1.The clinicopathological significance of transforming growth factor-β1, transforming growth factor-β1 receptor Ⅱ ,Smad4 and Smad7

Objective To investigate the clinicopathological significance of transforming growth factor-β1 (TGF-β1), transforming growth factor-β1 receptor Ⅱ (TGF-βRⅡ ), Smad4 and Smad7 protein expressions in gastric carcinomas. Methods 109 cases of gastric carcinomas,28 of high grade dysplasia,20 of low grade dysplasia,30 of chronic atrophic gastritis,29 of intestinal metaplasia, and 21 of normal gastric mucosa were selected for the detection of the expression level of TGF-β1,TGF-βRⅡ , Smad4 and Smad7 proteins by immunohistochemistry. Results The expression of TGF-β1,TGF-βRⅡ and Smad7 proteins in gastric carcinomas and dysplasia (high and low grade) were significantly higher than those in chronic atrophic gastritis, intestinal metaplasia and normal gastric mucosa (all P <0. 001 ) ,whereas the expression of Smad4 was significantly increased in high and low grade dysplasia (cytoplas-mic IS :4.89±2.38,5.80±1.54, respectively; nuclear IS: 3.89± 1.52,3.80±1.33, respectively), and decreased in gastric carcinomas (cytoplasmic IS:2.41±2.27,nuclear IS:2.02±2. 14) (P <0.001 ,P <0.001 ,respective-ly). Meanwhile,the expressions of TGF-β1,TGF-βRⅡ and Smad7 were higher in advanced gastric cancer (4.36± 2.66,3.05±1.93,4.84±3.06, respectively) than those in early gastric cancer ( 2.93±1.85,2. 17±1.87, 4. 14±2.46, respectively) ( P < 0.01, P < 0.05, P < 0. 05, respectively ) ; however the expression of Smad4 protein was much higher in early gastric cancer (P < 0. 001 ). The expression of Smad4 protein was significantly correlated with tumor size( P < 0.05 ), lymph node metastasis ( P < 0. 001 ), T stage ( P < 0. 001 ) and chnical stage ( P < 0.001) ,and the expressions of TGF-βRⅡ and Smad4 proteins were correlated with patients' 5-year survival rate (P<0.05 ,P < 0.01 ). TGF-β1 showed the positive correlation with TGF-βRⅡ and Smad7 (r1= 0.45 ,P <0. 05; r2 = 0.49, P <0. 05), but negative correlation with Smad4 ( r = - 0.21,P < 0.05 ). Conclusion TGF-β1, TGF-βR Ⅱ, Smad4 and Smad7 protein may play an important role in the carcinogenesis of gastric carcinoma,and smad4 is the key in TGF-β pathway. Also, the detection of these proteins expression can be a useful marker for predicting the gas-tile cancer invasion,metastasis and patients' prognosis.
2.Effect of Zhixuebao particles combined with Marvelon in the treatment of dysfunctional uterine bleeding in adolescent

Objective To observe the effect of Zhixuebao particles combined with Marvelon in the treatment of dysfunctional uterine bleeding in adolescent.Methods69 cases from October 2013 to March 2015 were randomly divided into observation group with 34 cases and control group with 35 patients.The control group was treated with Marvelon and observation group was another treated with Zhixuebao particles.Related indicators of two groups were compared.ResultsEndometrial thickness, menstrual volume and menstrual period of observation group were lower than that of control group(P<0.05).The bleeding time, stop bleeding time of observation group were shorter than that of control group (P<0.05).After treatment,hormone LH, FSH, P levels of observation group were lower than that of control group(P<0.05);Total effective rate of bservation group was 94.12%, higher than control group 74.29%(P<0.05).ConclusionZhixuebao particles combined with Marvelon is effective in treating dysfunctional uterine bleeding in adolescent, can improve hormone levels and it is worthy of further research and application.
3.The effect of postoperative early aerosol inhalation on sore throat of patients after double lumen ;endobronchial intubation

Objective To evaluate the effect of early aerosol inhalation on sore throat of the patients after double lumen endobronchial intubation. Methods 90 patients scheduled for thoracic surgery were randomly assigned to 3 groups,30 cases in each group: control group (Group C), early aerosol inhalation group (group one) and later aerosol inhalation group (group two). All patients were sent to PACU after extubation.The patients in group C were intraveously injected with 6 mL saline , those in group one were treated with aerosol inhalation of 1mg budesonide, while the patients in group two with budesonide at the same dosage 2 h later. Patients were examined with indirect laryngoscopy 6 h after surgery. The cases of vocal cord congestion and glottis edema were recorded. The degree of pharyngolaryneal pain was assessed with the Visual Analogue Scale (VAS) when they were sent to PACU , 6 , 24 and 48 hours after surgery . Results The rates of vocal cord congestion and glottis edema in group one were lower than those in group C and group 2, 6 h after surgery (P < 0.05). The pharyngolaryneal VAS in group one was significantly lower than that in group C and group 2 , 6 and 24 h after operation (P < 0.05), but there was no statistical difference between them 48 h after surgery between 3 groups. Conclusions Aerosol inhalation of budesonide after double lumen endobronchial intubation for the patients scheduled for thoracic surgery during early postoperative stage can reduce the incidences of vocal cord congestion and glottis edema, inhibit airway inflammation, significantly reduce the extent of the POST. The effect is better than that of the lateraerosol inhalation of budesonide.
4.Kerion Caused by Geotrichum in China: First Case Report in China

Objective To report a case of kerion caused by Geotrichum in China. Methods A 9 year-old-boy had kerion-form lesion on his scalp with swollen posterior auricular lymph nodes, and did not show other definite underlying disease. The pathogenic fungus was identified according to culture, scanning electron microscopy, biochemical tests and DNA sequencing. The hair infection test was performed and the infected hairs were examined by scanning electron microscope. Animal test confirmed the pathogenicity of the fungus. Results The fungal colonies were the same when the tissue cultures were repeated. The colonies showed milky white to yellowish in color. The hyphae could be identified at the periphery on Sabouraud′s agar culture at 27 ℃, which were moist and smooth on the surface at 37 ℃. Under microscope, there were many rectangular arthrospores, round or oval spores with or without buddings, as well as branched hyphae. The isolated fungus was identified as a Geotrichum silvicola by culture, scanning electron microscope, biochemical test and DNA sequencing. The patient′s condition was improved markedly after treatment of terbinafine for 4 weeks. Conclusions This is the first case report of kerion caused by Geotrichum in China, and terbinafine is effective.
5.Maxillary anatomy and its influence on miniscrew implantation

BACKGROUND: Miniscrew plant attachment to maxillary sinus is an important factor for implant safety and bone density can influence implant stability. Studies have explored implantation location of implant anchorage, such as bone thickness or bone density. However, the idea on implantation angle remains controversial. OBJECTIVE: To investigate the relationship between miniscrew implant in the interradicular space between the second premolars and the first molars in the maxilla and the maxillary sinus as well as bone density in the surroundings. DESIGN, TIME AND SETTING: Observation experiment was performed at dinan University from October 2007 to May 2008. PARTICIPANTS: A total of 38 patients with cone beam CT data were selected from Uni-Care Dental Center, Macao, including 11 males and 27 females, aged 18-35 (24.8±3.8) years. METHODS: Three dimensional images were reconstructed from cone beam CT images. For each subject, the buccolingual bone thickness was measured by postulating that the miniscrews would be inserted at 16 positions (10, 12, 14 and 16 mm above the median sagittal planes of the interradicular spaces between the first molars and the second premolars in the maxilla, and 30°, 45°, 60°, 90° from the cortical bone surface). The danger rate representing for implantation sites where buccolingual bone thickness was less than 6 mm was calculated. The density of the bone around the entire circumference of each miniscrew was also measured. MAIN OUTCOME MEASURES: Height of maxillary sinus, buccolingual bone thickness at implantation sites and bone density. The implantation sites could be divided into three groups. Group 2 exhibited the greatest danger rate and bone density and group 1 had the lowest danger rate and bone density.
6.Electron-beam chest-wall irradiation in breast cancer patients after mastectomy

Objective To evaluate the effcacy of electron-beam chest-wall irradiation in patients with breast cancer after mastectomy.Methods From June 1999 to December 2007,280 women with localized breast cancer received postmastectomy radiotherapy using electron beam to chest wall.The effcacy and toxicity of these 280 women was compared with 118 women treated during the same period using tangential field with photon beam.Results The follow-up rate was 93.2%.140 patients had a minimum followed up time of 5 years and 12 patients had a minimum follow up time of 10 years.The 5-year and 10-year chest wall recurrence rates were 6.8%and 5.0%.14.8%and 10.1%for patients irradiated with electron and photon(X2=1.12,P=0.290).The corresponding 5-year and 10-year disease-free survival rates were 60.6%and 65.5%,47.6%and 57.3%(X2=0.97,P=0.325).The 5-year and 10-year overall survival rates were 77.5%and 79.6%,48.4%and 53.3%(X2=0.37,P=0.545).Grade Ⅱ or more acute skin toxicity occurred in 10.4%and 16.9%of patients irradiated with electron and photon(X2=3.34.P=0.090).Pulmonary fibrosis developed in 28.8%and 22.1% of patients irradiated with electron and photon(X2=1.27,P=0.300).Conclusion Electron-beam chest-wall irradiation is as effective as photon-beam irradiation in breast cancer after mustectomy.
7.Indications for postmastectomy radiotherapy in breast cancer patients with 1 -3 positive axillary nodes

Objective To analyze the prognosis of T1-T2 stage breast cancer with 1 -3 positive axillary nodes after mastectomy, and to explore a subgroup of patients who could benefit from adjuvant radiotherapy. Methods In the retrospective study of 412 eligible patients, survival analysis was performed using the Kaplan-Meier method. Univariate and multivariate analyses were performed using the Log-rank method and Cox regression analysis, respectively. Results The follow-up rate was 98. 7%. 215 and 41patients were followed up for 5 and 10 years,respectively. The 5-and 10-year overall survival (OS) rate was 90. 0% and 81.3%, respectively. The 5-and 10-year locoregional recurrence (LRR) rate was 10. 7% and 18. 6%, respectively. In univariate analysis, T2 statging, more than one positive node, hormone receptornegative ( ER&PR-negative), ratio of positive lymph nodes (LNR) > 25%, Her-2 positive, no hormonal therapy were associated with a significantly higher rate of LRR. T2 staging, more than one positive node,hormone receptor-negative were the risk factors for LRR with statistical significance in the multivariate analysis. Basing on these 3 risk factors, the high-risk group (with 2 -3 factors) had a 10-year LRR rate of 36. 9% compared with 3.9% in the low-risk group ( with 0 - 1 factors;x2 =20. 64,P =0. 000). The 5-year and 10-year distant metastasis (DM) rate was 12.9% and 24. 5%, respectively. LRR, and LNR >25%were statistically significant predictors of DM in the multivariate analysis. The 5-year DM rate for patients with LRR was 36. 6% compared with 9. 7% without LRR (x2 = 16. 34,P =0. 000). The 5-year OS rate for patients with LRR was 69. 9% compared with 92. 9% without LRR ( x2 = 20. 79, P = 0. 000). LRR was associated with a higher risk of distant metastasis and worse survival. Conclusions LRR after mastectomy has a significant impact on the outcome of patients with T1 -T2 breast cancer and 1 - 3 positive axillary nodes.Patients who have 2 -3 risk factors might benefit from radiotherapy.
8.Bystander effects of Guiqiyiyuan Ointment on lung and kidney of rats damaged by heavy ion radiation

Objective To investigate the mechanism of Guiqiyiyuan Ointment (GO) for preventing and treating 12C6+ beam radiation induced lung and kidney bystander effect to provide a new strategy for prevention and treatment of clinical radiation injury. Methods Sixty healthy male Wistar rats were randomly and equally divided into 3 groups: NC group, SR group (simple radiation 2ml/kg), GO group(GO 2ml/kg intragastric administration for 7 days). The right side of the lung was modeled by 12C6+beam radiation. After modeling, the rats were killed at 48h. The left lung, left and right kidney tissues were taken from the rats. The DNA methylation rate was detected by ELISA assay, pathological changes were observed by HE staining, and the expressions of Dnmt1, Dnmt3a and Dnmt3b were detected by immunohistochemistry. Results Compared with the NC group, the level of DNA methylation was decreased significantly (P<0.01), the left lung showed inflammation, no abnormal finding was seen in the left and right kidneys, and the expressions of Dnmt1, Dnmt3a and Dnmt3b were significantly increased in the SR group (P<0.01). Compared with the SR group, the level of DNA methylation was increased significantly (P<0.01), the left lung inflammation became better, and the expressions of Dnmt1, Dnmt3a and Dnmt3b were significantly decreased in the GO group (P<0.01). Dnmt1, Dnmt3a and Dnmt3b proteins were expressed in the cytoplasms of bronchial and renal tubular epithelial cells in all the groups. The NC group presented as light brown-brown staining, showing a weak positive expression, the SR group as brown-brown staining, showing astrong positive expression, and the GO group as light brown-brown staining, showing a moderate positive expression. Conclusion The GO can reduce the bystander effect caused by 12C6+ beam radiation, and its mechanism is related to improving the level of DNA methylation.
9.Efficacy of EUS-guided ethanol ablation in the treatment of insulinoma

Objective To evaluate the safety and efficacy of endoscopic ultrasonography(EUS) guided ethanol ablation in patients with insulinoma. Methods The data of 10 patients with insulinoma trea-ted at the First Affiliated Hospital of Guangxi Medical University from December 2013 to January 2015 were prospectively analyzed. Results The patients were given EUS-guided ethanol ablation with dose of 0. 10 to 2. 00 ml(average 0. 70 ± 0. 62 ml)in pancreatic lesions for 15 times. No complications were observed dur-ing and after the procedure. The blood glucose improved after the procedure[4. 8(3. 9-5. 5)mmol/ L VS 2. 4 (1. 9-2. 5)mmol/ L,P < 0. 05]and the serum insulin level significantly decreased[83. 7(40. 1-143. 5) pmol/ L VS 177. 3(66. 5-200. 6)pmol/ L,P<0. 05]. The average hospital stay was(4. 3±1. 5)days. The patients were followed up for 6-12 months. EUS indicated that the echo of pancreatic lesions changed from high to low. CE-EUS revealed low enhancement and lack of blood supply. Conclusion EUS-guided ethanol ablation may become a promising minimally invasive treatment for insulinoma because of its safety,efficacy and low price. 10.miR-124 suppresses multiple steps of breast cancer metastasis by targeting a cohort of pro-metastatic genes in vitro.

Metastasis is a multistep process involving modification of morphology to suit migration, reduction of tumor cell adhesion to the extracellular matrix, increase of cell mobility, tumor cell resistance to anoikis, and other steps. MicroRNAs are well-suited to regulate tumor metastasis due to their capacity to repress numerous target genes in a coordinated manner, thereby enabling their intervention at multiple steps of the invasion-metastasis cascade. In this study, we identified a microRNA exemplifying these attributes, miR-124, whose expression was reduced in aggressive MDA-MB-231 and SK-3rd breast cancer cells. Down-regulation of miR-124 expression in highly aggressive breast cancer cells contributed in part to DNA hypermethylation around the promoters of the three genes encoding miR-124. Ectopic expression of miR-124 in MDA-MB-231 cells suppressed metastasis-related traits including formation of spindle-like morphology, migratory capacity, adhesion to fibronectin, and anoikis. These findings indicate that miR-124 suppresses multiple steps of metastasis by diverse mechanisms in breast cancer cells and suggest a potential application of miR-124 in breast cancer treatment.
