1.Studies on the Biotransformation of Poria cocos by Ganoderma lucidum

Objective: To study the biotransformation of Poria cocos by Ganoderma lucidum and the changes of total polysaccharide and total triterpenes contents of the biotransformation products. Method: The experiment uses the microwave method to extract and sulfuric acid-phenol method to measure total polysaccharide content. And the ultrasonic wave method was used to extract total triterpenes and the content was measured by vanillin-perchloric method. Result: After treatment by G. lucidum, the total polysaccharide and triterpenes content of the biotransformation products were significantly higher than the non-conversed P. cocos (P<0.01), showing that the original performance of P. cocos was changed by way of biotransformation of G. lucidum; P. cocos had a promotion effect on the growth of G. lucidum. Conclusion: Biotransformation of P. cocos for Increasing the total polysaccharide and triterpenes contents by using G. lucidum has advantages of simple, low cost and so on. This method has a certain application value in modernization of Chinese traditional medicine.
2.Relationship between plasma adiponectin and TNF-? concentration in women with gestational diabetes

Objective To investigate the serum adiponectin levels and TNF-? levels in women with gestational diabetes and to study their clinical significance.Methods Plasma concentration of adiponectin,tumor necrosis factor alpha and correlate parameters were measured in 50 patients with gestational diabetes(GDM)and 36 pregnant women with normal glucose tolerance(NGT).Radio immunoassay was used to measure the adiponectin,TNF-? level was measured by ELISA.Serum insulin level was measured by Electrochemiluminescent immunoassays fasting glucose levels by glucose oxidase method.Results GDM patients had significantly lower concentrations of adiponectin(10.3?(2.4) mg/L) and elevated levels of TNF-?(4.6?(1.5)?g/L) in comparison to NGT women.In GDM group TNF-? level was correlated positively with serum insulin level,glucose level.Concentration of adiponectin was negatively correlated with corresponding parameters.Conclusion Elevated level of TNF-? and decreased adiponectin concentration may not simply reflect maternal adiposity and insulin resistant state,but may contribute to the impaired glucose metabolism during pregnancy and forecast the risk of metabolism syndrome.
3.Biological safety of Firebird~(TM) and Taxus Express2~(TM) in the percultaneous coronary intervention of coronary artery disease:Two-year follow-up

BACKGROUND:Many studies have demonstrated that FirebirdTM and Taxus Express2TM can effectively reduce intrastent restenosis.However,there are few data about long-term efficacy of two stents,and reports about middle and long-term follow up are rare.OBJECTIVE:To observe the safety and biocompatibility of FirebirdTM and Taxus Express2TM in percutaneous coronary intervention(PCI) for coronary artery disease.DESIGN,TIME AND SETTING:Non-randomized concurrent control clinical observation was performed at Department of Cardiology,Hongqi Hospital of Mudanjiang Medical College from April 2005 to April 2008.PARTICIPANTS:233 patients(268 lesions) undergoing PCI were divided into FirebirdTM(n =82),Taxus Express2TM group(n =80) and bare metal stent group(n =71).METHODS:Coronary arteriography was performed through femoral artery or radial artery.Vascular inner diameter was determined using quantitative computer analysis.The patients underwent FirebirdTM,Taxus Express2TM and bare metal stenting,respectively.The patients were reexamined and followed-up using telephone every 2-4 weeks after discharging and examined using coronary arteriongraphy after 9-12 months.The follow-up lasted for 24 months.MAIN OUTCOME MEASURES:Characteristics of arteriongraphy and stenting condition of all patients;biocompatibility of stent to host;major adverse cardiac events during hospitalization and follow-up,including death,angina pectoris attacks or heart failure;coronary artery diameter decreased ≥ 50% was regarded as restenosis.RESULTS:101 stents were implanted in Firebird group,98 in Taxus Express2 group and 85 in bare metal stent group.There was no stent defluxion,dislocation,or breakage.No noticeable platelet decrease,hemolysis or white blood cell increase was found.There were no significant differences among three groups in terms of Characteristics of arteriongraphy and stenting condition.The incidence of major adverse cardiac events and intravascular restenosis in Firebird and Taxus Express2 groups was fewer than bare metal stent group(P 0.05).CONCLUSION:No specific biocompatibility responses in treatment of coronary artery diseases using FirebirdTM and Taxus Express2TM.The two drug-eluting stents are superior over bare metal stent in reducing restenosis.The safety and efficacy of two drug-eluting stents are similar.
4.Function and formation mechanism of neutrophil extracellular traps in related diseases

Neutrophil extracellular traps (NETs) are fibrous structures released by neutrophils and the formation process is called NETosis. NETs participate in the host innate immunity. Recent research has found that NETs is a double-edged sword. Under normal conditions, the formation of NETs can play a role in clearing pathogens and maintain the host homeostasis. However, when NETs are overproduced or not cleared in time, they can take part in the pathogenesis of many diseases. This article reviewed the formation of NETs, the mechanisms involved in NETosis and the role of NETs in the secretion of multiple cytokines in different diseases.
5.Patch clamp study of ultra-rapid delayed rectifier-potassium channel and the effect of Ibulitide on rat isolated atrial myocytes

Objective To study the characteristic of ultra-rapid delayed rectifier-potassium channel K current (IKur) in rat isolated atrial myocytes using whole-cell patch clamp technique, and the effect of Ibulitide on rat isolated atrial myocytes. Methods Atrial myocytes of rats on an in vitro Langendorf perfusion system were obtained by double digestion (collagenase typeⅡ) method. Whole-cell patch-clamp recording technique was used to record IKur currents. The current-voltage (I-V) curves of K+currents of atrial myocytes were fitted. With filled Ibulitide (2μg/L, filled 3 minutes), IKur was recorded. Results The Results showed that IKur was activated rapidly, almost no lag,and deactivation slowly. This current was activated at about-20 mV, the peak current was present apparent voltage dependence. The peak current density was (2.01 ± 0.27) pA/pF. After filled with Ibulite, this current was no any changed activation. Ibulite decreased the peak current density from+30 mV to+50 mV,but no statistical significance. Conclusion The IKur of rats can activate rapidly with no delays and inactivate slowly. This current is ultra-rapid delayed rectify outside current,and obvious displays the characteristic of outward rectification of whole action potential duration. The 2 μg/L Ibulitide shows no statistical significance for this current.

Objective:To investigate the effects of Chinese Medicinals with right supporting and phlegm transforming actions on albumin synthesis and secretion of fibrous hepatic cells of rat in vitro. Methods: Amygdalin, the water soluble extract of Radix Salviae Miltiorrhizae, lordyceps, and colchicine were used to act on fibrous hepatic cells of rat produced by primary monolayer culture in vitro. The results were determined by ELISA method. Results: Besides colchicine, the others amygdalin and cordyceps. 7.Risk Factors on Gas Gangrene after Open Trauma and Countermeasures

OBJECTIVE To explore the risk factors of gas gangrene after open trauma for its early diagnosis and precaution.METHODS The data of 42 patients with gas gangrene after open trauma from Aug 2003 to Aug 2008,were collected.The risk factors of gas gangrene after open trauma had been retrospectively analyzed from the cause of the injuries,the distribution of latent period,wound,systemic symptoms,laboratory examination and treatment.RESULTS Gas gangrene after injury was related with open wounds for various reasons.Latent period of gas gangrene was 24-72 h.Early risk factors of gas gangrene among them mainly included degloved or avulsed skin-injury(100%),musce trauma(100.00%),contaminated wound and retained foreign bodies(95.24%),weak pulse of peripheral artery(95.24%),nervous injury(90.48%),and lower temperature of skin aound the wound(83.33%).There were obviously systemic symptoms in infective stage but in early stage were not.CONCLUSIONS Gas gangrene is extremely rare in injury patients but it is life-threatening.So more attention should be paid to the risk factors in order to decrease the incidence of gas gangrene after trauma.Improving the prognosis of patients;the wounded locations should be effectively treated early so as to improve the prognosis.
8.Effects of dexmedetomidine on mRNA expression of triggering receptor expressed on myeloid cells 1 in peripheral blood mononuclear cells of rats with acute obstructive suppurative cholangitis

Objective To investigate the effects of dexmedetomidine (DEX) on the mRNA expression of triggering receptor expressed on myeloid cells 1 (TREM-1) in peripheral blood mononuclear cells of rats with acute obstructive suppurative cholangitis (AOSC).Methods Sixty healthy male Wistar rat models of AOSC induced by complete common bile duct ligation and injection of E.coli into the bile duct through an intubation tube were replicated successfully.After modeling,the peripheral blood was collected and mononuclear cells were isolated and cultured.According to random number table method,the mononeuclear cells were divided into model group (no drug added in culture of mononuclear cells) and low,medium and high dose DEX groups (final concentrations 0.4,0.8,1.2 μg/L DEX were in low,medium and high DEX mononuclear cell cultures,respectively).After the mononuclear cells were cultured for 24 hours,the levels of tumor necrosis factor-α (TNF-α),interleukins (IL-1 and IL-6) in the supernatant of the cultured mononuclear cells were detected by enzyme linked immunosorbent assay (ELISA).The level of C-reactive protein (CRP) was detected by immunity transmission turbidimetry.The expression of TREM-1 mRNA in the mononuclear cells was detected by reverse trantscription-polymerase chain reaction (RT-PCR).Results Compared with the model group,the levels of TNF-α,IL-1,IL-6,CRP were decreased,the TREM-1 mRNA expressions were down-regulated in the different DEX dose groups,and the degrees of descent in medium and high dose groups were more significant than those in low dose group [TNF-oα (ng/L):95.5±8.6,88.9±5.3 vs.131.1 ± 14.2;IL-1 (ng/L):53.5±8.3,48.3 ± 6.7 vs.73.7 ± 12.8;IL-6 (ng/L):266.9±26.2,252.1 ± 17.7 vs.349.9±40.4;CRP (ng/L):4.3 ± 1.1,3.9 ±0.7 vs.5.6 ± 1.7;TREM-1 mRNA (A value):0.43 ± 0.18,0.39 ± 0.16 vs.0.65 ±0.25,all P < 0.05].Conclusion DEX can down-regulate the expression of TREM-1 mRNA and inhibit the formation and secretion of inflammatory factors TNF-α,IL-1,IL-6 and CRP in peripheral blood mononuclear cells of rats with ASOC.
9.Perioperative nursing for patients with cephalocervical tumors treated with CT-guided 125I seed implantation

Objective To investigate the optimal measures and clinical effect of perioperative nursing care for patients with cephalocervical cancer treated with CT-guided radioactive 125I seed implantation. Methods Preoperative, intraoperative and postoperative continuous and comprehensive nursing was strictly carried out in 93 patients with cephalocervical cancer who underwent CT-guided radioactive 125I seed implantation treatment. The clinical results were analyzed and evaluated. Results The 125I seed implantation procedure was successfully accomplished in all 93 patients; the success rate was 100%. Shedding of 6 particles was observed in one patient, and shedding of 5 particles was observed in another patient; grade Ⅳskin injury was seen in 3 cases, which were improved after symptomatic treatment. All patients were uneventful at the time of discharge from hospital. Conclusion To ensure a successful CT-guided radioactive 125I seed implantation for the treatment of cephalocervical cancer, professional, meticulous and standardized nursing is the important guarantee.
10.Reliability and validity of a questionnaire for job satisfaction with influenza vaccine recommendation of community diabetes management doctors

Objective:
To evaluate the reliability and validity of a questionnaire for job satisfaction with influenza vaccine recommendation of community diabetes management doctors.
Methods:
Through the literature review,expert interviews and pilot survey,a questionnaire for job satisfaction with influenza vaccine recommendation of community diabetes management doctors was initially formed. Cronbach's α coefficient was used to evaluate the reliability of the questionnaire; correlation analysis,exploratory factor analysis(EFA)and confirmatory factor analysis(CFA)were used to evaluate the validity of the questionnaire.
Results :
The questionnaire had 23 items, which were categorized into four dimensions: working environment,job itself,general satisfaction and occupational load. The Cronbach's α coefficient of working environment,job itself,general satisfaction, occupational load and the total questionnaire was 0.924,0.884,0.937,0.891 and 0.936,respectively. EFA showed that the characteristic roots of four dimensions were all more than one,and the cumulative variance proportion was 73.82%. The Pearson correlation coefficient between each item and the total questionnaire ranged from 0.339 to 0.818, and the correlation coefficient between each dimension and the total questionnaire ranged from 0.429 to 0.872(all P<0.05). CFA showed that the variances of parameter estimation errors were all positive,and the standard errors ranged from 0.035 to 0.099,which were statistically significant(P<0.05);the standardized parameters between each item and dimension ranged from 0.596 to 0.939; χ2/df was 4.627(P<0.05), the comparative fit index was 0.827,the root mean square error of approximation was 0.076,which indicated the model fit.
Conclusion
The questionnaire has good reliability and validity,so it can be used to evaluate the satisfaction with influenza vaccine recommendation of community diabetes management doctors.
