Objective To study the efficacy of repetitive transcranial magneticstimulation(rTMS) of the right prefrontal cortex combined paroxetine for patients with obsessive-compulsive disorder ( OCD) under doubleblind , sham-controlled conditions. Methods Patients were randomly assigned to 2 sessions of real ( n = 32) or sham ( n = 31) rTMS and both group received Paroxetine treatment. rTMS treatments lasted 20 minutes, and the frequency was 10 Hz and the intensity was 100% of motor threshold for real rTMS. Psychopathology was assessed by HAMA.Y-BOCS before the treatment, immediately after the experimental treatment, and 1,2,4,6 weeks after the experimental treatment by an independent reviewer. Results In combined therapy group,9(32. 14% ) recovered, 10(35.71% ) remitted,5( 17. 86% ) improved,4( 14. 29% ) remained unresponsive. In contrast, there were 4(16.36%) recovery,5(21.82%) remission, 10(38.18% ) improvement,6(23.64% ) unresponse in drug therapy group. The outcome of ITT showed all the patients in the two groups significant improvement (the scores of YBOCS( F=56.258, P=0.000) ;the scores of HAMA( F=41.675, P=0.000) after the treatment, but the effectiveness in the combined therapy group was better than that in the drug therapy group (the scores of Y-BOCS, F= 13.652, P=0.028;the scores of HAMA, F= 11.632, P=0.031). Conclusions Repetitive transcranial rTMS of the right prefrontal cortex combined paroxetine is more effective than paroxetine only in treatment of refractory OCD.

To study the mechanisms whereby cisplatin suppresses survival of human esophageal squamous cell carcinoma cells. The cytotoxicity of cisplatin in cisplatin-resistant cell line EC109 /CDDP and its parental cell line EC109 was measured by cell viability assay.Western blotting was used to investigate the protein expression of to-tal p53 and phosphorylated p53 at Ser15.Colony formation assay was employed to evaluate the ability of cells to recover from treatments and form colonies. The results indicated that EC109 /CDDP cells were more resistant to cisplatin-induced cytotoxicity than EC109 cells, with the IC50 values of (20.4 ±4.4)μmol /L and (5.7 ±0.1 )μmol /L,respectively.Although cisplatin did not alter the total protein level of p53,it obviously increased the phosphorylation of p53 at Ser15.Cisplatin inhibited survival of both EC109 /CDDP and EC109.Notably,inhibition of p53 by Pifithrin-αsignificantly promoted recovery of cisplatin-treated EC109 and EC109 /CDDP cells to differ-ent degrees. In this respect,p53 protein was found to be activated in response to cisplatin treatment in both EC109 /CDDP and EC109,which may contribute to the cytotoxic effect of cisplatin.

BACKGROUND:Bone marrow stem cell transplantation can improve heart function and prevent ventricle remodeling. At present, the adult bone marrow stem cells used for transplantation primarily included bone marrow mononuclear cells (BM-MNCs) and mesenchymal stem cells (MSCs),and endothelial progenitor cells. The curative effects and precise mechanisms of transplantation of various bone marrow stem cells remain unknown.OBJECTIVE:To compare the effects of transplantation of autologous BM-MNCs and MSCs via the coronary artery on ventricle remodeling subsequent to acute myocardial infarction (AMI). DESIGN,TIME AND SETTING:Randomized controlled animal experiment performed at the Center for Clinical Research,Hebei Provincial People's Hospital,Electron Microscope Room,Hebei Medical University between March 2005 and December 2006.MATERIALS:Thirty-six male Jizhong pigs, were randomly divided into 4 groups:control group (n = 6),infarct model group (n = 10),BM-MNC group (n = 10),and MSC group (n = 10).METHODS:Porcine autologous BM-MNCs were isolated by gradient density centrifugation, and MSCs were obtained by adherence method.Prior to transplantation, both BM-MNCs and MSCs were colloidal gold labeled.Except the infract model group, pigs in the other 3 groups were developed into AMI models by oppressing the left anterior descending branch with balloon catheter.Ninety minutes after modeling,(6.0±1.3)×107 autologous BM-MNCs and (4.5±2.1)x 107 MSCs were respectively transplanted into pigs in the BM-MNC group and the MSC group via the coronary artery and cultured for 28 days.MAIN OUTCOME MEASURES:Observation of pathological changes of cardiac muscle tissue by light and electron microscope;Examination of cardiac function by ultrasonograph;Detection of the number of blood vessels and apoptotic myocardial cells, and expression of nuclear factor-κB (NF-κB) and troponin Ⅰ and its correlation to cardiac function by immunohistochemistry;Detection of mRNA expression of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) in the cardiac tissue as well as its correlation to cardiac function by reverse transcription-polymerase chain reaction (RT-PCR).RESULTS:In the MSC group, there was proliferation of a great deal of blood vessels as well as growth of abnormal cell masses around the coronary vessels, while the BM-MNC group exhibited the "budding" of many capillary vessels.Prior to transplantation, cardiac function indices were basically similar among each group (F = 1.550,P>0.05).Twenty-eight days after transplantation, left ventricular ejection fraction was significantly lower in the control,BM-MNC,and MSC groups than in the infarct model group (F = 5.30,P<0.05),while endocardial fractional shortening was significantly higher (F = 10.67,P<0.01).Compared with the infarct model group, the number of blood vessels in the infarct zone and infarct border zone was increased in the BM-MNC group (F=29.56-34.87,P<0.01) and had no apparent change in the MSC group. In the BM-MNC and MSC groups, apoptotic myocardial cells in the infarct zone and infarct border zone were significantly reduced (F=14.31-35.34,P<0.01 ) and troponin I expression rate was significantly increased (F=19.05,P<0.01 ),as compared with the infarct model group. In addition,NF-κB positive rate in the infarct border zone was significantly lower in the BM-MNC and MSC groups than in the infarct model group (F=19.05,P<0.01).VEGF gene expression level in the infarct border zone was significandy higher in the BM-MNC group than in the infarct model group and MSC group (F = 49.41,P<0.01).bFGF gene expression level in the infarct border zone was significantly higher in the MSC group than in the infarct model and BM-MNC groups (F=4.71,P<0.01).LVEF was negatively correlated to myocardial cell apoptosis rate and NF-κB level (r=-0.441 1,P<0.05;r=-0.579 6,P<0.01 ).LVEF was positively correlated to number of blood vessels,VEGF and bFGF expression (r=0.775,P<0.01;r=0.565 1,P<0.05;r=0.573 5,P<0.05).CONCLUSION:Transplantation of both autologous BM-MNC and MSC via coronary artery can improve the condition of left ventricular remodeling subsequent to myocardial infarction. The improvement of cardiac functions is related to the increase of blood vessels,VEGF and bFGF expression, the decrease of myocardial cell apoptosis and NF-κ B level in cardiac muscle tissues after stem cell transplantation.BM-MNC transplantation better promotes blood vessel proliferation and VEGF expression in the cardiac tissue but produces worse effects on bFGF gene expression than MSC transplantation.

Objective To investigate the prevalence of common thyroid diseases and urinary iodine levels in the population migrated from inland to coastal areas and to evaluate the epidemiological trend of thyroid disease in Heibei Cangzhou. Methods A retrospective analysis method was adopted. All the persons who were employees of enterprises and institutions, were divided into three groups:the migrated group, who migrated from Cangzhou City and the west counties of Cangzhou City to the Port of Huanghua where they had settled permanently because of the job; the inland group, who lived in Cangzhou City all the time and the seaport group, who lived in the Port of Huanghua for a long-term. Migrated groups were divided into stable occupants and intermittent occupants. During 2010 - 2016, a follow-up survey was conducted in the three groups at Examination Center of Cangzhou Central Hospital Physical. We compared the cumulative incidences of thyroid diseases including: chronic lymphocytic thyroiditis (CLT), nodular thyroid disease and Graves's disease (GD). In migrated group, 28 persons who occasionally lived in the Port of Huanghua were detected urinary iodine levels intermittently. We compared the two parts of urinary iodine level changes for three times by collecting a urine sample, including one part from people who lived in the Port of Huanghua continuously for more than a month, and another part of people who lived in Cangzhou or Hejian County for more than half a month then returned to Port of Huanghua. Results In 2010 - 2016, a total of 7 180 residents were surveyed, of which 2 012 were in the migrated group, 2 858 were in the inland group and 2 310 in the seaport group. The incidences of CLT and nodular thyroid disease were 2.29% (46/2 012)and 8.60% (173/2 012)in the migrated group, which were much higher than that of the inland group and the seaport group [1.05% (30/2 858), 5.42% (155/2 858); 1.47% (34/2 310), 6.88% (159/2 310), P < 0.01 or < 0.05]. At the same time, the incidences of GD was 0.45%(9/2 012) in the migrated group, which was higher than that of the inland group [0.10% (3/2 858), P < 0.05]. In migrated group, there were 1 503 residencies, including 482 stable occupants and 1 021 intermittent residents. The incidences of CLT and nodular thyroid disease in intermittent residents were 3.23% (33/1 021) and 9.89% (101/1 021), which were much higher than that of stable occupants [1.45% (7/482), 6.64% (32/482), P < 0.05]. The urinary iodine levels in the persons who lived in the Port of Huanghua occasionally (464.6 μg/L) were much higher than that of the persons who lived in Cangzhou for a long time(301.4 μg/L, U = 4.648, P < 0.01).There was a marked fluctuation of the median of urinary iodine determined 6 times which were 485.7,285.2,431.7,310.6,470.3 and 304.4 μg/L,respectively.Conclusions The incidence of many thyroid diseases of people migrated from inland to coastal areas has increased, which is not only higher than that of inland people, but also higher than that of coastal people.Large fluctuations in iodine intake due to migration flow and residential instability may be part of the reason for the high incidence of thyroid diseases. In the future, we should pay more attention to thyroid diseases of migratory population.
