1.A prospective study of emergent endotracheal intubations in off-hour time in pediatric intensive care unite

Objective To assess the risk of emergent endotracheal intubations in pediatric intensive care unite (PICU ) and to de-termine the risk factors of complications .Methods A prospective study was conducted with unified tables to collected data of all e-mergent intubations occurring in PICU between November 2011 and May 2012 .All the children were divided into group of any com-plication and group of no complication .Results (1)69 .1% children of emergent intubations had complications including desatura-tion(67 .9% ) ,bradycardia(29 .8% ) ,vomiting (16 .7% ) .(2)Emergent endotracheal intubations were 1 .7 times more likely to occur off-hours .Off-hours intubations were associated with 2 .7 times the risk of complications as on-hours intubations .(3)In a multivari-ate logistic regression analysis ,the nasal tracheal intubation ,two or more attempts at intubation ,and off-hours intubation were asso-ciated with complications .Conclusion Emergent endotracheal intubation are 1 .7 times more likely to occur off-hours .off-hours in-tubations ,the nasal tracheal intubation ,two or more attempts at intubation ,are associated with complications .

3.Effect of Xiao-Chai-Hu Decoction with Different Doses of Bupleurum on Body Temperature and Content of IL-1β, IL-6, TNF-α in Serum Induced by LPS among Fever Rat Models

This study was aimed to observe the effect of Xiao-Chai-Hu Decoction (XCHD) with different doses of Bupleurum on body temperature and content of IL-1β, IL-6 and TNF-α in serum among fever rat models induced by lipopolysaccharide (LPS) in order to and investigate its action mechanism. A total of 50 rats were randomly divid-ed into 5 groups, including the normal control group, fever model group, XCHD group (high-dose), XCHD group (middle-dose), and XCHD group (low-dose). The fever rat model was established by intraperitoneal injection of LPS (20 μg/kg). At the same time, all rats were given corresponding XCHD or saline solution by gavage. The body tem-perature of rats (Ti) was measured every 30 min for 240 min after the injection of LPS. Change values of body tem-perature △T(△T=Ti-T0) were calculated. After the last measurement of body temperature, blood was taken for the de-tection of IL-1β, IL-6 and TNF-α level in serum with ELISA. The results showed that the body temperature of XCHD group (high-dose) was obviously decreased at the time point of 3 h, 3.5 h, 4 h (P< 0.01, or P< 0.05). This temperature reducing effect at the time point of 3.5 h and 4 h was better than the XCHD group (middle-dose) and XCHD group (low-dose, P< 0.01, or P< 0.05). Compared with the fever model group, all three XCHD groups can obviously reduce the IL-1β level (P< 0.05). Among them, the XCHD group (high-dose) had obvious effect (P<0.01). Compared with the fever model group, the XCHD group (high-dose) can obviously reduce contents of IL-6 and TNF-α in serum (P< 0.05). It was concluded that XCHD can reduce fever induced by LPS in rats and decrease contents of IL-1β, IL-6 and TNF-α in serum. This effect has a positive dose-effect relationship with the dose of Bu-pleurum in a certain range.
4.Anti-tumor mechanisms of plant alkaloids

Plant alkaloids have anti-tumor activities.In recent years,the anti-tumor mechanisms of plant alkaloid are demonstrated to be related to the block of tumor cell cycle,the induction of tumor cell apoptosis,the inhibition of tumor angiogenesis and multi-drug resistance.In addition,most recently,plant alkaloids are found to inhibit telomerase activity and induce autophagy.Therefore,plant alkaloids may be developed into a sort of potential antitumor drugs.
5.Analysis of pathogen distribution and antibiotic resistance in 1606 blood cultures from neonates

Objective To determine the pathogen profile and antibiotic resistance in aerobic isolates from blood cultures of neonates. Methods All blood culture reports (n=28120) from newborns admitted to the Department of Neonatology during 2002-2012 were analyzed, and the sensitivity patterns were recorded. Results A total of 1665 bacteria were isolated from 1606 blood culture-positive samples and the positive rate of blood cultures was 5.7%(1606/28120). Gram-positive bacteria were iso-lated in 1336 cases, with Staphylococcus epidermidis (902 cases) and Staphylococcus haemolyticus (206 cases) being the com-mon bacteria. Klebsiella pneumoniae (108 cases), followed by Escherichia coli (73 cases), were the major Gram-negative bacte-ria (235 cases). The determination of the antibiotic resistance of aerobic isolates was performed in 2012. Most Gram-positive iso-lates were sensitive to vancomycin and moxifloxacin, and more than 90%were resistant to penicillin while most of Gram-nega-tive isolates were sensitive to amikacin and imipenem. Conclusions Staphylococcus epidermidis, Staphylococcus haemolyticus, Klebsiella pneumoniae and Escherichia coli remain to be the principal organisms responsible for neonatal sepsis.
6.Experimental study on effect of Xiaozhensan on H22 cells-bearing Mice

Objective:To observe the effect of prolonging life and inhibiting tumor growth of Xiaozhensan on H22 cells-bearing mice and study the mechanism of action. Methods:①H22 cells-bearing mice models were made by subcutaneous injection and were divided into six groups:model group,low,middle and high dosage groups of Xiaozhensan,5-FU group,5-FU plus Xiaozhensan middle dosage group. After 10 days' treatment,the mice were killed and the anti-tumor rates were calculated,to detect the content of IL-2 and observe pathological sections of tumor and expression of Bcl-2 by immunohistochemical method. ②H22 cells-bearing ascites mice models were divided as the above. The living days of the mice were recorded,the prolongation of life span were calculated. Results:Xiaozhensan had the function of inhibiting the growth of tumor,and there was the dependability between quantity and effect. In pathomorphology,the sections of Xiaozhensan groups showed nuclear shrinkage,necrosis,hemorrhage,inflammatory cell infiltration on the periphery,etc. Xiaozhensan could increase the content of IL-2 and inhibit the expression of Bcl-2. Xiaozhensan combined with 5-FU can prolong the life span of the mice bearing ascites tumor. Conclusion:Xiaozhensan had the function of inhibiting the growth of tumor. The mechanism of action may be concerned with these aspects:enhancing the cellullar immunologic response and strengthening the ability to resist tumor,inducing apoptosis and weakening reproductive activity of tumor,etc. Middle dosage group of Xiao Zhen San combined with anticarcinogenic western medicin can prolong the life span of the mice bearing ascites tumor. This study provided dependable experimental documents for clinical application of Xiaozhensan,and had some guiding significance in clinic.

New progress of myelodysplastic syndomes (MDS) in the 55th ASH annual meeting on 5-azacytidine and decitabine was reviewed.Except in older females,the survival time for a diagnosis of MDS in the United States does not seem to have substantially improved since the introduction of 5-azacytidine into the market in 2004.However,clinical and mechanism data suggest that judicious,mechanism-based application and optimization of these agents will have increasingly meaningful impact.Moreover,these drugs are exemplars of an inspiring alternative to the traditional cytotoxic paradigm and should pave the way for additional relatively nontoxic but broadly effective agents.

New progress of myelodysplastic syndomes (MDS) in the 55th ASH annual meeting on allogeneic transplantation were reviewed.Even though the IPSS was developed mainly to determine the prognostic risk in newly diagnosed MDS patients,its predictive value concerning posttransplantation outcome has bcen confirmed in several studies.According to a decision model published a decade ago,patients with intermediate-2 or high-risk MDS by IPSS criteria should be considered for hematopoietic cell transplantation (HCT) at the time of diagnosis if an HLA-matched donor is available at this time.Given the age of most MDS patients,therapeutic interventions are per se often limited to rather nonintensive treatment approaches.This article reviews the current evidence for allogeneic HCT as a therapeutic option in the context of disease-specific characteristics and current available alternative treatments.
9.H_2S:the third endogenous gaseous transmitter

It is hypothesized that H 2S is the third endogenous signaling gasotransmitter, after NO and CO. Endogenous H 2S may regulate some aspects of synaptic activity via cAMP mediated pathways and function as a neuromodulator or transmitter. H 2S is an endogenous vasorelaxant factor that activates K ATP channels and hyperpolarizes membrane potential of vascular SMCs. By directly acting on vascular SMCs, H 2S may reduce the extracellular calcium entry and relax vascular tissues.
10.The response of GABA eliciting the rats outer hair cells during development.

OBJECTIVE:
We used electrophysiological methods to study that whether GABA could elicit OHCs outward currents provide evidence for exsitence of GABA-A receptor and investige the relationship between the effect of GABA and the development of OHCs.
METHOD:
We used whole-cell recording OHCs at current-clamp or voltage-clamp to verify the function of GABA receptor on OHCs. Then we counteds the responsive cells vs. total number cells, and according to results to study the relationships between the GABA receptor and development of OHCs.
RESULT:
OHC was elicited outward current or hyperpolarized by GABA and the responsive cells were decreased with development.
CONCLUSION
The result of GABA receptor decreasing with development suggested that the receptor may draw efferents to OHCs or facilitate the MOC-OHC synapse formation.
