1.THE CLINICAL FEATURE OF SUBRETINAL AND VITREOUS HEMORRHAGE ASSOCIATED WITH SENILE MACULAR DEGENERATION

To study the clinical feature and surgical outcome of subretina and vitreous hemorrhage associated with senile macular degeneration (SMD). Vitrectomy was performed in 29 eyes of 29 patients with subretina and vitreous hemorrhage associated with SMD. Changes in the retina were observed, and the results of surgery were analyzed. The results showed that there was submacular hemorrhage in 16 patients, in whom 4 showed periomacular subretinal hematoma. There was peri macular subretinal hematoma only in 13 cases. The pathological changes in the macular region included discoid degeneration, submacular neovascularization, fixed foldings, exudation, scar and atrophic discoid cicatrization. All patients got visual acuity improvement after surgery. However those patients who had subretinal hemorrhage got less improvement of visual acuity than the others. It suggested that the SMD patients who was complicated by subretinal hemorrhage had a severer macular lesion. Pars plana vitrectomy could restore central vision of patients who suffered from subretinal and vitreous hemorrhage. But the outcome was related with the position of subretinal hemorrhage.
2.Comparative study of patient comfort and nursing workload in different PICC catheter sites

Objective To explore the differences of patient comfort and nursing workload between ultrasound guidance on upper arm placement of PICC and traditional placement of PICC, and to provide a reference for correct selection of catheter way. Methods Using a nonrandomized controlled study, a total of 1 116 patients were divided into the control group and the observation group, patients with traditional placement of PICC were set as the control group, and the patients with upper arm placement of PICC were named as the observation group. The patient comfort and workload of use and maintenance of PICC by nurses were compared between two groups. Results The incidence of catheter sliding out of PICC in the observation group was 1.9%(7/365), obviously higher than that of the control group, which was 0.3%(2/751), the difference was significant (χ2=8.37,P=0.007). The patient comfort had no statistical significance between two groups (t=2.13,P=0.082). The workload of use and maintenance of PICC in the observation group were (1.87±0.31) min and (9.16±1.07) min ,which were obviously higher than that of the control group [(0.85±0.16) min and(7.22±2.13) min] in winter (t=4.39, P=0.022;t=3.44, P=0.041), but there was no statistical significance between two groups in summer (P>0.05). Conclusions The workload of use and maintenance of PICC on upper arm placement was higher than traditional placement of PICC in winter, but the obvious differences of patient comfort and nursing workload in summer between the two PICC groups were not been found.
3.Effect of pramipexole in the treatment of Parkinson after stroke

Objective To investigate the effect of pramipexole in the treatment of Parkinson after stroke .Methods 94 cases with Parkinson after stroke in our hospital from June 2013 to November 2015 were randomly selected and divided into two groups,47 cases respectively.Control group received routine etiological and symptomatic treatment, the study group received conventional therapy and symptomatic treatment and pramipexole treatment,two groups were treated for eight weeks.Parkinson’s disease scores,inflammatory cytokines and oxidative stress index,and the clinical effect and complications of contrast were compared after the treatment.Results Compared with before treatment,scores of daily activities of emotion integral integral,the motor function in two groups decreased,levels of serum IL-1β,IL-6,TNF-αand Hs-CRP decreased,levels of CAT,T-SOD,T-GSH in plasma increased (P<0.05),and compared with the control group,scores of daily activities of emotion integral integral,the motor function in the study group were lower,levels of serum IL-1β,IL-6,TNF-αand Hs-CRP were lower,levels of CAT,T-SOD,T-GSH in plasma were higher (P<0.05),and the total effective rate in the study group 91.49% was higher than the control group 74.47% (P<0.05).Conclusion Pramipexole in the treatment of Parkinson after stroke was effective with high safety,and it can reduce Inflammatory factors and increase oxidative stress.
4.Study on Optimization and Characterization of Dihydromyricetin Liposome

This study was aimed to optimize dihydromyricetin liposome formulations by orthogonal design in order to study its characterization. Dihydromyricetin liposomes were prepared with thin-film ultrasonic dispersion technology. Formulations of dihydromyricetin liposomes were optimized with entrapment efficiency as the optimized index. The formulation and technology were evaluated by the single factor. Based on this, orthogonal design was made on the screening and optimization of dihydromyricetin liposome. Its morphology was observed by transmission electron micro-scope. Its in vitro drug-release behavior was studied by equilibrium dialysis. The preliminary stability was studied. The results showed that the optimized formulation and technology of dihydromyricetin liposome was when the molar ratio of lecithin and cholesterol was 1:1, the dosage of dihydromyricetin was 4.0 mg. The pH of PBS was 5.0, ultra-sonic time was 3 min. The encapsulation efficiency of dihydromyricetin liposome was 58.1%. Its morphology was small spherical or nearly spherical vesicle with observation in transmission electron microscope. It showed that the in vitro release of dihydromyricetin liposome arrived 76.29% in 48 h. The drug content was still 96.57% of dosage for 30 days at 4oC in the dark place. It was concluded that the optimized formulation and preparation technology of di-hydromyricetin liposome were simple, replicable and stable.
5.The influence of trunk control training on motor function and the ability in the activities of daily living of patients with cerebral palsy

Objective To observe the influence of trunk control training on motor function and the ability of cerebral palsy (CP) patients in the activities of daily living (ADL).Methods Forty patients with CP were randomly divided into a treatment group (n =20) and a control group (n =20).Both groups were treated with routine rehabilitation,while the treatment group also received trunk control training.All patients were assessed with function ambulation category (FAC) classification,time to walk 10 m,the Berg balance scale (BBS),and the modified Barthel index (MBI) at the beginning and eight weeks later.Results Before the intervention there was no significant difference between the two groups in terms of any of the assessments.Eight weeks later,all the assessment scores were significantly better in the treatment group than in the control group.Conclusion Trunk control training can significantly improve motor function and the ADL ability of patients with CP.
6.Safety and immunogenicity in 3-5 months-old infants after primary and boosting immunization with Hib PRP-TT conjugate vaccine

Objectives To evaluate the safety and immunogenicity of Haemophilus influenzae type b capsular-tetanus toxoid(Hib-TT) conjugate vaccine.Methods In an open-controlled,randomized trial,the eligible and consented infants of 3 to 5 months-old received 3 doses of Hib-TT or a licensed Hib-TT conjugate vaccine(Anerbao) as the control vaccine to evaluate safety; The serum anti-Hib PRP IgG antibody mean geometric concentration (GMC) in both groups after primary and boosting vaccination were measured by ELISA.Results No apparent difference in the frequency of total adverse reactions observed between two groups (study vaccine 23.85% vs.comparator 31.40%) (x2=0.5,P>0.05).The mild and severe fever reaction of both vaccines was 3.67% and 4.48% respectively,with no significant difference.The local reactions including erythema,swelling and induration reported was 1.22% in study vaccine group.After 3 injections,the serum anti-Hib PRP IgG antibody GMC was 6.6686 μg/ml in study group and 7.5346 μg/ml in control group,with no significant difference in both of the antibody GMC (x2 =0.147,P=0.702).After one dose boosting injection,the serum antibody GMC in study group increased from 2.6396 μg/ml of preboosting to 6.2044 μg/ml of post-boosting.Conclusion The Hib-TT conjugate vaccine is proved to be safe in 3-5 months-old infants.The primary immune schedule for 3-5 months-old infants of 3 injections 1 month apart with the Hib-TT conjugate vaccine could induce IgG antibody response against Hib PRP with the GMC of long-term protection concentration in serum.A boosting dose after primary vaccination elicited obviously immunological memory.
7.The correlation between the serum resistin and the collapse process of femoral head necrosis

Objective We aimed to explor the correlation between the serum resistin levels and the collapse process of femoral head necrosis. Methods Eighty-eight patients with osteonecrosis of the femoral head were included in this study (26, 34 and 28 cases at ARCO stage Ⅱ, Ⅲ and Ⅳ, respectively). Fifty healthy controls were enrolled. The serum resistin levels were detected with ELISA method. We compared the serum resistin levels between the patient group and control group. The differences of serum resistin levels between different ARCO stagesand various disease causes were analyzed in the patient group. Results The resistin levels were significantly higher in patients with osteonecrosis of the femoral than healthy control group (P = 0.026). Compared with control group, the resistin levels significantly increased in patients at ARCO stage Ⅲ and ARCO stage Ⅳ respectively (P = 0.001).The resistin levels of procollapse group (ARCO stage Ⅲ and Ⅳ) were significantly higher than that of precollapse group (ARCO stage Ⅱ) (P = 0.000). There was no statistic difference between ARCO stage Ⅲ andⅣ in resistin levels (P> 0.05). No statistical significance was found between different disease causes. ROC curve analysis of resisrin level indicated theertain accuracy (AUC = 0.749) , sensitivity and significant specificity (77.4%, 61.5%, respectively) in the diagnosis of femoral head necrosis. Conclusions Resistin is closely related to the collapse process of femoral head necrosis. The level of resistin was significantly increased after the collapse of femoral head, which could be useful for the clinical diagnosis of the collapse of femoral head necrosis.
8.Influence of treatment combining feedforward control and trunk exercise on the integrated ability and activity in children with cerebral palsy

Objective To investigate the influence of treatment combining feedforward control and trunk exercise on the integrated ability and activity in children with cerebral palsy (CP). Methods 100 children with CP were divided into control group and observation group, with 50 patients in each group. Patients in control group were treated with routine trunk exercise while patients in observation group were treated with additional feedforward control. The follow-up lasted 6 months. The scores of WeeFIM, GMFM-88, activity of daily living (ADL) and quality of life in two groups before and 6 months after intervention were observed. Results Scores of WeeFIM and GMFM-88 of 6 months after interven tion were higher than those before intervention in two groups respectively(P<0. 05). Scores of WeeFIM and GMFM-88 in observation group 6 months after intervention were higher than those in control group(P<0. 05). Scores of ADL 6 months after intervention in two groups were significantly higher than before intervention respectively (P<0. 05). Score of ADL 6 months after intervention in intervention group was significantly higher than that in control group(P<0. 05). Results of two groups 6 months after intervention were significantly different from their results before intervention. Scores of social function, physiological function, emotional function and total score of PedsQL were higher than those before intervention respectively (P<0. 05). These scores in observation group 6 months after intervention were significantly higher than those in control group (P<0. 05). Conclusion The effectiveness of treatment combining trunk exercise and feedforward control in children with CP was significant. This treatment could improve the motor function, ADL and life quality of patients.
9.Application of bibliometrics and visualization techniques to analyze the global research status and trends of rehabilitation after arthroplasty

Objective To investigate the global research status and trends ofrehabilitation after arthroplasty.Methods The Wed of Science database was used to search the publications on rehabilitation after arthroplasty from 1994 to 2018.The included publish items were statistically analyzed by bibliometrics.VOSviewer software was used to analyze the visual transformation of literature coupling (including author coupling,mechanism coupling and country coupling) and co-occurrence analysis.The research status and trends of rehabilitation after arthrop]asty in recent years were analyzed and predicted.Results A total of 1 702 studies were included in the present study.The number of literatures increased year by year globally,including 612 in the United States as the top number of studies in the world.The total citation frequency (15 433 times) and H index (61) of the research publications were also the highest in the world.China (79 literatures) ranked 6th in the number of global research publications,with total citation frequency (451 times) and H index (12) ranked 14th.The number of publications published by JOURNAL OF ARTHROPLASTY and ARCHIVES OF PHYSICAL MEDICINE AND REHABILITATION on rehabilitation after arthroplasty was the highest.The University of Pittsburgh and the University of Toronto were the biggest contributors to publications on rehabilitation after arthroplasty.The theme of rehabilitation after arthroplasty can be divided into five categories:pain management,functional exercise,hospital management,complications and clinical trials.Hospital management wasthe main research field recently and the orthopedic specialty hospital would become a hot research topic in the fulure.Conclusion According to the current global trends,rehabilitation study is deepening and the number of publications will increase continuously.The United States is the largest contributor in this area.The current researches focus on the "hospital management" after arthroplasty.The new type of orthopedic specialty hospital may be the next research hotspot for arthroplasty.

On August 2-4,2023,the"Third Summit Forum on'Building a Community of Shared Future for Doctors and Patients'"was jointly organized by institutions such as the Chinese Medical Ethics,the Hospital Humanities Management and Talent Training Special Committee of the China Population and Culture Promotion Association,Center for Ethical Studies of Renmin University of China,the Newspaper for China's Physicians,the China Health Law Society,the China Anti-Cancer Association,and the China Association For Ethical Studies in Harbin.The conference arranged a sub-forum for the"Seminar on the Construction of Chinese Medical Humanities",with domestic medical humanities scholars attending the conference.After heated discussions at the seminar,the Scholars'Consensus on the Construction and Development of Chinese Medical Humanities was formed.It was proposed that in the new era,it is urgent to build the medical humanities discipline,as well as lead the academic integration and development of medical humanities under the core socialist values.At the same time,for the construction of the medical humanities discipline,it is necessary to optimize the organizational mechanism,prosper and develop the overall framework of the medical humanities discipline,accelerate the construction of a professional teaching team for the medical humanities discipline,promote the establishment of a new carrier medical humanities education and teaching in cultivating morality and nurturing talents,as well as focus on solving problems related to the cultivation of medical humanities graduate students.
