2.Expression and clinical significance of small fragments of P28 and P43 in papillary thyroid carcinoma after TRa1 gene transcription

Objective The objective of this study was to investigate the expression of P28 and P43 in papillary thyroid carcinoma(PTC)and its relationship with clinicopathological features after TRa1 gene transcription.Methods Real-time fluorescence quantitative PCR(RT-PCR)and gel electrophoresis were used to determine the target fragments and to isolate the bands of different fragments.The optical density of each band was scanned by UV transmittance analyzer to detect 31 cases of PTC tissue and expression levels of P28 and P43 in para-cancerous tissues.Results The average gray value of P28 in thyroid carcinoma group was 0.77±0.34,which was significantly higher than that in para-cancer group(0.31±0.18).The average gray value of P43(0.85+0.21)in thyroid carcinoma group was significantly higher than that in para-cancer group(0.34±0.15)(P<0.05).The expression levels of P28 were not correlated with gender,age,tumor size,lymph node metastasis and clinical pathologic stage(P>0.05),The expression levels of P43 were not correlated with gender,age,tumor size and lymph node metastasis.(P>0.05)but they were related to clinical pathologic stage(P<0.05).There was no correlation between expression levels of P28 and P43(r=0.266,P=0.071).Conclusion The increased expression of P28 and P43 may have a high degree of malignancy and a certain clinical value in predicting the adverse prognosis of PTC.Both factors are helpful for the prevention and treatment of PTC.
3.Comparison study of the distracted walking condition between ordinary high school students and vocational high school students in Shanghai

Objective:
To investigate and compare the prevalence of distracted walking and determinants between general high school and vocational high school students and to provide a reference for the targeted interventions.
Methods:
A total of 1 777 senior high school students from 4 general high schools and 2 vocational high schools in Shanghai were selected for online questionnaire survey in December 2018 through convenience sampling. The questionnaire included demographic information and behaviors related to distracted walking.
Results:
In different road environments and crosswalk type, cell phone was used most frequently when waiting for traffic lights and when walking in housing estate roads in general high school students(24.6%,21.2%) and vocational high school student(18.8%,21.4%). More students from vocational schools reported smartphone use while walking in all road type except housing estate road(χ2=30.47,13.22,90.72,79.01,29.88,P<0.01). Listening to music was the main use of mobile phone when walking, students who spent more than half of their walking time listening to music were 31.2% at general high school students and 24.2% at vocational high schools; the need to access information is the primary motivator for smartphone use while walking. Nearly 50% participants realized smartphone use while walking could increase the risk of risky behaviors including inattention to traffic signals and status. About 30% general school students and 50% vocational school students agreed that tips, sprains and traffic collisions associated with distracted walking.
Conclusion
Currently, distracted walking is common among high school students, especially for vocational high school students. Pedestrian safety education should be incorporated into school-based education, together with education from parents, peers and the society, with the aim of increasing safety awareness.
4. Epidemiologic characteristics of fall in the elderly in urban and rural areas in Shanghai

Objective:
To understand the epidemiologic characteristics of fall in the elderly in Shanghai, as well as the differences between urban and rural areas, and provide evidence for targeted fall prevention and intervention.
Methods:
From January to March in 2017, a questionnaire survey was conducted in the elderly aged 60 or above selected from 7 urban communities and 6 rural communities in Shanghai to understand the epidemiologic characteristics of fall in the elderly and analyze the gender and urban-rural differences.

5. Diagnostic value of ultrasound-guided 14 gauge-core needle biopsy in breast nodules

Objective:
To analyze the diagnostic value of ultrasound guided 14 gauge coreneedle biopsy (US-CNB) in breast nodules.
Methods:
We retrospectively analyzed the pathological results of US-CNB and surgical excision from 373 breast nodules in Peking University International Hospital from Sep 2016 to Nov 2018 to evaluate the accuracy of 14g US-CNB.
Results:
A total of 349 patients(373 nodules)underwent US-CNB. US-CNB reported 282 benign lesions(75.6%, 282/373), 20 high-risklesions(5.4%, 20/373), and 71 malignant lesions(19.0%, 71/373). For 282 CNB reported benign lesions, the surgical pathology confirmed 235 lesions , 46 for high-risk lesions and 1 for malignant lesion with a concordancy of 83.3%(235/282)and the underestimation rate was 16.7%(47/282). US-CNB identified 20 high-risk lesions. According to surgical results, 15 were high-risk lesions and 5 were malignant lesions with a concordancy of 75% (15/20)and the underestimation rate was 25%(5/20). When it comes to malignant lesions, the excision results showed that 70 were malignant lesions and 1was high-risk lesion with a concordancy of 98.6%(70/71)and the overestimation rate was 1.4%(1/71). The concordance of the histological type , calculated for 50 invasive carcinomas, was 92% (46/50) with a kappa value of 0.77.The concordance of the histological grade could be calculated for 38 invasive ductal carcinomas with the Elston-Elllis Method . It was 89.5% (34/38) with a kappa value of 0.57.
Conclusions
The pathology result of 14gUS-CNB is in good consistency with surgical excision for breast benign and malignant lesions.
