Objective To determine the relationship between angiotensin converting enzyme gene polymorphism and ? myosin heavy chain gene and dilated cardiomyopathy (DCM) among people of Han nationality in Liaoning province. Methods The insertion/deletion (I/D) polymorphism of ACE gene was detected by the polymerase chain reaction (PCR) in 43 patients with DCM and 53 normal controls. The exons 13,14 of cardiac ?-MHC gene from 43 patients and 53 controls were amplified restrictively by using PCR. The amplified products of PCR were digested by restrictive en-donuclease DdeⅠ and NlaⅢ to identify the mutations at No. 8848 and 9124 nucleotide of ?-MHC gene. Objective To study the psychological status of patients having undergone eyeball enucleation,and put forward the corresponding strategy for psychological nursing.Method The symptom checklist-90(SCL-90)was used to investigate their psychological status after eyeball enucleation.Result The scores on interpersonal relationship sensitivity factor,depression factor, anxiety factor and terror factor(28?78±0?61),(40?76±0?65),(33?86±0?62)and(23?88±0?46),respectively.Conclusions Pertinent psychological nursing may help patients to eliminate the adverse emotional and psychological pressures,promoting them to develop a good state of mind,and return to the society better?
3.Effect of ulinastatin on serum concentration of S100βprotein and NSE in patients undergoing supratento-rial tumor resection

Objective To observe the serum concentration of S100βprotein (S100β)and neuron specific enolase (NSE)in patients undergoing supratentorial tumor resection with ulinastatin treat-ment.Methods Twenty-four patients with supratentorial tumor resection,aged 18-65 years,ASA Ⅰor Ⅱ,were randomly divided into the control group (group A,n =12)and ulinastatin group (group U,n =12).Patients in group U received ulinastatin (2 kU/kg)at the beginning of the surgery,with the continuous dose of 1 kU·kg-1 ·h-1 till the end of the operation.Group A received equivalent volume of saline solution as the vehicle control.Blood samples were taken from the artery and jugular venous bulb before induction of anesthesia (T1 ),skin incision (T2 ),1 h after dura openning (T3 ),at the closure of dura (T4 ),at the end of operation (T5 )and 24 h after operation (T6 )to analyze the concentration of S100β and NSE.The concentration of S100β and NSE were determined by ELISA. Results The concentration of serum S100β and NSE increased more significantly higher at T3-T6 in group A than group U (P <0.01).The concentration of serum S100βand NSE in group U were lower than those in group A at T3-T5 (P < 0.01 ).Conclusion Ulinastatin reduces the concentration of serum S100βand NSE during surgery,indicating it alleviates brain injury during supratentorial tumor resection.
4.Drug resistance and homology of diarrheagenic Escherichia coli in sentinel hospital of Fengxian District of Shanghai, 2019‒2022

ObjectiveWe conducted a drug resistance and homology analysis of diarrheagenic Escherichia coli (DEC) in Fengxian District of Shanghai in order to provide a basis for clinical rational drug use, risk monitoring and early warning. MethodsDEC were isolated from diarrheal patients in Fengxian District, Shanghai from 2019 to 2022. The minimum inhibitory concentrations (MIC) of 21 drugs to the DEC were determined. Genotyping and homology analysis were conducted with pulsed-field gel electrophoresis (PFGE). ResultsThe DEC detection rate of diarrhea cases was 18.99% (131/690), including enteroaggregative E.coli (EAEC) 64.89% (85/131), enterotoxigenic E.coli (ETEC) 22.14% (29/131), enteropathogenic E.coli (EPEC) 12.21% (16/131), and enterohemorrhagic E.coli (EHEC) 0.76%(1/131). The DEC detection showed obvious seasonal characteristics with a high incidence in summer. The DEC multidrug resistance rate was 66.41% with a total of 65 drug resistance profiles. The five antimicrobial drugs with the highest resistance rate were ampicillin (60.31%), nalidixic acid (51.91%), cefazolin (50.38%), tetracycline (44.27%), and cotrimoxazole (35.11%). The rate of DEC resistance to levofloxacin was significantly increased from 2019 to 2022. Cluster analysis showed that the similarity of 85 EAEC cluster was 58.4%‒100.0%, and 69 band patterns were obtained. The similarity of 29 ETEC cluster was 58.5%‒100.0%, and 13 band patterns were obtained, including 2 dominant band types. The similarity of 16 EAEC clusters was 53.9%‒100.0%, and 15 band patterns were obtained. Five groups of homologous strains were found, consistent with the resistance phenotypes. ConclusionAmong the diarrhea cases, the DEC epidemic intensity is high, the drug resistance situation is severe, and the risk of outbreak infection is high in Fengxian District, Shanghai. Therefore, health monitoring and prevention need to be strengthened.
5.Analysis of the risk of reinfection and influencing factors of SARS-CoV-2 nucleic acid test positive individuals in Fengxian District, Shanghai, 2022

ObjectiveTo investigate reinfection or the third time infection with SARS-CoV-2 among the people tested for positive from December 2022 to January 2023 and the influencing factors through a follow-up survey on previous novel coronavirus nucleic acid positive individuals between March to May, 2022. MethodsEpidemiological data of 2 583 novel coronavirus nucleic acid test positive cases were analyzed from March to May, 2022, following a follow-up survey at the 8th and 12th month after the first nucleic acid test positivity. Pearson chi-square method was used to analyze the differences of reinfection and the third time infection rates among first-positive patients with different characteristics. Kaplan-Meier survival analysis and Cox regression were used to analyze the influencing factors of reinfection. ResultsA total of 2 264 valid questionnaires were collected in the 8th month after nucleic acid tested positive, with a recovery rate of 87.7% and a reinfection rate of 9.7%. The third time infection was investigated among the individuals infected twice at the 12th month after the first nucleic acid test positivity, with a third time infection rate of 4.6%. The median interval (P25, P75) between reinfection and the first nucleic acid test positive for the novel coronavirus was 261 (252, 268) days and the interval (P25, P75) between the third time infection and reinfection was 135 (111,157) days. Gender, age, occupation, smoking, drinking and underlying diseases were not statistically associated with the risk of reinfection (P>0.05). However, the the third time infection rate for medical staffs (20.0%) was higher than that for student /teachers (14.3%) and corporate employees (9.5%), with a statistically significant difference in the third infection rate between different occupations (P<0.05). The risk of reinfection in self-employed individual was lower than that in corporate employees (HR=0.52, 95%CI: 0.33‒0.83), and which was still lower after adjustment for gender and age. The risk of reinfection among those with underlying diseases was 1.54 times (95%CI: 1.08‒2.02) higher than those without underlying diseases, but even 1.85 times (95%CI: 1.25‒2.75) higher after adjustment for gender and age. ConclusionDue to the constant mutation and variants of the novel coronavirus, the risk of reinfection and the third time infection is unavoidable. The presence of underlying diseases and occupation are the main factors influencing reinfection or third time infection.
6.Risk characteristics of hypertension in high-risk population: an analysis based on the surveillance data of chronic diseases in Shanghai

Objective:To understand the epidemiological characteristics of high-risk population of hypertension in Shanghai, and to provide references for health management and targeted lifestyle intervention of the high-risk population in community. Methods:The data from the 2013 Shanghai Non-communicable and Chronic Diseases Surveillance were used. The subjects who met the criteria of high-risk population of hypertension were included, and 11 753 subjects were finally identified, to analyze the distribution and level of risk characteristics. Result:Among six high-risk characteristics of hypertension, the detection rate of high salt in diet was the highest (55.3%), followed by overweight/obesity (44.0%), family history of hypertension (41.6%), and blood pressure level of (130-139)/(85-89) mmHg (31.7%). The detection rate of excessive drinking was the lowest (5.0%). 7. Teaching clinic for training of general practitioners in suburban of a Shanghai community

In order to improve the quality of clinical teaching, Xidu Community Health Service Center of Shanghai Fengxian District established a teaching clinic for training of general practitioners in 2015. The clinic served as a suburban teaching station of Department of General Practice of Zhongshan Hospital, a tertiary university hospital. This article introduces the implementation methods, achievements, practical experiences and work prospects of the clinic. The issues of system evaluation and the generalization of this teaching model for training of general practitioners are also discussed in the article.

Gastric cancer is one of the common tumors in the world and a major cause of cancer death.Although the 5-year survival rate of gastric cancer patients has increased greatly with the improvement levels of diagnosis and treatment,the high malnutri-tion rate of gastric cancer patients still has a significant impact on their overall survival and quality of life.Malnutrition is considered an independent prognostic factor for cancer patients,early detection of malnutrition in gastric cancer patients and more reasonable peri-operative nutritional support play an important role in the survival and prognosis of gastric cancer patients.This article combines exist-ing research at domestic and abroad to review the nutritional risk screening and assessment of gastric cancer patients during periopera-tive period,as well as the research progress of perioperative nutritional support and immunonutrition,in order to provide more compre-hensive nutritional management strategies for patients with gastric cancer during the perioperative period.
9.Risk characteristics of chronic non-communicable diseases in high-risk groups in Shanghai

Objective:To understand the risk characteristics of high-risk population of chronic disease in Shanghai, and provide theoretical basis for screening management and health intervention. Methods:The data from the 2013 Shanghai Non-communicable and Chronic Diseases Surveillance was used for analysis. People (aged 35 and over) who had no diagnosis of hypertension, diabetes and dyslipidemia but met the criteria for high-risk groups of chronic diseases were included in the study. Finally, 3 682 participants were included. Results:In this high-risk population, 62.90% of the subjects has total serum cholesterol level of 5.2-6.2 mmol/L, 35.88% had blood pressure level of (130-139)/(85-89) mmHg, 30.50% had abdominal obesity, 27.68% were current smokers, and 8.75% had fasting blood glucose level of 6.1-7.0 mmol/L.
