Vascular cognitive impairment refers to all cognitive impairment syndromes associated with vascular diseases,including severe cognitive impairment from mild cognitive impairment to vascular dementia.Citicoline is a necessary intermediate of the phosphatidylcholine biosynthesis,an important component of the neuronal membranes.Experimental studies have shown that citicoline has neuroprotective and neurorepair effects in cerebral ischemia.Clinical studies have shown that citicoline may improve cognitive function in patients with vascular cognitive impairment.
2.High-resolution ultrasonography for distinguishing benign from malignant thyroid nodules

Objective The extensive use of high-resolution thyroid ultrasonography has markedly improved the detection rate of thyroid nodules ( TNs) .However, the ultrasonographic features of benign and malignant TNs usually overlap and have yet to be de-fined.This study aimed to investigate the ultrasonographic features and risk factors of malignant TNs by comparing their ultrasonograph-ic manifestations with those of benign TNs. Methods A total of 119 TNs (63 benign and 56 malignant) in 64 patients were includ-ed in this retrospective study.The gender, age, location of the TNs, and other variables were subjected to univariate analysis, the risk factors of malignant TNs were screened by multivariate logistic regression analysis, and the predictive value of the logistic regression model was assessed using the ROC curve. Results Multivariate logistic regression analysis indicated that the risk factors of malig-nant TNs in ultrasonography were irregular shape (OR:6.166, 95%CI:1.681-22.617), micro calcification (OR:3.593, 95%CI:1.800-7.169), and A/T ratio≥1 (OR:17.577, 95%CI:1.739-166.468).ROC curve analysis showed the area under the curve (AUC) to be 0.922 (95%CI:0.868-0.976). Conclusion Such ultrasonographic manifestations as irregular shape, micro-calcification, and A/T ratio≥1 are valuable in identifying the nature of TNs.
3.DETECTION OF GENE MUTATIONS IN HBV PRECORE AND BASIC CORE PROMOTER REGION USING MICROARRAY TECHNIQUE

Objective To investigate the clinical value of micro-array technique in the detection of HBV pre-core and basic core promoter region variants. Methods Four spot mutations of A1896(nt1896G→A) and A1899( nt1899G→A) in precore region and T1762A1764(nt1762A→T,nt1764G→A) in basic core promoter region in 46 patients of acute and chronic liver diseases were detected by gene chips to investigate the clinical value of micro-array technique. Results Micro-array technique had a high specificity in the detection of specific mutation, and the positive rate was 87.0%. A1896 mutation was found in 18 cases (45.0%), A1899 mutation in 10 cases (25.0%), T1762 and A1764 double variants in 30 cases (75.0%), and poly-sites variants in 14 cases (35.0%). Bioeleotrical activities of human cerebral cortex which control and regalate movements get to increase during voluntary movements, the metabolic level of the cells of motor areas change at the same time,and consequently regiond cerebrd blood flow (rCBF)changes. Different changes of rCBF indicate the range of motor areas with which the relavant movemats are involved.And studies of this take great significance in finding out the basic active regalarity and features of human brain. 5.Effect of Exercise on Skeletal Muscle Metabolism

Influenced by the intensity, volume of exercise, the basic metabolic features of skeletal muscle are changed to such structural and functional adaptations as there are increases in the amount of contractile components, muscle glycogen and myoglobin, in the volume and quantity of mitochondria and in the density of capillary in skeletal muscle, which result in enhanced aerobic metabolism, contraction efficiency and prolonged time defore failure. It is also pointed out that the structural changes and decrease of function of sarcoplasmic reticula during acute exercise are important reasons for skeletal muscle fatigue , whereas, high- intensity training can improve sarcoplasmic reticulum function and its Ca2+ -intake rate, so that to improve the physiological efficacy of excitation- contraltion coupling. So it is put forward that anti- resistance training, as a means of exercise therapy, can efficiently increase muscle strength and prevent muscle atrophy. 6.BONE RECONSTRUCTED BY SEEDING HUMAN BONE MARROW STROMAL CELLS ON POROUS CERAMIC

To assess the ability of new bone formation with composite human bone marrow stromal cell (HBMSC)/coralline hydroxyapatite(CHA) in vivo. Human bone marrow collected from adult ilium by needle aspiration was cultured in vitro to expand and differentiate the stromal cells. The cells were seeded on CHA discs, then the HBMSC/CHA composites were implanted subcutaneously into athymic nude mice (CHA alone as control). At 4 and 8 weeks after implantation, the effectiveness of bone formation was evaluated by means of gross and histological observations. The results showed that HBMSC/CHA composite could form bone tissue in vivo, in which a small amount of HBMSC could be used as seed cells in bone tissue engineering, and CHA could be used as a kind of scaffold.
7.Catheter ablation guided by three-dimensional electroanatomic mapping system in combination with pulmonary vein antrum potential in patients with atrial fibrillation

BACKGROUND:Presently used pulmonary vein antrum location methods mainly performed by three-dimensional electroanatomy combined with X-ray image or CT image fusion.These methods conducted vein antrum location and ablation by anatomy instructions.It is still poorly understood whether the ablation hit the key part of atrial fibrillation.OBJECTIVE:To explore the feasibility of catheter ablation guided by three-dimensional electroanatomic mapping system in conjunct with pulmonary vein antrum potential in patients with atrial fibrillation.DESIGN,TIME AND SETTING:The verification clinical study was performed at the Department of Cardiology of Jinan Fourth People's Hospital and Hebei People's Hospital from March 2007 to June 2009.PARTICIPANTS:Fifty-one patients with drugs refractory,paroxysmal atrial fibrillation were included.METHODS:All patients underwent circumferential pulmonary vein antrum ablation guided by three-dimensional electroanatomic mapping system (CARTO) in conjunct with pulmonary vein antrum potential with the endpoint of electrical isolation.Relevant parameters and ablation success rate were observed.MAIN OUTCOME MEASURES:Procedure-related parameters,such as procedure duration,fluoroscopy duration,cumulative success rate and complication were observed.RESULTS:Pulmonary veins were isolated in all 51 patients.The mean procedure duration,fluoroscopy time and radiofrequency ablation duration are respectively (207±36.7) minutes,(38.2±14.3) minutes,(56.4±15.7) minutes.After (17.5±3.8)-months follow-up,forty (78.4%) patients did not have recurrence of atrial fibrillation,atrial flutter or atrial tachycardia.No severe procedure-related complication had happened.CONCLUSION:Pulmonary vein antrum potential can be used as a landmark to define pulmonary vein antrum,that combined with CARTO system to guide pulmonary vein antrum ablation is effective,safety and feasible for catheter ablation of atrial fibrillation.
8.Dynamic phosphoproteomics of insulin and epidermal growth factor signaling in mouse hepatocytes

BACKGROUND: Both epidermal growth factor (EGF) and insulin transfer their signals into cells by two primary signal transduction pathways,including phosphatidylinositol 3-kinase (PI3K) pathway and mitogen activated protein kinases (MAPK) pathway.But they have different physiological functions.OBJECTIVE: To comparatively assay the dynamic behaviors of phosphoproteomes between EGF and insulin signal transductions in mouse hepatocytes and find key signal proteins.DESIGN,TIME AND SETTING: Randomized grouping controlled observation experiment was performed in the laboratory of Molecular Biology,Luzhou Medical College between July 2005 and April 2006.MATERIALS: Hepatocytes were from Kunming mice of closed population.METHODS: The primarily cultured mouse hepatocytes were labeled with 32p isotope and then randomly divided into three groups: control,EGF-stimulated (received 10 μg/L EGF),and insulin-stimulated (received 100 nmol/L insulin) groups.MAIN OUTCOME MEASURES: After mouse hepatocytes were treated with EGF and insulin for 0,5,20,60 and 120 minutes,the dynamic behaviors of phosphoproteomes(I.e,phosphorylated level) between EGF and insulin signal transductions were comparatively analyzed by two-dimensional electrophoresis method.RESULTS: The categories of all phosphorylated proteins between EGF and insulin-stimulated phosphoproteomes had no apparent difference.The dynamic behaviors of phosphoproteomes of most proteins during EGF signal transduction are parallel with those during insulin stimulation,except the dynamic behaviors of 4 proteins are different significantly.CONCLUSION: Aforementioned 4 phosphorylated proteins were most probably the key members that could distinguish between two signal transduction pathways ornetworks,and determined their major physiological functions respectively.
9.Delta three-dimensional semiconductor array verification for intensity modulated planning of helical tomotherapy

Objective To investigate the feasibility of dose verification of intensity modulated planning of helical tomotherapy (HT) using three-dimensional semiconductor array (Delta4) and find a more time-consuming and accurate method to validate the delivery dose.Methods Delta4 detector array was used to verify the HT plan dose distribution of 10 patients.The precise setup of detector array was guided and registered by MVCT imaging.After the implementation of delivery,the measured dose distributions on the corresponding planes were compared with those calculated by the treatment planning system (TPS).According to the different acceptance criteria combination of dose difference or distance to agreement at different threshold level including 3%/2 mm,3%/3 mm,3%/4 mm,3%/5 mm at TH5,TH10,TH20,TH30,TH70,TH90,the γ analysis method was utilized to evaluate the dose verification.Results The dose distribution measured by the Delta4 was well consistent with that calculated by the TPS.The mean γpassing rates were all above 94.89%.Conclusions Delta4 detector array can get satisfying results when used in HT plan dose verification and is a viable tool to replace traditional film dosimetry system in clinical quality assurance work.
10.Analysis of risk factors of peripheral neuropathy in patients with type 2 diabetes

Objective To analyze the risk factors of type 2 diabetic peripheral neuropathy (DPN),in order to provide incidence for clinical treatment and prevention.Methods Two hundred and eighty-seven patients with type 2 diabetes were divided into DPN group(113 cases) and non-DPN group(174 cases)according to electrophysiological examination and diagnosis.The clinical information were collected including body mass index (BMI),blood pressure,fasting plasma glucose (FPG),2 h postprandial plasma glucose (2 h PG),fasting plasma insulin (FINS),C peptide,glycosylated hemoglobin (HbAl c),blood fat and cholesterin.The DPN prevalence in patients with type 2 diabetes was calculated,and t or x2 analysis and multivariate logistic regression analysis were applied.Results Among the 287 patients with type 2,the DPN prevalence was 39.4% (113/287).The level of age,duration of diabetes and smoking in DPN group were significantly higher than those of non-DPN group,while exercise,income situation and educational background were significantly lower than those of non-DPN group (x2 =4.378,8.430,4.525,4.500,4.203,6.890,P < 0.05 or < 0.01).Systolic blood pressure((137.52 ± 16.10) mmHg),FPG ((11.42 ± 3.08) mmol/L),2 hPG ((18.70 ± 4.61) mmol/L),HbA1c ((10.21 ± 2.50)%) in DPN group were higher than those of non DPN group ((systolic pressure (132.67± 15.80) mmHg,FPG(9.96 ±3.76) mmol/L,2 hPG(15.38 ±5.26) mmol/L,HbA1c(9.54 ±2.83)%).In DPN group,Fasting insulin,2 h insulin,fasting C peptide were (13.52 ± 4.92) mmol/L,(36.20 ± 17.52) mmol/L,(1.44 ± 0.62) mmol/L,(3.89 ± 3.01) mmol/L,lower than those of non DPN group ((16.76 ± 5.24) mmol/L,(47.95 ± t5.04) mmol/L,(1.83 ± 0.57) mmol/L,(5.24 ± 3.45) mmol/L),and the differences were significant (t =2.512,3.592,5.635,2.105,5.312,5.863,5.372,3.502,P <0.05 or P < 0.001).Multiple logistic regression analysis revealed that the level of duration of diabetes,HbA1c,2 hPG were positively correlated with DPN prevalence,and those were the independent risk factors of DPN (OR(95%CI) 1.040(1.018-1.062),1.331(1.032-1.717),1.366(1.044-1.787),P<0.05).The level of FINS,Fasting C peptide,2 h C peptide were negatively correlated with DPN prevalence,and those were independent protective factors of DPN (OR (95 % CI) 0.803 (0.725-0.889),0.923 (0.731-0.954),0.863 (0.801-0.930),P < 0.05).Conclusion The occurrence of DPN is common in patients with type 2 diabetes.The indices of duration of diabetes,HbA1c,2 hPG were risk factors of DPN,and there is more dangerous with the lower level of FINS,Fasting C peptide,2 h C peptide.
