1.Jiangtang Mixture Combined with Ear Acupuncture for Type 2 Diabetic Insulin Resistance:A Clinical Observation of 40 Cases

Objective To study the therapeutic effect and mechanism of Jiangtang Mixture combined with ear acupuncture for type 2 diabetic insulin resistance(IR).Methods Totally 120 cases of type 2 diabetic IR were randomly and equally divided into western medicine(Metformin) group,Chinese medicine (Jiangtang Mixture) group and Chinese medicine with ear acupuncture group.The treatment lasted for one month.The therapeutic effect,fasting plasma glucose(FPG),fasting insulin(FINS),postprandial 2 hours plasma glucose(P2hPG),hemoglobin A1c(HbA1c),blood fat,insulin Sensitivity index(ISI) and insulin resistance index(IRI) were observed before and after treatment.Results The total effective rate in Chinese medicine with ear acupuncture group was 92.5%,which was superior to those in Chinese medicine group (P
2.Genetic variability and interrelationships of mainly quantitative traits in Glycyrrhiza uralensis cultivated population.

OBJECTIVEThe main aim of the research was to evaluate genetic variability and interrelationships of mainly quantitative traits in 2-year population, and provide a basis for high-yield breeding of Glycyrrhiza uralensis.
METHODFour genotype G. uralensis population were transplanting in four different environment using complete randomized block design with three replication, and the 10 quantitative traits, including plant height (PH), stem diameter (SD), tiller number (TN), taproot length (TRL), root length (RL), root diameter (RD), diameter of 20 cm below the root head (D20), taperingness (TR), lateral root number (LRN) and root fresh weight (RFW) were measured in field.
RESULTThe difference among population for all evaluated traits were significant (P<0.05) through Duncan's multiple range tests, and the coefficient of variation of RFW and LRN were above 25%. The analysis of variance was used to evaluate the traits of four populations across to four different environment Genotype, environment and their interaction effect were significant (P<0.05) or highly significant (P<0.01) for mainly evaluated traits. Simple correlation between traits showed that PH, SD, LRN, RL, RD and D20 had highly significant (P<0.01) and positive correlation with RFW. Results of the path coefficient analyses showed that D20 had the greatest positive direct effect on RFW, followed by the traits of PH and RL.
CONCLUSIONSelection for increased D20, RL and PH would be the best indirect selection traits for increasing root yield. Meanwhile, ample genetic variability exists in the G. uralensis 2-year population, it could be used for breeding improvement of root yield.

3.Health status and needs of occupational population in an industrial park in Shanghai, China

Objective:To investigate the health status and needs of the occupational population in an industrial park in Shanghai, and to explore the causes.Methods:The study was a mixed method study. First, a list of health needs of the occupational population was developed through literature review. Then, a questionnaire was prepared on the basis of the list of health needs of the occupational population, which consisted of 4 parts: basic personal information, health status, medical care, and health needs. From September 16 to October 9, 2022, 3 enterprises of representative size and industry in Shanghai Xinzhuang Industrial Park were selected by convenience sampling method, and the target sample size was allocated in equal proportions according to the total number of employees, and the Quick Response code of the electronic questionnaire was distributed (Questionnaire Star). Finally, an interview outline was created based on the results of the questionnaire survey to gain a deeper understanding of the connotations and reasons for the health needs of employees, etc. The interviews were conducted from October 15 to 22, 2022.Results:A total of 819 enterprise employees were surveyed, of whom 580 (70.82%) were male, and their age was mainly concentrated in the 19-44 age group (621(75.82%)). Among them, 348 (42.49%) were overweight or obese, 388 (47.37%) had regular medical check-ups, and only 224 (27.35%) had active health interventions. The top 5 health needs of the surveyed population were relief of visual fatigue (365(44.57%)), improvement of sleep (355(43.35%)), relief of physical fatigue (343(41.88%)), oral health (333(40.66%)), and immunity enhancement (332(40.54%)). The interviewees were from 2 large enterprises, totaling 12 people. The results of the interviews showed that excessive use of mobile phones, high stress, interpersonal and social confusion, retaliatory late nights, sedentary lifestyles, and socializing were the main causes of health problems.Conclusions:The health status of the occupational population in Shanghai Xinzhuang Industrial Parks is not optimistic, and their health needs are obvious. Their health problems and needs are related to the characteristics of their work patterns.
4.Antibacterial effect of the carboxymethyl chitosan zinc peptide on several periodontal pathogens in vitro

Objective To determine the antimicrobial effects of carboxymethyl chitosan zinc (CMC-Zn +) and CMC-Zn +-peptide (CMC-Zn +-P) on four kinds of periodontal pathogens.Methods Dilution method was used to determine the minimum inhibitory concentration(MIC) of CMC-Zn + for Porphyromonas gingivalis(Pg),Actinobacillusactinomycetemcomitans(Aa),Prevotella intermedia(Pi) and Actinomyces viscosus(Av).The antimicrobial characters of CMC-Zn+ and CMC-Zn+-P on these four kinds of pathogens were evaluated by disk diffusion method.Results The MIC of CMC-Zn+ for Pg,Aa,Pi and Av was 0.312 5%,0.156 25%,0.156 25% and 0.078 125% respectively.Significant antimicrobial effects were improved along with the increased concentration of CMC-Zn+ and CMC-Zn+-P(P<0.01),while the best antimicrobial concentrations of both CMC-Zn+ and CMC-Zn+-P were 5%.The antimicrobial effect of CMC-Zn+-P was better than that of CMC-Zn+ on the same kind of periodontal pathogen(P<0.01).CMC-Zn+-P showed different antimicrobial effects on the four periodontal pathogens(Av>Aa>Pi or Av>Aa>Pg,P<0.01).Conclusions CMC-Zn + and CMC-Zn +-P have inhibition effect on Pg,Aa,Pi and Av,and the best antibacterial concentration was 5%.The CMC-Zn+-P has better antibacterial effect than CMC-Zn+ on Pg,Aa,Pi and Av.
5.Study on quality improvement in organisational structure and management model of integrated community child health services

Objective:To explore the quality improvement in the organization structure and management model of the integrated community child health services.Methods:This was a qualitative study, including two parts: cause analysis and service improvement suggestions. In the analysis part the data mining was conducted to identify valuable patterns and relationships in the comprehensive child health services. Semi-structured interviews were conducted with 12 relevant department heads and health workers of the comprehensive child health service team at Gumei Community Health Service Center in December 2023, and the causes of the key problems were explored. In the service improvement part, focus group discussions were held to propose suggestions, then improvement measures were formulated to address the identified problems.Results:Through data mining and semi-structured interviews, the key problems were identified: information isolation among multiple departments and lack of coordination mechanism in the comprehensive child health service team. A team organizational structure based on the "three definite" principle was established. The organizational structure should include the pediatric family doctor team, general practitioner management team and departments of pediatrics, maternal and child health care, immunization and child rehabilitation; the management model should include a cross-department resource and information sharing mechanism, the pediatric family doctor model, optimization and integration of physical space, and enhancement of publicity activities for the comprehensive child health services.Conclusion:Based on the analysis in Gumei health service center, this study identified key problems in community integrated child health services, and proposes the quality improvement measure in the organizational structure and management model of the service team.
6.Practice and thinking of building active learning general practice team based on learning health system

By sorting out the differences and connections between family doctor teams and specialized disease teams, role competency and mutual collaboration, and introducing the learning health system (LHS) mechanism, a comprehensive operating system for community general practice learning organizations based on LHS was constructed, focusing on five single disease types. The system includes a combination of general and specialized medicine that links three levels of medical institutions, thereby opening up the business cooperation process between professionals in different institutions, and establishing a sustainable collaboration mechanism. This allows medical institutions at three levels to continuously tap the potential of their disciplines, achieve their own ability growth and feel higher work value, and also bring better health solutions to residents, guided by the common goal of "health centered, patient centered".
