Objective To investigate the feasibility and effectiveness of laparoscopic extraperitoneal inguinal hernia repair with low-pressure pneumoperitoneum without using stapling devices. Methods A total of 38 patients with reducible inguinal hernia underwent laparoscopic extraperitoneal inguinal hernia repair in our hospital from January 2006 to February 2007.Three abdominal trocars were introduced into the extraperitoneal cavity. An 11-mm trocar was placed at the lower border of the umbilicus, and the other two sized 5.5 mm were at the upper and lower 1/3 of the line between the umbilicus and the pubic symphysis,respectively.Then,low-pressure(6-8 mm Hg) CO2 was insufflated into the extraperitoneal cavity.Non-traumatic forceps was used to create an operative space from the hilum to the pubic symphysis and the diseased inguinal area. The hernia sac and the fabrics around it were disconnected. For big hernia sacs, the sac was ligated, cut at the cervix, and then left at its original site. While for small hernia sacs,it was dissociated and put back into the abdominal cavity.Afterwards,a patch sized 12 cm ? 15 cm was placed into the cavity, stretched, and attached to the abdominal wall, covering the annulus inguinalis profundus,Hesselbach triangle, and the femoral ring, without using stapling devices. After the operation, the patients received sandbag compression at the operative area and were kept in bed with urethral catheter for 24 hours. Results The operation was successfully accomplished in all the 38 cases without conversion to open surgery. The operation time was 45-85 min(mean,56 min). The intraoperative blood loss was 5-10 ml.After the operation, seroma occurred in 4 cases, and perforation in 2.No subcutaneous emphysema, hypercapnia, intestinal injury, neuralgia,or hemorrhage was found. The patients were followed up for 6 to 19 months(mean,10.5 months),and no recurrence occurred. Conclusions It is feasible to use laparoscopic extraperitoneal inguinal hernia repair with low-pressure pneumoperitoneum without using stapling devices. The method is associated with few complications and confirmed efficacy.

Objective To study the feasibility, superiority and curative effect of percutaneous nephrolithotomy with fibrous sheath and Swiss LithoClast Master (EMS Ⅲ) through T-tube tract for retained choledocholithiasis. Methods This study involved 32 patients with hepatolithiasis who were treated in our hospital from August 2004 to August 2007. By combining percutaneous nephrolithotomy (with a fibrous sheath on) and Swiss LithoClast Master (EMS Ⅲ) through a T-tube tract, retained choledocholithiasis was removed. Results The 32 patients totally underwent 36 operations. The mean operation time was 43 minutes (ranged from 33 to 78 minutes). In 28 of the cases, the stones were extracted completely by one session; and 2 were cured by two operations. The final stone-free rate was 94% (30/32). Follow-up was carried out in all the patients for 26 to 48 months (mean,29 months),during which no patients developed abdominal pain, jaundice or fever,no recurrence was detected by B-ultrasonography. Two patients showed residual stones after the treatment, one of them received Roux-en-Y hepatocholangioenterostomy because of extensive multiple biliary stones in the liver, which could no be removed completely after three sessions of nephrolithotomy; another patient who was a 70-year old man refused the secondary operation and thus retained a few stones in the right inferior lobe of the liver. In this series of 32 cases,no bile duct tear, massive hemorrhage, biliary leakage, cholangitis or abdominal distension occurred. Conclusions It is a safe, convenient, and simple method to combine percutaneous nephrolithotomy with Swiss LithoClast Master via T-Tube tract for retained choledocholithiasis. The procedure results in less pain in patients, and reduced equipment spoilage, while the cost is low. As the surgery is easy to perform,it is worth being widely used.

Aim To investigate the specific binding sites that histone deacetylases 1(HDAC1) and estrogen receptor α(ERα)can be recruited to regulate the transcriptional activity of p21WAF1/CIP1 promoter in the breast cancer MCF-7 cells, and to clarify the molecular mechanism of suberoylanilide hydroxamic acid(SAHA) and leptin regulating p21WAF1/CIP1 promoter function. Methods The breast cancer MCF-7 cells in logarithmic growth phase were starved with FBS free medium for 24 hours, and treated with 20 μmol·L-1 SAHA(SAHA group) or 0.625 nmol·L-1 leptin(Leptin group) for 24 hours. The cells that were cultured in complete RPMI 1640 medium without any treatment were assigned as control group(Basal group). The cell lysis was prepared and incubated respectively with anti-HDAC1 and anti-ERα antibody by chromatin-immunoprecipitation(ChIP) method overnight at 4℃.The DNA-ChIP was followed the manufacturer′s protocol for the assay.DNA fragments binding anti-HDAC1 and anti-ERα antibody were gathered and purified. The relative expression level of DNA fragments from TSS to the upstream of the p21WAF1/CIP1 promoter region(+2~-4 000 bp) binding with antibody was detected by real-time PCR and analyzed by 2-△△CT method. Results In basal group, HDAC1 and ERα had high affinity with the f1 and f8 fragments of p21WAF1/CIP1 promoter compared to the f4 fragment. In SAHA group, the binding ability of HDAC1 and ERα to the f1 and f8 fragments of p21WAF1/CIP1 promoter was significantly lower than that of the control, while reversing to reach the peak after leptin treatment. Conclusions HDAC1 and ERα can be recruited to p21WAF1/CIP1 promoter by the cell proliferation signal during the proliferation of breast cancer MCF-7 cells. The DNA f1(from 0 to-400 bp) and f8(from-2 800 to-3 200 bp) fragment in the upstream of p21WAF1/CIP1 promoter are the target functional region for the binding with HDAC1and ERα.

Purpose:To study the secretion and gene expression of Angiogenesis factors in the patients with CML and study the effect of Angiogenesis in the occurrence and development of CML. Methods:Concentration of VEGF in peripheral blood was determined by using ELISA. Myeloid tissue was extracted from all CML cases and ITP patients to detect MVD by using CD34 labelling. At the same time the level of VEGF,b-FGF were detected by using RT-PCR in both peripheral blood and myeloid cells. Results:Our results showed that the concentration of VEGF was obviously higher in the peripheral blood of CML patients(177.53?153.45 pg/ml) than in those of control group(73.12?19.82 pg/ml). The gene expression of VEGF and b-FGF were both higher than those of control group. The difference has statistical significance(P
9.Protective effect of omapatrilat on human umbilical vein endothelial cell injury induced by angiotensin Ⅱ in culture
Chinese Pharmacological Bulletin 2003;0(12):-
Aim To investigate the protective effects of omapatrilat on human umbilical vein endothelial cells (HUVECs) injury induced by angiotensin Ⅱ (AngⅡ) in culture. Methods Cultured HUVECs were randomly divided into 4 groups:①control; ②AngⅡ;③omapatrilat and ④AngⅡ+omapatrilat. Lactate dehydrogenase (LDH) leakage was evaluated by spectrophotometer and cell cycle by flow cytometry (FCM); Nitric oxide (NO) was measured by colorimetry and endothelin-1 (ET-1) by radioimmunoassay. Results 10 -7 mol?L -1 AngⅡ significantly increased LDH leakage and ET-1 release, and this increase was inhibited by omapatrilat (10 -6 mol?L -1). Omapatrilat promoted HUVECs proliferation and NO release. Conclusion Omapatrilat can protect human umbilical vein endothelial cells injury induced by angiotensin Ⅱ in culture, suggesting that it may play an important role in prevention and treatment of vascular diseases.

Journal of Chinese Physician 2001;0(06):-
Objective To observe effect mechanism of L-arginine (L-arg,a precurory substance of nitric oxide sythesis) in proveting proteinuria in adriamycin-induced nephrosis model of rats. Methods Nephrosis model of 16 rats was established by tail vein injetion with adriamycin, then the 16 rats were divided into two groups;model group and treatment group with L-arg administration (treated group),the levels of nitric oxide(NO),endothelin-1(ET-1),renal function, and pathological changes were observed in the two groups. Results The successful nephrosis model of rats was indentified with optical and electron microscope technique.Proteinuria was occurred in the two groups, but proteinuria degree in treated group was lighter than those of model group, however NO level was higher than that of model group, there was negatively correlation in the levels of NO and ET-1 between both groups, pathological alteration under electron microscope observation was obviously less in the treatment group than those of the model group. Conclusions On basis of these findings, author consider that the L-arg could reduce proteinuria output by promoting NO synthesis and suppress the ET-1 production.
