1.Curative Effect Observation of Ulinastatin Combined with Somatostatin in the Treatment of Acute Severe Pancreatitis

Objective:To observe the clinical effect of ulinastatin combined with somatostatin in the treatment of acute severe pan-creatitis. Methods:Totally 130 cases of severe acute pancreatitis patients were randomly divided into two groups with 65 cases in each. The control group was given the routine treatment and 3mg somatostatin in 250ml sodium chloride injections, ivd, bid, and the treatment group was given 10ku ulinastatin in 250ml sodium chloride injections additionally, ivd, bid. After the 10-day treatment, the clinical symptom disappearance time, the recovery time of respiratory and heart rate, length of stay, blood and urine amylase, and clin-ical total effective rate were compared between the two groups. Results:The symptom disappearance time, recovery time of respiratory and heart rate and length of stay in the treatment group were all shorter than those in the control group (P<0. 05). The recovery time of serum and urine amylase in the two groups was without significant difference (P>0. 05). The total effective rate of the treatment group was 95. 38%, while that of the control group was 73. 84% with significant difference (P<0. 05). Conclusion:The application of ulinastatin in the treatment of severe acute pancreatitis shows obviously curative effect, which can effectively improve the clinical symptoms and shorten the course of disease, and is valuable in clinical use.
2.Effect of TSH inhibition on sIL-2 R, TSGF, CD44 V6 and T lymphocyte subsets in elderly patients with thyroid cancer

Objective To analyze the effect of TSH inhibition on soluble interleukin 2 receptor ( sIL-2R), interleukin 44 variant 6 (CD44V6), tumor specific growth factor (TSGF) and T lymphocyte subsets in elderly patients with thyroid cancer.Methods 50 elderly patients with thyroid cancer in our hospital were collected.The patients were randomly assigned to experimental and control groups, 25 cases in each group, patients in the experimental group were given TSH suppression therapy after surgery , patients in control group were given thyroxine replacement therapy after operation, treatment for 1 month, serum SIL-2R, CD44v6, TSGF and peripheral blood T lymphocyte subsets situation were detected in all patients with .ResuIts After treatment, compared with the control group, ①in the experimental group, the level of serum sIL-2R was significantly lower, the difference was statistically significant(P<0.05);②the serum CD44V6 level was lower in the experimental group (P<0.05);③the serum TSGF level was lower in the experimental group (P<0.05);③in the experimental group,the levels of CD3 + and CD4 +were higher, levels of CD8 + were lower ( P<0.05 ) .ConcIusions TSH suppression therapy can reduce senile thyroid cancer patients serum SIL-2R, CD44v6, TSGF and CD8 +lymphocyte level, peripheral blood T lymphocyte CD3 +, CD4 +levels were elevated,and improve the immune function of patients , the clinical has guiding significance .
3.Clinical observation of proximal humeral locking plate combined with injectable bone graft in the treatment of proximal humeral fractures

Objective To explore the treatment effect and application value of proximal humeral locking plate combined with injectable bone graft in the treatment of proximal humeral fractures.Methods 86 patients with proximal humeral fractures were selected,using the random number table method they were randomly divided into the observation group and control group,each group had 43 cases.The control group was treated with locking plate method,and the observation group was treated by the combination of artificial bone and locking plate.The therapeutic effect of the two groups was observed.Results The excellent and good rate of the observation group was 93.03%,that of the control group was 74.42%,the difference was statistically significant between the two groups(x2 =5.460 3,P <0.05).In observation group,the fracture healing time was (11.78 ± 3.64) weeks,the shoulder joint function score was (96.87 ± 4.23) ; those of the control group were (19.65 ± 6.27) weeks,(78.73 ± 8.28),the differences were statistically significant between the two groups (t =7.118 2,12.793 4,all P < 0.05).The incidence rate of complications of the observation group was 6.98%,which was significantly lower than 30.23% of the control group (x2 =7.678 6,P < 0.05).Conclusion Proximal humeral locking plate combined with injectable bone graft in the treatment of proximal humeral fracture has reliable curative effect,short fracture healing time and mild surgical complications,which is worth popularizing in clinical.

ObjectiveTo explore the relationship between fat content of postmenopausal osteoporosis patients and bone mineral density (BMD).Methods144 female osteoporotic patients with the age of 50~75 years were choosen from guangdong province from October 2010 to January 2011.According to TCM syndrome differentiations,these patients were divided into kidney Yang deficiency group,liver-kidney Yin deficiency group,spleen and kidney Yang deficiency group and Qi stagnancy and blood stasis group.Four groups of patients were performed total body fat content detection.The data were analyzed by software SPSS 16.0.ResultsComparing of L1~4 average BMD in four groups showed:BMD of Kidney Yang deficiency group was maximum,followed by spleen and kidney Yang deficiency group,and the BMD of Qi stagnancy and blood stasis group was minimum.Comparison between the four groups showed statistical significance (P<0.05).Comparing of body fat content Iin the four groups showed:the body fat content in the kidney Yang deficiency group was minimum,followed by spleen and kidney Yang deficiency group,and body fat content in Qi stagnancy and blood stasis group was maximum.Pair-comparison in kidney Yang deficiency group,liver-kidney Yin deficiency group,spleen and kidney Yang deficiency group showed no statistical significance (P>0.05).While these three groups showed statistical difference when comparing with Qi stagnancy and blood stasis group (P<0.05).The body fat content and L1~4 average BMD had negative correlation.ConclusionOsteoporosis in Qi stagnancy and blood stasis group was more serious than the other three other groups.Fat content was negatively related with bone density.
7.Double-guide wire technique for difficult bile duct cannulation in patients due to biliary complications after liver transplantation

Objective To compare the double-guide wire technique (DGT) with the standard cannulation technique (SCT) in patients with difficult access due to biliary complications after liver transplantation.Methods Difficult CBD cannulation is characterized by unsuccessful cannulation in 10 minutes.A total of 91 patients with biliary complications after liver transplantation were assigned to the DGT group (44patients,including 6 difficult cannulation,41 males and 3 females,30 to 61 years) and the SCT group (47patients,41 males and 6 females,33 to 56 years).An extra 20-minute cannulation was performed on the two groups.Success rate,procedure time and complications were compared.Results CBD cannulation was successful in 36 (81.8%) patients of DGT group and 33 (70.2% ) patients of SCT group,which was not different ( P > 0.05 ).The time of successful CBD cannulation in the DGT group ( 11.7 ± 3.2 minutes) was shorter than that in the SGT group ( 16.8 ±2.8 minutes,P <0.05).The incidence of post-ERCP hyperamylnsemia had no difference in the two groups ( P > 0.05 ).There were no serious complications like infection,hemorrhage or perforation in either group.Mild pancreatitis occurred in 2 cases in the SCT group,but none in DGT.Conclusion DGT is an effective and safe technique in patients with biliary complications after liver transplantation,with no more complications than the SCT group.It is recommended in difficult cannulation of common bile duct (CBD).
8.Clinical Significance of FGFR1OP and p57 (Kip2) Proteins in Gliomas: An Immunohistochemical Study

Objective: To investigate the effects of FGFR1OP and p57/Kip2 proteins on the genesis and progression of gliomas and their clinical significance. Methods: The expression of FGFR1OP and p57/Kip2 in 54 glioma specimens was detected by SP immunohistochemical technique. The relationship between the ex-pression levels of those proteins and various clinical pathologic factors was evaluated. Results: The expres-sion of FGFR1OP and p57/Kip2 was found in 66.7% and 44.4% gliomas, respectively. The OD value of FG-FR1OP was 0.131±0.010 in high grade gliomas, and 0.118±0.010 in low grade ones, with a statistical signifi-cance (t=-5.497, P=0.000), showing that higher expression of FGFR1OP was significantly associated with glo-ma cell differentiation. The OD value of p57/Kip2 was 0.156±0.008 in high grade gliomas, and 0.165±0.006 in low grade ones, with a statistical significance (t=0.296, P=0.014), showing that lower p57/Kip2 expression was correlated with high grade gliomas. FGFR1OP was negatively correlated with p57/Kip2 in gliomas (r=-0.732, P<0.01). Conclusion: Increased expression of FGFR1OP and/or decreased expression of p57/Kip2 may play an important role in the genesis and progression of gliomas and may indicate a poor prognosis.
9.Gene expression and calcium ion concentration variation in bone marrow mesenchymal stem cells differentiating into neuron-like cells

BACKGROUND: Monosialotetrahexosylganglioside plays an important role in a variety of physiological processes, such as the nerve cel growth and development, differentiation, regeneration and intracel ular and extracel ular information transmission. OBJECTIVE: To investigate the effects of monosialotetrahexosylganglioside on the changes of gene expression and calcium ion concentration in the course of inducing the mesenchymal stem cells into neuron-like cells with Woodbury’s method. METHODS: The mesenchymal stem cells from Sprague Dawley rats were cultured after isolated and purified, After 5 passages in culture, the cel integrated into a dense monolayer, and treated with 50 mmol/L monosialotetrahexosylganglioside for 24 hours as the monosialotetrahexosylganglioside group; then the mesenchymal stem cells were induced into neuron-like cells with the methods of Woodbury after pre-cultured for 24 hours, and set the control group. The protein and mRNA expression levels of growth-associated protein 43, neuron-specific enolase, neurofilament and nestin were detected by immunocytochemistry and real-time PCR, respectively. The fluorescence intensity of intracel ular free calcium ion before and after inducing was detected by laser scan confocal microscope. RESULTS AND CONCLUSION: After induction, the expression levels of growth-associated protein 43, neuron-specific enolase, neurofilament and nestin of the mesenchymal stem cells in the monosialotetrahexosylganglioside group were higher than those in the control group (P < 0.05), demonstrating that monosialotetrahexosylganglioside could promote the differentiation of mesenchymal stem cells into neuron-like cells. The fluorescence intensity in mesenchymal stem cells was increased gradual y in two groups after the medium was replaced by the induction medium, attained its peak value at 100 seconds and then decreased gradual y, but the fluorescence intensity was stil higher than that before the induction at 20 minutes. The fluorescence intensity of intracel ular free Ca2+ was increased significantly in the monosialotetrahexosylganglioside group when compared with the control group (P < 0.05), suggesting that monosialotetrahexosylganglioside could increase the concentration of intracel ular free Ca2+, and intracel ular free Ca2+ may be useful in the course of induction. The changes of protein expression levels of growth-associated protein 43, neuron-specific enolase, neurofilament and nestin were not significant after induction, indicating that Woodbury classic induced programme could regulate the post-transcriptive protein level.
10.The preventive effect of acarbose on the imapired glucose tolerance population from developing diabetes and cardiovascular diseases
