2.Vitamin C enhanced myocardial differentiation of dedifferentiated fat cells

[ ABSTRACT] AIM:In order to observe the myocardial differentiation capacity of the dedifferentiated fat ( DFAT) cells treated with vitamin C in vitro.METHODS: DFAT cells were dedifferentiated from the mature rat adipocytes with ceiling adherent culture.The DFAT cells of passage 3 were used in the study.Vitamin C and/or neonatal rat heart tissue lysate were added into the culture medium to induce myocardial differentiation for 3 weeks.The cell morphology was ob-served under microscope.The myocardial-specific markers, such as cTnT, GATA-4 and NKx2.5, were examined by the methods of immunofluorescence, PCR and Western blot.RESULTS:Mature rat adipocytes dedifferentiated into fibroblast-like DFAT cells after ceiling adherent culture.The DFAT cells spontaneously differentiated into cardiomyocyte-like cells under normal culture condition with a low incidence.After treated with neonatal rat heart cell lysate, the DFAT cells be-came cardiomyocyte-like cells that had bigger size, longer shape and myotubule-structure.The expression of cTnT, GATA-4 and NKx2.5 was remarkably increased at both mRNA and protein levels as compared with the normal cultured DFAT cells.The expression of cTnT, GATA-4 and NKx2.5 was further increased in DFAT cells after treating with vitamin C.No spontaneous beating cell was observed.CONCLUSION:Vitamin C enhances the differentiation of DFAT cells into cardio-myocyte-like cells.
3.The influence of integrated-CT artifacts on the attenuation correction results of SPECT/CT bone imaging

Objective To evaluate the influence of integrated-CT artifacts on attenuation-corrected (AC) images of SPECT bone imaging. Methods Imaging documents of 78 patients who underwent SPECT/CT bone imaging were retrospectively analyzed, and the artifacts on CT images and CT attenuation maps were visually studied. Compared with the non-attenuation corrected (NC) images, the coefficient of variation (CV) and percentage difference (PD) of radioactive count of regional bone influenced by CT artifacts were calculated and statistically analyzed to estimate the influence of CT artifacts on AC images of SPECT bone imaging. Results The integrated-CT artifacts were found in 38 patients of 78, and appeared the same image findings as those on CT attenuation maps respectively, including truncation artifact, thoraco-abdominal gas artifact, photon starvation artifact, etc. On all the AC images with integrated-CT artifacts, regional bones were influenced not only on uniformity (CVAC 17.62%±4.13%, CVNC 11.19%±3.81%;t=2.13, P<0.05), but also by the distribution (PDAC 16.98%±3.31%, PDNC 9.84%±1.62%;t=2.46, P<0.05) of radioactive count. Conclusion Artifacts on integrated-CT images can induce false AC information on CT attenuation maps, therefore, a comparative analysis with NC images is recommended if necessary.

5.Analysis of setup errors in helical tomotherapy for bone metastases

Objective To analyze the setup errors of bone metastases patients by tomotherapy with megavoltage CT (MVCT) and calculate the CTV-PTV margins.Methods 30 patients with bone metastases were enrolled.All patients received tomotherapy,fixed with body net and received MVCT scanning before radiation.The MVCT images were registered with the kilovoltage CT (kVCT) images,the setup errors of X (lateral),Y (vertical),Z (longitudina) and Roll (transverse profile rotation) were obtained according to the formula M =2.5Σ+0.7σ calculated CTV-PTV margin.Results 30 patients were received 494 MVCT images.The errors of systemic±random were (2.85±0.77) mm,(3.11±0.95) mm,(2.21±0.55) mm,and (0,55±0.24)° on X,Y,Z and Roll directions,respectively.The CTV-PTV margins were 3.64 mm,4.17 mm,and 2.86 mm on X,Y,Z directions,respectively.Conclusion The application of image-guided technology for bone metastases can correct positioning in time,which greatly reduces setup errors of the fractionated treatment,further improves the treat accuracy and has a positive value in guiding clinical radiotherapy.
6.Correlation between stroke volume variation and blood volume during hypovolemia

Objective To investigate the correlation between stroke volume variation (SVV) and blood volume during hypovolemia.Methods Twenty ASA Ⅰ or Ⅱ patients,aged 20-64 years,with body mass index (BMI) of 20-30 kg/m2,scheduled for elective orthopedic operation were enrolled in this study.Anesthesia was induced with dexamethasone,midazolam,propofol,fentanyl and cisatracurium,and maintained with sevoflurane,fentanyl and cisatracurium.Then the patients received endotracheal intubation and mechanical ventilation.Heart rate (HR),mean arterial blood pressure (MAP),central venous pressure (CVP),arterial pressure-based cardiac output (APCO),SW,systemic vascular resistance (SVR) and cardiac index (Cl) were recorded 5 minutes after endotracheal intubation.Blood was taken from the central vein at a rate of 30-50 ml/min and the volume was 5% of the whole blood volume,and then haemodynamic parameters mentioned above were recorded after the haemodynamics were kept stable for 5 minutes.Blood was taken again with the method mentioned above and the haemodynamic parameters were recorded.Then 6% hydroxyethyl starch (HES) 130/0.4 was infused at 50-70 ml/min via the right internal jugular vein,and the volume was equal to 5% of the whole blood volume,and then haemodynamic parameters were recorded after the haemodynamics was kept stable for 5 minutes.Fluid replacement was performed again using the method mentioned above and the haemodynamic parameters were recorded.Linear correlation of the changes in blood volume (difference between the blood volume at each time point and the baseline value) with dSVV (difference between the value monitored at each time point and the baseline value) was analyzed.Results Significant changes were found in SW,APCO and Cl after each change in blood volume (P < 0.05 or 0.01),while no significant changes were found in HR,MAP,CVP and SVR after each change in blood volume.The change in blood volume was negatively correlated with dSVV (r =-0.875,P < 0.01).Conclusion There is high correlation between SVV and blood volume during hypovolemia.And SVV can reflect the changes in blood volume accurately and can be used for volume therapy during hypovolemia.

8.Transumbilical laparotomy: a cosmetic and minimally invasive operative route in pediatric patients

Objective:The feasibility of a minimally invasive cosmetic transbilical laparotomy was studied in fifty-seven pediatric patients.The advantages of this technique will be discussed.Methods:The circumferential incision was made following the ring fold of the right umblilicus,which was less than 2700.The peritoneum was opened in the midline via rectus abdominis.Intra-peritoneal lesions were detected and explored.The surgical procedures were able to be carried out either inside or outside of the peritoneum.After closure of peritoneum,the umbilical incision was closed subcutaneously with an absorbable suture.Results:From January 2001 to January 2005,58 procedures via transumbilical route have been done in 57 pediatric patients.None of those cases required conversion to a traditional abdominal incision.The duration of surgery was from 20 to 170 minutes.All patients were discharged five days postoperatively and follow-up was conducted from 3 months to 4 years.No complications were observed in all but one patient.This patient had a leaking anastamosis.No visible scars were present and the cosmetic results were excellent.All patients had shorter hospital stay and lower operating cost.Conclusion:Transumbilical approach is a feasible,safe,economical,minimally invasive procedure with excellent cosmetic results which appears to be superior to traditional procedures.It suggests that the transumbilical route should be widely used clinically and might be applicable for elder children and possibly would be of benefit in the treatment of other conditions.
9.Predictive value of the stimulated thyroglobulin before and after 131I therapy for curative effect in patients with papillary thyroid carcinoma

Objective To analyze the stimulated thyroglobulin (sTg) level and variation tendency before and after 131I therapy in papillary thyroid carcinoma (PTC),and evaluate the predictive value of sTg for the efficacy of 131I therapy.Methods From January 2013 to September 2016,a total of 178 PTC patients (53 males,125 females;average age (44.1±11.4) years) without distant metastases who received 131I treatment in Tianjin Medical University General Hospital were reviewed.The patients were divided into two groups:group 1 (n=23) with residual thyroid detected by 131I whole body scan (WBS) and group 2 (n=155) without residual thyroid.Both thyroid stimulating hormone (TSH) and sTg were measured and defined as TSH1,sTg1 before 131I therapy,and TSH2,sTg2 after 131I therapy.Data of the two groups were compared with two-sample t test,x2 test and Mann-Whitney u test.The receiver operating characteristic (ROC) curve and diagnostic critical point (DCP) were used to evaluate the predictive value of sTg in radioiodine ablation efficacy.Results There were no significant differences in age (t=1.007),gender (x2 =1.419),tumor diameter (u=1 385),multifocal cancer (x2 =0.371),extrathyroidal extension (x2 =0.020) or lymph node metastasis (x2=0.391,all P>0.05) between group 1 and group 2.The levels of sTg1 and sTg2 of group 1 were 6.3(2.0,16.9) and 1.7(0.8,4.2) μg/L,which were higher than those of group 2 (2.7(0.6,6.4) and 0.3(0.2,1.3) μg/L;u=1 118.5,817.0,both P<0.01).△sTg and △sTg/△TSH showed no statistical differences between the two groups (u =1 359.5,1 195.0,both P>0.05).The area under ROC curve,DCP,sensitivity,specificity,positive predictive value,and negative predictive value of sTg1 were 0.686,4.435 μg/L,60.9%(14/23),67.7%(105/155),21.9% (14/64) and 92.1%(105/144),respectively.Meanwhile,the parameters of sTg2 were 0.771,0.460 μg/L,91.3% (21/23),58.1% (90/ 155),24.4%(21/86) and 97.8%(90/92),respectively.Conclusions The level of sTg could be used to predict the efficacy of 131I therapy in PTC patients.Preablative sTg1 (<4.435 μg/L) or postablative sTg2 (< 0.460 μg/L) holds high negative predictive value in identifying the efficacy of single 131I therapy.
10. Comparison between ablation efficacy of 1.1 GBq and 3.7 GBq 131I for low- and intermediate-risk differentiated thyroid carcinoma

Objective:
To compare the ablation efficacy and therapy response with 1.1 GBq and 3.7 GBq 131I in postoperative patients with low- and intermediate-risk differentiated thyroid carcinoma(DTC).
Methods:
A total of 190 patients (43 males, 147 females, age: (45.8±11.1)years) were enrolled from July 2016 to July 2017. Among them, 96 patients received 1.1 GBq 131I and 94 were given 3.7 GBq 131I. Diagnostic whole-body scan was performed 6 months after 131I ablation for treatment response evaluation, and the successful rate of 131I ablation was calculated.
