1.Effect of tiotropium bromide on pulmonary function and airway remodeling in elder chronic obstructive pulmonary disease stabilization patients

Objective To analyze the effect of tiotropium bromide on pulmonary function and airway remodeling in elder chronic obstructive pulmonary disease (COPD) stabilization patients.Methods Ninety-two elder patients diagnosed as COPD stabilization were divided into treatment group (50 patients) and control group (42 patients) by random digits table method.The patients in two groups were given basic treatment such as antimicrobial therapy,spasmolysis,antiasthma,expelling phlegm,and the patients in treatment group were given tiotropium bromide every day for 12 months besides the basic treatment.Meanwhile,40 healthy persons whose ages matched with those of COPD stabilization patients were as healthy control group.High resolution computerized tomography (HRCT) was employed to measure the parameters such as the airway wall thickness,the outer and inner diameter of the airway,airway wall area to assessed the airway remodeling before and after treatment in order to evaluate the drug etficacy.Meanwhile,the lung function,the inflammatory cytokines [interleukin (IL)-8,tumor necrosis factor (TNF)-α] in blood plasma and T lymphocyte subsets were also determined.Results The index of lung function in treatment group was significantly better than that before treatment and control group,and there was significant difference (P < 0.05).The airway wall thickness in treatment group had no significant difference before and after treatment (P >0.05),and other index of airway remodeling was significantly improved compared with that before treatment (P < 0.05).After treatment,the distance of 6 min walk test in treatment group was significantly longer than that in control group,and there was significant difference (P < 0.05).The level of serum IL-8 and TNF-αt before treatment had no significant difference between two groups (P >0.05),and decreased significantly after treatment and the improvement was more pronounced in treatment group compared with that in control group (P < 0.05).Compared with that in healthy control group,the level of CD3+,CD4+,CD4+/CD8+ was significantly lower and the level of CD8+ was significantly higher before treatment in treatment group and control group (P < 0.05).After treatment,the level of CD3+,CD4+,CD4+/CD8+ was significantly increased and the level of CD8+ was significantly decreased (P < 0.05),and the improvement was more pronounced in treatment group compared with that in control group (P < 0.05).Conclusion Tiotropium bromide can improve airway remodeling and lung function in elder COPD stabilization patients.
2.Relation between myocardial injury and left ventricular function in rhe umatic mitral diseases

Objective To study the relation between myocardia l changes and left ventricular function in preoperative patients with combined m itral diseases provide theoretical basis for clinical therapy and evaluate the effects of treatment. Methods A total of patients with combined mitral lesions were selected randomly for study. The cases were divided into tw o groups according to the degree of ultrastructural damage of the myocardium bef ore aortic occlusion in mitral valve replacement (Flameny cla ssification). The preoperative pulmonary artery catheterization(Swan-Ganz cathe ter), color Doppler echocardiography, roentgenologic measurement of heart size, preoperative clinical data and postoperative follow-up data were collected for analysis. Results The results showed that myocardial lesion in mitral disease had intimate correlation with the cardiac enlargement, especial c orrelated with left ventricular enlargement and cardiac pumping. Conclus ion Results suggest that the myocardial lesions in mitral valvular dise ases mainly involved the process of rheumatism and reflected in the disturbance of left ventricular function. Operation should be performed before the appeare nce of non-reversible myocardium injury in patients with rheumatic mitral va lvul ar diseases.
3.Thoughts on Improving Data Integrity of Pharmaceutical Production Enterprises

OBJECTIVE:To provide reference for improving the data integrity management system of pharmaceutical produc-tion enterprises. METHODS:According to related reports in US Food and Drug Administration(FDA)and China Food and Drug Administration(CFDA),the source of data integrity problems was analyzed,its reasons were summarized and solutions were put forward. RESULTS&CONCLUSIONS:The reasons why there were data integrity problems in enterprises can be attributed to 3 as-pects(staff,hardware/software system and quality management),and the 3 aspects showed large gap with current standards. It is suggested that enterprises evaluate the existing system by adopting the gap analysis,establish a data integrity management project team,strengthen personnel training,upgrade hardware/software system to ensure its safety,stability and effectiveness;and opti-mize the quality management system by developing good ducument specification,special regulation system of data integrity. In addi-tion,the enterprise should establish quality culture,pay attention to industry and regulatory trends in real time to guarantee the data integrity effectively.
4.Netrin-1:the new tumor markers in renal clear cell carcinoma

Objective:To explore the expression of Netrin-1 protein in human renal clear cell carcinoma (RCCC) and the relationships between Netrin-1, pathology and prognosis. Methods:72 cases of RCCC admitted in our hospital from 2008 June to 2009 June and their adjacent tissues were selected for study. They included 30 cases in stage I-II, 42 cases in stageⅢ-IV;9 cases in grade I, 9 cases in grade II, 40 cases in gradeⅢand 14 cases in grade IV. All cases were followed up for more than 5 years. Survival analysis lines were made by Kaplan—Meier method and the difference between groups was tested by the Log-rank test. The expression of Netrin-1 protein was detected by immunohistochemistry staining and its clinical significance was analyzed. Results:Renal clear cell carcinoma:51 cases in high expression of Netrin-1 and 21 cases in low expression, normal tissues: 12 cases in high expression of Netrin-1 and 60 cases in low expression, the difference between the two groups is significant (χ2=42.921, P<0.01). The difference of the expression of Netrin-1in Fuhrman grade and AJCC clinical stage is significant (χ2=8.000,χ2=6.203;P<0.05). The 5-year survival rate in low protein expression group and in high protein expression group was 79%(17/21) and 62%(32/51). The survival curve had different trend, with no significant difference between groups (χ2=1.360, P=0.245). Conclusions: Netrin-1 protein plays an important role in the development of RCCC. It might be a new specific tumor marker of RCCC, and might become a new target in treatment of RCCC.
5.Quality evaluation on different specifications of cervi cornu pantotrichum with its effect on ovariectomized osteoporosis model rats.

OBJECTIVETo study the effect of eight specifications of Cervi Cornu Pantotrichum on the osteoporosis of ovariectomized rats and grade the eight different specifications of Cervi Cornu Pantotrichum.
METHODTotally 100 SD female rats were divided randomly into 10 groups, namely the normal group, the model group and eight Cervi Cornu Pantotrichum groups of different specifications. Their bilateral ovaries were excised to reproduce the osteoporosis model. Meanwhile, the rats were given the eight different specifications of Cervi Cornu Pantotrichum for consecutively 12 weeks. Subsequently, the effects of the different specifications of Cervi Cornu Pantotrichum on bone mineral density and serum biochemical indicators of rats were observed. A clustering analysis was made for the eight specifications of Cervi Cornu Pantotrichum, with the serum content of ALP, BMP-2 and BGP as influencing factors.
RESULTAfter 12 weeks, the eight different specifications of Cervi Cornu Pantotrichum could significantly improve ALP, BMP-2, BGP in serum and bone mineral density of ovariectomized rats. And the cluster analysis showed similar results to the quality classification of traditional commercial herbs Cervi Cornu Pantotrichum.
CONCLUSIONDifferent specifications of Cervi Cornu Pantotrichum can antagonize the osteoporosis of ovariectomized rats, and their effects are related to the quality of commercial herbs.

6.Progress in research on preparation of antibodies against rhesus D antigen

The RhD antigen is expressed only in human red blood cells (RBC).Its immunogenicity and clinical application are only next to ABO blood group system, and is widely used both for blood typing and prevention of hemolytic disease of the newborn. The traditional anti-Rh(D) is derived by fractionation of plasma from individuals who have been sensitized by pregnancy or transfusion, or have been deliberately immunized to produce anti-Rh(D). Because of the limited source of plasma, researchers began the study of monoclonal and recombinant antibody. Monoclonal and recombinant anti-Rh(D) antibodies may provide alternatives to the current plasma derived polyclonal IgG anti-Rh(D), but up to now,none of them have yet proved effective in humans for prevention of RhD immunity and hemolytic disease of the newborn.
7.The research and application on management system of cold chain facilities temperature control in the hospital

Objective:To have a research and application on management system of cold chain facilities temperature control in the hospital and real-time monitor the temperature and humidity changes of cold-chain facilities.Methods: Based on RFID radio frequency, Internet of things and computer technology to realize the construction of the system hardware and software platform, through commissioning and case analysis for system improvement, the system was popularized and applied comprehensively.Results: After one year’s trial run, it showed that the system could accurately and effectively monitor the abnormal temperature and humidity of the hospital’s cold chain facilities and give the acousto-optic hint and remote alarm in time, effectively improving the management level of cold chain temperature control.Conclusion: The system could guarantee the fine management of the cold chain facilities temperature control, provide safe and traceable historical data, and put forward a new solution for building the quality and safety of cold chain facilities temperature control system.
8.The national rare diseases registry system of China and the related cohorts studies:vision and roadmap

[Summary] Rare diseases are one of the major challenges we face today in the era of precision medicine, because of the low incidence and prevalence, difficulty in diagnosis, lack of sufficient therapeutic methods, as well as their significant impacts on affected individuals, families and the society. Integration of clinical phenotypic and biological omics data and the further analysis are providing a way to illustrate the mechanisms of rare diseases, discovering novel diagnostic and prognostic biomarkers, developing orphan drugs and other therapeutics, and improving clinical outcomes and quality of life for the patients. A nation-wide registry system and the cohorts studies based on the registry are vital to the research of rare diseases. National Rare Diseases Registry System ( NRDRS) of China will provide this essential platform to promote the rare diseases research in China. With the collaboration of 20 leading medical institutes and innovation in medical informatics technologies, this system will, for the first time in China, collect the epidemiological, clinical, socio-economical, genomics and metabolomics data of more than 50 rare diseases and not less than 50 000 cases. As a national strategy for enhancing the development of medical sciences and the improvement of population health in China, NRDRS and its cohort studies will provide the pivotal support to policy making, clinical care, novel drug discovery, patient advocacy, and finally scientific progress in the field of rare diseases.
9.The Construction of Recombinant Plasmids of Toxoplasma gondii P30 and the Immunoprotective Activity of the P30 Constructs

Objective To identify the potential DNA vaccine candidate which can induce the protective immune response to Toxoplasma gondii by inoculating mice with plasmid DNAs encoding three different forms of P30 antigen (membranous secretory,and intracellular). Methods Three forms of recombinant plasmid: pcDNA3 P30Mb(contain the whole P30 gene sequence,including the gene encoding signal peptide and hydrophobic tail),pcDNA3 P30Se(contain the whole P30 gene sequence, without the gene encoding hydrophobic tail) and pcDNA3 P30In(contain the whole P30 gene sequence,without the gene encoding signal peptide) were constructed by PCR and subcloning technique. The mice were immunized with different forms of recombinant plasmids and IgG antibodies in the mice were detected by ELISA and Western blotting. Results Three forms of expression recombinant plasmid of Toxoplasma gondii P30 gene were successfully constructed. The P30 inserts were identified by restrictive enzyme digestion and sequencing. ELISA and Western blotting analysis demonstrated that specific IgG antibody could be induced in three immunized groups, but there was some difference in appearence time and intensity of IgG.Conclusion Genetically immunization of mice with the recombinant plasmids could elicit specific IgG antibodies. In respect to IgG response, the immune efficiency of the three forms of recombinant plasmids was different at the beginning (2 wk),but 4 wk later approximately same.
