Breast cancer susceptibiIity gene 1( BRCA1)is a tumor suppressor, but mutated get BRCA1 is cIoseIy reIated to the deveIopment of breast cancer. Current study has reveaIed that BRCA1 can get invoIved in the DNA damage repair process as a key mediator. DNA doubIe-stranded break (DSB)is the most serious form in DNA Iesions, and BRCA1 pIays an important roIe in repairing DSB through reguIation of homoIogous recombination( HR). In this articIe,the moIecuIar mechanism of BRCA1 in reguIating HR is reviewed with reference to the activity of functionaI domains in BRCA1 struc-ture,the mutations occurring in main domains of BRCA1,the reIationship of BRCA1 with BRCA2, Rad51 or CtIP,and phosphoryIation of BRCA1. In addition, the association of the defective BRCA1-mediated HR repair mechanism with the sensitivity to different DNA damaging agents and synthetic IethaIity in tumor ceIIs is aIso addressed.

Objective To observe the inhibitory action of annonaceous acetogenins AS-4 on human liver cancer cell line HepG2 in vitro. Method Using different concentration of annonaceous acetagenins group(6.25,12.5,25,50 ?g/mL),control group(25 ?g/mL) of masculine(DDP),control blank group, after routine culture of 24 or 48 hours, the effect of annonaceous acetogenins AS-4 on HepG2 was analyzed by colorimetry assay(MTT) and inverted phase-contrast microscope observing. Result AS-4 had the best inhibitory effect on HepG2,the highest inhibitory rate was 91.68%,and the effect was time-dosage dependent, and the 48 h IC50 was 6.67 ?g/mL. Conclusion AS-4 has lethal effect on HepG2 in vitro, and shows obvious cytotoxic effect.

Objective:To investigate and analyze the current status of patient satisfaction and the influencing factors at private hospitals in Guizhou Province, and thus to provide a scientific basis for the management of medical service at private hospitals. Methods:The satisfaction scores were statistically described and analyzed by the factor analysis and multiple linear regression analysis. Results:The overall satisfaction score of patients was (3.87 ± 0.85),of which the satisfaction of medical technology, drug price, inspection fee,and medical facilities was lower. The factor analysis showed that satisfaction of patients at private hospitals could be classified into the environment, the quality of service, medical expense, and service flow.Multivariate linear regression results showed that its standardized partial regression coefficient by the parameter descending order followed as medical expense, the environment, the quality of service, and then service flow. Conclusions:Private hospitals should set up a fair medical expenditure mechanism, improve the environment and quality of medical services, and optimize service flow in order to improve patient satisfaction.

Objective To investigate the effects of pravastatin on the blood biochemical index, the levels of serum resistin, tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) in patients with impaired glucose tolerance (IGT) and metabolic syndrome (MS).Methods 60 patients with IGT and MS were random treated with pravastatin or remedial life-style intervention, and the levels of lipids profiles, insulin resistance index (HOMA-IR),serum resistin,TNF-α and IL-6 were measured before and after treatment. Results The levels of total cholesterol [ (4.45 ±0.60)mmol/L vs (5.58 ±0.96) mmol/L,t =-5.42,P <0.01],HOMA-IR [3.22 ±0.64 vs 3.58 ±0.71,t =-2.05,P <0.05) ],resistin[ (1.97 ±0.72) μg/L vs (2.76 ±0.73) μg/L,t=-4.26,P <0.01 ],TNF-α[ (9.36 ±2.03) μg/L vs (13.87 ±2.30)μg/L,t =-8.06,P <0.01] and IL-6[ (3.50 ±0.99) μg/L vs (6.32 ±1.17) μg/L,t =-10.06,P <0.01 ] in pravastatin group were significantly lower than those in life-style intervention group. Conclusion Pravastatin could improve insulin sensitivity of the patients with IGT and MS,significantly decreased the serum levels of resistin,TNF-α,IL-6,and it had anti-inflammatory effect.

Sixty patients with early diabetic nephropathy (DN) and normal blood pressure were randomly divided into telmisartan treatment and control groups. And the blood biochemical parameters, urinary albumin excretion rate (UAER),the serum levels of homocysteine (HCY) and transforming growth factor-β1 (TGF-β1) were measured at both pre- and post-treatment of 24 weeks. After treatment, the levels ofUAER,HCY [(61 ±26) vs.(98±38) mg/L,(11.70 ±2.86) vs.(14.4±2.8) μmol/L (P< 0.01 ) ]and homeostasis model of insulin resistance index,TGF-β1 [ ( 3.17 ± 0.66) vs.( 3.60 ± 0.87 ),(66 ± 14) vs.(77 ± 17 )U/L respectively (P < 0.05 )]in the telmisartan group were significantly lower than those in the control group. It indicated that telmisartan could improve the insulin sensitivity, significantly decrease the serum levels of HCY and TGF-β1,reduce UAER and exert kidney protection effects in the patients with early DN.

Objective To calculate the biological reference of plasma amino acid on L-8900 amino acid analyzer and to provide reference for clinical diagnosis with domestic reagents replacing original reagent .Methods By testing the original standards and the same batch of plasma (50 cases) ,we compared original reagents with domestic reagents for their performance (including resolution , repeatability ,accuracy ) .We tested the plasma free amino acids of 400 cases of healthy people using the domestic reagents to estab-lish biological reference interval of plasma amino acid ,and do correlation analysis between the amino acids level and liver function . Results (1)Domestic reagents showed high accuracy in the results of 5 consecutive detection of amino acids were high peak separa-tion and high peak retention time and high peak area .(2)Statistics derived biological reference interval of 19 amino acids and 10 kinds of amino acids had significant differences .(3)Correlation analysis showed that ALT and liver function were negatively corre-lated with threonine(P<0 .05) .GLU and valine ,isoleucine ,leucine were positively correlated(P<0 .05) .CHO and negatively cor-related with isoleucine(P<0 .05) .Conclusion Domestic agents can replace the original reagents ,on the basis ,the biological refer-ence intervals of plasma amino acids have great importance to clinical diagnosis and prognosis .

Objective To evaluate the clinic effects of perioperative psychological intervention combined appropriate sedation by target-controlled infusion of midazolam during operation via symptom check List 90 (SCL-90) and observers assessment of alertness/sedation scale (OAA/S) in patients undergoing thumb or finger reconstruction. Methods Eighty patients undergoing thumb or finger reconstruction were randomly divided into group A,B,C,and D (n =20 cases per group). All patients were received brachial plexus block and spinal-epidural combined anesthesia.Patients in group A were given perioperative psychological intervention combined appropriate sedation by target-controlled infusion of midazolam during operation; patients in group B were received routine treatment; patients in group C were given perioperative psychological intervention; and patients in group D were sedated with midazolam during operation. The vital signs and the scores of OAA/S were observed during operation. The scores of visual analogue scale (VAS) and SCL-90 were recorded after operation. Results The OAA/S grade in groups A and D were stability than that in groups B and C (P < 0.05) ; the scores of VAS and the results of psychological status assessment by SCL-90 in group A were superior to groups B,C,and D (P < 0.05).Conclusions Perioperative psychological intervention combined appropriate sedation in operation on thumb or finger reconstruction is a excellence anaesthesia processing method. It might have beneficial effects on postoperative recovery profile, and strengthen the tolerance to pain and improve the patient's psychological status during operative period.
