1.Clinical effect of small incision phacoemulsification combined with trabeculectomy in tunnel in the treatment of glaucoma with cataract

Objective To explore the clinical effect of small incision phacoemulsification combined with trabeculectomy in tunnel in the treatment of glaucoma with cataract.Methods Thirty-six cases(38 eyes)who performed 2.8 mm small incision phacoemulsification combined with trabeculectomy in tunnel were enrolled in this study.The visual acuity,intraocular pressure(IOP),astigmatism and complications were assessed before and after operation.Results The visual acuity ≥ 0.4 was in 29 eyes(76.32%)postoperatively.After followed up 3-6 months,the IOP of all the patients had been effectively controlled.The IOP at 1 week[(12.24 ± 3.59)mm Hg(1 mm Hg =0.133 kPa)]and 3-6 months[(15.53 ± 5.26)mm Hg]after operation was significantly lower compared with before operation[(36.72 ± 4.34)mm Hg](P < 0.05).There was no significant difference in astigmatism among before operation,1 month and 3-6 months after operation(P > 0.05).Conclusion Small incision phacoemulsification combined with trabeculectomy in tunnel is an effective surgery for glaucoma with cataract,which can improve visual acuity and IOP.
2.Clinical efficacy of lens trabeculectomy combined with phacoemulsification in the treatment of angle-closure glaucoma with cataract

ObjectiveTo explore the clinical efficacy of trabeculectomy combined with phacoemulsification in the treatment of angle-closure glaucoma with cataract.Methods56 cases of angle-closure glaucoma with cataract were selected in department of ophthalmology and were received trabeculectomy combined with phacoemulsification.Preoperative and postoperative visual acuity,intraocular pressure,anterior chamber angle,anterior chamber depth,filtering bleb and complications were observed.ResultsSix months after operation,53 (94.64%) were improved in visual acuity.There was significant difference between preoperative and postoperative visual acuity( Z =-5.074,P <0.05).IOP was( 15.26 ± 3.37 )mm Hg 6 months after operation,which was significant lower than(34.60 ± 3.85 ) mm Hg before operation( t =28.29,P <0.05 ).The anterior chamber depth was( 3.27 ± 0.64 ) mm 6 months after operation,which was significant larger than( 1.58 ± 0.39 )mm before operation (t =16.90,P < 0.05 ).All patients had bleb formation with no serious complications.ConclusionTrabeculectomy combined with phacoemulsification was effective in treating angle-closure glaucoma and cataract.
3.Effect of Empirically Applying ?-Lactam Antibiotics for Treatment of Hospital-acquired Pneumonia on Pathogenic Bacteria

4.Protective effects of deproteinized extract of calf blood on hypoxia-reoxygenation-induced injury in cultured myocardial cells

AIM : To explore the effects of DECB (deproteinized extract of calf blood, DECB) on hypoxia-reoxygenation-induced injury in cultured cardiac myocytes. METHODS : By using cultured myocardial cells as models, thenumber of death cells, beating frequency and the content of LDH were observed after hypoxia-reoxygenation-induced injury. RESULTS : 0.1 and 0.3 g?L -1 of DECB decreased the number of death cells and the content of lactate dehydrogenase (LDH), and restored the cellular beating frequency in the supernatant. CONCLUSION : DECB can protect the cultured myocardial cells against hypoxia-reoxygenation-induced injury.
5.Peri-operative Examination for Traumatic Cyclodialysis with Ultrasonic Biomicroscopy

Objective To explore the use of Ultrasonic Biomicroscopy(UBM)for peri-operative examination of traumatic cyclodialysis.MethodsUBM was used in 33 eyes of 33 cases who were diagnosed as traumatic cyclodialysis.The morphologic characters were observed peri-operatively.ResultsCiliary detachment of 360° was verified in the 33 eyes with cyclodialysis of more than 2 clock hours.1 month after cyclopexy,the space of ciliary detachment disappeared or diminished and all reattached 3 months after operation.ConclusionUBM is safe and effective for checking traumatic cyclodialysis.
6.Ultrasound-guided aspiration biopsy for differential diagnosis of cervical mass

Objective To assess the clinical application value of ultrasound-guided aspiration biopsy in the diagnosis and differential diagnosis of cervical tumors.Methods Seventy-two patients with cervical tumors underwent ultrasonography and ultrasound-guided aspiration biopsy;and the obtained samples were examined histopathologically.Results The successful rate of aspiration biopsy was 98.15%,and the accurate rate of histopathological diagnosis was 97.22%.No complications occurred.Conclusion Being a minimally invasive,rapid,safe and simple method in diagnosis and differential diagnosis of cervical tumor,ultrasound-guided aspiration biopsy has important clinical value and can be used as the first choice of qualitative diagnosis of cervical tumor.
7.A new naphthaquinone derivative from Pyrola calliantha H. Andres.

To investigate the chemical constituents of hemostatic extract of Pyrola calliantha H. Andres, the extract was subjected to chromatographic separation and purification. Along with some known compounds, a new naphthaquinone derivative was isolated and identified as 2-(1, 4-dihydro-2, 6-dimethyl-1, 4-dioxo-3-naphthalenyl)-3, 4, 5-trihydroxylbenzoic acid by physicochemical and spectroscopic analysis.
8.Effects of Hippophae rhamnoides L. subsp. chinensis Rousi polysaccharide on alpha-glucosidase enzyme activity and level of blood glucose.

The enzyme-inhibitor model and the sugar tolerance mouse model were used to evaluate the relationship between the inhibition rate of enzyme activity and concentration of Hippophae rhamnoides L. subsp. chinensis Rousi polysaccharide (HRP). The inhibitory patterns of enzyme and dose-dependent effects of HRP's effect on blood glucose using acarbose tablets as control were also examined. The mechanism underlying hypoglycemic effects of HRP was discussed. The results showed: in the enzyme-inhibitor model, the inhibitory activity of different concentrations of HRP (9.80, 19.60, 39.20, 78.40, 156.80 and 312.50 mg x L(-1)) on alpha-glucosaminidase (AG) inhibitory activity were 6.62%, 18.02%, 33.26%, 48.23%, 62.11%, 76.31%, 90.12%, IC50 was 31.59 mg x L(-1). The inhibitory rate of 25.00 x 10(3) mg x L(-1) acarbose tablets was only 64.87%, and IC50 was 10.75 x 10(3) mg x L(-1). In the sugar tolerance mouse model, different doses of HRP (240, 480, 960 mg x kg(-1)) tended to decrease levels of blood glucose compared with control group (acarbose tablets 375 mg x kg(-1)) at 15, 30, 60 and 120 min. It's further confirmed that HRP is a kind of competitive inhibitor of AG activity. Its inhibition rate increases with the increase of concentration in normal mice, and it subsequently improves the sugar tolerance showing the effect of reducing blood sugar.
9.Distribution and Resistance of AmpC Enzyme Producing Gram-negative Bacilli

OBJECTIVE To study the isolation,distributive characteristics and drug resistance of AmpC enzyme producing Gram-negative bacilli in nosocomial infection of two years and provide the evidence for treatment. METHODS A total of 528 strains of Gram-negative bacilli collected from daily specimens were identified with Bio-Fosun-Ⅰ,and AmpC enzyme was screened by cefoxitin disk and then corroborated by EDTA disk method. All data were analyzed statistically. RESULTS Among 528 strains collected,136 (25.75%) were AmpC enzyme producing strains,the respective percentage of Pseudomonas aerugionsa,Echerichia coli,Enterobacter cloacae,Klebsiella pneumoniae and Citrobacter was 32.35%,28.67%,18.38%,8.09% and 5.15%,respectively. Most strains (38.9%) were detected in ICU. The common infection sites were lungs. The resistance rate of AmpC enzyme producing strains to the first,second and third-generations cephalosporins was 71.3-99.5%. The susceptive rate of AmpC enzyme producing strains to imipenem,cefepine,amikacin and piperacillin/tazobactant were low. CONCLUSIONS For effective supervision and control of AmpC enzyme producing Gram-negative bacilli in nosocomial infection,detection of AmpC enzyme shoud be paid much attention by clinical microbiology laboratory.
10.Pathogens in Hospital-acquired Pneumonia:Analysis of Stages and Clinical Study

OBJECTIVE To sum up the distribution and drug-resistance of pathogens in hospital-acquired pneumonia(HAP) at each stage,and then to offer references for treatments of HAP.METHODS To retrospectively analyze 46 patients whose course was over 30 day and who were in hospital from Jan 2000 to Oct 2005,and to divide course into prophase,metaphase and anaphase,and then to clinically study the drug-resistance and distribution of pathogens in each phase.RESULTS In prophase the most pathogens were Gram-negative bacilli(accounted for 87.5%),in metaphase the Gram-positive cocci were manifold distinctly;and in metaphase and anaphase drug-resistance of Gram-negative bacilli increased distinctly;and the fungi were not detected in prophase,but were positive in metaphase and anaphase.CONCLUSIONS In prophase application of third generation cephalosporin which is more sensitive to Gram-negative bacilli as an empirical antibacterial therapy is right;based on the drug-resistance and distribution of pathogens,the antibacterial drugs and treatment must be readjusted.
