1.Clinical Observation of Recombinant Human Brain Natriuretic Peptide on the Treatment of Acute Decompen-sated Heart Failure

OBJECTIVE:To investigate the effects,prognosis and safery of recombinant human brain natriuretic peptide on the treatment of patients with acute decompensated heart failure. METHODS:90 inpatients with acute decompensated heart failure were divided into observation group and control group according to the use of recombinant human brain natriuretic peptide or not,with 45 cases in each group. Control group received rountine anti-heart failure therapy and intravenous dripping of Nitroglycerin injection at 20 μg/min continuously for 72 h. Observation group was additionally given Lyohilized recombinant human brain natriuretic peptide with initial loading amount of 1.5 μg/kg,iv,then intravenous dropping at 0.007 5 μg/(kg·min)continuously for 72 h,on the basis of control group. Hemodynamic parameters,brain natriuretic peptide level,the difference of liquid intake and output,dyspnea,lung rales,systemic edema,the improvement of cardiac function were observed and compared between 2 groups before and after treat-ment as well as re-hospitalization rate and surriral rate 30 d after medication,the occarrence of ADR. RESULTS:After treatment, left ventricular ejection fraction of observation group was significantly higher than that of control group,and central venous pressure and brain natriuretic peptide level were lower than those of control group,with statistically significance (P<0.05). The difference of liquid intake and output after treatment was (1 171.60 ± 444.80)ml in observation group and (822.30 ± 268.40)ml in control group,with statistical significance between 2 groups(P<0.05). After treatment,the scores of dyspnea,lung rales and systemic ede-ma were decreased in 2 groups;the improvement of observation group was significantly better than that of control group,with statis-tical significance(P<0.05). Total effective rates of observation group and control group were 95.56% and 82.22%;re-hospitaliza-tion rates 30 d after medication were 17.78%and 26.67%;survival rates 30 d after medication were 88.89%and 75.56%,with sta-tistical significance between 2 groups (P<0.05). There was no statistical significanse in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Recombinant human brain natriuretic peptide can significantly improve hemodynamic parameters and heart function of patients with acute decompensated heart failure,and effectively relieve dyspnea,lung rales and systemic edema. It is better than routine treatment in prognosis with good safety.

Objective Our aim is to study the clinical manifestation and endocrine disorder of the bulimia nervosa (BN). Methods 2 cases of BN were clinically analysed. Results Most of the patients were anorexia before the disease happened, with many aspects of endocrine disorders. Conclusions BN and amenorrea could result from hypothalamus function disorders. However, the alteration of corticoid,thyroid hormones may be results of emotion stress and body weight loss.

5.Analysis of 1120 cases of thyroid cancer with thyroid disease

Objective To investigate the clinical changing trend and CT imaging diagnosis of thyroid cancer associated with thy-roid disease .Methods 1 120 cases of thyroid cancer patients admitted in our hospital in the past 10 years were selected and devided into different groups according to their admission times :patients admitted before 2003 were in one group;from 2003 on ,patients were divided into groups according to their admission time by a time span of 2 years ;then the changing trend and CT imaging diag-nosis of thyroid carcinoma with nodular goiter (NG) ,thyroid adenoma (TA) and merger trend of Hashimoto′sthyroiditis (HT ) of different times were compared and analyzed .Results There were 518 cases of papillary carcinoma(46 .25% ) ,345 cases of follicular adenocarcinoma ,(30 .80% ) ,158 cases of medullary carcinoma (14 .11% ) ,99 cases of undifferentiated carcinoma (8 .84% ) in 1120 patients .The occurrence rate of thyroid carcinoma with nodular goiter (NG) in 2009-2010 ,2011-2012 and the occurrence rate of thyroid carcinoma with HT in 2007-2008 ,2009-2010 ,2011-2012 were significantly higher than that of 2005-2006 (P<0 .05) . Take 2007-2008 as the deviding line of thyroid combined TA ,and it shows that the occurrence rate increased continuously before this line and started to drop down after that .CT of thyroid cancer shows uneven low-density imaging ,blur edge ,and inhomogeneous enhancement .The CT characteristics differ from different diseases .Conclusion In recent years the incidence rates of thyroid with nodular thyroid and HT increased significantly compared with the years before ,while incidence rate of TA decreased .
6.A Comparative Study of CT and MRI in the Follow-up of Hepatocellular Carcinoma after Transcatheter Arterial Chemoembolization with Lipiodol

Objective To compare the ability of CT and MRI in evaluating the residual of tumor of hepatocellular carcinoma (HCC) and its stability after treated by transcatheter arterial chemoembolization (TACE) with Lipiodol.Methods 28 patients with HCC underwent CT, MRI and angiography within 2~6 months after 1~4 procedure(s) of TACE. These three examinations were completed sequentially within one month. CT and MRI findings were thus compared on the basis of the results of arteriography and clinical follow-up at least 6 months.Results 46 lesions were found in 28 patients. 31 lesions with and the other 15 lesions without residual viable tumor were confirmed by arteriography and clinical follow-up. 20 lesions with and 26 lesions without residual viable tumor were found on CT images, and CT displayed 64.5% sensitivity, 100% specificity, and 76.1% accuracy respectively. 29 lesions with and 17 lesions without residual viable tumor were found on MRI images, and its sensitivity, specificity, and accuracy were 93.5%, 100%, and 95.7% respectively. Objective To construct shRNA expression vector of SURVIVIN for RNAi-mediated therapy of lung cancer.Methods DNA templates of SURVIVIN shRNA were designed,synthesized and cloned into the shuttle vector to get recombinant plasmids.After plasmids were transfected into A549 cells,the one with the most repression was screened by means of RT-PCR.Then MTT and Western blot were done to ascertain whether the proliferation of A549 cells were inhibited and SURVIVIN was down-regulated.Results The recombinants were constructed and screened successfully.The proliferation of A549 cells and SURVIVIN expression were repressed.Conclusion Mediated by RNAi,SURVIVIN expression was down-regulated,which suggested a potential gene therapy of huaman lung cancer.

Tetraspanins belongs to the transmembrane 4 superfamily(TM4SF). They can be as a bridge to connect the proteins outside or inside the cell membrane. A tetraspanins web is formed by the tetraspnins-proteins complex, and the web is believed to involve in fundamental functions of immunity system, and consequnently, signaling between cells and inside cells, regulating cell activation and adhesion, participating in the identification and infection of some virus. As a family of conservative transmembrane proteins, tetraspanins play multiplex roles in invertebrate. It was described how tetraspanin microdomains might have functions in the immune system, and how they contact with virus. In addition, the important role of tetraspanins in the innate immune system of invertebrate were discussed.
9.Impacting factors on the professional identity of railway young workers and the relationship with the professional identity and subjective well-being

Objective To investigate the impacting factors on the professional identity of railway young workers and the relationship of the professional identity and subjective well-being.Methods The sample consisting of 5 762 young workers was selected from railway enterprises by stratified and random sampling according to the proportion.Occupational professional identity Scale and Index of Well-being,Index of General Affect Scale were used.The affect of the variable,such as sex,age,working years,educational background and income on professional identity were studied.Moreover,the relationship between professional identity and subjective well-being was reported.Results (1) Professional identity ascended witb the increase of ages,and there was significance between three groups(F=6.667,P<0.05).(2)Professional identity ascended with the increase of length of service (F=5.147,P<0.01),but it dropped at the point of 9-12 years.(3) The professional identity of workers with bachelor degree was higher than that with junior college education background.and there was significance between three groups (F=3.538,P<0.05).(4)The workers who had more than 3 500 RMB monthly salary was regarded on the top in professional identity,while those whose monthly salary were under 2 500 RMB was in the lowest level (F=42.275,P<0.01).(5)The result of linear regression indicated that the degree of professional identity can forecast the degree of subjective well-being(R =0.534,P<0.01).Conclusion The professional identity of railway young works was affected by their ages,the length of service,the educational degree and salary,The professional identity can forecast subjective well-being.
10.Effect of Percutaneous Coronary Intervention on Blood Levels of Vascular Endothelial Growth Factor, High Sensitivity C-reactive Protein and Fibrinogen in Patients With Acute Myocardial Infarction

Objective: To study the effects of percutaneous coronary intervention (PCI) on blood levels of vascular endothelial growth factor (VEGF), high sensitivity C-reaction protein (hs-CRP) and fibrinogen (Fg) in patients with acute myocardial infarction (AMI).
Methods: Our research included 2 groups: AMI group,n=110 patients with PCI and Control group,n=53 healthy subjects from physical examination. Blood levels of VEGF, hs-CRP and Fg were compared between pre-operation and 30 min, 24 h, 72 h post-operation; the peak values of VEGF, hs-CRP and Fg were compared between the patients with post-operative MACE occurrence at 24 h and the patients without MACE occurrence.
Results:①Compared with Control group, blood levels of VEGF, hs-CRP and Fg in AMI group were increased at pre-operation and 30 min, 24 h, 72 h post-operation, for VEGF (pg/ml) as (84.7±21.4) vs (130.5±40.2) and (143.2±49.7), (107.1±31.8), (97.3±24.4); for hs-CRP (mg/L) as (1.03±0.34) vs (3.11±0.56) and (4.67±0.62), (6.42±0.72), (2.83±0.56); for Fg (mg/L) as (3.86±0.32) vs (4.79±0.52) and (5.02±0.54), (5.54±0.67), (4.11±0.44) respectively, allP<0.05. In AMI group, the post-operative value of VEGF was ifrst increased, it reached the peak at 30 min and then gradually decreasing; while hs-CRP and Fg were increased gradually, they reached the peak values at 24h time point, allP<0.05.②In AMI group, compared with the patients without post-operative MACE (72/110), the patients with MACE (38) had elevated 24h post-operative levels of VEGF as (131.7±36.5 vs 155.5±42.8), hs-CRP as (5.68±0.62 vs 6.89±0.53) and Fg as (5.23±0.60 vs 5.66±0.55) respectively, all P<0.01.
Conclusion: Post-operative monitoring of blood levels of VEGF, hs-CRP and Fg had the important clinical value in AMI patients after PCI.
