1.A survey of the cognition of nursing teachers and students on the integration of clinical courses in the context of nursing professional certification

Objective:To explore the cognition of nursing undergraduates and teachers on the integration of clinical curriculums under the background of nursing professional certification.Methods:With the Dundee Ready Education Environment Measure (DREEM) translated by the center for education and research of China Medical University as reference, two self-made questionnaires were used for evaluating the integrated nursing clinical curriculum (student and teacher Editions), which included self-evaluation of teachers or students, assessment of practice teaching environment, classroom harvest and self-learning, learning attitude and emotions etc. From May to July 2017, 300 questionnaires were given to300 nursing undergraduates from grades 2013, 2014, 2015 of, and 295 copies were returned. 60 questionnaires were given to 30 nursing teachers who gave clinical practice teaching and 30 medical nursing teachers wwo teach only, and 58 copies were collected. SPSS 21.0 software was used for t-test, difference analysis and linear correlation description. Results:The results of the survey showed that ①the scores of the items of "save time and energy", "the satisfaction with curriculum integration" and "reduce learning burden" in the survey were higher than those of other items. ②The scores of "the students' adaptation degree", "interns' adaptation degree" and "the mastery of the content of experiment course" were higher than those of other items in the investigation of the clinical teaching teachers. The three items with top scores evaluated by professional nursing teachers who teach only in medical college and universities were "interns' adaptation degree", "the mastery of the teaching content" and "the amount of information of the teaching content". ③There were significant differences in the average scores of the items of students from different grade ( F=6.648, P=0.002), with the increase of grade, the average score was higher with higher linear correlation ( r=0.202, P=0.005). ④There was statistically significant difference in the cognition of clinical course integration of nursing teachers who gave clinical practice teaching and medical nursing teachers who teach only ( P<0.05). Conclusion:Based on the background of nursing professional certification, nursing undergraduates care about the reduction of time, energy and other learning-related factors brought by curriculum integration, while nursing teachers pay more attention to the application of clinical curriculum integration.

Autoimmune pancreatitis (AIP) is an autoimmune-mediated abnormal chronic inflammatory disorder and is often misdiagnosed as pancreatic neoplastic lesions. With in-depth studies of this disease in recent years, it has been taken seriously by hepatobiliary physicians and surgeons. This article summarizes the clinical features, diagnostic criteria, and treatment methods for autoimmune pancreatitis at the present stage, so as to provide clinicians with diagnosis and treatment experience to reduce clinical misdiagnosis.
3.Prognostic factors of sudden sensorineural hearing loss in children.

OBJECTIVE:
The aim of this retrospective study was to analyze the recovery rate of sudden sensorineural hearing loss in children, and explore the prognostic factors in order to guide the clinical diagnosis and treatment.
METHOD:
A retrospective review was conducted for the prognosis of children with sudden sensorineural hearing loss during the past 5 years (from November 2010 to May 2015) in Chinese PLA General Hospital. This paper have a complete clinical data of 101 patients (113 ears)with sudden hearing loss, ranging from 0 to 18 years old Patients were divided into four groups according to hearing recovery and eight putative prognostic factors were analyzed.
RESULT:
Among 101 patients (113 ears), the ratio of male and female was 60:53. Treatment was initiated from 1 to 183 days after disease onset, with an average of (18.5 ± 22.1) d. Bilateral and unilateral hearing loss were 24 ears and 89 ears, respectively. The proportion of mild hearing loss, moderate hearing loss, severe hearing loss and profound hearing loss were 7.1%, 6.2%, 23.9% and 62.8%, respectively. Vertigo and tinnitus occurred in 54.9% and 77.9% of the patients, respectively. After the treatment, the complete recovery rate was 9.7% and the overall recovery rate was 36.3%. The degree of hearing loss, earlier treatment onset, sex and bilateral involvement were significantly associated with hearing recovery (P < 0.05).
CONCLUSION
Sudden sensorineural hearing loss in children was generally identified as severe and profound hearing loss, but after positive and timely treatment, it can be improved or even cured. The mild hearing loss, earlier treatment onset, unilateral hearing loss and female were positive prognostic factors. The concurrence of tinnitus or vertigo, the results of ABR and DPOAE had no significant influence on prognosis.

4.Application of structured hypothermia intervention program in elderly intestinal cancer patients with frailty

Objective? To explore the effects of structured hypothermia intervention program in elderly intestinal cancer patients with frailty. Methods? From November 2017 to July 2018, we selected 96 patients with surgery for intestinal cancer scored more than two points in Fried frailty assessment for the aged before surgery at the First Affiliated Hospital of Hu'nan Normal University & People's Hospital of Hu'nan Province by cluster sampling. All of the patients were divided into experimental group (n=48) and control group (n=48) with the method of random number table. Experimental group carried out the structured management model, formulated and implemented the structured hypothermia intervention program, while control group provided warm nursing based on nursing routine. We compared the respiration, blood pressure, heart rate and blood oxygen saturation before, during and after surgery as well as temperature, incidence of hypothermia and related indicators (extubation time, retention time in resuscitation room, urine volume and intraoperative blood loss) at each time points during surgery. Results were analyzed with the repeated measure variance analysis, t test and χ2 test. Results? There were interactions between time and groups in temperature, blood pressure, heart rate, respiration and blood oxygen saturation (P< 0.05). The fluctuation with time of the above vital signs in the experimental group was lower than that of control group. In experimental group, the incidence of hypothermia was lower than that in control group; the extubation time and retention time in resuscitation room were shorter than those in control group; the intraoperative blood loss was less than that in control group and the urine volume was more than that in control group;the differences were all statistically significant (P< 0.01). Conclusions? The application of structured hypothermia intervention program in elderly intestinal cancer patients with frailty can effectively maintain the stable intraoperative vital signs, shorten the time anesthesia resuscitation, reduce the incidences of hypothermia and related complications, standardize preventive measures for intraoperative hypothermia and guarantee patient intraoperative safety among elderly intestinal cancer patients with frailty.
5.Study on the mechanism of a pyroptosis of renal podocytes in hypoxia and high glucose environment

Objective :
To investigate the effects of hypoxia , high glucose single factor and hypoxia high glucose
compound factors on the pyroptosis of rat glomerular podocytes .
Methods :
Rat glomerular podocytes were cultured in vitro and randomly divided into control group , high glucose group , hypoxia group and hypoxia high glucose group , EdU method was used to detect the cell proliferation , transmission electron microscope was used to observe the morphology and size changes of nucleus and mitochondria , and Western blot was used to detect pyroptosis related proteins nucleotide⁃binding oligomerization domain⁃like receptor protein 3(NLRP3) , Cysteinyl aspartate specific proteinase⁃1(Caspase⁃1) , gasdermin( GSDMD) and inflammatory factor pro⁃interleukin⁃1β( Pro⁃IL⁃1β) , interleukin(IL) Ⅳ1β , IL⁃18 . The effect of hypoxia and high glucose on the pyroptosis of rat glomerular podocytes was analyzed .
Results :
EdU results showed that hypoxia and high glucose inhibited the proliferation ability of rat glomerlar podocytes (P < 0. 05) . The results of transmission electron microscopy suggested that hypoxia and high glucose promoted the occurrence of pyroptosis of rat glomerular podocytes . Western blot showed that hypoxia and high glucose promoted pyroptosis of rat glomerular podocytes , and increased the expression of pyroptosis related proteins NLRP3 , Caspase⁃1 and GSDMD , among which the expression of pyroptosis protein increased most significantly in hypoxia and high glucose group (P < 0. 05) . At the same time , it also increased the expression of pro⁃inflammatory factor Pro⁃IL⁃1β , IL⁃1β and IL⁃18 (P < 0. 05) .
Conclusion
hypoxia and high glucose can induce pyroptosis of rat glomerular podocytes , one of the mechanisms may be through affecting NLRP3 ⁃Caspase⁃1 ⁃GSDMD and its down⁃ stream inflammatory factors .
6.Effect of exosomes derived from Echinococcus multilocularis on macrophage polarization: A preliminary study

Objective To investigate the effect of exosomes derived from Echinococcus multilocularis on macrophage polarization after treatment for different durations and concentrations. Methods A total of 60 BALB/c mice were used for modeling, among which 4 mice were selected to observe the growth of abdominal lesions on 7.0T MRI. The mice for modeling were dissected, and the protoscoleces was taken from the abdominal lesion and cultured in vitro ; ultracentrifugation was used to extract the exosomes from the supernatant, and transmission electron microscopy and Western blotting were used for the characterization of exosomes. The macrophages without exosome treatment were established as control group, and the macrophages co-cultured with different concentrations of exosomes derived from Echinococcus multilocularis were established as experimental group (10 μg/mL group and 50 μg/mL group) and were cultured for 48 and 72 hours. The morphological changes of macrophages were observed under a microscope, and flow cytometry and ELISA were used to observe polarization state. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t -test was used for further comparison between two groups. Results The results of 7.0T MRI showed the formation of diffuse lesions with different sizes in the abdominal cavity of mice, and the exosomes derived from Echinococcus multilocularis were approximately 100 nm in diameter and were cup-shaped or saucer-shaped, with the positive expression of the surface markers CD9, TSG101, and CD63. After co-culture, most of the cells in the experimental group were elongated with an irregular and polygonal shape. Flow cytometry showed that after 48 hours of co-culture, the positive rates of CD16/32, CD206, and CD369 in the control group were 99.53%±0.06%, 90.27%±0.21%, and 2.40%±0.20%, respectively; compared with the control group, except that the 10 μg/mL exosome group had a significant reduction in the positive rate of CD369 (0.80%±0.00%) ( P < 0.05), all the other groups had a significant increase in the positive rates of CD16/32, CD206, and CD369 (all P < 0.000 1); after 72 hours of co-culture, the positive rates of CD16/32, CD206, and CD369 in the control group were 99.67%±0.06%, 85.47%±0.55%, and 6.60%±0.20%, respectively, and compared with the control group, the experimental group had significant increases in the positive rates of CD16/32, CD206, and CD369 (all P < 0.05). ELISA showed that after 48 hours of co-culture, the levels of IL-6 and TNFα in the control group were 58.53±15.52 pg/mL and 320.70±5.30 pg/mL, respectively, and when the exosome concentration was 50 μg/mL, the level of IL-6 in the experimental group was 98.81±15.55 pg/mL, which was higher than that in the control group ( P < 0.05); after 72 hours of co-culture, the levels of IL-6 and TNFα in the control group were 76.22±9.68 pg/mL and 323.90±87.37 pg/mL, respectively, and when the exosome concentration was 10 μg/mL, the level of TNFα was 164.20±14.17 pg/mL, which was significantly lower than that in the control group ( P < 0.05); when the exosome concentration was 50 μg/mL, the level of IL-6 was 99.52±8.35 pg/mL, which was significantly higher than that in the control group ( P < 0.05). Conclusion Exosomes derived from Echinococcus multilocularis can regulate macrophage polarization and induce M2-like polarization of macrophages after co-culture at a concentration of 10 μg /mL for 72 hours, and further studies are needed to clarify the specific method.
