1.Teaching discussion on basic course of computer application in military medical university

Objective:To explore and improve the teaching model and method of basic courses of computer application in Military Medical University so as to enhance the teaching quality and effect of the courses.Methods: Through analysis and discussion for the facing problem in basic teaching course of computer application of military medical university, and adopting series of teaching methods included that confirmed teaching aim, innovated teaching idea, explored group instruction and emphasized curriculum development, and combining teaching experiences, some opinions and suggestions were proposed to strengthen and improve the teaching quality of basis course of computer application.Results:After improved the teaching methods for basic courses of computer application, the project assessment and actual measurement of students' ability showed that the students' knowledge level and ability were greatly improved than formerly students, and the teaching level of the basic course of computer application also were enhanced.Conclusion: Under the new situation, basic course teaching of computer application in military medical university is facing new challenges, and the corresponding measurements should be adopted to enhance the teaching quality so as to satisfy the requirement of compound talent cultivation for military medicine under the informatization age.

Hepatitis B virus ( HBV) threatens human's health seriously, immune disorder is the main pathogenesis.HBV cannot naturally infect mouse liver, thus the researchers tried to established HBV mouse models to imitate the immunological pathogenesis of HBV infection.This review summarize various methods to establish HBV mouse models, including HBV transgenic technics, HBV in vivo liver-target transfection technics and HBV naturally infected humanized mouse technics etc.Their advantages, disadvantages and contributions to immunological studies were also analyzed, and the development of this area was also prospected.
3.The animal experiment for acute superior vena cava obstruction

Objective To investigat the safe time of SVC interruption in New Zealand rabbits, and help to find the safe time ofSVC interruption in chnical operation. Methods 25 New-Zealand rabbits were separated into 3 groups randomly. Bi-SVC was cross-clamped for one hour, two hours in animal models. The femoral artery or SVC pressures were monitored. The control group (sham op-eration group, 5) were opened chest both sides and monitored femoral artery pressure and SVC pressure for 2 hours. Samples of cere-brum, cerebellum and medulla of rabbits immediately post-operation were studied for water content, light and electric microscopies.The tiny change of brain cells in brain mantle, hippocampus and thalamencephal were observed under electron microscope. Results(1) The SVC pressure of rabbits varied regularly in occlusion groups, which displayed as "upgrade-degression- upgrade again-retain".(2) There was no obvious difference in water content of brain between the one hour group and the control group, both of which weremuch lower than two hours group. (3) The brain tissues were almost in normal construction in one hour group, but encephaledema wasobserved in two hours group. The pathological change of cony brain cells was light and reversible in one hour group. However, in twohours group, necrosis of neuron ceils in brain could be observed, which was irreversible. Concerto The animal model of acuteSVC obstruction was built successfully. Interruption of the SVC for 1 hour was proved safe, with longer interruption of SVC 2 hours,obvious encephaledema and necrosis of neuron cells can be observed.

Bone morphogenetic proteins (BMPs) were first studied as growth factors or morphogens of the transforming growth factor-beta super family. These growth molecules, originally associated with bone and cartilage development, are now known to play important roles in morphogenesis and homeostasis in many other tissues. Recently, signiifcant contributions of BMPs, their receptors, and interacting molecules have been linked to carcinogenesis and tumor progression. BMPs can sometimes play a role as a tumor suppressor. This article explains the composition and biological characteristics of BMPs, and investigates their new roles in the pathogenesis of cancer.
5.Efficacy and Safety of Intrathecal Injection of Vancomycin Combined with Dexamethasone in the Treat-ment of Intracranial Infection:A System Review

OBJECTIVE:To systematically review the efficacy and safety of intrathecal injection of vancomycin combined with dexamethasone in the treatment of intracranial infection,and to provide evidence-based reference in the clinic. METHODS:Re-trieved from PubMed,Medline,CJFD,VIP,Wanfang databases,randomized controlled trials(RCTs)about intrathecal injection of vancomycin combined with dexamethasone in the treatment of intracranial infection were collected. Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies according to Cochrane system review manual 5.0.1. Then Meta-analysis was conducted by Rev Man 5.2 statistical software. RESULTS:Total of 8 RCTs were included,involv-ing 543 patients. Meta-analysis showed that,compared with intravenous dripping of ceftriaxone or vancomycin,intrathecal injec-tion of vancomycin combined with dexamethasone could significantly increase response rate [RR=1.18,95%CI(1.11,1.26),P<0.001] and bacterial clearance rate of CSF[RR=1.13,95%CI(1.01,1.27),P<0.001] of intracranial infection patients,shortened treatment time [SMD=-1.60,95%CI(-1.89,-1.30),P<0.001],reduce the incidence of ADR [RR=0.48,95%CI(0.32,0.73), P<0.001]. At the same time,it also could improve changes of intracranial pressure[SMD=-1.78,95% CI(-2.10,-1.47),P<0.001],changes of protein quantitation of CSF[SMD=-0.18,95%CI(-0.25,-0.11),P<0.001] and changes of glucose quantita-tion of CSF[SMD=1.77,95%CI(0.91,2.63),P<0.001],with statistical significance. CONCLUSIONS:Intrathecal injection of van-comycin combined with dexamethasone shows good clinical efficacy for intracranial infection,improves bacterial clearance rate, shortens treatment duration,significantly lowers intracranial pressure and protein quantitation,improves glucose quantitation of cere-brospinal fluid,with good safety.
6.Comparison of POSSUM, P-POSSUM and Thoracoscore in forecast of the postoperative mortality in thoracic surgery patients

Objective To evaluate and compare the applicability and accuracy of POSSUM , P-POSSUM and Thora-coscore in predicting the postoperative mortality of thoracic surgery patients .Methods We collected the clinical data of 1 450 thoracic surgery patients during March 2012 to August 2013 in the Second Xiangya Hospital.We used the 3 kinds of risk sco-ring system to calculate the predicted postoperative mortality in these patients .Then we used H-L test and the area under the ROC curve to evaluate and compare their degrees of compliance and validities of identification independently .Results Within 1 450 cases,20 patients (1.4%) died in 30 days after surgery, POSSUM overestimated the mortality (3.84%,P<0.001), while the P-POSSUM (1.05%/1.10%) and Thoracoscore (0.90%) underestimated the mortality, P>0.05.The area under the ROC curve of POSSUM, P-POSSUM ( two kinds of death prediction formula ) and Thoracoscore to predict postoperative mortality in all thoracic surgery patients was 0.774,0.777,0.777 and 0.831 independently, P <0.05; the area under the ROC curve to predict postoperative mortality in patients after lung surgery was 0.755,0.771,0.771,0.849 independently, P<0.05; the area under the ROC curve to predict postoperative mortality of patients with esophageal surgery was 0.640,0.650,0. 650,0.764 independently, only Thoracoscore P<0.05;the area under the ROC curve to predict postoperative mortality of pa-tients with mediastinal and other surgical was 1.000,1.000,1.000,0.854 independently, only Thoracoscore P<0.05.Con-clusion Thoracoscore scoring system is the most suitable risk scoring system to predict postoperative mortality in thoracic sur-gery patients with high predictive accuracy and good identification validity .
7.Effects of Early Usage with Wulingjun Powder on the Behavior of Rats with Post-traumatic Stress Disor-der and the Level of IL-1βand IL-6 in Their Hippocampi

OBJECTIVE:To discuss the effect of early usage with Wulingjun powder on the behavior of rats with post-traumat-ic stress disorder(PTSD)and the level of interleukin-1β(IL-1β)and IL-6 in their hippocampi. METHODS:50 SD rats were ran-domized into control group(normal saline),model group(normal saline),positive control group(paroxetine,10 mg/kg)and the groups of low and high-dose(0.4,0.8 g/kg)Wulingjun powder. The rats in all groups except for control group,were exposed to single prolonged stress for the establishment of PTSD models. Corresponding drugs were given ig immediately after such establish-ment,qd,for 7 consecutive days. An open field test(the times of the rats’entering the central area and the percentage of the time the rats spent in exploring the central area within 10 min were recorded)was conducted 7 days after giving drugs;and an elevated plus-maze test(the times of the rats’in opening arms and the percentage of the time the rats spent in opening arms within 5 min were recorded)was carried out 8 days thereafter,to observe the behavior of the rats. Then the rats were sacrificed to determine the level of IL-1β and IL-6 in their hippocampi by ELISA. RESULTS:Compared to the normal group,the behavioral index of the rats in model group was significantly lower,and the level of IL-1β and IL-6 in their hippocampi was significantly higher(P<0.05 or P<0.01). Compared to model group,the behavioral index of the rats in positive control group and the group of high-dose Wuling-jun powder was significantly higher,and the level of IL-1β and IL-6 in their hippocampi was significantly lower (P<0.05),but no statistically significant difference was shown in all indexes in the group of low-dose Wulingjun powder (P>0.05). CONCLU-SIONS:High-dose Wulingjun power can improve the anxiety-like behavior of the rats with PTSD,which may be related to the re-duction in the level of IL-1βand IL-6 in the hippocampus.
8.Fan-shaped decompression and allograft fibula supporting internal fixation for treatment of early femoral head necrosis in adults

BACKGROUND:Traditional core decompression and al ograft fibula supporting can reduce the stress load within femoral head and improve mechanical properties of femoral head. However, it cannot provide supports for maintaining the stability of five pathological areas fol owing femoral head necrosis. OBJECTIVE:To observe the clinical effect of fan-shaped decompression and al ograft fibula supporting internal fixation in treatment of early femoral head necrosis in adults, taking al ograft fibula grants as the control. METHODS:Forty patients with early femoral head necrosis were randomly divided into treatment group and control group, receiving fan-shaped decompression plus al ograft fibula supporting internal fixation and traditional decompression plus al ograft fibula grafting, respectively. The therapeutic effects in two groups were observed and compared. After treatment, patients were detected by bilateral hip anteroposterior films, Harris scoring and X-ray ARCO staging to evaluate the col apse severity and restoration of necrosis. RESULTS AND CONCLUSION:At the last fol ow-up, Harris scores in the treatment group were significantly higher and the repairing effect was better than control group (P<0.05). In treatment group, 18 hips restored wel (72%) and 7 hips delayed or failed to restore (28%);in control group, 9 hips restored wel (60%) and 6 hips delayed or failed to restore (40%). Our findings indicate that, fan-shaped decompression plus al ograft fibula supporting internal fixation yields a more complete decompression, a higher stability of femoral head and a more reliable supporting, compared with traditional decompression plus al ograft fibula grafting.
9.Treatment for erectile dysfunction patients with obstructive sleep apnea syndrome by nasal continual positive airway pressure.

OBJECTIVETo investigate the influence of nasal continual positive airway pressure (nCPAP) on the erectile function of erectile dysfunction (ED) patients with obstructive sleep apnea syndrome(OSAS).
METHODSTwenty-seven patients with ED and OSAS were randomly divided into treatment (n = 15) and control (n = 12) groups, the former treated by nCPAP from 22:00 to 6:00 per night for 1 month. And the apnea-hypopnea index(AHI), the lowest oxygen saturation (SaO2) and IIEF were compared before and after treatment.
RESULTSAHI, the lowest SaO2, erectile function and IIEF showed no significant difference before treatment in the two groups, but displayed a significant improvement after treatment in the treated group compared with the control (P > 0.05), in which no significant changes were observed before and after treatment (P > 0.05).
CONCLUSIONnCPAP therapy can improve the erectile function of ED patients with OSAS.

10.Clinical efficacy of Deanxit in non-erosive gastroesophageal reflux disease with anxiety and depres-sion

Objective To investigate clinical effects of Deanxit in non-erosive gastroesophageal re-flux disease ( NERD) with anxiety and depression.Methods A total of 76 patients diagnosed NERD with anxiety and depression by endoscopy and hospital anxiety and depression scale ( HAD) were randomly divid-ed into regular treatment group and Deanxit treatment group.Clinical efficacy, symptom score, and anxiety and depression score were observed after one week, four weeks, and eight weeks.Results Deanxit signifi-cantly increased the clinical efficacy after one week ( P <0.01) and remission rate after one week ( P <0.05), four and eight weeks ( P <0.01).It improved symptoms, depression, and anxiety, which showed significant differences ( P <0.01) compared to control group after one week, four and eight weeks adminis-tration.Conclusions Deanxit can be applied in NERD with anxiety and depression treatment, which has quick effect and high remission rate, and improve patient's reflux symptoms and anxiety depression.
