1.Clinical application of esophageal stent loaded with ~(125)I seeds: recent progress in research

Esophageal cancer is a common malignant tumor. Most patients have already in the late stage of the disease when the diagnosis is confirmed and have lost the surgery chance. In recent years, esophageal stent has been widely applied in the treatment of esophageal stenosis caused by esophageal cancer. The clinical experience shows that the esophageal stent can effectively solve the problem of dysphagia. However, the ordinary esophageal stent has no therapeutic effect on the esophagus tumor. The esophageal stent loaded with ~(125)I seeds can not only solve the dysphagia problem but also can treat the primary esophagus cancer with its brachytherapy effect. This article aims to review the clinical application and the up-to-date research progress of the esophageal stent loaded with ~(125)I seeds.
2.STUDY OF THE CORRELATION BETWEEN ANGIOGENESIS AND PROGNOSIS OF BREAST CARCINOMA

92 specimens from patients with invasive ductal carcinoma were investigated. The expression of vascular endothelial growth factor (VEGF) was demonstrated by immunocytochemical staining with an antibody against VEGF and was quantitatively estimated by using computerized image analysis system. Vessels were immunohistochemically highlighted by using an antibody to CD34, and microvessel density (MVD) was quantified. The postoperative survey in univariate analysis showed that the relapse-free-survival (RFS) time of patients with more than 87 microvessels in single microscopic field was significantly worse compared to that of patients with less than 87 microvessels in node-negative patients (P<0.05). Multivariate analysis confirmed that MVD was an independent prognostic indicator for RFS in node-negative patients and all patients (P<0.05~0.01). In addition, there was a closely positive correlation between VEGF and MVD (P<0.01). The results suggested that VEGF plays crucial roles in the promotion of angiogenesis in primary breast carcinoma. VEGF and MVD are closely correlated with biological behavior of primary breast carcinomas. MVD can provide a useful message in predicting the recurrence or metastasis of tumors.

4.Diagnosis and treatment of calcifying pseudoneoplasm of thoracic spine

Objective To explore the characteristics of calcifying pseudoneoplasm of the neuraxis ( CAPNON) , and to improve the di-agnostic level and standard of treatment. Methods The clinical traits, imaging features and curative effect of one case of CAPNON who were treated in our departmen were analyzed combined with the pathological analysis and literature review. Results Transient back pain was the only clinical presentation of the patient. CT images showed obvious calcification. MRI showed low signal in T1-weigthed images and T2-weighted images. Examined with the contrast-enhanced MR images, the lesion showed inhomogeneous enhancement. No nervous symptom newly occurred in the 6-month follow-up. Conclusion Symptoms of patients with CAPNON are generally related to local compression or irri-tation of the adjacent tissue. CAPNON should be highly suspected in patients with dense calcification in the CT combined with low signal in T1-weigthed images and T2-weighted images and rim or internal inhomogeneous linear enhancement in the MRI. The final diagnosis depends on the histopathological analysis, and surgical resection is the major therapy.
5.Repair of bone defect with chitin/rhBMP2/collagen complex

Objective: To investigate the feasibility of chitin as bone substitute material and carrier of rhBMP2.Methods: Porous chitin and chitin/rhBMP2/collagen complex were implanted into calvarial defects in 8 rabbits. Bone repairing ability was assessed by radiographic and histological observation. 2 rabbits without implantation were served as controls. Results: Chitin had certain bone conductive ability. When combined with rhBMP2,a complex possessing both bone conductive activity and bone inductive activity was produced. The complex had greater bone repairing ability than chitin alone. Conclusion: Chitin may be used as a bone substitute material and carrier of BMP. But its mechanical strength and surface activity should be improved.
6.Acute kidney injury network classification system for evaluation of acute kidney injury in patients with severe traumatic brain injury

Objective To evaluate the incidence,severity,risk tactors and impact to prognosis of acute kidney injury (AKI) in patients with severe traumatic brain injury (sTBI) by using acute kidney injury network (AKIN) classification system.Methods A retrospective analysis was carried out in 136 patients with sTBI hospitalized between January 2007 and May 2011.Demographic data,admission evaluation (whether with hernia or not on admission,systolic pressure and mean arterial blood pressure,serum creatinine and urea nitrogen,and blood glucose),outcome at 6 months post-injury and mortality were collected.Renal function was assessed using AKIN criteria.The patients were divided into two groups based on the presence or absence of AKI (non-AKI group and AKI group).According to the severity of AKI,AKI group was further classified as AKI grade 1 group,AKI grade 2 group and AKI grade 3 group.The differences among groups were analyzed.Results According to AKIN classification system,31 (23%) out of the 136 patients were diagnosed as being with AKI,including 21 cases (68%) in AKI grade 1 group and 10 cases (32%) in AKI grade 2 and 3 groups.The patients at older age and with lower Glasgow coma scale (GCS) on admission,higher levels of serum creatinine and blood urea nitrogen on admission were prone to AKI.As compared with TBI patients with normal renal function,TBI patients associated with AKI had higher mortality and worse outcome.Conclusions AKI is a common complication of patients with sTBI.AKIN classification system can early diagnose AKI in sTBI patients and may contribute to improvement of the outcome.

8.Neuroradiologic and clinicopathologic features of dysembryoplastic neuroepithelial tumor

Objective To evaluate MRI or CT appearance and clinicopathologic features of dysembryoplastic neuroepithelial tumor (DNT). Methods MRI or CT appearance and clinicopathologic features in 12 cases of histopathology confirmed DNT were retrospectively studied. Results There were 6 men and 6 women, and the age ranged from 12 to 68 years with the average age of 36.7 years. Most patients had partial seizures, but the neurological deficits were absent. All lesions were located in the supratentorial region and involved the cerebral cortex on MRI scan. Localization of lesions was predominantly in the frontal (n=4) and temporal (n=3) lobes. The maximum lesion diameter ranged in size from 2 cm to 5 cm, and the lesion showed round, lobular- or irregular-shaped. The white matter was involved in 2 cases, and 7 were with cystic change (58.3%). All lesions were hypointense on T1-weighted images and hyperintense on T2-weighted images as well as devoid of peritumoral edema or mass effect. MRI signal intensity of cystic lesions was homogeneous on T1-weighted images which was equal to or slightly higher than that of cerebrospinal fluid. CT scan of 6 cases disclosed moderately hypodense lesion, 2 of which were markedly hypodense cystic-lobular shaped, and foci of calcification was observed in 1 case. Contrast enhancement was absent in 4 cases and only 1 case showed slight enhancement unevenly. Histologically, the DNT were classified into three subtypes: simple form (n=4), complex form (n=6), and non-specific variant (n=2). All patients experienced lesion gross total or subtotal surgical removal, and had received no chemo- or radiotherapy postoperatively. Conclusion DNT is a benign lesion, and its characterization is better disclosed by MRI compared to CT scan.
9.Mesocaval C-shunt plus ligation of splenic artery and esophagogastric (devascularization) in the treatment of type II Budd- Chiari syndrome

Objective To study a new operative method for treatment of hepatic venous occlasion without (associated) pathologic change of inferior vena cava or long-segment stricture. Methods A total of 44 cases of Budd-chiari syndrome with hepatic venous occlusion without pathologic change or long-segment stricture of (inferior) vena cava underwent combined mesocaval C-shunt, ligation of splenic artery, and esophagogastric (devascularization).Results Pre-shunt portal venous pressure was 36cmH2O(31~45 cmH2O, 1cmH2O=0.0098kPa) and post-shunt pressure fell to 26 cmH2O(21~33 cmH2O),the mean reduction was 10 cmH2O. One patient died of liver failure. A slight degree of hepatic encephalopathy occurred in 2patients who recovered after conservative treatment.Chylorrhea occurred in 4 patients, and it spontaneously disappeared 7d to 3.5months after operation. 39 patients(88.6% follow up) were followed up for 6months to 7years , and there was no case of recurrent bleeding nor hepatic encephalopathy. Ascites disappeared in 31cases,and was markedly reduced in 7 cases .The prosthetic grafts were patent as shown by color Doppler ultra sound in all followed-up patients.Conclusions This operation is simple and effective for B-CS with hepatic venous (occlusion) but not associated with inferior vena caval pathologic change or long-segment stricture.
10.Depression in systemic lupus erythematosus

Objective To investigate the level of depression in patients with systemic lupus erythematosus (SLE).Methods Fifty six in patients and out patients with SLE were investigated with questionnaire of self rating depressive scale (SDS).Results Of these patients 41% had clinical features of depression.Tiredness,hebetude,hyposexuality,despair,irritability,depression,thinking difficulty,insommia,uneasiness and vacancy were more common.Most of these patients are manual worker,poorly educated ( P 0 05).Conclusion Patients with SLE have symptoms of depression to a certain extent.Doctors should pay close attention to their mentality and emotion in diagnosis and treatment of the prtients with SLE.
