1.Imageologic foundation of ultraselection arterial embolism for pelvic fracture with hemorrhage

[Objective]To study the imageologic characteristic of pelvic arterial anastomosis and the method of ultraselection arterial embolism for pelvic fracture with hemorrhage.[Method]From 1999.1-2005.6,60 patients were undergone pelvic arteriography.The patient ages ranged from 21 to 52 years of age(mean,34.5),and 42 were male,18 were female.Measure the types and number of pelvic arterial anastomosis.[Result]The types and number of pelvic arterial anastomosis were observed by pelvic arteriography as follows: the lumbar artery with iliolumbal artery 100%;the iliolumbal artery with deep iliac circumflex artery 83.33%;the obturator artery with inferior epigastric artery(or external iliac artery) 36.67%;the lateral sacral artery with coccygeal artery 100%;the inferior gluteal artery with internal pudendal artery 26.67%;the inferior gluteal artery with deep femoral artery 56.67%;the superior gluteal artery with inferior gluteal artery 61.67%.[Conclusion]For pelvic fracture whith hemorrahge,the best treatment of the pelvic arterial disruption is embolization of injured arteries and their anastomosis.
2.Atlantoaxial Occult Fracture:Multi-slice Spiral CT Diagnosis

Objective To discuss the diagnostic value of multi-slice spiral CT 2D and 3D reconstruction for atlantoaxial occult fracture.Methods Multi-slice spiral CT scans were performed in 38 patients who were suspected of fracture but were not detected with radiography,and the images were post-processed with Vitrea workstation(including MPR,MIP,VR and SSD).The images were observed by two experienced radiologists.Results Of 38 cases,28 cases were detected having small fractures with spiral CT reconstruction.Conclusion Multi-slice spiral CT 2D and 3D reconstructions are of important value in diagnosing atlantoaxial occult fracture.
3.Analysis of Adverse Drug Reactions in Children Appeared Between Jan.2002 and Sept.2007 in Our City

OBJECTIVE: To explore the factors,general pattern and characteristics of adverse drug reactions(ADR) in children.METHODS: A total of 188 pediatric ADR reports collected in our city from Jan.2002 to Sept.2007 were statistically analyzed in respect of children's age and sex,route of administration,ADR severity,category of drugs involved,organs or systems involved and clinical manifestations etc.RESULTS: Of the 188 pediatric ADR cases,95(50.5%) were induced by intravenous route.A total of 112 kinds of drugs were involved in the ADR,with anti-infective agents showing the highest proportion.Digestive system,neural system,urinary system and skin etc were involved in the ADR.CONCLUSION: Clinic should attach great importance to the monitoring and reporting of pediatric ADR to reduce or avoid the occurrence of ADR.
4.High epidural blockade lowers serum free fatty acids level in patients with acute myocardial infarction

Objective: To explore whether high epidural blockade(HEB) can lower the elevated serum free fatty acids(FFA) level in the patients with acute myocardial infarction(AMI). Method:In 7 patients with AMI,HEB at T 15 was performed by injecting 5 to 7ml of 0.5%-0.75% lidocaine every 2 to 4 hours for 7 successive days;while the other 7 patients with AMI didn't use HEB as control. only morphine or dolantine was used for relieving angina cordis. Result: The elevated FFA level returned to normal range in 12 hours following HEB in the study group, but in one week in the control group. Conclusion:HEB(T1-5) can lower the elevated FFA level in AMI patients to normal within12 hours.
5.Sudy about complications of iliac crest bone graft harvesting

[Objective]To discuss the causes and preventive measures about complications of iliac crest bone graft harvesting.[Method]From Jan.1990 to Jan.2005,828 patients with iliac crest bone graft harvesting were reviewed retrospectively,their complications were taken statistical analysis.[Result]Fifteen patients were found with superficial infections;10 deep infections;20 patients with superficial seromas and 10 patients with minor hematomas;4 deep hematomas;30 nerve injuries;3 vascular injuries,all 3 cases occurred secondary to harvesting of posterior iliac crest bone grafts;1 donor defect hernias,and 2 iliac wing fractures.[Conclusion]The causes of complications are incorrectly harvesting and rude operation.The preventive measures are that operators should know the anatomic characteristic of ilium and microinvasion.
6.Surgical treatment of multiple thoracolumbar spinal fractures by pedicle screw/rod instrumentation system

[Objective]To evaluate the clinical value of surgical treatment of multiple thoracolumbar spinal fractures by pedicle screw/rod Instrumentation system.[Method]Ninteen cases of male 14 and femal 5 of multiple injuries involving thoracic and lumbar spine were surgically treated through anterior approach by open reduction,selective vertebral canal decompression,internal fixation with pedicle screw/Rod spine instrumentation,and followed by anterior lateral bone graft fusion according to complex classification system 19 cases,Ba.m1T_(11) 1 cases,Ba.m.p2T_(11) 2 cases,Ba.m1T_(12) 1 cases,Ba.m.p2T_(12) 1cases,Ba.m.p3T_(12) 1cases,Ba.m1_L 4 cases,Ba.m.p2L_1 3cases,Fa.m.p3(T_(12),L_1) 2cases,Sa.m.p3L_2 1cases,Ba.m.p1L_2 1 cases.[Result]All cases were followed up form 6 to 15 months averageing 10.5 months.No implant failure and late kyphosis deformity was observed.The average vertebra height was improved from preoperative 48.14% to postoperative 92.14%.The postoperative neurological function in two cases of complete spina lcord injuries and 11 incomplete spina lcord injuries were improved according to JOA criterion.Astable bone fusion according to radiological criteria was achieved in call cases postoperatively,and progressive compression did not occur.[Conclusion]Pedicle screw/rod instrumentation system can achieve effective reduction for the fractured vertebrae and simultaneously restore spine stability on the basis of adequate vertebral canal decompression.Posterior spine instrumentation is an ideal choice for surgical treatment of thoracolumbar unstable fractures with or without spinal cord dysfunction.
7.Hallux valgus deformity treated with minimally invasive the first metatarsal distal osteotomy

Objective To discuss the clinical effect of hallux valgus deformity treated with minimally invasive the first metatarsal distal osteotomy. Methods The data of 375 cases (626 feet) treated with minimally invasive the first metatarsal distal osteotomy was retrospectively analyzed. Hallux valgus angle was21°-65°(mean37.61°±9.25°), intermetatarsal (IM) anglewas7° -21° (mean 13.03°± 3.73°). The preoperative American Orthopaedic Foot and Ankle Society (AOFAS) score was (46.5 ± 9.4) pints. Results The patients were followed up 12-30 months [mean (18.5 ± 6.8) months]postoperation, hallux valgus angle was 7.18°±4.55°,intennetatarsal angle was 5.07°± 1.70°,with a mean angle decreasing 30.54° and 12.33°.The AOFAS score was (84.8±7.6) points (P <0.01). Conclusion According to the hallux valgus angle,intermetatarsal angle and the length of the first metatarsal,decide the place and method of osteotomy, the hallux valgus can be corrected in three-dimensional and get an excellent effect.

9.Imaging characteristics of computed tomography examination in colon cancer combined with obstruction

Objective To summarize the imaging characteristics and clinical diagnostic value of computed tomography (CT) examination in colon cancer combined with obstruction.Methods The clinical data of 33 patients with colon cancer combined with obstruction who were admitted to the Wuxi No.2 People's Hospital from June 2012 to March 2015 were retrospectively analyzed.All the patients received abdomianl plain scan and dualphase enhanced scan of CT,and the imaging data were summarized.Results Of 33 patients with colon cancer combined with obstruction,obstruction was located at the ascending colon in 9 patients,hepatic flexure of the colon in 2 patients,colon transverse in 7 patients,splenic flexure of the colon in 3 patients,descending colon in 3 patients,sigmoid colon in 7 patients and proctosigmoid in 2 patients.Thirty-three patients had irregular and thickened colonic wall at the obstructive segment and irregular constriction of colonic lumen,with a mean coloric wall thickness of 1.9 cm (range,0.6-3.2 cm).The length of constriction was 2.0-3.0 cm in 3 patients,3.1-4.0 cm in 3 patients,4.1-5.0 cm in 11 patients,5.1-8.0 cm in 12 patients and more than 8.0 cm in 4 patients.The mean length,diameter and mean diameter of the constriction of 33 patients were 5.8 cm,0-0.5 cm and 0.2 cm.The performance at the junction of the normal wall and the stenosis was scuff syndrome in 7 patients and shoulder symptoms in 5 patients.Colonic dilatation above the obstructive segment was detected in 26 patients with multiple gas-fluid levels and in 7 patients with mainly gas.Twenty-nine patients had colonic dilatation at the proximal end of obstruction with the diameter of right colonic dilatation of 6.0 cm,diameter of left colonic dilatation of 4.0 cm and colonic collapse at the distal end of obstruction.Thirty-three patients had different grades of cecal dilatation with a mean diameter of 6.9 cm.Three patients had ischemic colitis including 1 patient with colonic perforation and free underarm gas.The serosal invasion or organ involvement of colon cancer was detected in 25 patients with unclear boundary and cable-strip or nodular opacities.Fourteen patients had lymph node metastases,with surrounding tumor or retroperitoneal heterogeneous enhancement,sacvariable necrosis area,short-tempered edge of lymph node,bar-type exudation surrounding some of lymph node.There was 3 patients with hepatic metastases and 1 with pulmonary metastasis.Conclusion Irregular and thickened colonic wall,irregular constriction of colonic lumen,scuff syndrome and shoulder symptoms at the junction of the normal wall,colonic dilatation above the obstructive segment and multiple gas-fluid levels are mainly characterics of CT examination for colon cancer combined with obstruction.
10.Clinical value of multi-slice computed tomography in the diagnosis of colonic neoplasms

In order to investigate the clinical value of multi-slice computed tomography (MSCT) in the diagnosis of colonic neoplasms,the clinical data of 57 patients who were confirmed as with colonic neoplasms were admitted to the Wuxi No.2 People's Hospital from June 2013 to June 2014.Patients received plain and enhanced scan of MSCT,multiplannar reconstruction (MPR),sliding thin-slab maximum intensity projection (STS-MIP) and curved planar reformation (CPR),and the results of these examinations were compared with the postoperative TNM stage.Of the 57 patients,tumors located at the cecum in 1 patient,ascending colon in 23 patients,hepatic region of the colon in 9 patients,transverse colon in 11 patients,splenic region of the colon in 1 patient,descending colon in 3 patients and sigmoid colon in 9 patients.The results of the CT examination were in accordance with the results of exploratory laparotomy.The wall of the intestine was irregular ring-like thickened in 57 patients.The thickening of the intestine ranged between 0.6-3.2 cm,and patients had intestinal stricture in different degrees.Sixteen patients had gas-fluid level and dilatation of the intestine and were presented with intestinal obstruction signs.Forty-two patients had nodular or mass shadow in sofi tissues,and the nodules or mass protruded into the intestinal cavity or outside of the intestinal cavity,and resulting in intestinal stricture.The junction of the mass and the adjacent intestinal wall was blurred or clear,and the thickness of the intestinal walls was normal.Carcinomatous ulcer was observed in 20 patients.The shape of the ulcer was crateriform.The serosa and (or) adjacent organs were invaded by the tumors in 47 patients.Twenty-seven patients had lymph node metastasis,including 6 had distal metastasis,4 had hepatic metastasis and 2 had pulmonary metastasis.The sensitivity and accuracy of the preoperative CT examination for T stage were 100.0% (57/57) and 77.2% (44/57),respectively.The sensitivity,specificity and accuracy of the CT examination for N stage were 67.9% (19/28),72.4% (21/29) and 70.2% (40/57),respectively.The sensitivity,specificity and accuracy of the CT examination for M stage were 100.0% (6/6),100.0% (51/51) and 100.0% (57/57),respectively.The results of CT examination for TNM stage were well correlated with the pathological examination for TNM stage (κ =0.592,0.514,1.000,P <0.05).MSCT scanning and post-processing technique are of important clinical value in depicting tumor location,size and morphology,delineating tumor extension,revealing lymph nodes and metastases,and confirming preoperative tumor TNM stage in patients with colonic neoplasms.
