1.Establishment of our hospital's contingency plan for clearing partial troubles in the HIS

The paper expounds the connotation of HIS troubles, the methods for determining HIS troubles and the necessity of establishing a contingency plan for clearing such troubles. It also puts forward some specific measures for managing the contingency plan: ①defining the accountability and division of work in clearing various kinds of troubles; ②doing a good job of coordination in clearing troubles; ③carrying out strict training for the relevant personnel; and ④conducting checks and supervision all the time. Establishment of the plan makes it clear to HIS managers that enough attention ought to be paid to the prevention of troubles.
2.The building and application of Bank-hospital connection system

Objective:Through the construction of the Bank-Hospital connecting project, the enduring problems in clinic such as the long waiting time at registration, check out, payment, and other non-clinic issues are solved. Meanwhile, the clinic medication procedure is further regulated, which provides a faster medical assistance service for patients and elevates the ability to apply medical informationization.Methods:Based on the current HIS platform, the banking and mobile systems connected as a whole network and constructed the banking-medication platform. Results: The waiting time of patients is largely reduced by expanding the channels of registration, applying multiple cards as medication card, and introducing local charging, self-payment systems at every floor, and mobile intelligent platform.Conclusion: The Bank-Hospital connecting project brings convenience to patients and increases our efficiency.
3.Radiological Diagnosis of Hemangioma of Long Bones in Children

Objective To improve the knowledge of radiological manifestations , pathologic basis and differential diagnosis of hemangioma of long bones in children.Methods 7 cases with hemangioma of long bones proved by surgery and pathology were retrospectively analyzed .Results The X-ray features mainly included : (1) "soap bubbles" or cystic appearance;(2) "mesh" sign or "honeycomb" appearane,with or without cystic bone destruction;(3) Localized cortical thickening.Conclusion Radiographic findings of hemangioma of long bone is varied and relevant to pathological type, growth speed and pattern of tumor ; Though these will provide some help in diagnosis but it should be based on pathological evidence .
4.Relationship between levels of serum C-reactive protein, leucocyte count and carotid plaque in patients with ischemic stroke.

In order to study the relationship between serum C-reactive protein (CRP) levels, leukocyte count and carotid plaque in patients with ischemic stroke, carotid duplex examination was performed by high-definition imaging (HDI) 5000 triplex system. Serum CRP was measured by nephelometry within 72 h after index ischemic stroke. A lesion was considered a plaque in the presence of a maximum intimal-medial wall thickness (IMT) 1.2 mm. Results of carotid ultrasonography were divided into two groups: M1, normal (IMT < 1.2 mm) and M2, abnormal (IMT > or = 1.2 mm). The results showed that the mean age of M2 was significantly older than that of M1 (69.7 +/- 10.4 versus 62.5 +/- 9.6, P = 0.001). The patients with hypertension and diabetes mellitus (78%, 35% respectively) in M2 were significantly more than those (52%, 18% respectively) in M1 (P < 0.01, P < 0.05). There were 32 (65%) patients with elevated CRP levels in M2, but 33 (46%) patients with elevated CRP levels in M1, with the difference being significant between the two groups (P < 0.05). The levels of serum glucose and leukocyte count (8.1 +/- 5.5, 10.3 +/- 4.0, respectively) in abnormal CRP group were significantly higher than that of normal CRP group (6.4 +/- 2.8, 8.7 +/- 3.4) (P < 0.05, P < 0.05); elevated CRP levels was found in 42 (62%) patients with territory infarction and 23 (43%) patients with lacunar infarction respectively, with the difference being significant between these two groups (P < 0.05). It was concluded that the elevation of CRP levels was an significant clinical index for carotid plaque in patients with acute cerebral infarction.

5.Clinical features of primary bilateral macronodular adrenal hyperplasia

Objective:To investigate the clinical profile of primary bilateral macronodular adrenal hyperplasia(PBMAH) and sex difference.Methods:One hundred and forty cases of PBMAH were recruited in our center from 2014, and all patients were evaluated for hormone secretion, adrenal imaging, and metabolic parameters.Results:Overt Cushing′s syndrome accounted for 76.4% of PBMAH cohort and 47.9% were female. The overt group had higher serum cortisol and 24 h urinary free cortisol levels, lower adrenocorticotropic hormone, higher serum cortisol after low-dose dexamethasone suppression tests, larger total adrenal size, and a higher percentage of obesity, hypertension, diabetes mellitus, and hypokalemia than the subclinical group(all P<0.05). When compared with the male group, the female group had smaller adrenal size( P<0.001), lower HbA 1C( P=0.003), higher total cholesterol( P=0.005), and lower density lipoprotein-cholesterol levels( P=0.035). Further, 24 h urinary free cortisol in the male group was found to be positively correlated with diastolic blood pressure, fasting glucose, 2 h postprandial glucose after oral glucose tolerance test(OGTT), and HbA 1C after adjusted for age, body mass index, and onset duration, and was negatively correlated with body mass index and potassium levels. While 24 h urinary free cortisol in the female group was positively correlated only with diastolic blood pressure, fasting glucose, and 2 h postprandial glucose after OGTT(all P<0.05). During follow-up, 80.0% of patients achieved remission after unilateral adrenalectomy, with a recurrence rate of 17.9%. Conclusion:PBMAH related metabolic disorder is more pronounced in overt Cushing′s syndrome and males. Unilateral adrenalectomy as an effective treatment can benefit the majority of patients.
