Objective To investigate clinical characteristics of aerobic vaginitis AV) and its mixed infections for diagnosis efficiently. Methods From April 2008 to December 2008, 516 patients with vaginitis treated in Tianjin Medical University General Hospital were enrolled in this study. AV, bacterial vaginosis (BV) , vulvovaginal candidiasis (VVC), trichemonal vaginitis (TV) , and cytolytic vaginosis (CV) were diagnosed based on symptoms, sign and vaginal discharge examination. Results Among 516cases, AV cases were found in 14. 7% (76/516), and AV was common vaginal infection. AV mixed infections was diagnosed in 58% (44/76), including mixed with BV (45%, 20/44), mixed with VVC (30% , 13/44) , and mixed with TV (25% , 11/44). Those common symptom of AV were yellow vaginal discharge (63%, 20/32), more vaginal discharge (44%, 14/32). Vaginal pH value was usually more than 4. 5 (84% , 27/32). Vaginal cleanliness mainly was grade Ⅲ - V (88% , 28/32). Six cases with enterococcus faecium and 4 cases with streptococci were frequently isolated. The symptom and sign of mixed AV infection was atypical. Conclusions Aerobic vaginitis is a common lower vaginal infection and easily mixed with other pathogens, especially with BV, VVC or TV. When patients were diagnosed with AV or other vaginal infection, it should be mentioned whether those patients have mixed vaginal infection or AV.

Obesity,hypertension,and diabetes mellitus are main risk factors of endometrial cancer.The basis of metabolic syndrome is insulin resistance and hyperinsulinemia. Insulin resistance may contribute to an increased risk for endometrial cancer according to worldwide researches.

Background and objective Endometrial carcinoma is the most common gynecologic malignancy in the world. Although the insulin-resistant state or hyperinsulinemia was recently suggested as a potent risk factor for endometrial carcinogenesis and progression, there is only limited supporting evidence and the mechanism is unclear. In this study, we explored the roles of phosphatidylinositol 3-k/nase (PI3K)/Akt signaling pathway in the response of a human endometrial cancer cell line, Ishikawa3-H-12 cells, to insulin.Methods The Ishikawa 3-H-12 cells were serum-starved and then stimulated by insulin at various concentrations and for different time periods. To identify the insnlin-mediated signal pathway in the cells, LY294002, a selective inhibitor of PI3K, was used. The proliferation and the apoptotic rates were determined with methyl thiazolyl tetrazolium (MTT) and flow cytometric assays, respectively.Results The insulin receptor positive Ishikawa 3-H-12 cells had enhanced proliferation upon insulin stimulation in a rinse-and time-dependent manner. The growth promoting effect of insulin was blocked when the cells were pre-incubated with LY294002 for 60 rains.Insulin was able to protect the cells from serum-starvation-induced apoptosis in a concentration-dependent manner, while the anti-apoptotic effects of insulin was reversed by adding LY294002. Treatment with insulin at 1 μM for 15 rain resulted in an increased level of activated Akt The insulin-induced Akt activation was inhibited by LY294002 in a dose-dependent manner.Conclusion Insulin activates PI3K/Akt signaling pathway and is a mitogenic and anti-apoptotic agent for Ishikewa 3-H-12 endometrial cancer cells.

Lynch syndrome.(LS) is an autosomal dominant condition caused by a mutation in the mismatch repair genes (MMR).Endometrial cancer (EC) is the most common extra-colonic cancers of LS Ⅱ type.Among the female members of LS family with the MMR gene mutation,EC has an overall lifetime risk more than 35％.LS-associated EC is more likely present as the clinical pathological characteristics of early age of onset,endometrioid adenocarcinoma,lower grade,lower uterine segment involvement,and better prognosis.LS patients with family history and early age of onset should be performed with a combined screening and genetic testing,while high-risk patients should be taken clinical monitoring methods as endometrial biopsy.Oral contraceptives mav be reasonable chemopreventive agents in the patients with LS.Although prophylactic hysterectomy with bilateral salpingo-oophorectomy is an effective strategy to prevent EC in women with LS,preoperative counseling should address the trade-offs between the reduction in the risk of cancer,and the risks and side effects of surgery.The patients need hormone replacement therapy after surgery.

Objective To investigate how to optimize the protocol of embryo cryopreservation to improve the success of frozen-thawed embryo transfer (FET), reduce multiple pregnancy rate and increase the cumulative pregnancy rate from one oocyte retrieval process. Methods The clinical data of 1 166 FET cycles were retrospectively analyzed and separated into different groups:445 for vitrification and 721 for slow-freezing. The vitrification group was divided into single embryo (28 cy?cles), double embryos (71 cycles) and triple embryos (346 cycles). 0-1 optimal embryo was called O0-1 group (235 cycles), 2 optimal embryos were called O2 group (80 cycles), 3 optimal embryos were called O3 group (130 cycles). The difference preg?nancy outcomes (implantation rate, clinical pregnancy, abortion rate and live-birth rate) were compared between groups. Results (1) There were significantly higher embryo survival rate(98.3%vs 73.1%), embryo recovery rate without damaging (83.3%vs 62.1%), implantation rate(36.8%vs 29.9%), clinical pregnancy(57.1%vs 44.0%) and live-birth rate(47.9%vs 34.5%) in vitrification group than those of slow freezing group(P<0.05). (2) There were significantly higher clinical pregnan?cy rate(62.7%vs 39.4%vs 32.1%), multiple pregnancy rate(37.6%vs 10.7%vs 0%)and live-birth rate(52.6%vs 31.0%vs 21.4%)in triple embryos group than those of the single and double embryos groups (P<0.05). (3) There were significantly higher implantation rate(33.0%vs 27.1%), clinical pregnancy(65.2%vs 48.1%), multiple pregnancy rate(38.0%vs 20.4%) and live-birth rate(56.2%vs 39.2%) in O2-3 group than those of O0-1 group(P<0.05). (4) There were significantly higher im?plantation rate(36.8%vs 26.3%) and multiple pregnancy rate (46.7%vs 21.3%) in O3 group than those of O2 group (P<0.05). There were no significant differences in clinical pregnancy(69.2%vs 58.8%), abortion rate(15.6%vs 10.6%), ectopic preg?nancy rate(0 vs 0), live-birth rate(58.5%vs 52.5%) and preterm birth rate(26.3%vs 14.3%) between two groups (P>0.05). Conclusion Vitrification technology can improve the clinical pregnancy and live-birth rate, and decrease multiple preg?nancy rate. Two optimal embryos in one tube are supposed to be the preferred method for embryo cryopreservation.

Objective To investigate the mRNA,protein expression and tyrosine phosphorylation of insulin receptor substrate.1(IRS-1)in endometrial carcinoma.Methods Sixty-three endometrial carcinoma(EC)patients,21 endometrial atypical hyperplasia(AHE)patients and 22 normal control(NE) entered this smdy.Their clinical information were colleeted Fasting serum C-peptide concentration was measured.Expression of IRS-1 in endometrium was examined by RT-PCR and western blol Immunoprecipitation was used to measure the tyrosine phosphorylation of IRS-1.Results C-peptide 0.007].There were no significant differences in IRS-l mRNA and protein expression among the three grouDs.Tyrosine phosphorylation of IRS-1 in EC group[(62±36)%]was higher than that in AHE and NE groups[(53 4-34)%and(35 4-33)%;P=0.048,0.002].IRS-1 activation in AHE group was also higher than nornlal control(P=0.045).IRs-1 activation in endometrioid carcinoma[(69 4-33)%]was higher than that in other histological types[(34±31)%;t=2.300,P=0.025].IRs-l tyrosine phosphorylation was significantly higher in patients with advanced stage,high grade,deep myometrial invasion and pelvic lymph node metastasis.IRS-l activation in endometrium was positively correlated with fasting sertlm C-peptide concentration(r=0.491,P=0.001).Conclusions There is excessive activation of IRS-1 in endometrial carcinoma and atypical hyperplasia.Activation of IRS-I in endometrial carcinoma is related with poor clinical-pathologic fleatures and may be a prognostic predictor for this tumor.Over-activationof IRS-1 may be an intermediate event linking the hyperinsulinemia and endometrial carcinoma.

Objective To study clinicopathological characteristics of hereditary nonpolyposis colorectal carcinoma(HNPCC)-associated endometrial carcinoma(EC).Methods Totally 421 EC patients admitted to General Hospital of Tianjin Medical University from 1981 to 2006 were divided into three groups:group A:sporadic EC;group B:familial aggregation of EC;group C:HNPCC-associated EC.Results HNPCC-associated EC accounted for 6.4%(27/421).Mean age at time of diagnosis was 49.7years in group C,earlier than 56.3 years in group A(P=0.004)and 55.2 years in group B(P=0.035).There were 33.3%(9/27)patients with multiple primary carcinomas in group C.It was higher than 14.3%(9/63)in group B and 5.1%(17/331)in group A respectively(P=0.038,P＜0.01).There was no difference among the three groups in histological type or menopausal status(P＞0.05).The numbers of patients with low grade EC in group C(70.4%,19/27)and group B(61.3%,38/62)were more than that in Group A(P=0.013,P=0.023).Prognosis for group C was better than that in group A(P=0.021),and 5-and 10-year survival rate in groups A,B and C was 80%,70%;88%,85%;and 96%,96%respectively.Conclusions Mean age at time of diagnosis is earlier in HNPCC-associated EC than that in sporadic EC;patients with HNPCC-associated EC are more frequently complicated with multiple primary carcinomas and of low grade;prognosis is better.
