1.Experimental study:the effect of dexamethasone on the adult osteoblast proliferation and differentiation

Objective To observe and analyze the effect of dexamethasone on the adult osteoblast proliferation and differentiation in vitro. Methods Iliac trabecular bone specimens were obtained from eight adult patients undergoing scheduled surgery. After the bone pieces were treated with collagenase trypsin, larage numbers of osteoblasts were released out. Then the osteoblasts were purified and their passages were fed with DMED-F12 and fetal bovine serum. The cells were incubated under the condition of 37 ℃, 95% relative humidity and 5% CO2. Subsequently, 1?10-8 mol/L dexamethasone were added into culture medium to feed eight groups of osteoblasts for three days and the control groups were cultured not to add medicine at the same time. All the groups were observed continually under light microscope and transmission electron microscope. Finally, apoptosis detection was performed by use of the terminal deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL) and biochemical indexes alkaline phosphatase (ALP) and osteocalcin (OCN), were assayed to check if dexamethasone has any effect on proliferation and differentiation of osteoblasts in vitro. Results Of the adult osteoblasts obtained by collagenase trypsin method, the survival condition was good, the biochemical indexes were stable, the degree of purification was excellent, and they were fit for the next step study. The following data demonstrated that under the condition of dexamethasone 1?10-8 mol/L and osteoblasts 10 000 /ml, there were significant increase of ALP and OCN secretion without apoptosis alteration. Conclusion Dexamethasone has significant promoting effect on proliferation and differentiation of adult osteoblasts in vitro without apoptosis alteration and could be used as positive control during screening research of osteoporosis.
2.A randomized controlled comparison of efficacy of 4 and 8 weeks lansoprazole treatment of gastric ulcer induced by endoscopic submucosal dissection

Objective To compare the efficacy between the treatment courses of 4 weeks and 8 weeks of lansoprazole in the treatment of endoscopic mucosal stripping ( ESD ) induced iatrogenic gastric ulcer.Methods 84 gastric adenoma or early gastric cancer patients were randomly divided into two groups after lansoprazole(30 mg/day) treatment for 4 weeks and 8 weeks.After 8 weeks of ESD, the efficacy between two groups were compared.Results 69 patients from the total 84 patients were included into the final analysis, 34 patents in 4 week-treatment group and 35 patients in 8 week-treatment group.There was no significant difference between the two groups of the ulcer stage(68% in the scar stage of 4 week-treatment group and 69% in the scar stage of 8 week-treatment group, P=0.93 ).There was no significant difference between the two groups of ulcer reduction radio (0.0081 ±0.015 in 4 week-treatment group and 0.0037 ±0.008 in 8 week-treatment group, P=0.15) 8 weeks after ESD.There was no difference in scar stage and ulcer reduction radio between 4 week-treatment group and 8 week-treatment group of patients with large ulcers ( >30 mm ) . Conclusion For ESD-induced gastric ulcer, there was no significant difference in efficacy of lansoprazole between 4 week-treatment and 8 week-treatment.It may be sufficient for proton pump inhibitors only 4 weeks’ treatment in ESD-induced gastric ulcers.
3.Mechanism of β-catenin/Akt signaling pathway in apoptosis of gastric cancer cells induced by selenite sodium

Objective To explore the mechanism of β-catenin/Akt signaling pathway in the apoptosis of gastric cancer cells induced by selenite sodium.Methods Gastric cancer cell line SGC-7901 was cultured, and 0 mol/L, 5 mol/L, 10 mol/L and 20 mol/L selenite sodium treated cells; flow cytometry was used to detect the cell apoptosis rate after 24 h;10 mol/L sodium selenite treated cells, flow cytometry was used to detect the cell cycle changes after 24 h;10 mol/L sodium selenite treated cells,protein expression ofβ-catenin,cyclin D1 and protein kinase B ( Akt) activity were detected by Western blot after 0,6,12,24 h.Results Cells apoptosis rate was significantly higher than 0 mol/L after cells was treated by 5 mol/L, 10 mol/L and 20 mol/L sodium selenite, due to cells apoptosis rate was higher by 10 mol/L sodium selenite treated than 5 mol/L and 20 mol/L, 10 mol/L sodium selenite treated cells for follow-up study;gastric cancer cells was treated by 10 mol/L sodium selenite for 24 h, compared with the control group, G0/G1 phase cells decreased, cells in S phase and G2/M phase significantly increased(P<0.05);gastric cancer cells was treated by 10 mol/L sodium selenite for 0 h, 6 h, 12 h and 24 h,protein expression of β-catenin and cyclin D1 and Survivin in 6 h, 12 h and 24 h was significantly lower than that in 0 hour (P<0.01),and with the extension of time, protein expression gradually decreased;gastric cancer cells was treated by 10 mol/L sodium selenite could significantly decrease the phosphorylation of p-Akt in 6,12,24 h, while there was no significant difference of Akt among difference time points( P <0.01).Conclusion Sodium selenite could induce apoptosis of gastric cancer cells, and the cells were arrested in S phase.The mechanism may be associated with beta-catenin/Akt signaling pathway.
4.miR-146a expression and its effect on the biological characteristics in gastric carcinoma

Objective To explore the expression of miR-146a and its effect on the biological characteristics in gastric carcinoma. Methods The expressions of miR-146a in gastric cancer tissue and carcinoma adjacent tissue were detected by RT-qPCR , and human gastric cancer cell line MKN-28, SGC-7901, MKN-45 and immortalized gastric epithelial cell GES-1 were cultured,GES-1 as a reference, the expressions of miR-146a in each cell line was detected by RT-qPCR; miR-146a mimics and miR-146a control were transfected into gastric cancer cell line MKN-45, and the expression of miR-146a was detected by RT-qPCR;CCK8 was used to detect the effect of miR-146a on the proliferation of cells;flow cytometry was used to detect the effect of miR-146a on cell apoptosis. Western blot was used to detect the expressions of the apoptosis-related proteins. Results The expression of miR-146a in gastric cancer tissue was significantly lower than carcinoma adjacent tissue(P<0.01); expression of miR-146a in gastric cancer cell MKN-45 was the lowest campared to GES-1, so the gastric cancer cell MKN-45 was selected as a follow-up study. The expression of miR-146a in miR-146a mimics group was significantly higher than control group(P<0.01); CCK8results showed that the proliferation rateof miR-146a mimics group was significantly lower thanmiR-146a control on 24 hour (P<0.05),and was significantly lower than miR-146a control on 48 hour and 72 hour (P<0.01).The results of flow cytometry showed that the apoptosis rate in miR-146a mimics group was significantly higher than miR-146a control;Western blot showed that the expressions of Cleaved-caspase-3 and Bax protein in miR-146a mimics group was significantly lower than those in miR-146a control, and Bcl-2 protein expression was significantly higher than miR-146a control (P< 0.05). Conclusion The expression of miR-146a in gastric carcinoma was lower than carcinoma adjacent tissue, and the miR-146a could inhibit cell proliferation and promote cell apoptosis after transfected.
5.Comparison of X-ray film and MRI in the diagnosis of posterior ankle impingement syndrome

Objective To compare the diagnostic value of X-ray film and MRI in the posterior ankle impingement syndrome (PAIS).Methods X-ray film and MRI data of 72 cases with posterior ankle pain were analyzed retrospectively.Passive plantar flexion test and diagnostic blocking were used as the diagnostic criteria.72 patients with posterior ankle pain were divided into PAIS group and non PAIS group.X-ray film and MRI findings in the PAIS group and non PAIS group were compared,and features which had differential diagnostic value were selected for calculating and comparing the efficacy of X-ray film and MRI in diagnosing or excluding PAIS.Results There was no significant difference for X-ray film findings such as os trigonum osteosclerosis(χ2 =2.947,P =0.086),os trigonum cystic changes(χ2 = 3.031,P =0.082)and posterior ankle soft tissue swelling(χ2 = 1.610,P =0.205 )between the PAIS group and the non PAIS group.There was significant difference for MRI features such as os trigonum or posterior talus bone marrow edema(χ2 =38.868,P =0.000 ),edema around os trigonum(χ2 =39.919,P =0.000 )and tenosynovitis of the flexor hallucis longus (χ2 =8.854,P =0.003)between the PAIS group and the non PAIS group.There was no significant difference for MRI features such as posterior ankle synovitis(χ2 =2.534,P =0.119)and posterior ankle ligament thickening(χ2 =1.515,P =0.218)between the PAIS group and the non PAIS group.Conclusion Using passive plantar flexion test and diagnostic blocking as the gold standard,the diagnostic efficacy of MRI on PAIS is obviously higher than that of X-ray film.MRI can significantly improve the diagnostic accuracy of PAIS,and avoid unnecessary diagnostic blocking.

Objective:To describe the epidemiological characteristics of mental disorders in community resi-dents aged 18 years and over in the Chifeng City of Inner Mongolia Autonomous Region.Methods:Six thousand three hundred and seventy six individuals aged 18 years and over were sampled using stratified Probability-Propor-tional-to-Size Sampling in Chifeng City in 2010.All respondents were investigated by face-to-face interview.The Composite International Diagnostic Interview-3.0 Computer Assisted Personal Interview (CIDI-3.0-CAPI)was used to make diagnoses based on the definition and criteria of the Diagnostic and Statistical Manual of Mental Dis-orders,Fourth Edition (DMS-IV).Results:A total of 4528 subjects completed the CIDI-3.0-CAPI.Regarding anxi-ety disorder,mood disorder,substance use disorder,and impulse control disorder,the 30-day adjusted prevalence rates (95%CI)were 2.5%(2.08% -2.99%,0.9%(0.67% -1.23%),0.5%(0.34% -0.76%),and 0.6%(0.41% -0.86%)respectively,and the 12 -month prevalence rates were 4.5% (3.93% -5.15%),2.4%(2.0% -2.90%),1.1% (0.84% -1.45%),and 1.0% (0.74% -1.32%)respectively.The lifetime adjusted prevalence rates were 6.6% (5.92% -7.36%),6.5% (5.81% -7.24%),2.7% (2.26% -3.21%),and 1.4%(1.23% -1.95%)respectively.Conclusion:According to morbidity,anxiety disorders,mood disorder,substance use disorders and impulse control disorders are common in Chifeng City of Inner Mongolia Autonomous Region se-quencely,being a prominent public health problem.
7.Construction of influencing factors model and policy recommendations for quality of primary traditional Chinese medicine health management services

Objective:To construct the impact factor model of primary traditional Chinese medicine (TCM) health management service quality, and put forward corresponding policy suggestions.Methods:In this cross-sectional study, the typical sampling and random sampling method were used to select 39 insiders of primary TCM health management service from 15 communities in Zhejiang Province from August to October in 2022. Interviews on service projects, implementation effects and impact factors were conducted, the three-level coding of interview record was carried out using the grounded theory research method, a model of impact factor for health management service quality of basic TCM was constructed, and the corresponding policy recommendations were put forward.Results:Based on the open coding of 39 original interview data, a total of 516 reference points were obtained, and 53 initial concepts related to the topic were formed and summarized into 17 first-level categories. Through the summary of main axis coding, 6 main categories of “policy environment”,“health literacy”,“community orientation”,“capacity building”,“health preference” and “conflict of interest” were extracted. The logical correlation between the six main categories were analyzed with selective coding, an impact factor model was constructed in accordance with the story line of factors affecting service quality. The story line of this model was as follows: first, the policy environment was the external guarantee of community TCM service quality; second, as the demand-side of the services, the health literacy and requirement of residents was the regulatory factor for service quality; in addition, the community played the role of the supply-side of the services, the service ability construction was the key factor, and the community functions and target orientation was the internal driving factor, meanwhile, the internal/external conflicts of interest had a negative constraint on the service quality.Conclusions:The guarantee intensity of external policy environment is limited, the service demand side pays insufficient attention, and the service supply side functions are absent at the present stage. It is necessary to improve the external policy environment, deepen the connection between supply and demand sides in the field of TCM health service, promote the capacity building of TCM service at the grass-roots level, balance the interests of relevant departments of TCM service, in order to improve the quality of TCM health management service at the grass-roots level.
8.Expression of CENPF and miR-1-3p in the serum of patients with advanced gastric cancer and their correla-tion with prognosis

Objective To investigate the expression of centromeric protein F(CENPF)and microribonucleic acid 1-3p(miR-1-3p)in the serum of patients with advanced gastric cancer and their correlation with prognosis.Methods Sixty patients with advanced gastric cancer admitted to our hospital from March 2019 to March 2020 were collected as the study group,while 60 healthy volunteers who underwent physical examinations at our hospital's physical examination center during the same period were collected as the control group.Real-time fluorescence quantitative PCR(qRT-PCR)method was applied to detect the expression levels of serum CENPF and miR-1-3p in each group;Pearson method was applied to analyze the correlation between serum levels of CENPF and miR-1-3p;Kaplan-Meier method was applied to analyze the relationship between the expression of CENPF,miR-1-3p,and prognosis in patients with advanced gastric cancer;and COX regression was applied to analyze risk factors affecting the prognosis of patients with advanced gastric cancer.Results Compared with the control group,the CENPF level in the study group was obviously increased,while the miR-1-3p level was obviously reduced(P<0.05).The correlation analysis results showed that there was a negative correlation between serum CENPF and miR-1-3p levels in patients with advanced gastric cancer(r =-0.650,P<0.001).There were obvious differences in CENPF and miR-1-3p levels among different TNM stages and lymph node metastasis status(P<0.05).The 3-year survival rate of patients in the high expression group of CENPF was 19/30(63.33%),which was obviously lower than that in the low expression group,28/30(93.33%)(χ2 = 7.954,P<0.001);the 3-year survival rate of patients in high expression group of miR-1-3p was 29/30(96.67%),which was obviously higher than that in the low expression group,18/30(60.00%)(χ2 = 11.882,P = 0.001).Multivariate COX regression analysis showed that TNM staging,lymph node metastasis,CENPF,and miR-1-3p expression were risk factors affecting the prognosis of patients with advanced gastric cancer(P<0.05).Conclusion The serum CENPF level in patients with advanced gastric cancer obviously increase,while miR-1-3p level obviously decrease,both of which are related to prognosis.
9.Characteristics and influencing factors of cognitive impairment in children with attention deficit hyperactivity disorder

Objective :
The differences of cognitive impairment between children with attention deficit hyperactivity
disorder (ADHD) and normal children were compared , the influencing factors of cognitive impairment of children with ADHD were analyzed .
Methods :
A total of 133 children with ADHD were selected as the ADHD group , and 117 normal children were recruited as the control group . The severity of the subjects ′ clinical symptoms was assessed using the Swanson , Nolan , & Pelham Rating Scale⁃Fourth Edition ( SNAP⁃IV) and the Weiss Functional Impairment Rating Scale . The degree of cognitive functional impairment of subjects was assessed using the MATRICS cognitive suite (MCBB) , and the executive function impairment of the subjects was assessed by the Behavior Rating Inventory of Executive Function ( BRIEF) . The differences in cognitive functional impairment of the two groups were compared . The children with ADHD were further divided into three groups according to their clinical phenotype and age , respectively , and the differences of cognitive impairment among the three groups were compared . The influencing factors of the degree of cognitive impairment in children with ADHD were analyzed by multiple linear regression . The improvement of methylphenidate sustained⁃release tablets on cognitive and executive functional impairment in children with ADHD was observed .
Results :
The scores of SNAP⁃IV , Weiss Functional Impairment Rating Scale , connection test and BRIEF of ADHD patients were significantly higher than those of normal controls (P < 0. 05) . The scores of symbol coding test and maze test of ADHD patients were significantly lower than those of normal controls (P < 0. 05) . The score of symbol encoding test in children with ADHD⁃HI was significantly higher than that in ADHD⁃I and ADHD⁃C groups (P < 0. 05) , and the total BRIEF score of ADHD⁃C was significantly higher than that in ADHD⁃I and ADHD⁃HI groups (P < 0. 05) . With the increase of age , the score of connection test of ADHD children gradually decreased , while the scores of symbol coding test and maze test gradually increased (P < 0. 05) . The results of multiple linear regression analysis showed that age was the influencing factor of ADHD children ′s score in the connection test , symbol coding test and maze test (P < 0. 05) ; the scores of SNAP⁃IV and Weiss Functional Impairment Rating Scale were the influencing factors of BRIEF score of ADHD children (P < 0. 05) . After methylphenidate treatment , the scores of connection testand BRIEF significantly decreased (P < 0. 001) , while the scores of symbol coding test and maze test significantly increased (P < 0. 001) .
Conclusion
The younger the age and the more serious the clinical symptoms related to ADHD suggest that the cognitive impairment of children with ADHD is more serious . After methylphenidate treatment , the degree of cognitive and executive impairment in children with ADHD are improved .
