1.Anterior approach operation of thoracolumbar fractures through limited incision

[Objective]To investigate the surgical method and clinical effects of the anterior approach operation in the treatment of thoraco-lumbar vertebral fractures through limited incision.[Method]Thirty-four cases of bursting fracture of thoracolumbar vertebral body were treated with the operation of anterior decompression,bone grafting and internal fixation by the anterior retropleuro-peritoneum approach.The MRI combined with clinical examination,X-ray,CT results were taken as selection criteria of treatment.[Result]Thirty-four cases were treated with this operative technich.Hemorrhage capacity is about 300-800ml with average 600ml.The operation time was 2-2.5 hours.All patients were followed-up with an average period of 18 months(ranged,6-24months).No aggravation of nerve injury appeared in 34 cases.X-ray manifested good fusion,no broken nails and no recurrence of kyphosis was found in all cases.[Conclusion]Anterior approach operation through limited incision is an effective way for severe thoracolumbar fractures. It has significantly low complication and allow early recovery of function.
2.Risk Factors and Outcome of Stroke-associated Pneumonia in Emergency Ward

Objective To explore the risk factors and outcome of stroke-associated pneumonia (SAP) in emergency ward. Methods The baseline characteristics and laboratory data of 306 patients with stroke in emergency ward of our hospital from April, 2014 to December, 2016 were reviewed. Patients were allocated into SAP group (n=120) and non-SAP group (n=186). The multivariable Logistic regression was applied to investigate risk factors for the progression and death of SAP. Results Totally, 120 (39.2%) patients developed SAP, in which 34 (28.3%) cases died. There were significant differences in age, the scores of Glasgow Coma Score (GCS), Sequential Organ Failure As-sessment (SOFA) and National Institute of Health Stroke Scale (NIHSS), and mechanical ventilation, dysphagia, antiacid and tube feeding (P<0.05). The multivariable Logistic regression analysis revealed that age over 65 years, hypertension, GCS<9, NIHSS, dysphagia and anti-acid were the risk factors for the development of SAP;the SOFA, NIHSS>15 and mechanical ventilation were the risk factors for 30-day mortality in SAP group. Conclusion SAP is a severe complication of stroke, it is associated with a variety of risk factors that affect its out-come in emergency ward.
3.THE ANTIATHEROSCLEROTIC EFFECT OF THE EXTRACT FROM GRAPE SEED IN C57BL/6J MICE

Objective: To investigate the antiatherosclerotic effect of the extract from grape seed in C57BL/6J mice. Method: The mice were fed with atherosclerotic diet for 20 w and then with basic diet or different dosage of grape seed extract for 4 w. The blood samples were examined for blood lipid. The sections of aorta were examined and scored microscopically. Results: After fed with atherogenic diet for 20 w, the aortic score was 7.2?1.3 and TG, Chol and MDA in serum were significantly increased. After fed with the extract of grape seed for 4 w, the aortic score was reduced to 6.0?1.2, and TG, TC, and MDA levels also decreased. Conclusion: The extract of grape seed has antiatherosclerotic effect.
4.Determination of sabinene in ledum terpenes from Ledum palustry L by gas chromatography

AIM: To establish a gas chromatography method for the determination of sabinene in ledum terpenes from Ledum palustry L. METHODS: 5% SE 30 was adopted as the stationary phase, the temperature of column was set at 65 ?C and FID detector with termperature at 200 ?C . RESULTS: Sabinene in the preparation was gained satisfactory separation and assay under the chromatographic condition. The calibration curve of sabinene was good linear in the range of 0.06 0.60 mg?mL -1 . with the corelation coefficient r=0.9998(n=6) . The average recovery of sabinene was 100.3%, RSD=1.4%, (n=5) . CONCLUSION: The method is simple, highly repeatable and accurate, and it can be used to control the quality of the preparation.
5.The study of Yinaotongluo capsule preventing the microcirculation dysfunction in rats

ObjectiveTo explore the effect of Yinaotongluo capsule on preventing the microcirculation dysfunction in rats.Methods40 SD rats were randomly divided into the thrombosis model group, three Yinaotongluo capsule groups (low, moderate and high dosages) and control group with 8 rats in each group. Photosensitizer was injected through vessel. Endothelium damage, cell adhesion and thrombosis were induced by photochemistry. The change of microcirculation was recorded and analyzed by microphotography technique.ResultsYinaotongluo capsule groups could prolong the time to thrombosis induced by photochemistry compared with control group (P<0.05). However, the area of thrombus to vessel significantly decreased in all Yinaotongluo capsule groups compared with control group (P<0.01). The effect of moderate dosage group of Yinaotongluo capsule was superior to Buchangnaoxintong group.ConclusionYinaotongluo capsule can improve the microcirculation of mesentery of rats, in part it can improve the systematic microcirculation of rats.

Objective To investigate the abnormal expression of microRNA (miRNA)-25 in the serum of gastric cancer patients and its clinical significance. Methods In Xijing Hospital of Digestive Diseases,Fourth Military Medical University,86 gastric cancer patients with operation and completed follow-up data,70 gastric adenoma patients and 80 healthy controls were selected as study objects.Total RNA was isolated from the serum. After the stable and sensitive miRNA-25 absolute quantity detection method established,the serum levels of miRNA-25 in gastric carcinoma patients,gastric adenoma patients and healthy controls were tested according to this method. The expression differences of miRNA-25 in the serum of patients with gastric cancer and gastric adenoma and healthy controls were analyzed with statistic analysis,and the correlation between miRNA-25 expression level and clinic pathological features of gastric cancer was also analyzed. Results The expression level of miRNA-25 in the serum of gastric cancer patients (135. 6 fmol/μg total RNA) was significantly higher than that of gastric adenoma patients (67. 7 fmol/μg total RNA) and healthy controls (62. 2 fmol/μg total RNA)(P<0. 01). The receiver operating characterisstic curve of miRNA-25 indicated that serum miRNA-25 with good specificity and sensitivity in gastric cancer diagnosis (AUC=0. 827). The serum level of miRNA-25 in gastric cancer patients with lymph node metastasis [(148. 3±10. 2) fmol/μg total RNA] or clinicopathological stage Ⅲ /Ⅳ patients [(146. 7±9.5) fmol/μg total RNA] was significantly higher than that of gastric cancer patients without lymph node metastasis [(120. 3±10. 1)fmol/μg total RNA] or clinicopathological stage Ⅰ/Ⅱpatients [(119. 4±12. 2) fmol/μg total RNA] (P<0.05). The correlation statistical analysis result indicated that there was no significant difference in survival period between serum miRNA-25 highly expressed and lowly expressed gastric cancer patients (P>0. 05).Conclusion Serum miRNA-25 testing maybe helpful in diagnosis and prognosis of gastric cancer.
8.The efficacy and safety of mismatched hematopoietic stem cell transplantation for treatment of severe aplastic anemia

Objective To study the efficacy and safety of human leukocyte antigen (HLA)mismatched hematopoietic stem cell transplantation (HSCT) on severe aplastic anemia(SAA). Methods From January 2006 to May 2010, 17 patients received mismatched HSCT. HLA antigens were 3-locimismatched in 9 patients, 2-loci-mismatched in 8. All patients received recombinant human granulocyte colony-stimulating factor (rhG-CSF) primed bone marrow cells plus peripheral blood stem cells after modified busulfan/cyclophosphamide + antithymocyte immunoglobulin (BU/CY + ATG ) conditioning regimen. Results All patients achieved full donor type engraftment. Grade Ⅲ-Ⅳ graft versus host disease (GVHD) occurred in 3 patients and extensive chronic GVHD in 1. With a median following-up time of 285(60-1670) d, 11 patients were alive, 9 of them had normal blood counts and the other 2 were blood transfusion independent. Six patients died of transplant-related complications. Conclusion Mismatched HSCT is a feasible and safe option for SAA patients without sibling identical donors.
9.Outcomes following HSCT using fludarabine replacing cyclophosphamide as a new preconditioning regimen for treatment of hematologic malignancies

Objective To evaluate the hypothesis of fludarabine replacing cyclophosphamide as a new myeloablative preconditioning for the treatment of malignant hematologic diseases in aged and/or intolerable patients receiving allogeniec stem cell transplantation (allo-HSCT). Methods Between January 2008 and November 2008,12 patients, who were intolerant to standard conditioning regimen, received allo-HSCT with HLA identical sibling (n = 9) or mismatched family donors (n = 3),including 1 case of acute lymphoblastic leukemia with Ph chromosome (Ph+ ALL) ,6 cases of acute myelogenous leukemia (AMD,3 cases of myelodysplastic syndrome-refractory anemia with excess blasts (MDS-RAEB) and 2 cases of chronic myelogenous leukemia (CML). Stem cell sources were G-CSF mobilized peripheral blood alone (n = 1) ,or with G-CSF mobilized bone marrow (n - 11), with a median of 6. 68 (4. 35 - 7. 86) × 10~8/kg MNC and 1. 50 (0. 31 - 3. 91) × 10~6/kg CD34~+ cells. Eleven patients received revised busulfan and fludarabine regimen with/without antithymocyteglobulin(ATG),and the rest one received TBI and fludarabine regimen. GVHD prophylaxis included cyclosporin A, mycophenolate mofetil and methotrexate. Results Results All patients were well tolerated to the regimen without serious regimen related toxicity. The median time of ANC≥0. 5 × 10~9/L was day 17. 5 (11 - 23), and that of BPC≥20. 0 ×10~9/L was day 14. All patients except one got donor engraftment successfully and attained CR. With a median follow-up of 418 (62-554) days, 10 of 12 patients were alive and disease-free. Conclusion Fludarabine replacing cyclophosphamide as a new preconditioning regimen is well tolerated and safe for allo-HSCT, especially in older patients or/and those with severe concurrent medical conditions.
10.Characteristics of fiver enzyme abnormalities in acute graft-versus-host disease after bone marrow transplantation

Objective To investigate the characteristics of liver enzyme between acute graft-versushost disease (aGVHD) and non-aGVHD groups after allogeneic hematopoietie stem cell transplantation (allo-HSCT). Methods The liver enzyme data of patients with or without aGVHD was analyzed. Results Among the 371 patients, 158 developed aGVHD(41.6%) with a median time of 29. 5 d. In non-aGVHD group, the median elevating times of ALT, AST, GGT, LDH were all ≤ 20 d after transplantation, which were significantly earlier than those of aGVHD group. The peak value of AST was much higher in aGVHD group, but the remaining liver enzyme showed no significant difference. In aGVHD group, the duration of AST, GGT and LDH was significantly longer than the non-occurrence of aGVHD group, but ALT and ALP showed no difference in duration. In ALT, AST, ALP and LDH elevating group, the occurrence rate of aGVHD was significantly higher, but only ALT and LDH elevation could enter logistic regression model. The sensitivity and veracity of ALT or LDH elevating only were not very good for the diagnosis of aGVHD, but if ALT and LDH beth elevated after day 24 and persisted more than 22 d, the sensitivity and veracity were better. Conclusions The change of liver enzyme is common in allo-HSCT and associates with the occurrence of aGVHD, and the liver enzyme elevating only may not be a good diagnostic index of aGVHD. If the dynamic characteristics of liver enzyme is taken into account and the effect of drug-induced liver injury could be excluded, the elevation of liver enzyme still have the diagnostic value of aGVHD.
