1. Diagnosis and treatment of neuroendocrine breast cancer: A retrospective analysis of ten cases and review of the literature

Objective: To investigate the characteristics of clinical manifestations, diagnosis and treatment of NEBC (neuroendocrine breast cancer). Methods: This was a retrospective study. Ten consecutive female patients with NEBC were admitted in Department of Breast Surgery, Peking Union Medical College Hospital between January 2004 and December 2010. The average age of the ten patients was 52 years, and the follow-up period was 24-177 months (median 84 months). Results: Clinically, most of the NEBC manifested as breast lump, sometimes as NPBL (non-palpable breast lesion) or nipple discharge. An ultrasound examination usually revealed heterogeneous echoic mass with irregular shape and clear boundary. Mammography showed no significantly different changes from other kinds of breast cancer. Nine patients received curative resection for NEBC, among whom, seven underwent modified radical mastectomy, one underwent nipple and areola-sparing modified radical mastectomy, one with nipple discharge underwent excision of duct and relevant lobule and hook-wire-guided biopsy of breast lump, and later right mastectomy with sentinel lymph node biopsy. The expressions of hormone receptors like ER (estrogen receptor) /PR (progesterone receptor) were usually positive in NEBC, while the expression of HER2 (human epidermal growth factor receptor 2) was usually negative (-) to positive (+). At least one neuroendocrine marker like Syn (synapsin) or CgA (chromgranin A) was positive. During the follow-up period, metastasis was found in two patients and one of them had passed away. Conclusion: NEBC is a clinical entity different from breast cancer with BC-NE (breast cancer with neuroendocrine differentiation). NEBC may affect younger patients in China, ultrasound examination is valuable for diagnosis, and surgical and comprehensive therapy may achieve satisfactory outcome with relatively good prognosis, although there might be cases of metastasis. 2.Study on early-warning of Chinese materia medica price base on ARMA model.

This study sets up an early-warning system framework of Chinese materia medica price, using price index as early warning indicator to establish black early-warning model, with indicator of price index volatility and limit line of "price principal". The research divides warning degree into 5 parts named negative heavy warning, negative light warning, no warning, positive light warning and positive heavy warning, with 5 corresponding lights to describe the change level of the medicine price. Then make an early-warning empirical research based on Chengdu Chinese materia medica price index from December in 2010 to October in 2013. ARMA model is applied to forecast index and the result of early-warning is analyzed, and finally farmer households, companies, customers and the government are recommended respectively.

3.Epidemiology of 10 000 mammary gland disease cases in Yangquan city

Objective To investigate the epidemiology of 10 000 patients with mammary gland disease.Methods 10 000 mammary gland disease cages during 1987~2006 in Yangquan Tumor Institute were collected,all patients had integrity case files and were diagnosed by molybdenum target radiography,near infrared ray,sonography,aspiration-needle biopsy or polyrrhea smear examination.Results In 8919 outpatient treated cases,7493 suffered cyclomastopathy(84%).1081 cases were inpatient cared(11%),the top five mammary gland disease are:breast cancer 342 cases,cyclomastopathy 252 cases,adenofibroma 104 cases,intraductal papilloma 86 cases and ductal ectasia 76 cages.For breast cancer patients,there were 125 cases during 1987~1996,3 were 21~30 years old and 23 were 31~40 years old.The number during 1997~2006 was 217,24 were 21~30 years old and 69 were 31~40 years old.Conclusion In 10000 cases,there are 7745 cyclomastopathy patients(77.45%)which takes the first place.Especially for 342(3.42%)breast cancer cases,the incidence grew up and patients age was much younger.
4.Transnasal endoscopic removal of lacrimal sac and intraorbital foreign body.

We reported a case of a work-related accident involving periorbital and intraorbital metal foreign bodies. The patient was a 43-year-old woman whose nasal dorsum was struck while she was sawing wood with nails. X-ray plain films of skull bone and computed tomographic scan of nasal sinus revealed two 5-mm-long metal objects lodged in right lacrimal sac and medial bulbar space of the right orbit. No deficits were noted in her visual acuities, visual fields, and ocular movements. Based on accurate positioning by imaging examination, transnasal endoscopic removal of metal foreign bodies was successfully accomplished. The patient recovered well without any complications after surgery.

Objective To explore the better diagnostic method for acute splenic arteral injury through comparative analysis of CT and DSA. Methods Fifty-seven patients with acute splenic injury were examined by CT and DSA,treated with splenic arterial embolization and then undertook follow up. Objective:To study the effects of Naoling tablet on Aristolochia acid A from Aristolochia fangchi,preliminary explore the deintoxication of Aristolochia fangchi.Methods:Absorption value of Aristolochhia aicd A was determined by HPLC after Aristolochi fangchi decocted with Naoling tablet,the change of content of Aristolochia acid A was observed .Results:the content of Aristolochia acid A from Aristolochia fangchi was markedly decreased after Naoling tablet was added.Conclusion:Naoling tablet can markedly decreased the content of Aristolochhia aicd A from Aristolochia fangchi.
10.Study on dual-source and image reconstruction technique application in lumbar spondyloschisis without slid-ing

Objective To explore the technical advantages and clinical value of dual-source and image re-construction technique application in lumbar spondyloschisis without sliding .Methods 36 cases of patients with LSWS were collected who were examined by dual-source CT scan and diagnosed definitely .18 cases of patients were examined by X-ray( including lumbar vertebrae and double oblique ) .The diagnosis rate of X-ray and CT scanning in diagnosing LSWS were calculated ,which was made statistical analysis .The image of 36 cases patients with LSWS were reconstructed by multi-planar reconstruction ( MPR) ,volume rendering technique ( VR) and curved planar reconstruc-tion( CPR) .The display rate was calculated , which was made statistical analysis according to various image recon-struction in diagnosing LSWS .Results 7 cases of patients with LSWS were found by X-ray examination .36 cases of patients with LSWS were found by dual-source CT examination .The display rate was 38.9%and 100.0%.71 LSWS were found in 36 cases of patients with LSWS ,35 cases were bilateral spondylolysis ,1 case was unilateral spondylolys-is.In several image reconstruction methods ,CPR and cutting VR showed the highest rate in diagnosing LSWS ,which was 100.0%.The symptom of LSWS in X-ray examination:local thinning and structural disorder in lumbar spondylol-ysis,cortical discontinuity .The symptom of LSWS in CT examination were as follows:clear linear low density shadow in lumbar,ends hardening and bone fragments in some case .Conclusion The dual-source CT and its image recon-struction technology have the technological superiority and higher clinical value in diagnosing LSWS ,which is crucial to prevent LSWS misdiagnosed ,and could become the preferred examination in screening and diagnosing LSWS .
