1.Application of molecular recognition theory in thyrotropin and its receptor system

AIM: The selective recognition of the sense peptides which are located in special regions of thyrotropin receptor (TSHR) by their corresponding antisense peptides has been investigated. Three pairs of sense and antisense peptides were named TR1 (aa37-45) and RT1 (aa45-37), TR2 (aa353-366) and RT2 (aa366-353), TR3 (aa648-655) and RT3 (aa655-648). METHODS: To prepare three affinity chromatography columns, antisense peptides were immobilized, called RT1-sepharose 4B, RT2-sepharose 4B and RT3-sepharose 4B, respectively and investigate the retardative behavior for each of native peptide TR1, TR2 or TR3 on above columns with stepwise elution. RESULTS: Each of the three immobilized antisense peptides recognized and retarded its corresponding sense peptide-TR1, TR2 or TR3 instead of those non-complementary peptides. Immobilized RT1 recognized free TSHR protein molecule as well. In additional, bovine thyrotropin was recognized by immobilized TR1. CONCLUSION: The results indicate that molecular recognition theory exsits in thyrotropin receptor system. It may be useful to isolate biological molecules and to locate epitopes of TSH on TSHR molecule. Otherwise, antisense peptide may be used for treatment of experimental autoimmunolized thyroid disease (AITD) in the rat. [
2.Immune regulation of antisense peptides of thyrotropin receptor activity fragments

Objective To study the immune regulation of antisense peptide in rats by observing immune function of activity fragments of thyrotropin receptor (TSHR) and their corresponding antisense peptides. Methods TSHR peptides TR1, TR2, TR3 and their antisense peptides RT1, RT2, RT3, and three pairs of complementary peptides were injected into rats of different groups respectively, and the serum levels of TT_3, TT_4, TSHR antibody (TRAb), thyroid stimmulating antibody, thyroid blocking antibody and TSH antibody (TSHAb) and pathological changes in thyroid tissue were investigated. Results Serum TRAb could be induced when each of three fragments of TSHR was injected into rats; TRAb and TSHAb were induced by RT1 or RT2; epithelial hyperplasia and lymphocytic infiltration observed in thyroid tissue of rats injected with TR2 could be abated by injecting RT2 subsequently. Conclusion The results suggest that all 3 TSHR fragments are shown to be immunogenic and are capable to induce TRAb; both RT1 and RT2 show their effect on immune regulation and are idiotypic of TSHR peptides; On the other hand, the humoural and cell immunities are ameliarated by injection of antisense peptides. Therefore, it is possible that antisense peptides may be involved in immune regulation via immune network.
3.Expression and significance of ArhGAP29 and E-cadherin in intrauterine adhesions of endometrium

Purpose To investigate the expression and significance of ArhGAP29 and E-cadherin in endometrial tissue of intrauterine adhesions (IUAs) and to explore their roles in the pathogenesis of IUAs.Methods The expression of ArhGAP29 and E-cadherin was detected by immunohistochemical PV 9000 two-step method.The correlation between ArhGAP29 and E-cadherin expression and clinical features was analyzed.Results (1) The immunoreactive score (IRS) of ArhGAP29 and E-cadherin in normal endometrial tissue were higher than those in IUAs endometrial tissue (P =0.017,P =0.004).(2) IRS of ArhGAP 29 and E-cadherin in moderate IUAs patients were higher than that in severe IUAs patients (P =0.020,P =0.026).In IUAs patients without amenorrhea,the IRS of ArhGAP29 and Ecadherin were higher (P =0.019,P =0.031) than that in IUAs patients with amenorrhea.(3) The decrease of ArhGAP29 expression had a significantly parallel relationship with the negative expression of E-cadherin (r =0.725,P < 0.001).Conclusion The expression of ArhGAP29 and E-cadherin decreases in endometrial tissue of IUAs patients,which is related with degree of IUAs severity.ArhGAP29 and E-cadherin may participate in the IUAs formation.
4.The treatment and prognosis of patients with phyllodes tumor of the breast

ObjectiveTo study the treatment and prognosis of the breast phyllodes tumors.MethodsKG1Clinical data, and the results of follow-up in 203 cases of breast phyllodes tumors were analyzed using Logistic analysis and Cox regression in SPSS statistic software.ResultWT5”BZLocal recurrence and tumor mortality were in direct proportion to the pathologic grade of the tumor. There was a direct proportion between local recurrence and tumor infiltration; and between tumor caused-mortality and tumor mitosis and necrosis.Mitotic activity and tumor necrosis were the independent prognostic factors. After surgical resection 21 1% of patients with benign tumor, 45 2% of patients with borderline tumor, 64 3% of patients with malignant tumor suffered recurrence. The 5-year survival rate was 100% in patients with benign phyllodes tumor, 92 0% with borderline tumor, and 33 3% with malignance.KG2Conclusion Local excision is not the appropriate treatment for phyllodes tumor. Patients with benign or borderline phyllodes tumor should undergo wide local excision including a margin of uninvolved tissue, and that with local recurrent borderline and malignant phyllodes tumor receive mastectomy.
5.Risk characteristics of hypertension in high-risk population: an analysis based on the surveillance data of chronic diseases in Shanghai

Objective:To understand the epidemiological characteristics of high-risk population of hypertension in Shanghai, and to provide references for health management and targeted lifestyle intervention of the high-risk population in community. Methods:The data from the 2013 Shanghai Non-communicable and Chronic Diseases Surveillance were used. The subjects who met the criteria of high-risk population of hypertension were included, and 11 753 subjects were finally identified, to analyze the distribution and level of risk characteristics. Result:Among six high-risk characteristics of hypertension, the detection rate of high salt in diet was the highest (55.3%), followed by overweight/obesity (44.0%), family history of hypertension (41.6%), and blood pressure level of (130-139)/(85-89) mmHg (31.7%). The detection rate of excessive drinking was the lowest (5.0%). The percentage of population with one or two high-risk characteristics was higher than that with three or more high-risk characteristics (76.9%
6.Evaluation on the application of community hypertension screening model by automated blood pressure measurement

ObjectiveTo evaluate the effect of standardized blood pressure measurement in consulting room (SBPM) model on blood pressure screening of non-hypertensive patients in community. MethodsFour communities were randomly selected from Fengxian District of Shanghai, and non-hypertensive patients in the communities were included for screening. Based on the communities, participants were further classified into the intervention group and control group. A one-year intervention study was conducted from January 1, 2021 to December 31, 2021. The intervention group received the intervention measures of standardized measurement, and the control group remained the routine measurement. The distribution of blood pressure values and last digit of the values between the intervention group and control group were tested using Chi-square test and normality test. Then changes in abnormal blood pressure rate before and after the intervention were determined by double difference method. Statistical analysis was performed using SPSS 20.0. ResultsA total of 15 368 participants were included in the intervention group, and 19 811 participants in the control group. After the intervention, range of the last digit of blood pressure values in the intervention group was 9.55%‒10.41%, of which that of systolic and diastolic blood pressure were equally distributed (P=0.932 and 0.871, respectively). The range of the last digit in the control group was 1.31%‒42.58%, of which that of systolic and diastolic blood pressure showed unequal distribution (P<0.001). Through one-year standardized measurement intervention, the abnormal rate of blood pressure in the intervention group was 26.29%, which was 7.61 times as high as that in the control group (OR=7.55, 95%CI: 6.75‒8.57, P<0.001). ConclusionStandardized blood pressure measurement in consulting room is suitable for the screening of blood pressure measurement in community, which has higher data quality than that of routine measurement.
7.Effect of Compoud Qingqin Liquids on Renal Function of Uric Acid Nephropathy Rats

Objective To observe the effect of Compoud Qingqin Liquids on renal function of rat model of uric acid nephropathy, and to discuss its protection of renal function. Methods The rat model was induced by gavaging adenine and feeding yeast. SD rats were randomly divided into control group, model group, positive group, and high-, medium-, low-dose groups of Chinese medicine. Blank control group and model group were daily gavaged with distilled water, positive control group was daily gavaged with allopurinol by 9.33 mg/kg, and high-, medium-, low-dose group of Chinese medicine was daily gavaged with Compound Qinggin Liguids by 3.77, 1.89, 0.09 g/(kg·d) respectively for 6 weeks. General condition of rats were observed, renal pathological changes were observed with light and electron microscope. Urine protein concentration, blood uric acid, blood urea nitrogen, creatinine and kidney weight index were respectively tested before and after treatment. Results There were no significant differences in eating, drinking and body weight between before and after modeling. Compoud Qingqin Liquids can obviously decrease the concentration of urine protein, blood uric acid, serum creatinine, blood urea nitrogen, and kidney weight index (P<0.05) of rats with uric acid nephropathy. Renal tubular epithelial cells atrophy and renal interstitial fibrosis of high-dose group of Chinese medicine were not evident. Conclusion Compoud Qingqin Liquids can protect the rats renal function against uric acid renal injury.

9.Application effect of Precede-Proceed model nursing in postoperative anti-osteoporosis treatment of patients with osteoporotic thoracolumbar fracture

Objective:To investigate the application effect of Precede-Proceed model nursing in postoperative anti-osteoporosis treatment of patients with osteoporotic thoracolumbar fracture (OTLF).Methods:A retrospective cohort study was conducted to analyze the clinical data of 168 patients with OTLF admitted to the Second Affiliated Hospital of Soochow University from September 2021 to June 2022, including 32 males and 136 females, aged 56-81 years [(72.0±6.6)years]. The fractured segments were T 1-T 10 in 29 patients, T 11-L 2 in 114 and L 3-L 5 in 25, all of whom were treated with percutaneous kyphoplasty (PKP). The 86 patients admitted from September 2021 to January 2022 were treated with conventional care (conventional nursing group) and the 82 patients admitted from February to June 2022 with Precede-Proceed model care (Precede-Proceed model nursing group). The compliance rates of anti-osteoporosis treatment at 1 and 6 months postoperatively and at the last follow-up were compared between the two groups, mainly including standardized medication, balanced diet, exercise and regular return visit. Chinese Osteoporosis Quality of Life (COQOL) questionnaire, including pain degree, physical function, social interaction ability, psychological and mental function, and total score were assessed before, at 6 months after surgery and at the last follow-up. Bone mineral density was measured preoperatively and at the last follow-up. Recurrence rate was recorded at the last follow-up. Results:All the patients were followed up for 12-16 months [(13.2±1.0)months]. There were 3 patients in the conventional nursing group and 2 in the Precede-Proceed model nursing group who were lost to follow-up. The compliance rates of standardized medication and regular return visit at 1 month after surgery were 97.5% (78/80) and 98.8% (79/80) in the Precede-Proceed model nursing group, which had no significant difference compared with 90.3% (75/83) and 96.4% (80/83) in the conventional nursing group ( P>0.05). The compliance rates of balanced diet and exercise at 1 month after surgery were 92.5% (74/80) and 91.3% (73/80) in the Precede-Proceed model nursing group, which were higher than those in the conventional nursing group [78.3% (65/83) and 73.5% (61/83)] ( P<0.05 or 0.01). The compliance rates of standardized medication, balanced diet, exercise and regular return visit at 6 months after surgery were 86.3% (69/80), 83.8% (67/80), 82.5% (66/80) and 90% (72/80) in the Precede-Proceed model nursing group, which were higher than those in the conventional nursing group [57.8% (48/83), 60.2% (50/83), 38.6% (32/83) and 37.3% (31/83)] ( P<0.01). At the last follow-up, the compliance rates of all the aspects in the Precede-Proceed model nursing group were 80% (64/80), 75% (60/80), 70% (56/80) and 82.5% (66/80), which were all higher than those of the conventional nursing group [36.1% (30/83), 54.2% (45/83), 26.5% (22/83) and 27.7% (23/83)] ( P<0.01). There were no statistical differences in COQOL scores including pain degree, physical function, social interaction ability, psychological and mental function and total scores between the two groups before surgery ( P>0.05). The pain degree, social interaction ability, psychological and mental function and total scores of COQOL in the Precede-Proceed model nursing group were lower than those in conventional nursing group at 6 months after surgery and at the last follow-up ( P<0.05 or 0.01). There was no statistical difference in the physical function of COQOL scores at 6 months after surgery ( P>0.05). The physical function of COQOL scores in the Precede-Proceed model nursing group were significantly lower than that of the conventional nursing group at the last follow-up ( P<0.05). There was no statistical difference in preoperative bone mineral density between the two groups ( P>0.05). Bone mineral density in the Precede-Proceed model nursing group was (-2.7±0.9)SD at the last follow-up, which was higher than that in the conventional nursing group [(-3.1±0.9)SD] ( P<0.05). At the last follow-up, bone mineral density of the Precede-Proceed model nursing group was significantly improved compared with that before surgery ( P<0.01), while there was no significant difference in the conventional nursing group ( P>0.05). The incidence of refracture in the Precede-Proceed model nursing group was 3.8% (3/80), which was lower than that in conventional nursing group [13.3% (11/83)] ( P<0.05). Conclusion:Compared with the conventional nursing mode, the Precede-Proceed model nursing can significantly improve the compliance rate of OTLF patients with postoperative anti osteoporosis treatment, improve their quality of life and bone quality, and reduce the incidence of refracture.
10.Risk characteristics of chronic non-communicable diseases in high-risk groups in Shanghai

Objective:To understand the risk characteristics of high-risk population of chronic disease in Shanghai, and provide theoretical basis for screening management and health intervention. Methods:The data from the 2013 Shanghai Non-communicable and Chronic Diseases Surveillance was used for analysis. People (aged 35 and over) who had no diagnosis of hypertension, diabetes and dyslipidemia but met the criteria for high-risk groups of chronic diseases were included in the study. Finally, 3 682 participants were included. Results:In this high-risk population, 62.90% of the subjects has total serum cholesterol level of 5.2-6.2 mmol/L, 35.88% had blood pressure level of (130-139)/(85-89) mmHg, 30.50% had abdominal obesity, 27.68% were current smokers, and 8.75% had fasting blood glucose level of 6.1-7.0 mmol/L. Among the different risk characteristics, the detection rate of smoking risk in men was higher than that in women (64.51%
