1.Clinical application of esophageal stent loaded with ~(125)I seeds: recent progress in research

Esophageal cancer is a common malignant tumor. Most patients have already in the late stage of the disease when the diagnosis is confirmed and have lost the surgery chance. In recent years, esophageal stent has been widely applied in the treatment of esophageal stenosis caused by esophageal cancer. The clinical experience shows that the esophageal stent can effectively solve the problem of dysphagia. However, the ordinary esophageal stent has no therapeutic effect on the esophagus tumor. The esophageal stent loaded with ~(125)I seeds can not only solve the dysphagia problem but also can treat the primary esophagus cancer with its brachytherapy effect. This article aims to review the clinical application and the up-to-date research progress of the esophageal stent loaded with ~(125)I seeds.
2.Mesocaval C-shunt plus ligation of splenic artery and esophagogastric (devascularization) in the treatment of type II Budd- Chiari syndrome

Objective To study a new operative method for treatment of hepatic venous occlasion without (associated) pathologic change of inferior vena cava or long-segment stricture. Methods A total of 44 cases of Budd-chiari syndrome with hepatic venous occlusion without pathologic change or long-segment stricture of (inferior) vena cava underwent combined mesocaval C-shunt, ligation of splenic artery, and esophagogastric (devascularization).Results Pre-shunt portal venous pressure was 36cmH2O(31~45 cmH2O, 1cmH2O=0.0098kPa) and post-shunt pressure fell to 26 cmH2O(21~33 cmH2O),the mean reduction was 10 cmH2O. One patient died of liver failure. A slight degree of hepatic encephalopathy occurred in 2patients who recovered after conservative treatment.Chylorrhea occurred in 4 patients, and it spontaneously disappeared 7d to 3.5months after operation. 39 patients(88.6% follow up) were followed up for 6months to 7years , and there was no case of recurrent bleeding nor hepatic encephalopathy. Ascites disappeared in 31cases,and was markedly reduced in 7 cases .The prosthetic grafts were patent as shown by color Doppler ultra sound in all followed-up patients.Conclusions This operation is simple and effective for B-CS with hepatic venous (occlusion) but not associated with inferior vena caval pathologic change or long-segment stricture.
3.Acute kidney injury network classification system for evaluation of acute kidney injury in patients with severe traumatic brain injury

Objective To evaluate the incidence,severity,risk tactors and impact to prognosis of acute kidney injury (AKI) in patients with severe traumatic brain injury (sTBI) by using acute kidney injury network (AKIN) classification system.Methods A retrospective analysis was carried out in 136 patients with sTBI hospitalized between January 2007 and May 2011.Demographic data,admission evaluation (whether with hernia or not on admission,systolic pressure and mean arterial blood pressure,serum creatinine and urea nitrogen,and blood glucose),outcome at 6 months post-injury and mortality were collected.Renal function was assessed using AKIN criteria.The patients were divided into two groups based on the presence or absence of AKI (non-AKI group and AKI group).According to the severity of AKI,AKI group was further classified as AKI grade 1 group,AKI grade 2 group and AKI grade 3 group.The differences among groups were analyzed.Results According to AKIN classification system,31 (23%) out of the 136 patients were diagnosed as being with AKI,including 21 cases (68%) in AKI grade 1 group and 10 cases (32%) in AKI grade 2 and 3 groups.The patients at older age and with lower Glasgow coma scale (GCS) on admission,higher levels of serum creatinine and blood urea nitrogen on admission were prone to AKI.As compared with TBI patients with normal renal function,TBI patients associated with AKI had higher mortality and worse outcome.Conclusions AKI is a common complication of patients with sTBI.AKIN classification system can early diagnose AKI in sTBI patients and may contribute to improvement of the outcome.
4.Effects of Hedan tablet on cytokins and oxidative stress factors in APOE-/-mouse

Objective To observe the effects of Hedan tablet on cytokines and oxidation factors in APOE-/-mouse, and to explore its effect on atherosclerosis and to explore its behind mechanism. Methods APOE-/-mice (n=50) were randomly divided into control group, model group, low dose Hedan tablet treatment group, high dose Hedan tablet treatment group and simvastatin treatment group. Mice in control group were given normal feed while mice in other groups were fed with high cho?lesterol diet. Hedan or Simvastatin was administrated intra-gastrically while normal saline was given to model group in the same route. After 12 weeks, mice were sacrificed to observe the mRNA level of tumor necrosis factor-α(TNF-αmRNA) in aorta by RT-PCR. Mean while, serum levels of interleukin-1 (IL-1), interleukin-10 (IL-10), malonaldehyde (MDA) and su?peroxide dismutase (SOD) were determined in different groups. Results Compared with control group, TNF-αmRNA tran?scription level as well as serum levels of IL-1 and MDA significantly increase while serum levels of IL-10 and SOD de?creased remarkably in model group, (P<0.01). Compared with model group, mRNA levels of TNF-αas well as serum levels of IL-1 and MDA were significantly decreased while serum levels of IL-10, SOD were greatly increased in low dose and high dose Hedan tablet treatment groups as well as in simvastatin treatment group (P<0.01). Conclusion Hedan tablet inhibit the formation of atherosclerosis through its anti-oxidation role and anti-inflammation role.
5.Syphilis seroconversion among drug addicts during methadone maintenance treatment in Maanshan city,Anhui province: incidence and risk factors

Objective To evaluate the incidence of and risk factors for syphilis seroconversion amomg drug addicts during methadone maintenance treatment (MMT).Methods A retrospective study was performed on drug addicts receiving MMT at MMT clinics in Maanshan,who were seronegative for syphilis before the treatment and followed up by serological tests for syphilis for more than two years.The incidence of and risk factors for syphilis seroconversion during the two years of follow up were analyzed in all the subjects.Results A total of 161 drug addicts were included in this study,of whom,15 experienced seroconversion for syphilis with the average seroconversion rate being 4.66 per 100 person-years.The seroconversion tended to occurr in the early stage of MMT.Both Chi-square analysis and multivariate logistic regression analysis showed that syphilis seroconversion was independently associated with nonuse of condoms during the last sexual behavior before receiving MMT.Conclusions Interventions should be undertaken as early as possible to promote safe sexual behaviors among drug addicts at methadone clinics,and to promote condom use is especially important for blocking sexual transmission of diseases.
6.Diagnosis and treatment of primary angitis of central nervous system.

Objective To investigate methods for the diagnosis and treatment of primary angitis of central nervous system (PACNS). Methods Radiological and clinical feature, operative resutls of 12 patients with mass lesion presentations of PACNS were analyzed retrospectively. Results The lesions showed low density on CT and long T1 and long T2 signal on MRI. The ring-wall enhanced lesions on MRI were flowery in 7 patients. Gross total resection of the lesions were performed in 10 patients with excellent recovery postoperatively. Growth of lesions were observed in 2 patients who underwent subtotal resection. Conclusions Lesions of PACNS have special appearance on enhanced MRI. More patients with mass lesion presentations of PACNS could be diagnosed preoperatively according to radiological and clinical feature.For these patients, surgery is the optimal treatment at present.
7.The clinical characteristics of pneumomediastinum in patients with dermatomyositis and polymyositis

Objective To analyze the clinical characteristics of pneumomediastinum in patients with dermatomyositis and polymyositis for demonstrating its pathogenesis and for predicting its prognosis. Methods The clinical records of 96 patients with PM/DM were reviewed, focusing on for perdicting its pneumomediastinum. Five patients with pneumomediastinum are described in detail. Case reports of pneumomediastinum in PM/DM in English publications are reviewed. Results Five DM cases complicated by pneumomediastinum all had lung infections. Twenty-nine cases (including our five cases) of DM/PM with pneumomediastinum have taken methylprednisolone, four cases alive, and six died. Nine cases have taken CsA,seven cases alive and two died. Conclusion The infections was strongly suspected as being responsible for the pneumomediastinum. Methylprednisolone has poor effect. CsA can be an effective therapeutic agent in PM/DM.

Seventy two diabetic subjects type Ⅱ were given konjac food for 65 days.The data analysed by multiple F-test indicated that the fasting blood glucosc(FBG), 2-h postprandial blood glucose (PBG) at the 30th and 65th day after consuming the food were significantly reduced (P = 0.001, P200mg%) decreased on the average by 51.8 and 84.6 mg% respectively, those with FBG-O 150-200 mg% by 24.1 and 68.7mg%, and those with FBG-0200 mg%).It could be concluded that konjac food was very useful in both prevention and treatment of hyperglycemia.

10.EFFECT OF ANISODINE ON BRAIN MONOAMINE

The effect of anisodine on brain monoamine was studied in 30 rats. The brain monoamine levels of caudate nucleus, hippocampus, diencephalon, brain stem and cerebral cortex were estimated by fluoro-metric method in drug-treated rats and saline-treated controls. Only the NE level of the brain stem was significantly increased in anisodine-treated animals 48 hours after injection. There were no significant differences in the dopamine values between anisodine-treated rats and saline-treated controls, while the 5HT and 5HIAA levels were significantly increased in anisodine-treated animals. The relation of brain monoamines to learning and memory is discussed. The increased 5HT and 5HIAA levels caused by anisodine may play a role in the impairment of memory.
