1.Supplementary Effect of Huafu Shengji Paste on Anal Fissure After Anal Dilation Surgery

Objective To observe the supplementary effect of Huafu Shengji Paste on anal fissure patients after anal dilation surgery. Methods One hundred and six anal fissure patients after anal dilation treatment were randomized into treatment group and control group, 53 cases in each group. The control group was given conventional local nursing therapy, and the treatment group was given local application of Huafu Shengji Paste. After treatment, the clinical efficiency and the relief of clinical symptoms of the two groups were evaluated. Results (1) The cure rate and the total effective rate in the treatment group were 58.49%, 94.34%, and those in the control group were 35.71%, 73.21% respectively, the difference being significant(P < 0.05).(2) After treatment, the scores of symptoms of edema, exudation, pain, erosion, bleeding and prolapse in the two groups were much decreased(P<0.01 compared with those before treatment), and the decrease of the scores in the treatment group was superior to that in the control group(P<0.05). Conclusion Application of Huafu Shengji Paste has obvious effect on relieving the clinical symptoms of anal fissure patients after anal dilation surgery ,and its effect is superior to that of conventional local nursing therapy.

3.The diagnostic values of bile cytology and blood tumor markers for malignant bile duct obstruction

Objective To explore the diagnostic values of bile cytology obtained in endoscopic retrograde cholangiopancreatography (ERCP) and blood tumor markers for malignant bile duct obstruction.Methods From August 2005 to April 2009, bile cytology and blood tumor markers measurement were performed in 47 cases with bile duct obstruction, in which 39 cases were confirmed to be malignant and 8 were benign. Results Malignant cells were found in bile from 26 of 39 malignant obstruction (66. 7% ), and serum CA19-9 was positive in 28 (71.8%). No malignant cells were detected in bile cytology from 8 cases with benign obstruction (with a specificity of 100% ) and serum CA19-9 was negative in 7 (87.5%). In 26 patients with cholangiocarcinoma, serum CA19-9 was positive in 18 (69. 2% ). For bile cytology and blood tumor markers test, sensitivity of parallel combination was 89. 7%, and the specificity of serial combination was 100. 0%. Conclusion Bile cytology testing during ERCP can provide the pathological evidence for malignant bile duct obstruction. The sensitivity is improved by combination tests of bile cytology and blood tumor markers.
4.Clinical study of the surgical approaches for intracranial arachnoid cysts

Objective To investigate the surgical approaches indication, technique and efficacy for treatment of intracranial arachnoid cysts (IAC). Methods The clinical data of 56 patients with IAC who were diagnosed with MRI and/or CT techniques were retrospectively analyzed. According to the surgical approaches adopted, these cases were classified into 4 groups, group A with expectant treatment (12 cases), group B with endoscopic cystocisternal fenestration (15 cases), group C with cystoperitoneal shunting (21 cases), and group D with surgical cyst removal (8 cases). The outcomes of the patients were compared. Results The patients were followed up for 3 months to 46 months (mean 15 months). The rate of symptomatic improvement in group B was 14/15, in group C was 95.24%(20/21) and in group D was 5/8, and there was significant difference ( χ2=8.335, P<0.05). The rate of radiographic improvement in group B was 14/15, in group C was 90.48%(19/21) and in group D was 5/8, and there was significant difference (χ2=8.235, P<0.05). The rate of complication in group B was 1/15, in group C was 14.29%(3/21), and in group D was 5/8, and there was significant difference ( χ2=8.256, P<0.05) Conclusions In terms of therapeutic effect, surgical safety and minimal invasiveness, cystoperitoneal shunting and endoscopic cystocisternal fenestration should be the primary choice .
5.Effects of low molecular heparin combined with dexamethasone on coagulation and fibrinolysis at early stage in rats with acute respiratory distress syndrome

Objective To observe the effects of low molecular weight heparin (LMWH) combined with dexamethasone (Dex) on coagulation and fibrinolysis at the early stage in rats with acute respiratory distress syndrome (ARDS) induced by two-hit of oleic acid (OA) and lipopolysaccharide (LPS).Methods Forty healthy adult male Sprague-Dawley (SD) rats were randomly divided into sham operation, ARDS model, Dex, LMWH and combining therapy groups (8 rats in each group). The rat ARDS model was established by sequential two-hit with intravenous injection of OA 0.2 mL/kg in a tail vein and intra-peritoneal injection of LPS 5 mg/kg. After model establishment, the rats in Dex group were intra-peritoneally injected with 10 mg/kg Dex, and those in LMWH group were intravenously injected with 200 U/kg LMWH, while the rats in combining therapy group were given Dex and LMWH simultaneously with the same dosages and methods as above mentioned respectively. In sham operation group, however, the rats were intravenously injected with 0.4 mL of normal saline (NS) and were given 2 mL/kg of intra-peritoneal NS injection, and they accepted another 1 mL/kg NS intra-peritoneal injection 4 hours later, the other procedures being the same as those in the model group. The experiment was ended at 6 hours after the establishment of ARDS model. A light microscope was used to observe the pathological changes in lung tissues, partial pressure of arterial blood oxygen (PaO2) was measured, and the oxygenation index (PaO2/FiO2) was calculated. Wet to dry weight (W/D) ratio of lung tissues was also checked. Coagulation indexes were measured by solidification method, and the serum level of plasminogen activator inhibitor-1 (PAI-1) as well as the content of procollagen type Ⅲ (PC-Ⅲ) was determined by enzyme linked immunosorbent assay (ELISA).Results Under the light microscope, effusion of red blood cells, fibrin deposit in the lung interstitium and alveoli, formation of transparent membrane at alveolar wall, inflammatory cells infiltration in pulmonary interstitial tissue, and fibrinous thrombi in lung capillaries or lung arterioles were seen in the model group. Compared with model group, the red blood cells effusion and fibrin deposition in the lung interstitium and alveoli were less, inflammatory cells infiltration in pulmonary interstitium was alleviated and the fibrinous blood emboli in the lung capillaries or lung small arterioles were also decreased in Dex, LMWH and combining therapy groups, among the three groups, the best results being in the combining therapy group. Compared with sham operation group, the PaO2/FiO2 was significantly lowered [mmHg (1 mmHg = 0.133 kPa): 272.02±28.28 vs. 420.24±35.52,P < 0.01], the lung W/D ratio was obviously higher (5.59±0.40 vs. 3.82±0.28,P < 0.01), prothrombin time (PT) as well as thrombin time (TT) were markedly longer [PT (s): 18.78±1.57 vs. 16.36±0.97, TT (s): 39.02±5.03 vs. 29.22±8.83, bothP < 0.05], fibrinogen (Fib) content was significantly decreased (g/L: 1.82±0.26 vs. 2.69±0.40,P < 0.01), but both the serum contents of PAI-1 and PC-Ⅲ were remarkably elevated in model rats [PAI-1 (ng/L): 719.04±103.74 vs. 517.25±119.18, PC-Ⅲ (μg/L): 29.93±3.24 vs. 22.97±6.26, bothP < 0.01); Compared with model group, the level of PaO2/FiO2 was significantly elevated, and the lung W/D ratio was obviously decreased in Dex, LMWH and combining therapy groups respectively, the most significant changes being in combining therapy group [PaO2/FiO2 (mmHg): 376.78±25.25 vs. 272.02±28.28, lung W/D ratio: 4.14±0.42 vs. 5.59±0.4, bothP < 0.01] , in LMWH group, the prolongation of TT was the longest (s: 52.00±4.24 vs. 39.02±5.03,P < 0.05), while Fib contents in the three treatment groups were all obviously increased (g/L: 2.37±0.38, 2.59±0.51, 2.59±0.24 vs. 1.82±0.26,P < 0.05 orP < 0.01); meanwhile, the decrease of PAI-1 in combining therapy group was the greatest (ng/L: 546.02±93.94 vs. 719.04±103.74,P < 0.01). The indexes showed no statistically significant differences among the three treatment groups, except that PT in combining therapy group which was significantly longer than that in Dex and LMWH groups (s: 19.98±1.61 vs. 17.20±1.48, 17.02±2.34, bothP < 0.05). Conclusions The rats with ARDS induced by two-hit of OA combined with LPS have coagulation dysfunction and fibrinolytic inhibition. Using LMWH early can improve coagulation and fibrinolytic status in the rats with ARDS, and the therapeutic effects of LMWH plus Dex for treatment of ARDS are better than those of using each of them alone.
6.Clinical Study on the Treatment of Subluxation of the Small Joint of the Cervical Vertebrae by Acupuncture plus Tuina

To observe the clinical therapeutic effect of the subluxation of the small joints of the cervical vertebrae by acupuncture plus Tuina, 178 subjects were divided randomly into acupuncture group, Tuina group and acupuncture plus Tuina group to receive the corresponding treatment. The cure rate in acupuncture plus Tuina was 68.8%, which is higher than that in acupuncture group (27.8%) and that in Tuina group(28.6%), and the statistical management showed significant differences. Acupuncture combined with Tuina could enhance the therapeutic effect in the treatment of the subluxation of the small joints of the cervical vertebrae.
7.TCM Treatment on Chloasma in Terms of the Five Organs

[Objective]To summarize the clinical experience of treating chloasma with the theory of the Zangfu-organs by Pro.Sun Yinong. [Method]By analyzing the idea of Sun’s therapy and medication of the proven case, it elaborates Pro.Sun Yinong’s clinical experience on using Chinese herbs to treat chloasma under the theory of the Zangfu-organs. [Results]The five Zang-organs perform their respective duties, any pathological changes in the five Zang-organs al can cause chloasma. We adapted at different herbs therapy to treat chloasma, which relieved the patient ’s clinical symptoms, and achieved satisfactory clinical outcome.[Conclusion] Starting from the five Zang-organs on the treatment based on syndrome differentiation, not only provides a new thought for the treatment of chloasma,but also ful y embodies the concept of holism and syndrome differentiation on TCM treatment of chloasma, which has practical effect on clinical application.
8.Determination of Methionine,Vitamin B1 and Vitamin B2 in Lingzhi Erwei Methionine Capsules by HPLC

Objective:To establish an HPLC method for the simultaneous determination of methionine, vitamin B1 and vitamin B2 in Lingzhi Erwei methionine capsules. Methods:The determination was performed on a Ultimate? AQ-C18 column(250 mm × 4. 6 mm, 5 μm,Welch Inc. ) with the detection wavelength at 220 nm. The mobile phase was composed of methanol and 0. 07 mol·L-1 sodium heptanesulfonate solution (14 ml triethylamine diluted with water to1 000ml, adjusting pH to 3. 5 with diluted orthophosphoric acid) with gradient elution, and the flow rate was 1. 0 ml·min-1. Results: Methionine, vitamin B1 and vitamin B2 was in good linearity within the range of 0.527 0-1.510 0 mg·ml-1 - (r =0.999 9), 0.045 52-0.136 56 mg·ml-1 (r =0.999 9) and 0.010 10-0. 060 58 mg·ml-1(r=0. 999 9), respectively. The mean recovery was 100. 5%, 97. 7% and 101. 5% with RSD of 0. 5%, 1. 0%and 1. 4%(n=9) accordingly. Conclusion:The method is simple, accurate, reliable and appropriate in the simultaneous determina-tion of methionine,vitamin B1 and vitamin B2 in Lingzhi Erwei methionine capsules.
9.Reflection on the establishment of medical and health curriculum standard for higher voca-tional education

One of the biggest differences between ordinary undergraduate education and higher vocational education is more emphasis on the cultivation of students' vocational skills so as to adapt to so-cial demand and employment. In order to overcome the deficiencies in current teaching outline of voca-tional education and to change from subject knowledge teaching to applied skill cultivating , medical and health curriculum standard including 'curriculum philosophy, curriculum goal, curriculum content, curriculum implementation' four elements was formulated based on concepts of 'education as base , practice orientation and lifelong learning' and cultivation mode combining industry standard with cur-riculum teaching goal and combing work with study. All measures taken above were aimed at promoting coordinated development of students' knowledge, ability and emotion and satisfying the social demand for talent.
10.Effect of Combination of Epigallocatechin-3-gallate and Cisplatin on Proliferation and Apoptosis of SW620

Objective To investigate the effect of different composed combination of EGCG and cisplatin on human colon adenocarcinoma cell line SW620 and the related mechanisms. Methods SW620 cells at exponential phase were treated with cisplatin(5 mmol.L-1)and EGCG(0, 25, 50 and 100 mmol.L-1).Seventy hours after the cultivation, cell proliferation was detected with MTT;the percentage of apoptotic cells was determined by flow cytometry;expression levels of JAK2, p-JAK2, p-STAT3, STAT3, Bax, Bcl-2, cleavage-PARP, and cleavage-caspase3 were detected by Western blotting. Results The SW620 cell proliferation rate was(95±5)%,(67±5)%,(42±5)%(22±4)% and the apoptotic rat was 5%, 15%, 35%, 57%in the 0, 25,50, 100 mmol.L-1 EGCG groups, respectively.Moreover, EGCG up-regulated the expression of Bax, cleavage-PARP and cleavage-caspase3(P<0.05)and down-regulated the expression of p-JAK2, p-STAT3 and Bcl-2(P<0.05)in a dose-dependent manner, but had no influence on the expression of JAK2 and STAT3(P>0.05). Conclusion EGCG can promote cisplatin-induced apoptosis and proliferation inhibition of human colon adenocarcinoma cell lines of SW620 by suppressing JAK2/STAT3 gignaling pathway.
