International Journal of Cerebrovascular Diseases 2012;20(6):413-417
Objective To investigate the value of T cell subsets predicting stroke-associated infection (SAI) in patients with acute ischemic stroke. Methods The consecutive patients with acute ischemic stroke within 36 hours after symptom onset were included. The peripheral venous blood samples were collected the next morning after admission.A flow cytometry was used to detect T cell subsets. The patients were divided into either an infected group or a non-infected group according to whether they had infection or not in hospital within 2 weeks. The information of the risk factors for ischemic stroke was collected, and the relationship between SAI and related risk factors was analyzed. Results A total of 55 patients with acute ischemic stroke were included,28 of them were in the infected group and 27 were in the non-infected group. The percentages of CD3 +T cells (63.42 ± 8.84% vs.69.55 ± 10.05% ; P =0.022) and CD4+ T cells (35.34 ± 7.10% vs,40.46 ±7.24%; P =0.014),and CD4+/CD8+ ratio (1.23 ±0.38 vs.1.55 ±0.56; P =0.023) in the infected group were significantly lower than those in the non-infected group. The multiple regression analysis showed that the higher NIHSS score (odds ratio [ OR] 1.295,95% confidence interval [ CI] 1.084 - 1.574; P =0.004) and t he decreased percent age of CD4+ T cells (OR 0.874,95 % CI 0.784 - 0.974,P =0.015) were the independent predicting factors for SA1.The percentage of CD4+ T cells < 70.35% had the best predictive value for the occurrence of SAI (sensitivity 77.8% and specificity 92.3%).Conclusions The decreased proportion of the auxiliary T cells after the onset of acute ischemic stroke may increase the susceptibility of SAI.The detection of T cell subsets may predict the SAI in a certain degree.

Objective To establish the screening platform of circulating tumor cells (CTCs) using acridine orange fluo?rescent (AO-F) dyeing method, and to apply it in the screening of peripheral blood CTCs in patients with kidney cancer. Methods Twenty-seven patients with metastatic renal cell carcinoma was included in this study. Primitive tumor cells and kidney cancer cell line 769-P were cultured with different concentrations of fetal bovine serum. Smears were prepared and observed under fluorescence microscopy. The percentage of AO-F positive staining of 769-P cells under 5 random sights was calculated. The sensitivity of AO-F staining to cells was evaluated. The 5 mL morning fasting venous blood was obtained from 10 subjects with healthy check-up. The 1×106 cell suspension was prepared. The logarithmic phase of renal tumor cells was used to prepare tube containing 500, 200, 100, 50 and 10 tumor cell suspension, which were mixed with 1×106 nucleated cells to establish CTCs model of renal cancer. AO-F staining method was used to detect the expression of AO-F positive cells. The correlation between expression of AO-F positive cells and clinical parameters was analyzed. Results The prima?ry cells and cell line 769-P showed similar bright color and morphological characteristics. The percentage of AO-F positive staining in 769-P cells was 93%±3%under 5 random sights. The recovery rates (%) of four groups (500, 200, 100 and 50 tu?mor cell suspension) were 10.2±3.8, 9.2±2.3, 10.8±2.6 and 10.5±1.9, respectively. There were no significant differences in recovery rates between four groups (P>0.05). The group of 10 tumor cell suspension could find AO-F positive staining cells occasionally. Zero case was positive in controls. Nine of 27 patients were positive and the rate was 33.33%. There were no significant statistical differences in AO-F positive rates between gender, age, tumor size, pathological pattern, Furhman stage, metastasis of lung and presence of tumor (P>0.05). Conclusion It is confirmed that the method of CTCs staining with AO-F, which has high specificity and reproducibility, is feasible to detect CTCs and worthy of being studied. There is a certain reference value to predict tumor recurrence and metastasis.

To establish a high performance liquid chromatography (HPLC) method for simultaneous determination of four alkaloids(arecoline, guvacoline, arecaidine, and guvacine) in Arecae Pericarpium (AP) and Arecae Semen (AS), and compare the contents of these four alkaloids between different medicinal parts. The chromatographic conditions were as follows:Welch SCX(4.6 mm×250 mm, 5 μm)column, with acetonitrile-0.2% phosphoric acid solution (adjusted to pH 3.85-3.90 with ammonium hydroxide) at 50:50 as the mobile phase, at a flow rate of 0.5 mL·min⁻¹. The column temperature was set at 35 °C, and the detection wavelength was 215 nm. The results of content determination in 7 batches of AS and 10 batches of AP showed that, the contents of 4 alkaloids in AS (arecaidine 0.020%-0.045%, guvacine 0.031%-0.086%, arecoline 0.194%-0.346%, and guvacoline 0.065%-0.094%) were generally higher than those in AP (arecaidine 0.10%-0.032%, guvacine 0.006%-0.029% arecoline 0.00%-0.070%, and guvacoline 0.00%-0.020%), and most of the APs had no arecoline and arecaidine at all in fruit peel. The above results indicated that different alkaloids can be used to distinguish the different medicinal parts of Arera catechu. Arecoline, guvacoline, arecaidine, and guvacine can be used as the quality control markers of AS, while for AP, only arecaidine and guvacine were needed.
