1.Effect of tocotrienol rich fraction of palm oil on glucose metabolism in atherosclerotic mice

AIM To investigate the effect of tocotrienol rich fraction of palm oil (TRF) on glucose metabolism in atherosclerotic mice and the possible mechanism. METHODS Apolipoprotein E gene deficient(ApoE~(-/-)) mice were divided into 3 groups as model control, TRF 0.05% and 0.2%(W/W) groups. 10% (W/W) fat and 0.2% (W/W) cholesterol were added into the diets to induce atherosclerosis formation. Oral glucose tolerance test and insulin tolerance test were conducted after mice were treated by TRF for 12 and 14 weeks respectively. Serum cholesterol, triglyceride, free fatty acid, and insulin levels were measured using corresponding kits. The mRNA expression levels for peroxisome proliferator-activated receptor γ(PPARγ), adiponectin and glucose transporter 4 (Glut4) in white adipose tissue (WAT) were determined by using quantitative real-time PCR. Activation of PPARγ by TRF was tested using luciferase reporter assays. RESULTS Compared with the model control group, TRF decreased non-fasting or fasting blood glucose levels and improved insulin sensitivity of ApoE~(-/-) mice. Both TRF groups showed decreased levels of triglyceride and free fatty acid. The mRNA level of adiponectin in WAT was up-regulated by (1.73±0.32) times in TRF 0.2% group compared with the control group. Glut4 mRNA level was increased (1.89±0.24) and (2.01±0.61) times compared with control group in TRF 0.05% group and TRF 0.2% group respectively. The fold inductions of TRF on PPARγ-ligand-binding domain, PPARγ1 and PPARγ2 activities were (2.7±0.2), (6.1±0.65) and (5.3±0.1) times compared with DMSO by using luciferase reporter assay. CONCLUSION TRF can improve glucose metabolism in atherosclerotic mice and this effect may be partly due to modulating the activity of PPARγ.
2.Schwannoma of submandibular gland: a case report.

A case of Schwannoma in the submandibular gland was reported which had been misdiagnosed as pleomorphic adenoma before operation. The tumor was originated from lingual nerve which in turn invaded the Schwann membrane near the submandibular ganglion.

3.A qualitative research on the feelings of nursing masters in initial stage of clinical practice

Objective To describe the feelings,career development needs and the coping styles of stress experience of nurses with master's degree within one year after graduation.Methods The semistructured in-depth interview was conducted in 15 nurses with master's degree individually.The data were analyzed by qualitative content analysis method and the themes were abstractod.Results Three themes were extracted,including feelings,their career development needs and coping styles of stress.The nurses with master's degree reported feelings of heavy work pressure; lack of a sense of accomplishment; strong sense of loss.Their career development needs included improvement of professional knowledge and clinical skills; organizational supports; improvement of professional English and received a doctor' s degree.There were three main coping styles of stress:adjusting attitude and self role orientation; competence improvement; adjusting interpersonal relationship.Conclusions Nursing masters suffer from heavy pressure in clinical practice within one year after graduation.They have some occupation development needs.Nursing managers should explore positive and effective methods to relieve the nurses' stress and ratioually use and actively guide nursing masters,and provide direction and space for achieving their occupation value.
4.Investigation of functional ankle instability in nurses and exploration of its preventive and protective methods

Objective To investigate the functional ankle instability in nurses,and provide references for increase preventive and protective awareness and establishment of preventive measures.Methods 100 nurses recruited from 10 wards of Provincial Hospital Affiliated to Shandong University were included into this study.Chinese version of the Cumberland Ankle Instability Tool (CAIT) was used to investigate the condition of functional ankle instability of these nurses.A self-designed questionnaire was used to investigate cognitive degree of ankle sprain prevention and treatment knowledge.Results Only 16% of these nurses had no unstable problems in both of their two ankles.There were functional ankle instable problems in either single or both of the two ankles of the other 84% nurses.And most of the nurses had little knowledge of how to prevent or treat ankle sprain properly.Conclusions There is a high rote of functional ankle instability in nurses.It is necessary to pay great attention to the prevention of ankle sprain and instability in nurses,which is of great importance to health and occupational safety assurance of nurses.
5.Pasteurization affects the physicochemical properties and biological function of hemoglobin?

BACKGROUND:Pasteurization is a perfect method for albumin virus inactivation, which may not be required for virus inactivation validation. However, there are no systematical reports concerning virus inactivation of hemoglobin blood substitutes. OBJECTIVE:To explore the effects of pasteurization on the physicochemical properties and biological function of hemoglobin blood substitutes. METHODS:Appropriate cord blood samples were taken fol owed by centrifugation, washing blood, rupture of membranes, stabilizer treatment. In the control group, the samples were placed in 55℃water bath, and when the temperature of hemoglobin solution reached (55±1)℃, a heat treatment began and lasted for 2 hours. In the pasteurization group, the samples were placed in 60℃water bath, and when the temperature of hemoglobin solution reached (60±1)℃, a heat treatment began and lasted for 10 hours. The heating process was under continues nitrogen protection. Then, the hemoglobin solution was placed in ice bath and cooled to below 4℃fol owed by low-speed centrifugation and filtration via microporous membrane, purification and viral inactivation thereby to obtain cord blood hemoglobin. RESULTS AND CONCLUSION:The products in the pasteurization group were al red clear liquid. There was no significant difference between the two groups in the yield, methemoglobin concentration, and oxygen-carrying capacity. The purification of the two groups was more than 98%. Two kinds of purification methods had no effects on the oxygen-carrying capacity of hemoglobin. Therefore, pasteurization method can replace thermosensitive purification method of 55℃, 2 hours. The pasteurization method wil not only ensure the physicochemical and biological properties of hemoglobin, but also achieve the purpose of virus inactivation.
6.Effect of high risk human papillomavirus test in screening of the patients with ASCUS

Objective To explore the clinical value of high risk human papillomavirus(HR-HPV) test in shunting monitoring of atypical squamous cell of undetermined significance (ASCUS) and to seek the best treatment for patients with ASCUS.Methods 470 patients with ASCUS tested by TCT were given HR-HPV testing and biopsy under colposcopy.The clinical characteristics were analyzed.Results 470 cases of ASCUS contained a variety of cervical lesions:inflammation/acuminate accounted for 69.15% (325/470),cervical intraepithelial neoplasia (CIN)accounted for 29.57% (139/470),and invasive carcinoma accounted for 1.28% (6/470).The positive rate of cervical pathological examination was 30.85% (≥ CIN Ⅰ,145/470).The positive rate of HR-HPV was 50.43%(237/470).The detection rate of ≥CIN Ⅰ in HR-HPV positive group and negative group was 55.27% (131/237)and 6.01% (14/233),while the detection rate of ≥ CIN Ⅱ in HR-HPV positive group and negative group was 36.71% (87/237) and 1.29% (3/233).The differences were statistically significant(P < 0.01).Incidence of ≥ CIN Ⅰ and invasive carcinoma in HR-HPV positive group was about 19.332 fold of that in HR-HPV negative group(95% CI =10.632 ~ 35.152),while the incidence of ≥CIN Ⅱ and invasive carcinoma was about 44.467 fold of HR-HPV negative group (95% CI =13.812 ~ 143.152).The detection sensitivity,specificity,positive predictive value,negative predictive value(NPV) of HR-HPV in the≥ CIN Ⅰ patients were 90.34%,67.38%,61.60%,93.99%.Those of ≥ CIN Ⅱ patients were 96.67%,60.53%,36.71%,98.71%.Conclusion ASCUS included partial CIN of high grades and cervical cancer,and the proportion of HPV infection is large,detection of high risk HPV-DNA is an effective shunting management method of ASCUS.
7.Clinical study of two kinds of operation method treatment of recurrent pterygium

Objective To compare the therapeutic effects of recurrent pterygium treated by with corneal limbal autograft con -junctival transplantation combined with mitomycin C and amniotic membrane transplantation combined with mitomycin C .Methods A total of 153 eyes of 153 cases with recurrent pterygium were divided into 2 groups ,A group :97 eyes who treated with amniotic membrane transplantation combined with mitomycin C ,B group :56 eyes treated with corneal limbal autograft conjunctival trans -plantation combined with mitomycin C ,follow up 12 months after operation .Results 12 eyes had recurrence in A group ,the recur-rent rate was 12 .37% ;5 eyes had recurrence in B group ,the recurrent rate was 8 .93% ,there was no significant different between 2 groups(P> 0 .05) .Conclusion This study indicates amniotic membrane transplantation or corneal limbal autograft conjunctival transplantation combined with mitomycin C can effectively reduce the recurrent rate of recurrent pterygium ,which are safe and ef-fective method .
8.Investigation on the cognition about organ donation of ICU medical staff in 3A hospital in Chongqing

Objective To investigate the cognition and participation on organ donation in ICU medical staff and analyze the influencing factors, thus to provide reference for the development organ donation promotion measures. Methods 265 ICU medical staff were chosen by convenient sampling method from 10 three-level hospitals in Chongqing and were investigated with a self-designed questionnaire about the cognition, willingness and its influencing factors towards organ donation. Results ICU medical staff′s understanding of brain death, significance, basic conditions and concepts of organ donation were 67.2% (178/265), 61.1% (162/265), 60.0% (159/265), 55.8% (148/265) respectively, but understanding of Principles , registration methods, laws and regulations, and procedures of organ donation were 24.5%(65/265), 12.1%(32/265), 7.9%(21/265), 4.5%(12/265) respectively. 47.5%(126/265) ICU medical staff were willing to donate their own organs, 35.8%(95/265) were consented to relative′s organ donation and 4.5% (12/265) were organ donation applications. 89.1% (236/265) were affected by traditional ethical concepts, 70.2%(186/265) were for the publicity not enough, 51.7%(137/265) did not know about the procedures of organ donation, 37.0%(98/265) were for too much troubled procedures, and 43.8%(116/265) were for the ineffective work of relevant organizations. Conclusions Lacking in-depth understanding of organ donation and low willingness to donate organs in ICU medical staff in Chongqing. Mainly associated with the effect of traditional concept and lack of propaganda to the donation.
9.The management of laryngotracheal defect derived from thyroid gland papillary carcinoma resection.

OBJECTIVE:
To investigate the preservation of the structure and function of the trachea and larynx, the management of laryngotracheal defect when trachea and larynx was involved.
METHOD:
To review the management and clinical results of 13 cases of thyroid papillary carcinoma with larynx and trachea involvement, the preservation of laryngotracheal structure and relative defect reconstruction of our department from 2007-2014. Those patients being performed total laryngectomy was excluded. 3 males and 10 females, aged from 46 to 67 years old with median age of 53 were included. Among them 8 cases were recurrent. The extent of the tumor foci was estimated with the help of computed tomography and laryngofiberoscope before surgery. Selective neck dissection, total thyroidectomy and related laryngotracheal resection was performed for the first time operation patient, while selective neck dissection, recurrent foci and related larynx and trachea resection was performed for those recurrent patients. Three kinds of modalities were applied to manage the laryngotracheal defect including to reconstruct with pedicled sternocleidomastoid clavicular periosteocutaneous flap, pedicled trapizius muscular flap and to preserve the remaining larynx and trachea and perform a stoma of larynx and trachea which repaired by a second-stage procedure. The patients were followed-up from half an year to 3 years.
RESULT:
Ten patients out of 13 decannulated while another 3 cases, 2 of which were performed local flap to reduce the stoma, wore tracheal tubes all time.
CONCLUSION
Either flap transfer or laryngotracheal stoma before second stage repair might preserve partial laryngotacheal anatomy and function in selected cases thus improve the life quality of the patients.

10.Pathogenic Bacteria in Respiratory Infection in Newborns:Their Distribution and Drug Resistance

OBJECTIVE To investigate the distribution and drug resistance of pathogenic bacteria which caused respiratory infection among suscepted patients and offer scientific basis for reasonable usage of antibiotics.METHODS Oropharyngeal swabs among 709 cases of respiratory infection neonates were investigated by the routine methods and drug resistance was analyzed by K-B method.RESULTS Totally 438 bacterial strains were isolated from 709 neonates.most of these bacteria were Gram-negative bacilli(70.3%),among which Haemophilus influenzae and Klebsiella pneumoniae were accounted for 39.8% and 10.3%,respectively;fungi and Gram-positive cocci were accounted for 23.5% and 6.2%.CONCLUSIONS Most strains present higher resistance rates to penicillin and ampicillin;but cefoxitin,amikacin,vancomycin,imipenem and the third generation cephalosporins are revealed with higher sensitivity rates for pathogenic bacteria in newborns.
