1.ANALYSIS OF EPIDEMIC CHANGES IN THE EPIDEMIOLOGICAL OBSERVATION POINTS OF SCHISTOSOMIASIS IN HUBEI PROVINCE FROM 1990 TO 1998

Objective To master the trend of schistosomiasis epidemic by means of established observation points and give the scientific reference for the control decision making henceforth. Methods Three villages of schistosomiasis with highly prevalence in Hubei Province were selected as observation points, and their epidemiological status were observed from 1990-1998. Results The schistosomiasis prevalence was considerably decreasing in the 3 villages, from high epidemic areas into medium or light areas. Conclusion It is a right tactic to use the selective population chemotherapy and control snail in high transmission areas of schistosomiasis in high epidemic areas.
2.Effect of mild hypothermia on the expression of the inducible HSP70 in canine brain tissue after globai cerebral ischemia reperfusion

Objective:To study the mechnism of mild hypothermia during cerebral resuscitation. Method:Fourteen dogs were anaesthetized with ketamine and fentanyl and were divided randomly into three groups. Four dogs as nonischemic control(group A).recived general care for 24 hours after operation without cardiac arrest. The other ten dogs underwant a 18-minute cardiac arrest.were resuscitated with standard CPR.and supported by intensive care for 24 hours,then divided into two groups. Five dogs were treated with routine cerebral resuscitation measures (RCRM) (group B):five dogs were treated with RCRM and mild hypothermia started 10 min after CPR and mantained at 34 C?0.5 C for 12 hours (group C). At the end of experiment,the parietal cerebral cortex was obtained to detect the expression of heat shock proteins with immunohistochemical technique and to observe the change of histomorphology. Result: The pathologic change in group C was more obviously mitigated than that in group B. 3.Hospital Onset of Lower Respiratory Infection and Drug Resistance:Analysis of 141 cases

OBJECTIVE To investigate the epidemiological characteristic of lower respiratory infection among the hospitalized patients in Tianlin Community Health Service Center so as to take effective measures to control the infection and reduce the emergence of drug resistant strains.METHODS Totally 6150 cases of patients discharged from our Center during from Jan 2004 to Sept 2006 were collected and among them 141 cases occurred hospital onset of lower respiratory infection and drug resistance was analyzed through contrasting.RESULTS The incidence rate of hospital onset of lower respiratory infection and drug resistance were 2.29% and 58.62%.Among the 141 cases,the incidence rate of ESBLs was 28.36% which mainly produced from Escherichia coli and Klebsiella pneumoniae.CONCLUSIONS Primary affection should actively treated and antibiotics should use rationally according to the drug sensitive cultivation to control the abuse of antibiotics and reduce the emergence of drug resistant strains.
4.STUDY ON INCREASING MOLLUSCICIDAL EFFECT BY COMPLEX NICOTINANILIDE WITH NICLOSAMIDE

Objective To explore the increment of molluscicidal effect and reduction of the cost as well as increasing snail control area by complex commercial molluscicides. Methods Nicotinanilide and niclosamide were combined with the proportion of 1∶1 (The complex of short term was complex molluscicide), and then the snail control tests by immersion and spraying methods were performed using the complex molluscicide in laboratory and field. Meanwhile, the snails climbing from water as they contacted with molluscicide was observed. Results Under the temperature 23-25℃ in lab and 8-19℃ in the field, 0.4 mg/L of the complex by immersion for 48 h, the mortalities of the snails were 100% and 97% respectively. Using 0.4 g/m 2 of complex by spraying for 48 h in lab and the field, the mortalities of the snails were 100% and 81.1% respectively. To investigate the distribution characteristics and linkage disequilibrium of T cell immunoglobulin domain and mucin domain protein 4 (TIM4) promoter polymorphisms in asthma patients of Chinese Han population, the promoter region of TIM4 was re-sequenced by PCR-sequencing, and linkage disequilibrium was analyzed by SHEsis software. Four single nucleotide polymorphisms (SNPs) in the promoter region of TIM4 were detected, including two new SNPs (at positions-1609,-153) and two reported SNPs (rs6874202, rs6882076). The frequency distribution of rs6882076 was different among different races (P<0.05). In addition, linkage disequilibrium among the SNPs of the promoter region of TIM4 was found and GGTG was the predominant haplotype. There were four SNPs in the promoter region of TIM4 in asthma patients of Chinese Han population, which were in linkage disequilibrium.
6.MR spectroscopy for differentiating benign from malignant solid adnexal tumors

Objective To investigate the ability of proton magnetic resonance spectroscopy (1H-MRS) for differentiating benign from malignant solid adnexal tumors. Methods One-hundred and six patients (114 tumors) with surgically and histologically proven solid adnexal tumors (44 benign, 70 malignant) underwent conventional MR imaging and 1H-MRS. Single-voxel spectroscopy was performed using the point resolved spectroscopy localization technique with a voxel size of 2.0 cm × 2.0 cm × 2.0 cm. Resonance peak integrals of choline (Cho), N-acetyl aspartate (NAA), creatine (Cr), lactate (Lac), and lipid (Lip) were analyzed and the Cho/Cr, NAA/Cr, Lac/Cr and Lip/Cr ratios were recorded and compared between benign and malignant tumors using independent two-sample t test. Receiver operating characteristic (ROC) curve analysis was used to evaluate the diagnostic performance of Cho/Cr ratio for differentiating benign from malignant tumors. Results A Cho peak was detected in all 114 tumors, NAA peak in 112 tumors (43 benign and 69 malignant), Lip peak in 70 tumors (21 benign and 49 malignant), and Lac peak in 16 tumors (7 benign and 9 malignant). The Cho/Cr and Lip/Cr ratios were 4.8±2.5, 6.4±4.0 in benign versus 9.6 ± 3.3, 10.5 ± 4.6 in malignant solid adnexal tumors, respectively, with a statistically significant difference (t values were-8.826 and-2.915,P<0.05). The NAA/Cr ratio were 1.4 ± 0.7 in benign versus 1.6 ± 1.0 in
malignant solid adnexal tumors, with no statistically significant difference (t=-1.523,P>0.05). When the Cho/Cr threshold was 7.2 for differentiating between benign and malignant tumors, the sensitivity, specificity, accuracy were 80.0%(56/70),88.6%(39/44) and 83.3%(95/114) respectively. Conclusions The 1H-MRS patterns of benign and malignant solid adnexal tumors differ. The Cho/Cr ratio can help clinicians differentiate benign from malignant tumors.
7.Study of optimization for the cream base of benzhydramine hydroehloride

Objective To optimize the cream base of benzhydramine hydroehloride .Methods Five nonionic surfactants as an emulsifier were used to prepare benzhydramine hydroehloride creams .The five sorts of cream were evaluated from the ap-pearance ,preliminary stability ,accelerated test stability and room temperature stability .Results Benzhydramine hydroehloride cream could be prepared using five nonionic surfactants .On appearance of smoothness ,gloss and spread ,formula E was best but formula A was worst ,while formulas B and D were close and worse than formula E only .In the stability tests ,all of creams had no obvious change after the centrifugal experiment and the low temperature experiment .In the high temperature ex-periment ,the formula E was stable ,formulas A ,B and D showed a little appearance change ,and the formula C became layered bleeding .In room temperature ,the formula C became bleeding after five days ,which happened to the formula A after one month ,and formula B after two months .The formulas D and E had no obvious change until six months in room temperature . Conclusion The formulas D and E were stable ,which had clinical application value .
8.Association of insulin resistance with spontaneous recanalization of infarct-related arteries in patients with a-cute myocardial infarction

Objective To clarify the role of insulin resistance on spontaneous recanalization of infarct-relat-ed arteries in the early phase of acute ST-elevation myocardial infarction (STEMI) in patients with normal glucose tolerance. Methods 141 consecutive patients with normal glucose tolerance and acute STEMI were enrolled in our study. Subjects were divided into TIMI 0-1 group (n =91 ) and TIMI 2-3 group (n =50) by primary coronary angi-ngraphy (CAG). The Gemini score and 0-3-vessel disease score estimated the severity and extent of coronary artery disease (CAD). Metabolic parameters and homeostasis model assessment for insulin resistance (IRI) were deter-mined. Results Serum level of fasting insulin, IRI and Gemini score were higher in TIMI 0-1 group than in TIMI 2-3 group [ (11.52±6.22)mU/L vs (7.54±3.65)mU/l,(2.79±2.32) vs (1.73±1.26),(59.17±26.95) vs ( 38.46±22.74) ( P <0.01)]. IRI was positively associated with Gemini score (r=0.185,P <0.05 ). Multivariate Logistic regression analysis revealed that IRI was independent risk factor influencing spontaneous recanalization of in-farct-related urteries(OR=2.87,95% CI=1.09-7.57,P<0.05). Conclusion Insulin resistance is independent risk factor influencing spontaneous recanalizafion of infarct-related arteries in the early phase of acute STEMI in pa-tients with normal glucose tolerance.
9.Effects of acupuncture, combined with phacoemulsification for cataract with glaucoma.

OBJECTIVE:
To observe the effects of acupuncture combined with phacoemulsification for cataract with glaucoma.
METHODS:
Sixty-eight patients with cataract and angle-closure glaucoma were randomly divided into an observation group and a control group, 34 cases in each one. The patients in the control group were treated with phacoemulsification. Based on the treatment in the control group, the patients in the observation group were treated with and acupuncture at acupoints around eyes; the treatment was given once every other day, and 15-day treatment constituted a course, and 2 courses (60 days) were given. The intraocular pressure (IOP), visual acuity, pain severity, aqueous outflow (F value) and clinical symptoms were observed in the two groups before treatment and after 1 d, 7 d, 30 d and 60 d of treatment.
RESULTS:
The IOP after 1 d, 7 d, 30 d and 60 d of treatment in the observation group was lower than that in the control group (all <0.05), while the visual acuity after 7 d, 30 d and 60 d of treatment in the observation group was higher than that in the control group (all <0.05). Compared before treatment, the IOP was significantly decreased after 1 d, 7 d, 30 d and 60 d of treatment in the two groups, and the visual acuity was significantly increased after 7 d, 30 d and 60 d of treatment in the two groups (all <0.05). The pain scores and F value after 7 d, 30 d and 60 d of treatment in the observation group were lower than those in the control group (all <0.05). Compared before treatment, the pain scores and F value after 7 d, 30 d and 60 d of treatment were significantly reduced in the two groups (all <0.05). After 60 d of treatment, the number of patients who presented general clinical symptoms in the observation group was significantly lower than that in the control group (<0.05).
CONCLUSION
Based on phacoemulsification, acupuncture and could significantly reduce the IOP and F value, relieve pain and improve visual acuity in patients with cataract and glaucoma.
