2.Hydroxyl radical and cerebral ischemia
Chinese Pharmacological Bulletin 1998;0(S1):-
Free radicals are extensively existed in aerobic organisms with high reactivity. In cerebral ischemia, free radicals can be generated in excess of cellular antioxidant capacity resulting in severe damage. Hydroxyl radical is one of the most potent and reactive species. It plays a key role in neuronal damage induced by ischemia and reperfu-sion. This review will focus on: possible routes forthe formation of hydroxyl radical during cerebral ischemia; changes of hydroxyl radical contents induced by ischemia; the effect of hydroxyl radical on ischemic neuronal damage; the protective effects of hydroxyl radical scavengers on neuronal damage induced by ischemia.

Objective To investigate the most appropriate low radiation dose in multi-slice CT (MSCT) scans for orbital trauma patients. Methods Thirty trauma patients with suspected orbital fractures who underwent helical CT scans with a 64-MSCT using regular dose were selected. Noise was artificially introduced to the axial images using an image space noise addition tool to simulate 6 sets of lower dose scans with tube current of 30, 70, 100, 140, 170 and 200 mA, respectively. The lowest tube current with adequate image quality for confident diagnosis was determined based on the evaluation of the overall image quality and fracture detection on images at different dose levels. The determined lowest tube current was then validated using clinical scans. Radiation dose related parameters CTDIvol, DLP, ED were also recorded.Image quality was evaluated according to its low-density resolution, noise and structure clarity and characterized into 5-grades of excellent, good, fair, worse and worst Rank sum test and X2 test were wsed for statistics. Results In 30 trauma patients with regular dose of 300 mA, there were 30 cases of orbital fracture, 19 cases of intraorbital emphysema, 12 cases of ocular muscle injury and 1 case of intraorbital foreign body. These changes could still be clearly observed and correctly diagnosed when the tube current was reduced to as low as 70 mA. However, the overall image quality was mostly fair. At the simulated dose of 100 mA, the majority of images were characterized as excellent or good, and there was no statistical difference compared with that of regular dose scans (P > 0.05 ) . In the clinical evaluation for 20 orbital trauma patients with the reduced tube current of 100 mA, the majority of images were judged to be excellent (9 cases) or good(17 cases) and fair (4 cases). The radiation dose(0. 29 mSv) was reduced by 70% compared with that of regular tube current of 300 mA(0. 86 mSv). Conclusion The tube current of MSCT may be used as low as 100 mA in orbital trauma patients to reduce radiation dose and keep the image quality diagnosable.

Objective To report a case of kerion caused by Geotrichum in China. Methods A 9 year-old-boy had kerion-form lesion on his scalp with swollen posterior auricular lymph nodes, and did not show other definite underlying disease. The pathogenic fungus was identified according to culture, scanning electron microscopy, biochemical tests and DNA sequencing. The hair infection test was performed and the infected hairs were examined by scanning electron microscope. Animal test confirmed the pathogenicity of the fungus. Results The fungal colonies were the same when the tissue cultures were repeated. The colonies showed milky white to yellowish in color. The hyphae could be identified at the periphery on Sabouraud′s agar culture at 27 ℃, which were moist and smooth on the surface at 37 ℃. Under microscope, there were many rectangular arthrospores, round or oval spores with or without buddings, as well as branched hyphae. The isolated fungus was identified as a Geotrichum silvicola by culture, scanning electron microscope, biochemical test and DNA sequencing. The patient′s condition was improved markedly after treatment of terbinafine for 4 weeks. Conclusions This is the first case report of kerion caused by Geotrichum in China, and terbinafine is effective.

Sepsis-associated encephalopathy(SAE)is a common complication with high mortality in patients with sepsis, but its pathogenesis is not clear, and there is no recognized diagnostic criteria and specific treatment.Intestinal tract plays an engine-like role in the occurrence and development of sepsis. The destruction of intestinal barrier and the disorder of intestinal microorganisms can affect the outcome of sepsis, in which gut microbiome affect the pathophysiology of intestine and brain through " the microbiome-gut-brain axis" (MGBA), and "gut microbiome-mitochondrial crosstalk" explains its role at the organelle level. The gut microbiome disorder exists in SAE animal model, while fecal bacteria transplantation can improve the symptoms and prognosis, suggesting that the exploration of gut microbiome may be of certain significance to understand the mechanism of SAE and explore its treatment. Here we review from three aspects: the gut microbiome, MGBA and the role of gut microbiome in SAE.

A 53-year-old male patient was admitted to the hospital on March 26, 2006. Ten years prior to the presentation, a small furuncle developed on the dorsum of his right hand, and subsided after 2-week treatment with erythromycin ointment; one month later, a broadbean-sized cutaneous ulcer developed on the dorsum of the same hand. After anti-infective treatment, the ulcer healed while the lesional skin thickened, and long-term topical treatment with compound dexamethasone acetate cream showed no obvious effect Dermatological examination revealed an irregular verrucous plaque measuring 2.5 cm × 4 cm with little exudation on the dorsum of the right hand. KOH preparation of the skin lesion revealed brown spores. Sabouraud's dextrose agar culture grew restricted, velvety and dark green colony, and microscopy revealed branched, globular conidiophores generated by cladospores. DNA sequencing showed that the isolate was different from Cladosporium sphaerospermum (AB100654) by 2 bases in the sequence of D1/D2 region of 26S rDNA, from Cladosporium sphaerospermum (AY625063) by 5 bases in the sequence of internal transcribed spacer 1 and 2 (ITS 1 and ITS 2), but fully consistent with Cladosporium sphaerospermum (AM 176719) in the sequence of ITS region. The isolate was identified as Cladosporium sphaerospermum. Hematoxylin-eosin stain of the lesional tissue revealed granulomatous changes, and PAS stain demonstrated brown spores. A diagnosis of phaeohyphomycosis was made. Antifungal susceptibility testing indicated that the isolate was highly sensitive to itraconazole. The lesion obviously subsided after treatment with oral itraconazole 0.2 g once daily for 8 weeks, but the patient was lost to follow up 2 months later.

Objective:To investigate the effects of rosuvastatin and lfuvastatin on patients with acute coronary syndrome (ACS) combing impaired glucose tolerance (IGT).
Methods: A total of 215 consecutive ACS patients combing IGT treated in our hospital from 2009-05 to 2011-05 were studied. The patients were randomly divided into 2 groups, Rosuvastatin group, the patients received rosuvastatin10mg/day, n=108 and Fluvastatin group, the patients received fluvastatin 40mg/day, n=107. The total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C) levels before and at 6, 12, 24 months after medication, fasting blood glucose (FBG), 2-hour postprandial glucose (2hPBG) and the number of new-onset of diabetes patients were compared between 2 groups.
Results: After treatment, the TC, LDL-C levels were decreased (6, 12, 24 months) and the HDL-C level (12, 24 months), 2hPBG (24 months) were increased in both groups. Compared with Fluvastatin group, Rosuvastatin group had decreased TC and LDL-C (6, 12, 24 months), and increased LDL-C (24 months). With 6, 12, 24 months treatment, the blood lipids reached the standard were more in Rosuvastatin group than those in Fluvastatin group as 35.3%vs 26.1%, 36.4% vs 22.0%, 43.1% vs 31.8% respectively, all P<0.05. With 12 and 24 months treatment, the new-onset diabetes patients in Rosuvastatin group were 11 and 18, in Fluvastatin group were 12 and 17. With 12, 24 months treatment, FBG, 2hPBG levels and the number of new-onset diabetes patients were similar between 2 groups, P>0.05.
Conclusion: Compared with lfuvastatin, the conventional dose of rosuvastatin could better reduce the blood lipids level in ACS patients combing IGT, the effects for preventing ACS patients from IGT to diabetes were similar for both drugs.
