ,provide an objective basis for clinical diagnosis and treatment early pneumoconiosis .Methods Strict control of the inclusion criteria :no smoking ,no pneumonia ,no tuberculosis ,no lung cancer and no lung surgery ,as a result ,total 33 early coal pneumoconiosis patients were chosen in the study .All of them were underwent paired CT obtained at full inspiratory position .High kv chest radiography ,and pulmonary function tests at the same day .Reference to the chest X-ray pneumoconiosis classification method as pneumoconiosis HRCT staging .Analyzed the correlation between early coal pneumoconiosis with HRCT staging and the parameters ,including Forced vital capacity (FVC) ,Forced expiratory volume in 1 second(FEV1) ,FEV1/FVC ,Maximum midexpi-ratory flow(MMEF) ,Peak expiratory flow(PEF) ,FEF75 ,FEF50 ,FEF25 ,Total lung capacity(TLC) ,Residual volume(RV) ,Carbon monoxide diffusion capacity(DLco) ,KCO and so on .Results HRCT could be more accurately display the shape ,size and distribu-tion of coal pneumoconiosis small nodules .There were significant correlation with MMEF ,PEF ,FEF75 ,FEF50 and FEF25 in different HRCT staging .Conclusion HRCT can early detection and validation early coal workers pneumoconiosis .Early coal worker′s pneu-moconiosis small airway flow rate decline caused by inhalation coal dust .

The effects of epigenetic modification on the differentiation of islet cells and the expression of associated genes (Pdx-1, Pax4, MafA, and Nkx6.1, etc) were investigated. The promoter methylation status of islet differentiation-associated genes (Pdx-1, Pax4, MafA and Nkx6.1), Oct4 and MLH1 genes of mouse embryonic stem cells, NIH3T3 cells and NIT-1 cells were profiled by methylated DNA immunoprecipitation, real-time quantitative PCR (MeDIP-qPCR) techniques. The histone modification status of these genes promoter region in different cell types was also measured by using chromatin immunoprecipitation real-time quantitative PCR methods. The expression of these genes in these cells was detected by using real-time quantitative PCR. The relationship between the epigenetic modification (DNA methylation, H3 acetylation, H3K4m3 and H3K9m3) of these genes and their expression was analyzed. The results showed that: (1) the transcription-initiation-sites of Pdx-1, MafA and Nkx6.1 were highly methylated in NIH3T3 cells; (2) NIH3T3 cells showed a significantly higher level of DNA methylation modification in the transcription-initiation-site of Pdx-1, Pax4, MafA and Nkx6.1 genes than that in mES cells and NIT-1 cells (P<0.05); (3) NIT-1 cells had a significantly higher level of H3K4m3 modification in the transcription-initiation-site of Pdx-1, Pax4, MafA and Nkx6.1 genes than that in mES cells and NIH3T3 cells (P<0.05), with significantly increased level of gene expression; (4) NIH3T3 cell had a significantly higher level of H3K9m3 modification in the transcription-initiation-site of Pdx-1, Pax4, MafA and Nkx6.1 genes than that in mES cells and with NIT-1 cell (P<0.05), with no detectable mRNA expression of these genes. It was concluded that histone modification (H3K4m3 and H3K9m3) and DNA methylation might have an intimate communication between each other in the differentiation process from embryonic stem cells into islet cells.

Objective To investigate the relationship between 9p21 single nucleotide polymorphism (SNP) and myocardial infarction(MI) in Yunnan Yi nationality. Methods One hundred and ten patients with MI and 110 controls were enrolled. DNA sequencing was used to detect 9p21 gene locus and SNP typing and analysis. Results Hardy-Weinberg equilibrium was found in 9 sites of chromosome 9p21 in myocardial infarction group and control group.The frequency of rs1333049 GG genotype was 30.91% and 18.18% in case group and control group respectively, G The frequencies of rs2383206 GG genotype were 31.82% and 18.18%, respectively, and the frequencies of G allele were 55.3% and 41.1%, respectively. There were significant differences between the two groups 58.3% and 44.9%, The difference between the two groups was statistically significant(P<0.05). Conclusion This study demonstrates an association of rs1333049 polymorphism locus on chromosome 9p21 with risk for MI in in Yunnan Yi nationality.

One hundred fifty-three patients with hypoxia of both sexes,underwent hip arthroplasty from January 2012 to January 2014,aged 65-97 yr,weighing 41-80 kg,living in areas at altitude above 1800 m,of American Society of Anesthesiologists physical status Ⅱ-Ⅵ,were selected.The hemoglobin (Hb) at 3-4 days after operation was collected,and the patients were divided into 3 groups depending on whether or not postoperative anemia occurred:no anemia group (Hb＞ 110 g/L);mild anemia group (90 g/L≤ Hb≤ 110 g/L);moderate anemia group (70 g/L ≤ H b＜ 90 g/L).The 36-item Short-Form Health Survey score and Harris Hip Score at 28 days after operation were collected.There was no significant difference between the three groups in the postoperative 36-item Short-Form Health Survey score and Harris Hip Score (P＞0.05).Postoperative mild and moderate anemia did not affect the short-term outcome after hip arthroplasty in elderly patients in high-altitude regions.

Objective To investigate the ED50 and ED9 5 of subarachnoid 0.375% bupivacaine in elderly patients undergoing elective total hip arthroplasty (THA ) in high altitude area. Methods Twenty-three elderly patients,aged 73-99 years old,ASA Ⅱ or Ⅲ,selected for total hip ar-throplasty (THA)under combined spinal-epidural (CSE)anesthesia were enrolled in this study and predose of 3.0 mg,0.375% (m/v)bupivacaine administered in subarachnoid,following dosage in-creased by ratio of 0.8.Calculate the ED50 and ED9 5 of subarachnoid 0.375% bupivacaine and 95%CI.Results Twenty of 23 underwent the test completely.The ED50 (95%CI)and ED9 5 (95%CI)of 0.375% (m/v)bupivacaine were 4.81 mg (3.98-5.81 )and 7.29 mg (6.03-8.81 ),respectively. Conclusion Subarachnoid 0.375% bupivacaine (7.5 mg)can be used effectively and safely in elderly patients undergoing total hip arthroplasty in high altitudes area.
