Objective To evaluate the incidence of and risk factors for syphilis seroconversion amomg drug addicts during methadone maintenance treatment (MMT).Methods A retrospective study was performed on drug addicts receiving MMT at MMT clinics in Maanshan, who were seronegative for syphilis before the treatment and followed up by serological tests for syphilis for more than two years. The incidence of and risk factors for syphilis seroconversion during the two years of follow up were analyzed in all the subjects. Results A total of 161 drug addicts were included in this study,of whom,15 experienced seroconversion for syphilis with the average seroconversion rate being 4.66 per 100 person-years.The seroconversion tended to occurr in the early stage of MMT.Both Chi-square analysis and multivariate logistic regression analysis showed that syphilis seroconversion was independently associated with nonuse of condoms during the last sexual behavior before receiving MMT.Conclusions Interventions should be undertaken as early as possible to promote safe sexual behaviors among drug addicts at methadone clinics, and to promote condom use is especially important for blocking sexual transmission of diseases.

Objective To investigate the clinical value of PET-CT in aiding CT guided percutaneous puncture biopsy for lung mas-ses.Methods The retrospective data were analyzed including 5 7 patients with lung masses who underwent CT guided percutaneous puncture biopsy combined with PET-CT (regarded as a research group)and other 75 similar patients with single CT-guided biopsy (control group). In two groups,it was divided with malignant, suspicious malignant, can not be diagnosed, inflammatory, benign by pathological findings, and was contrasted with the surgical pathology or final clinical results. To calculate sensitivity, specificity and accuracy of diagnosis for cancer through two puncture method.Recorded complication for pneumothorax and bleeding in two groups .Be compared analysis using the 2 test. Results In study group,it was higher for the diagnosis of malignancy sensitivity, specificity, accuracy. There were 96.3% (52/54),100.0%(3/3)and 96.5%(55/57)in research group, while those were 88.4%(61/69),83.3% (5/6)and 88.0% (66/75)in the control group, respectively. And no statistically significant.Between research and the control group, there was a significantly statistical difference in the rate of twice punctures in one mass (χ2= 10.122,P=0.001).Between the two groups, there was no statistically significant difference in the pneumothorax rate and the hemorrhage rate. Conclusion PET-CT in aiding CT-guided lung biopsy improves the sensitivity, specificity and accuracy of diagnosis.

Objective To observe the effects of constant light exposure on the obesity in high fat diet rats. Methods Thirty-two male SD rats were randomly divided into four groups:rats on a normal chow exposed to standard light-dark cycle ( group A) , rats on a normal chow exposed to constant light ( group B) , rats on a high fat diet exposed to standard light-dark cycle ( group C) , and rats on a high fat diet exposed to constant light ( group D) . Body weights and food intakes were recorded weekly throughout the 12-week study. Body weight, fat mass, visceral adipose tissue weight, intraperitoneal glucose tolerance test ( IPGTT) results, insulin resistance parameters, serum lipids and levels of interleukin 6 (IL-6), tumor necrosis factor-α(TNF-α) were compared among groups. Epididymal adipose tissues mRNA expression of circadian clock genes, i. e. clock, bmal1, rorα, rev-erbα, cry1, per1, and per2 were analyzed by realtime PCR. Results From the 9th week, body weights of rats in group D were significantly higher than those in group C (all P<0. 05). At the 12th week, area under curve of IPGTT (AUC-IPGTT) in groups B, C, and D were significantly higher than that in group A. AUC-IPGTT in group D was significantly higher than that in group C (all P<0.05). Compared with group C,asignificant increase in fat mass, visceral adipose tissue weight, homeostasis model assessment for insulin resistance, serum cholesterol, TNF-α levels were observed in group D ( all P<0. 05). And a significant decrease in quantitative insulin sensitivity check index ( QUICKI) and high density lipoprotein-cholesterol were observed in group D in comparison with group C (both P<0. 05). Circadian clock genes (clock, rorα, rev-erbα, cry1, per1) mRNA expressions in group B and D were significantly different from those in group A (all P<0. 05) . Expression of cry1 in group D was significantly higher than that in group C. In group C, rev-erbαmRNA expression was significantly down-regulated in comparison with group A (P<0. 05). Conclusion Constant light exposure exaggerates obesity, glycolipid metabolism abnormality, inflammation, and insulin resistance in high fat diet rats.

BACKGROUNDTo evaluate the reversal effect of high dose tamoxifen on multidrug resistance to EP regimen in patients with non-small cell lung cancer.
METHODSA total of 41 patients with NSCLC were studied, who were resistant to EP regimen and were proved to have P-gp protein overexpression. All patients were randomizedly divided into two arms. Reversal group (n=21) received oral tamoxifen 100?mg, 2 times everyday on D1-5, together with EP regimen. Control group (n=20) were only given EP regimen.
RESULTSIn reversal group, complete response occurred in 1 patient, and partial response in 5; disease remained stable in 11 patients, and tumor progression occurred in 4 patients. The response rate was 28.6%(6/21). In control group, no response occurred; 9 patients had stable diseases, and the other 11 progressed. There was a significant difference in response rate between the two groups (P=0.012?1). In reversal and control groups, the median survivals were 8.4 and 4.6 months respectively (P < 0.01), and 1-year survial rates were 38.1% and 35.0% respectively. Reversal of P-gp occurred in 7 cases of reversal group (33.3%),and none in control group (P= 0.005?2) . There was no significant difference in toxicities between the two groups (P > 0.05).
CONCLUSIONSHigh dose tamoxifen can remarkably downregulate the expression of P-gp and partially reverse the multidrug resistance to EP regimen for non-small cell lung cancer.
