BACKGROUND: As indicated by experiments on animal, cytokine has regulatory effects on sleep. Clinical research has found abnormal level of cytokine in patients with depression. Sleep rhythm disorder is a common symptom of depression; however, the relationship between cytokine and sleep has hardly been conducted at home and abroad.OBJECTIVE: To explore the relationship between abnormal level of cytokine and clinical characteristics of sleep disorder so as to provide theoretical basis for early intervention and functional detection of sleep disorder in patients with depression.DESIGN: A case-controlled study using the patients with depression as the subjects and normal persons as the controls.SETTING: Two wards of the Department of Psychiatry of a university hospital.PARTICIPANTS: The study was completed in the Department of Psychiatry, Second Affiliated Hospital of Xinxiang Medical College. The subjects in this study were divided into depression group and control group. The patients in depression group were hospitalized in the Second Affiliated Hospital of Xinxiang Medical College from September 2001 to December 2001. Inclusion International Classification of Diseases(10th edition) and Chinese Classifiliver function, electrocardiography and kidney function examination were quilizer or lithium salt were used in the past year; had the history of alcohol Had endocrine, heart, liver and kidney diseases and other serious somatic diseases. The 35 patients who met the above criteria were 14 males and 21females aged from 18 to 60 years with the average age of(36 ± 12) years in the depression group. In the control group there were 13 males and 17 females aged from 19 to 60 years with the average age of(34 ± 10)years.METHODS: The levels of plasma interleukin-2(IL-2) and solute interleukin-2 receptor(sIL-2R) were detected with enzyme-linked immunoadsorbent assay.disorder in patients with depression.RESULTS: The level of plasma IL-2 in the depression group(77.92± 36.85) pg/L was lower than that of the normal control group (98.98± 30. 72 ) pg/L( t = 2. 446, P < 0.05). Moreover, IL-2 level was positively correlated to depth of sleep level( r = 0. 364, P < 0. 05) . No significant difference in sIL-2R level was found between the two groups( P > 0.05).CONCLUSION: The development of sleep disorder in patients with depression may be related with the level of cytokine, and the decrease of IL-2level can be the neurobiological basis of sleep level in depression.
3.THE EFFECT OF ISCHEMIA-REPERFUSION ON MATRIX METALLOPROTEINASE-1 IN RAT HEART

Objective To investigate the effect of ischemia and ischemia/reperfusion(I/R)in rat heart on matrix metalloproteinase-1(MMP-1).Methods The I/R animal models were established by shutting down and reopening the anterior interventricular branch with a silver clamp,then the distribution and amount of MMP-1 of the normal and I/R rat hearts were observed by immunohistochemical staining and Western blotting and analyzed by computer image analysis.Results 1.Immunohistochemical staining showed MMP-1 existed mainly in the cardiac matrix.There were strong positive reactions in fibrocytes,smooth muscle cells of the blood vessel and endotheliaI cells of capillaries.MMP-1 didn't show distinct changes 30 minutes after ischemia,while its concentration increased dramatically 60 minutes after ischemia.The positive reaction of MMP-1 increased 30 minutes after I/R,and 60 minutes after I/R there was large fusion areas in MMP-1 existing reglons.2.Quantitative analysis showed no dramatic changes of MMP-1 after ischemia for 30 minutes(P>0.05),while dramatic changes were seen 60 minutes after ischemia(P<0.05).MMP-1 changed dramatically 30 minutes and 60 minutes after I/R.3.Western blotting showed that there were no distinct naked-eye-observable changes.The bands of MMP-1 became widened 30 minutes after I/R,and became obviously widened 60 minutes after I/R.Conclusion 1.MMP-1 is secreted by fibrocytes,smooth muscle cells and endothelial cells of cardiac tissue under physiological conditions,and cardiomyocytes has the potential to secrete MMP-1 under ischemia or I/R.2.The longer time the heart ischemia lasts,the greater MMP-1 concentration will increase.Reperfusion can increase MMP-1 concentration to an even higher level,which may be the main cause of the collagen destruction after heart I/R.
4.Interleukin-6 Inhibits the Expression of Apolipoprotein M in HepG2 Cells

Obiective:To investigate the effect of interleukin-6(IL-6)on apolipoprotein(apo)M expression in a human hepatoblastoma cell line(HepG2).Methods:HepG2 cells were cultured and incubated with different concentrations of IL-6 (0,1.25,2.5,5,10,20,40 and 80 μg/L)for 24 hours.After the incubations,total RNAs were extracted and applied to reverse transcript PCR and real-time quantitative PCR to detect the expression levels of apoM and apoA-I.The effect of IL-1α on apoM expression was also determined.Results:IL-6 significantly inhibited the expression of apoM(F=10.778,P < 0.01).Whereas IL-6 did not influence the expression of apoA-I(F=2.004,P > 0.05).IL-1α(0,1.25,2.5,5,10,20,40 and 80 μg/L)did not decrease the expression of apoM(F=2.038,P > 0.05).Cooclusion:IL-6 inhibits apoM mRNA transcription,which may contribute to the pathogenesis of abnormal lipid metabolism and macrovascular complications in type 2 diabetes.
5.Reconstruction of Maxillofacial Soft Tissue Defects After oral cancer operation by Free Forearm Flap.

Objective To evaluate the clinical application of vascularized free forearm flaps in reconstruction of oral and maxillofacial soft tissue defects after oral cancer operation.MethodsWe retrospectively analysed 12cases with oral and maxillofacial soft tissue defects which had been reconstructed with vascularized free forearm flaps.Data concerning the recipient vessels,the survived rate and complications was analysed.ResultsAll the flaps survived compeletely,with the success rate of 100 %(12/12).The vessel thrombosis rate was 8.3%(1/12) and the flap salvage rate was 100 %(1/1).The overall complication rate was 8.3 %(1/12).ConclusionThe free forearm flap is safe and reliable and is helpful to improve the function and shape in the reconstruction of oral and maxillofacial defects.
6.Clinical study of tissue-type plasminogen activator therapy for children's plastic bronchitis

Objective To evaluate the clinical efficacy of tissue-type plasminogen activator (tPA) treatment of children with plastic bronchitis.Methods The study retrospectively reviewed the clinical data of the children with plastic bronchitis who were admitted to Tianjin Children's Hospital from September 2013 to January 2015 and were treated with tissue-type plasminogen activator.This study analyzed the effect and safety of tPA treatment,including clinical and radiological changes and follow-ups.Results A lot of plastic secretions were safely removed from the bronchial tubes in all children and clinical manifestations including breathing,body temperature,transcutaneous oxygen saturation and image changes were significantly improved.Conclusions Bronchoscopy is an effective way to treat plastic bronchitis,but with the use of tPA a better clinical efficacy could be achieved.The method is safe and effective and should be applied early in the patients in order to prevent the occurrence of severe airway obstruction complications.
7.Construction of training bases for three tumor therapies in Guangdong province

Objective To study the construction of training bases for three tumor therapies. Methods Eight training bases in six third-level first-class hospitals with score of technology assessment higher than 90 were investigated. Results There were good hardware in all training bases and qualified teaching staff in six of them. Annual operative quantity of hyperthermia and radioactive particles implantation technology of all training bases were up to the standard , while the coincidence rate of ablation technology was 80%. Besides , quantity of ablation technology and radioactive particles implantation technology trainees participated in during training met the standard, but that of hyperthermia not. There were significant difference in theory and operational test results before and after training (P < 0.01). Conclusions Management system, operative quantity and teaching staff construction need to be improved. Clinical skills training and standardized training assessment should be strengthened in base construction.
8.Determination of national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases

Objective: To determine the national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases (CQQC) according patients with hypertension, coronary artery disease(CAD), and congestive heart failure(CHF)and to analyze the influence factors. Methods: The quality of life was estimated by CQQC to evaluate 7937 persons normal and with cardiovascular diseases(contain with hypertension, CAD, and CHF ) in 28 hospitals of 19 cities across China. The data were compared in gender, age, exercise, psychologic condition, education, number of family member and medical cost. Results: 1、The mean scores(national norms)of normal persons were (95.74±23.21); 2、The scores of hypertension patients were (73.75±23.62), the scores of hypertension stage 3 group (69.06±21.78)were least compared with other stages(P<0.01); 3、The scores of CAD patients were(64.67±20.59), the scores of unstable angina group(55.37±18.62)were least compared with other types in CAD patients(P<0.01); 4、The scores of CHF patients were (40.36±17.24), it was least compared with those of hypertension and CAD patients (P<0.01); 5、Influence factor: (1)Sex, age: The mean scores were (71.41±27.8)for 7937 persons, the scores of male persons were significantly higher than those of female (P=0.000). In normal persons, along with age increased, the scores decreased (P<0.05) ; except ≥70 ages group, the scores of male persons were higher than those of female(P<0.05) ; (2)Exercise: The scores of patients participating exercise were significantly higher than those of no participating exercise [(72.64±21.87)vs.(66.91±23.13), P=0.000]; (3)Psychological condition: The scores of patients with healthy psychological condition were significantly higher than those of with psychologic disorder [(70.13±25.66)vs.(68.91±20.44), P=0.001]; (4)Other: patients with better education, more than 1 family members and medical insurance or public expense had more scores (P<0.05~0.001). Conclusions: The sample size is large and the datum is reliable for Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases. It can reflect influence of age, sex, exercise, psychologic condition, family, education background, medical condition and disease, its item is brief, clear, and operation is easy, so it may well be spread.

The apphcation of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen has been 18 years in China.The teatment is applied by more and more hospitals,however,the operation procedure is not the same in each hospital,which affected the therapeutic effect of radioactive seeds implantation,and means more safety risks.This paper summarizes the operation procedures of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen,and minimizes the loopholes in clinical operation,in order to improve the therapeutic effect and reduce the safety risks.
10.Efficacy of fluvoxamine combined with extended-release methylphenidate on treatment-refractory obsessive-compulsive disorder.

To observe the clinical efficacy of dopamine modulator methylphenidate (MPH) of extended-release formulations (MPH-ER) augmentation of ongoing fluvoxamine treatment in refractory obsessive-compulsive disorder (OCD) and its effects on patient's anxiety and sleep quality.
Methods: A pilot randomized, placebo-controlled, and double-blind trial was conducted at an outpatient, single-center academic setting. Participants included 44 adults with serotonin reuptake inhibitor treatment-refractory OCD and they received a stable fluvoxamine pharmacotherapy with Yale-Brown Obsessive Compulsive Scale (Y-BOCS) scores higher than 20. The 44 patients were randomly assigned into a study group and a control group, with 22 patiencs in each group. Fluvoxamine and MPH-ER were given to the study group, while fluvoxamine and placebo were given to the control group, with 8 weeks of the treatment course. Y-BOCS, Hamilton Anxiety Scale (HAMA) were used to assess the efficacy, Pittsburgh Sleep Quality Index (PSQI) was used to evaluate the sleep quality, and Treatment Emergent Symptom Scale (TESS) was used to evaluate the side effects. Data were analyzed in the intention-to-treat sample.
Results: The improvement in the Y-BOCS total score, Y-BOCS obsession subscale score and HAMA score were more prominent in the study group than those in the control group (P<0.001). There was no significant difference in PSQI score and TESS score between the two groups. MPH-ER was well tolerated.
Conclusion: Fluvoxamine combined with MPH-ER is effective in the treatment of refractory obsessive-compulsive disorder. It can improve anxiety and has no adverse effect on sleep quality.
