1.Cross-lagged analysis of acute stress disorder, posttraumatic stress disorder and depressive symptoms among children after Lushan earthquake

Objective:To examine the relationships between acute stress disorder (ASD) symptoms,posttraumatic stress disorder (PTSD) symptoms and depressive symptoms in children survivors following the Lushan earthquake in China.Methods:One hundred ninety-seven children aged 1 to 14years (93 male and 104 female) were followed up at two and six weeks after Lushan earthquake.The ASDS,UCLA PTSD Reaction Index and DASS were used to measure the ASD,PTSD and depressive symptoms.Results:The average scores of ASDS and the depression subscale of DASS at two weeks after earthquake were(36.5 ± 10.9) and (3.3 ± 2.8),respectively.The average scores of UCLA PTSD Reaction Index and the depression subscale of DASS-21 at six weeks after earthquake were (18.5 ± 12.6) and (3.3 ± 3.5),respectively.The rates of ASD and PTSD were 28.6% and 8.2%,respectively.The cross-lagged structural equation analysis with latent variables indicated that the ASD symptoms at two weeks after earthquake positively predicted PTSD symptoms (β =0.59,p < 0.001) and depressive symptoms (β =0.29,p <0.001) at six weeks after earthquake.But there was no significant association between depressive symptoms at two weeks and PTSD symptoms at six weeks after earthquake.Conclusion:ASD symptoms in the early time postearthquake could predict subsequent PTSD and depressive symptoms,while depressive symptoms tend to remain stable within six weeks after earthquake.

4.Cloning and expression of Sj423 gene encoding the egg-shell precursor protein of Schistosoma japonicum

The gene fragment encoding the egg-shell precursor protein of Schistosoma japonicum was amplified with RT-PCR by using PCR primer designed according to the 423 bp cDNA fragment of the Philippine strain of S.japonicum, the corresponding mDNA fragment of Chinese strain as template and then the 5' and 3' ends of this gene cDNA were amplified with 5' RACE and 3' RACE by using a series of primers designed according to the result of sequencing. Result of sequence analysis showed that this fragment, named as Sj423, contained a complete open reading frame (ORF) of gene encoding the egg-shell precursor protein of S.japonicum.(Chinese strain). As demonstrated by sequencing analysis. No intron could be detected in this gene fragment. This gene was subsequently expressed in E.coli after cloning into the expression vector pET28c(+). The molecular mass of the expressed product of this gene was 20.9 kDa as revealed by SDS-PAGE analysis, and Western blot analysis showed that the recombinant protein expressed could react well with the rabbit antiserum against the worm antigen of S.japonicum;indicating the good antigenicity of this expressed product.
5.Dynamic hip screw fixation for femoral intertrochanteric fracture:failure reason

BACKGROUND:Intertrochanteric fracture showed shattered state of different degrees in the clinic. The medial cortex is often a lack of continuity. Indentation and lesser trochanter displacement often cause destruction of biomechanics of femoral calcar to different degrees. Under this condition, it is very important to perform detailed classification of fractures and to strictly master indication of dynamic hip screw. OBJECTIVE:To further analyze the reasons for failure of internal fixation with dynamic hip screw for intertrochanteric fracture. METHODS:Data of 82 patients with intertrochanteric fracture repaired by internal fixation with dynamic hip screw, who were treated at the Department of Orthopedics, Kangtai Branch of the Second Hospital of Lanzhou University from March 2004 to December 2013, were retrospectively analyzed. The reason for failure of internal fixation and prevention method were explored. RESULTS AND CONCLUSION:Al patients were fol owed up for 4-48 months. Time of fracture healing was 12-38 weeks. Fixation failure was found in 12 cases, with an incidence of 15%. Of 12 failure cases, 7 cases affected hip screw cutting out femoral head neck (including 1 case combined with avascular necrosis of the femoral head), 1 case suffered from compression screw slipping out of the tube, 3 cases experienced screw pul ing out and breaking, plate loosening, and 1 case affected steel plate breakage. There were 1 case of Evans II type (8%), 3 cases of type III (25%), 5 cases of type IV (42%), and 3 cases of type V (25%). Lesser trochanter was not completely reset in 5 cases (42%). There were tip-apex distance>25 mm in 7 cases (58%) and early weight loading (3 weeks after fixation) in 1 case (8%). These data confirmed that the selection of indications, the degree of stability after reduction, accuracy of implant position and postoperative unreasonable exercise wil cause fixation failure of dynamic hip screw. Preoperative careful and comprehensive analysis, intraoperative precise operation and postoperative reasonable functional exercise are the keys to ensure success of fixation.
6.Clinical characters and CT findings of steatohepatitis in highland area.

OBJECTIVETo explore the relationship between the CT findings of steatohepatitis and the special environmental factors in highland area, such as anoxia and life styles.
METHODSCT scan was performed for 64 patients, who were 44 males and 20 females in Tibet with a 3500-meter elevation.
RESULTSHypodensity in liver parenchyma and the invertion of liver density and spleen density were found in all the 64 patients. According to the degree and scope in their pathological changes, these patients were divided into three groups: severe 14, moderate 26, and mild 24.And diffused 43, sectional 14, regional 7 by CT findings. Among the 44 drinkers, 34 with more than 10 years were diffused by CT scan, the other 10 drinkers under 10 years, diffused 8, and sectional 2. In addition, the 27 steatohepatitis patients were all diffused by CT who were obese and had drinked more than 10 years.
CONCLUSIONanoxia and drinking excessively in highland area are relevant to fatty degeneration in liver.

7.Preliminary evaluation about efficacy and safety of sorafenib therapy in elderly patients with advanced renal cell carcinoma

Objective To evaluate the tolerance and safety of sorafenib for elderly patients with advanced renal cell carcinoma.Methods Forty cases with advanced renal cell carcinoma were enrolled,26 were males and 14 were females,the average age was 70 years.Recurrence or metastasis was found in 32 patients who had received nephrectomy,22 of the 32 cases had received cytokine therapy before recurrence or metastasis.Primary renal lesions of 8 cases could not be resected,so patients get renal tumor biopsy.Pathological type of all patients was clear cell carcinoma.KPS of all the patients were ≥70 points.Sorafenib was used as first-line treatment,with 400 mg twice per day,until intolerance or disease progression occurred.Results The average treatment time was 7.5 months (3-18 months),CR 0 case,PR 6 cases,SD 29 cases,PD 5 cases.The overall objective response rate and disease control rate were 15.0% (6/40)and 87.5%(35/40),respectively.The median follow-up period was 11 months.The adverse reaction included hand-foot skin reaction(70.0%),alopecia (62.5%),rash(52.5%),diarrhea(37.5%),loss of appetite(32.5%),fatigue(27.5%).Most adverse reactions occurred around the second week after drug therapy initiation,their duration did not equal.And most of these adverse reactions could be released by symptomatic treatment,they did not affect the treatment.Conclusions The types of adverse reactions of sorafenib for elderly patients with advanced renal cell carcinoma are similar to those reported in the literature.Generally the degree of adverse reactions is minor,with good tolerance and safety.
8.Screening for Causative Genes Involved in Children with Minimal Change Nephritic Syndrome

Objective To screen for the causative genes involved in the occurrence and development of minimal changes nephritic syndrome(MCNS) and to furtherly assist the genetic diagnosis and treatment of MCNS.Methods Human genome U133 Array Set from Affymetrix Inc was used to evaluate gene expression patterns in peripheral blood mononuclear cells(PBMC) isolated from 7 children with primary MCNS and 7 age-matched health volunteers.Reverse transcription-polymerase chain reaction(RT-PCR) and real-time PCR were performed to identify the findings of gene chip.Results Of 33 000 genes detected,969 genes showed significant difference between children with(MCNS) and healthy volunteers;552 genes were up-regulated,while 417 genes down-regulated significantly.Findings from RT-PCR and real-time PCR were consistent with those of gene chip.Conclusions Gene chip of expression patterns is a powerful method to detect expression difference of genes correlated with MCNS.Occurrence and development of MCNS can be a complicated process that many correlative genes may participate in.
9.Relative bioavailablity of cefaclor effervescent tabletsin human volunteers

Aim To study relative bioavailablity of cefaclor effervescent tablets in healthy volunteers. Methods According to the crossover design, A volunteers were each orally given a single does of the 0.75 g cefaclor effervescent tablets and cefaclor capsules with an interval of 5 days between the two formulations.The plasma concentrations of the drug were determined by RP-HPLC.Pharmacokinetic parameters were obtained by ATPK programe,and calculated on the basis of open single compartment model.Results After a single oral dose, the peak levels in plasma averaged Cmax(31.27?5.81)?g?ml-1 and(30.56?5.25) ?g?ml-1 at (0.58?0.12)h and(0.73?0.17)h and AUC0~4(35.48?4.65) ?g?h?ml-1 and (35.89?2.90) ?g?h?ml-1 for tablet and capsule,respectively. Conclusion The result shows that two formulations are bioequivalence.
10.Effects of Chinese herbs for replenishing shen and strengthening qi on some indexes of neuro-endocrino-immune network in asthmatic rats.

OBJECTIVETo study changes of several related indexes of neuro-endocrino-immune (NEI) network in rat asthma model, and to observe effects of epimedium herb (EH, a Chinese herb for replenishing Shen) and milkvetch root (MR, a Chinese herb for strengthening qi) on these indexes.
METHODSOne hundred and twenty male healthy Brown Norway rats of clean grade were randomly divided into 11 groups. Their mRNA expression of corticoid release hormone (CRH) in hypothalamus was tested by Realtime-PCR, serum ACTH and CORT, IL-6, IL-4, IFN-gamma were detected with radioimmunoassay and enzyme-linked immunosorbent assay respectively and pathological changes of lung tissue were observed with hematoxylin and eosin stain.
RESULTSAs compared with the normal group, mRNA expression of CRH in hypothalamic tissue, plasma ACTH and serum concentration of IFN-gamma not changed significantly, but serum level of CORT raised significantly and the pre-inflammatory factors IL-6 and IL-4 showed an increasing trend in the model rats. As compared with the model group, level of CRH mRNA expression in groups treated with low dose and moderate dose of both EH and the formula (HE and MR), IFN-gamma in group treated with moderate dose of EH and serum CORT in group treated with low dose of MR were higher, and serum levels of IL-4 and IL-6 in groups treated with high dose of MR were lower.
CONCLUSIONRats suffered from repeated asthmatic attack for three weeks had some disorders in indexes of NEI network. Chinese herbs for replenish Shen and strengthening qi could improve the function of hypothalamic-pituitary-adrenal axis and adjust the balance of Th1 and Th2 cytokines, which might be one of the mechanisms of Chinese herbs for treatment of repeated asthma attack.
