1.Clinical profile and outcomes of adult patients with echocardiographic evidence of cardiac tamponade at the Philippine General Hospital: A 5-year study (the captive-heart study).

BACKGROUND: Cardiac tamponade is a life-threatening hemodynamic condition from pericardial effusions that increase intrapericardial pressure suffeciently to externally compress and restrict cardiac chamber filling, constrain cardiac output, and induce backward failure. The number of pericardial effusions arising from cardiothoracic post-surgical and catheter-based procedures accounts for 70% of all the cases in one series. In the Philippines, tuberculous infection remains as one of the most common etiologies of pericarditis, and accounts for 25.1% of the 438 cases of pericardial effusion in a local review.
METHODS: This is a retrospective cross-sectional study. The inpatient adult echocardiograhy database of the Philippine General Hospital-Section of Cardiology from June 2007 to June 2012 was reviewed, and all confirmed studies with evidence of cardiac tamponade on echocardiography were included.
RESULTS: A total of 58 patients were included in this review. The age ranged from 18-75 years, with mean of 43±15 years. Thirty-eight (66%) patients were diagnosed cases of malignancy, with lung cancer as the most common type. The other concomitant conditions included pulmonary tuberculosis (12%), presence of pulmonary mass of undetermined etiology (7%), systemic lupus erythematosus (3%), endocrine disorder (3%), renal failure (3%), and post cardiac surgery status (2%). The most common clinical findings were tachycardia (84%) and elevated jugular venous pressure (57%). Hemodynamic compromise was seen in 8 patients (14%), and Beck's triad was present in only 5 (9%) of the cases. The pericardial effusions were moderate to large in size in 95% of the cases. All effusions were circumferencial, and 100% had right-sided chamber collapse, 38 (66%) of which had both right atrial and right ventricular diastolic collapse.
CONCLUSIONS: Malignancy and TB pericarditis account for most cases of cardiac tamponade in our setting. There should be a high index of suspicion for cardiac tamponade among patients presenting with difficulty of breathing and tachycardia, especially on a background of malignancy of TB infection. Cardiac tamponade carries a high in-hospital mortality rate and prompt recognition and intervention is warranted. There was a higher rate of intervention among those who survived their hospitalization, a finding that is similarly reflected by a previous study.

Introduction:
Ciguatera fish poisoning (CFP) is common in tropical and subtropical waters. On 13 November 2015, eight Filipino seafarers from a cargo ship sailing in the Caribbean Sea experienced a range of symptoms after consuming a barracuda. Upon their return to the Philippines, an investigation was conducted to describe the cases.
Methods:
A case-series was conducted. A CFP case was defined as a previously well individual on the ship who developed at least one gastrointestinal symptom and at least one neurologic manifestation after eating barracuda on 13 November 2015. All cases were admitted to hospital in Manila, Philippines and were interviewed using a standard questionnaire. Urine and serum samples of cases were collected for ciguatoxin (CTX) testing by radiological and receptor-binding assay.
Results:
Eight of the 25 seafarers on the ship ate the barracuda; all eight met the CFP case definition. The age of cases ranged from 37 to 58 years (median: 47 years) and all were males. Onset of symptoms ranged from 1 to 3 hours (median: 2 hours) from the time of ingestion of the barracuda. All cases experienced gastrointestinal (nausea, vomiting, diarrhoea) and neurologic (temperature allodynia, itchiness) symptoms but no cardiovascular manifestations. Urine and serum specimens of all eight cases showed CTX below the detection limit.
Discussion
The Philippines Epidemiology Bureau recommended that the Philippine Maritime Authority include CTX poisoning and its health risks in seafarers’ training to prevent future cases of CFP. The Event-based Surveillance and Response system will continue to provide a mechanism for the reporting and appropriate management of CFP cases.

Introduction :
Health care providers must develop awareness and understanding of the rights of
patients in order to render quality care. The study aimed to assess the awareness of students of the
College of Medicine of the Patient's Bill of Rights.
Methods :
Medical students selected by stratified sampling were asked to answer a questionnaire on
patients' rights. Respondents with scores of at least 80% were considered "aware". The proportion of
respondents who were "aware" was determined.
Results :
The results revealed that 84.6% of the respondents have >80% awareness level. More than
90% of the medical students sampled were aware of the right to choose a physician, followed by the
rights to be shown respect, dignity, and consideration, to be provided healthcare service of best
quality by skilled healthcare professionals, to be included in decisions and choices about one's care,
and to religious assistance. Less than 70% were aware of the right of the unconscious or legally
incompetent patient and right to access and transfer of care and continuity of care.
Conclusion
More than 80% of medical students exposed to patients are aware of the Patient's Bill of
Rights.

5.HIV screening among patients with newly diagnosed solid and hematologic malignancies in a Tertiary Hospital in the Philippines

Objectives:
This preliminary study determined the prevalence of HIV infection among patients with newly diagnosed solid and hematologic malignancies at the Philippine General Hospital - Cancer Institute.
Methods:
Adult Filipinos aged 19 years and above with biopsy- or imaging-confirmed malignancy and for
chemotherapy, seen at the adult medical oncology and hematology clinic from January to September 2021 were
included. Demographic and clinical data were obtained using a questionnaire. Rapid HIV screening was performed using blood extracted via finger prick. Pre- and post-test counselling were conducted.
Results:
Of the 124 patients included in our study, majority were female (91, 73.4%), and 45 years old and above with a median age of 49 (20 – 74). Majority had solid tumors (121, 97.6%) with breast cancer being the most common (67, 54.0%) followed by colorectal (18, 14.5%), and head and neck cancer (14, 11.3%). Among those with hematologic malignancies, two had acute myelogenous leukemia and one had multiple myeloma. Six patients had AIDS-defining malignancies (NHL, cervical cancer). HIV risk factors and associated conditions were present in 18 patients (14.5%). Ten patients reported prior HIV testing. None of the patients tested positive for HIV.
Conclusion
The absence of HIV cases detected in our cohort may be due to the low prevalence of HIV risk factors and associated conditions. At this time, there is insufficient evidence to routinely recommend HIV testing among newlydiagnosed cancer patients. However, physicians are encouraged to offer HIV testing to cancer patients, especially to those with HIV risk factors, given the benefits of early detection and management of HIV.

Objective:
Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), primarily targets the respiratory system. This study describes the characteristics associated with mortality among patients infected with SARS-CoV-2 at a single hospital in Baguio City, Philippines.
Methods:
We reviewed medical records (including history, laboratory results and treatment regimen) of 280 confirmed COVID-19 patients admitted to a single hospital during March–October 2020. Clinical characteristics and outcomes (frequency and type of complication, recovery rate and mortality) were evaluated. Multiple logistic regression was used to analyse factors associated with mortality.
Results:
The mean age of COVID-19 patients was 48.4 years and the female-to-male ratio was 1.8:1. Hypertension, cardiovascular disease (CVD) and diabetes were the most frequent comorbidities reported. Common presenting symptoms were respiratory and constitutional, with 41% of patients not reporting symptoms on admission. Patients with moderate, severe and critical disease comprised 45%, 8% and 4%, respectively. A total of 15% had complications, health care-associated pneumonia being the most frequent complication. The recovery rate was 95%; 5% of patients died, with multiorgan failure being the most common cause. The presence of CVD, chronic kidney disease, prolonged prothrombin time and elevated lactate dehydrogenase (LDH) were associated with mortality.
Discussion
Most COVID-19 patients in our population had asymptomatic to moderate disease on admission. Mortality from COVID-19 was associated with having CVD, chronic kidney disease, elevated LDH and prolonged prothrombin time. Based on these results, we emphasize that people should take all necessary precautions to avoid infection with SARS-CoV-2.

Background and Objective:
Convalescent plasma therapy (CPT) may reduce the risk of disease progression among patients with COVID-19. This study was undertaken to evaluate the efficacy and safety of CPT in preventing ICU admission among hospitalized COVID-19 patients.
Methods:
In this open-label randomized controlled trial, we randomly assigned hospitalized adult patients with
COVID-19 in a 1:1 ratio to receive convalescent plasma as an adjunct to standard of care or standard of care alone. The primary endpoint was ICU admission within first 28 days of enrolment. Primary safety endpoints include rapid deterioration of respiratory or clinical status within four hours of convalescent plasma transfusion and cumulative incidence of serious adverse events during the study period including transfusion-related acute lung injury (TRALI), transfusion-associated circulatory overload (TACO), severe allergic reactions, and transfusion-related infections.
Results:
A total of 22 patients were assigned to receive convalescent plasma as an adjunct to standard of care and 22 to receive standard of care alone. The median time from onset of COVID-19 symptoms to study enrolment was eight days (IQR, 4 to 10). Two patients (9.1%) in the CPT group and one patient (4.5%) in the control group were admitted to the ICU. The primary outcome measure, ICU admission, was not different between the two groups (q-value >0.9). No patient who received convalescent plasma had rapid deterioration of respiratory/clinical status within four hours of transfusion and none developed TRALI, TACO, anaphylaxis, severe allergic reactions, or transfusion-related infections. There was also no significant difference in the secondary outcomes of 28-day mortality (two patients in the CPT group and none in the control group, q-value >0.90), dialysis-free days, vasopressor-free days, and ICU-free days.
Conclusions
Among hospitalized COVID-19 patients, no significant differences were observed in the need for
ICU admission between patients given CPT as adjunct to standard of care and those who received standard of
care alone. Interpretation is limited by early termination of the trial which may have been underpowered to
detect a clinically important difference.
