1.Treament of fractures of distal humerus with open reduction and anatomical plate internal fixation

[Objective]To investigate the treatment of fractuees of distal end of humerus with open reduction and internal fixation with anatomical plate.[Method]From February 2001 to April 2006,open reduction and anatomical plate internal fixation were performed in 22 cases with distal end of humerus fracture,of which olecranon osteotomy approach was carried out in 11 while musculus triceps brachii linguiform flap approach in other 11.All the patients received combined therapy including fumigation and rinsing with Chinese herbs,CPM(continuous passive motion) and manipulation.[Result]All the cases were followed up for 13 to 22 months.All the fractures got union.The postoperative elbow functions were excellent in 9 cases,good in 7 cases according to Mayo standards.The satisfactory rate was 72.71%.[Conclusion]Open reduction and internal fixation with anatomical plate can provide stable fixation and allow early postoperative elbow functional training in treatment of distal humerua fracture.Good clinical results can be achieved using fumigation and rinsing with Chinese herbs,CPM and manipulation after operation.

3.Factors affecting the development of chronic diseases among cured leprosy patients in Huai'an City

Objective:
To investigate the prevalence and influencing factors of chronic diseases among cured leprosy patients in Huai'an City, so as to provide insights into chronic diseases control among cured leprosy patients.
Methods:
All cured leprosy patients in Huai'an City in 2021 were enrolled from the National Leprosy Management Information System in China. The demographic data, family status, leprosy diagnosis and treatment, health status and development of chronic diseases were collected using face-to-face questionnaire surveys and physical examinations, and the factors affecting the development of chronic diseases were identified among cured leprosy patients using a multivariable logistic regression model.
Results:
Totally 708 cured leprosy patients were included, with a male to female ratio of 2.75︰1, and a mean age of (71.84±10.09) years. The overall prevalence of disability was 48.45%. The prevalence of chronic diseases was 31.78% among participants, and the prevalence rates of hypertension, diabetes, coronary heart disease, stroke, dyslipidemia, tumor, chronic obstructive pulmonary diseases were 25.14%, 4.10%, 3.67%, 3.67%, 2.26%, 2.26% and 0.85%, respectively. There were 154 participants with a type of chronic diseases (21.75%), 61 with two types of chronic diseases (8.62%) and 10 with 3 and more types of chronic diseases (1.41%). Multivariable logistic regression analysis identified complete loss of labor ability (OR=1.484, 95%CI: 1.015-2.171), labor income as the major economic source (OR=0.621, 95%CI: 0.407-0.948), being cared of (OR=1.832, 95%CI: 1.203-2.788) and disability (OR=1.715, 95%CI: 1.127-2.608) as factors affecting the development of chronic diseases among cured leprosy patients.
Conclusions
There is a high risk of chronic diseases among cured leprosy patients with loss of labor ability, with government subsidy and others' funding as the major economic source, requiring to be cared of and with disability in Huai'an City.
4.Quantitative evaluation of live Enterococcus faecalis in failed endodontic root-well-filled teeth

Objective:To analyse viable Enterococcus faecalis(E.faecalis)in root-well-filled teeth associated with failed endodontic treatment by using propidium monoazide (PMA)in combination with real time qPCR.Methods:Bacterial samples were extracted from 34 root-canal-treated teeth with post-treatment apical periodontitis.Each sample was separated into 2 different tubes.PMA was added to one of the tubes,and the other was left untreated.Then,DNA extraction and qPCR were performed.Results:E.faecalis was found in 20 of the 34 samples(58.8%).In PMA treated and none-treaten samples the Ct value of E.faecalis was 25.1 2 ±2.04 and 24.62 ± 2.02 respectively(P =0.001 ).Conclusion:PMA may be feasible in differentiating viable and dead Enterococcus faecalis cells.
5.Assessment of two methods in evaluating alveolar bone loss by micro-CT based on periodontitis model in mice

Objective To evaluate two methods measuring alveolar bone loss by micro computed tomography (micro-CT)based on periodontitis model in mice.Methods The silk ligatures were tied around the right maxillary second molars of mice to induce periodontitis model.The right half maxillaries of mice model were harvested for micro-CT analysis.Three dentists were recruited for the measurement with two different methods:Modified tomography (T) method and reconstruction (R) method.Accuracy and consistency of each method were estimated by standard deviation (SD).Results The SDs of R method managed by the same operator (measurement for 3 times) or different operators (3 operators) were 34.87μm and 35.67 μm respectively,while that of T method was 7.82 μm and 14.24 μm respectively.The SDs of T method were significantly lower than those of R method (both P<0.05).Conclusion T method is more accurate and consistent than R method for evaluating alveolar bone loss in mice periodontitis model.
6.Bactericidal effect of nano-silver against E.faecalis growing in multi-species biofilm

Objective:To evaluate the bactericidal effects of nano-silver against Enterococcus faecalis(E.faecalis) growing in multi-species biofilm.Methods:85 biofilms were established using MBECTM P&G Assay with E.faecalis together with Fusobacterium nucleatum and P.melaninogenica.Thereafter,10 specimens were used as the controls,75 were randomly divided into 3 groups(n=25),and treated with 0.1% nano-silver (12-15 nm) solution,0.1% nano-silver (100 nm) solution and 2% hypochlorite solution,respectively.Each sample was then separated into 2 different tubes.PMA was added to one of the tubes,and the other was left untreated.Then,DNA extraction and qPCR were performed.The cycle threshold(Ct) values between samples were compared by paired t test.Results:The Ct values of the samples treated with PMA were higher than that without PMA(P=0.000) in the group of 0.1% nano-silver solution(12-15 nm)was higher than that in the group of 0.1% nano-silver solution(100 nm)and 2% sodium hypochlorite solution.(P<0.05);the value in the group of 0.1% nano-silver solution(100 nm)was larger than that in 2% sodium hypochlorite solution(P<0.05).Conclusion:0.1% nano silver solution might have a strong bactericidal effect against against E.faecalis growing in multi-species biofilm.The bactericidal effect may be enhanced with the small size of silver particles.
7.Effect of super selective intra-arterial infusion of verapamil for the treatment of cerebral vasospasm after subarachnoid hemorrhage:an efficacy analysis

Objective To investigate the efficacy of super selective intra-arterial infusion of verapamil for the treatment of cerebral vasospasm (CVS) after subarachnoid hemorrhage (SAH).Methods From January 2013 to February 2016,the clinical data of 15 patients with subarachnoid hemorrhage (SAH) who had CVS after intracranial aneurysm clipping (n=8) or endovascular treatment (n=7) were analyzed retrospectively.All patients received whole brain digital subtraction angiography (DSA).Microcatheter super selection to spastic arteries was used and verapamil (11.1±3.4 mg) was infused.The findings of whole brain DSA before and after treatments were compared.The blood pressure and heart rate were collected during the treatment and the findings of transcranial Doppler ultrasonography were recorded.The patients were followed up for 6 months and the Glasgow outcome scale (GCS) scores were obtained.Results (1) A total of 20 intra-arterial infusion treatments were performed in 15 cases.They were compared before and after perfusion.CVS was improved on DSA in 14 cases,there was no significant change in on cases.(2) Transcranial Doppler ultrasonography showed that the mean blood flow velocity (mBFV) of the middle cerebral artery was decreased from 181±4 cm/s before the super selective intra-arterial infusion to 126±4 cm/s within 1 hour after treatment.There was significant difference (t=42.46,P<0.01).No complications associated with the use of microcatheters were observed.(3) The right upper arm blood pressure of the patients was monitored in the process of perfusion.The systolic pressure was 138±8 mmHg at the beginning of the perfusion,it was 135±10 mmHg at the end of the perfusion,and it was 137±7 mmHg at 1 h after the perfusion.The heart rate was 83±6/min at the beginning of the operation,it was 79±8/min at the end of the operation,and it was 80±5/min at 1 h after the operation.There were no significant differences (P>0.05).(4) All patients were followed up for 6 months.The GOS score at 6 months:good recovery in 9 cases,moderate disability but could take care of themselves in 3 cases,severe disability and could not take care of themselves in 3 cases,no persistent vegetative state or death.Conclusion Super selective intra-arterial infusion of verapamil can effectively improve the treatment of aneurysmal SAH caused CVS.At the same time,it has no obvious effect on blood pressure and heart rate.
8. Compound brain peptide gangiioside can improve intrauterine hypoxia-induced neonatal brain injury and promote synapse regeneration in a mouse model

BACKGROUND: The most current application of compound porcine cerebroside and gangiioside injection (CPCGI) focuses on the clinical therapeutic efficacy in some craniocerebral injuries and neonatal hypoxic ischemic encephalopathy, but the molecular mechanism of CPCGI involved in the recovery of nerve function is rarely reported. OBJECTIVE: To study the protective effect and mechanism of CPCGI in a neonatal mouse model of intrauterine hypoxia-induced brain injury. METHODS: Fifteen Kunming mice were randomly divided into control group, hypoxia group and treatment group. At the 14th day after pregnancy, mice in hypoxia group and treatment group were put into a 10% oxygen incubator to make the model of intrauterine hypoxia. After delivery, neonatal mice were intraperitoneally injected with CPCGI and PBS respectively, and mouse development in each group was observed after treatment. Mice in the control group were not hypoxic and treated. The expression of glial fibrillary acidic protein, Neurocan, SynDIGI and collagen IV were observed by immunofluorescence staining and western blot assay. A step-down test was used to test animal memory function. The study protocol was approved by the Animal Ethic Committee of Luohe Medical College in China. RESULTS AND CONCLUSION: Compared with the control group, the expressions of glial fibrillary acidic protein and Neurocan in the hypoxia group were significantly increased, while the expressions of SynDIGI and collagen IV were significantly decreased. The expression of SynDIGI and collagen IV in the treatment group was significantly increased than that in the hypoxia group, while the expression of glial fibrillary acidic protein and Neuroncan was significantly decreased than that in the hypoxia group. The learning and memory ability of mice in the hypoxia group was significantly decreased compared with the control group, but was significantly improved after CPCGI treatment. These results suggest that CPCGI can alleviate brain injury and improve learning and memory ability after hypoxic ischemic encephalopathy. The detailed mechanism might be related to inhibiting activation of astrocytes, down-regulating Neuroncan expression, up-regulating collagen IV expression, promoting synaptic reconstruction and reducing cerebrovascular injury.
9.Clinical characteristics and risk factors of patients with uremic encephalopathy

Objective:To analyze and compare clinical characteristics and risk factors of patients with uremic encephalopathy (UE).Methods:A retrospective analysis was performed from January 2014 to January 2019 in our hospital. Seventy patients diagnosed with chronic kidney disease (CKD) at the end stage (according to diagnosis standard of CKD) complicated with UE were classified into the UE group. In principle matching with sex, age and duration of disease, seventy patients with chronic kidney disease at the end stage but without UE were classified into the non-UE group (NUE group). The demographic data, laboratory examination, CT or MRI examination were recorded and analyzed by using t or χ 2 test. In addition, independent risk factors of patients with UE were analyzed by using Logistic model. Results:A total of 70 patients in the UE group and 70 patients in the NUE group were collected. The proportion of patients with a history of alcohol comsumption, chronic obstructive pulmonary disease and polycystic kidney disease were higher in the UE group than in the NUE group ( P<0.05). There were no significant differences in hypertension, diabetes, and coronary artery disease history between the two groups ( P>0.05). The proportion of cerebral focus and lesions for brain white matter revealed by head CT or MRI in the UE group were significantly higher than that in the NUE group ( P<0.05). The serum NLR and UA levels in the UE group were higher than those in the NUE group ( P<0.05), but the serum ALB and FT3 levels in the UE group were lower than those in the NUE group ( P<0.05). Logistic regression analysis showed that serum NLR, ALB and FT3 levels were independent risk factors for UE patients. Conclusions:Cerebral focus and lesions for brain white matter revealed by CT or MRI are typical abnormal in UE patients. The serum NLR, FT3 and ALB levels are independent risk factor for UE patients.
10.Relationships between Notch1, DLL4, HES-1 expression and angiogenesis, lymphangiogenesis of gastric carcinoma and their significances

Objective To investigate the expression of Notch1,Delta-like ligand 4 (DLL4),hairy and enhancer of split-1 (HES-1),microvessel density (MVD),lymphatic vessel density (MLD) in gastric carcinoma,so as to discuss their roles in the development of gastric carcinoma.Methods Gastric carcinoma,paracancer tissues which was apart from the edge of cancer tissue > 60 mm obtained during operation and normal gastric mucosa obtained during gastroscopy were used as controls.All specimens were made tissue microarray.The expressions of Notchl,DLL4,HES-1 were detected by immunohistochemistry.Immunohistochenical double taining was used to detect MVD and MLD.The relationships between Notch1,DLL4,HES-1 expression and angiogenesis,lymphangiogenesis and their significances were analyzed.Results The positive rate of Notch1 in gastric carcinoma was 48.30%,significantly higher than that of paracancerous (25.00%,x2 =6.38,P < 0.05) and control group (16.67%,x2 =10.18,P <0.05).The differences of the positive rate of DLL4 in gastric carcinoma (55.94%),paracancerous (45.70%) and control group (56.67%) were not significant (x2 =1.18,P >0.05 ; x2 =0.005,P > 0.05).The differences of the positive rate of HES-1 in gastric carcinoma (36.64%),paracancerous (34.40%) and control group (33.33%) were not significant (x2 =0.05,P > 0.05 ;,x2 =0.11,P > 0.05).The mean of MVD in gastric carcinoma group was 28.84 ± 14.17,which was significantly higher than that in paracancerous group (17.02 ±8.54,t =4.03,P<0.05) and control group (16.69 ±7.21,t =5.01,P<0.05).The mean of MLD in gastric carcinoma group was 8.55 ±4.98,which was significantly higher than that in paracancerous group (4.05 ± 2.48,t =9.30,P < 0.05) and control group (3.99 ± 1.56,t =10.32,P < 0.05).The expression of DLL4 was correlated with MVD (t =2.77,P < 0.05),but wasn't correlated with MLD (t =1.89,P >0.05).There were no correlations between the expression of Notch1,HES-1 and tissues MVD,MLD (P >0.05).Conclusion Notch1 plays important roles in the development of gastric carcinoma.There are many angiogenesis and lymphangiogenesis in gastric carcinoma.The expression of DLL4 in gastric carcinoma has certain effect in the formation of microvessel.
