1.Hydrophilic acrylic intraocular lens:biocompatibility and complications following implantation

BACKGROUND:Hydrophilic acrylic materials with good biocompatibility have been widely used in clinic. However, there are some problems about the biocompatibility and safety of hydrophilic acrylic intraocular lens after long-term clinical application. OBJECTIVE:To discuss the biocompatibility and stability issues and corresponding processing methods after hydrophilic acrylic intraocular lens implantation. METHODS:A computer-based search of Wanfang and PubMed database was performed for articles related to clinical application of hydrophilic acrylic intraocular lens published from 2005 to 2014. The keywords were “hydrophilic acrylic,intraocular lens” in Chinese and English, respectively. A total of 618 articles were initialy retrieved, and finaly 35 articles were included in result analysis. RESULTS AND CONCLUSION:As the hydrophilic acrylic intraocular lens is foldable, a smal incision is required for implantation and the operation is simple that cause less damage to the patients. In addition, the hydrophilic acrylic intraocular lens has poor bacterial and inflammatory cel adhesion, which leads to a low probability of infective endophthalmitis after implantation. But there is a high incidence of posterior capsule opacification as wel as some refractive errors and visual quality problems after implantation. Taken together, to solve these problems depends on the continuous improvements and updates of intraocular lens materials and designs.
2.Oligodendrocyte transplantation for acute spinal cord injury:insulin-like growth factor 1 induces bone marrow mesenchymal stem cells differentiating into oligodendrocytes

BACKGROUND:Treatment and rehabilitation of spinal cord injury is a complicated problem,andthe reconstruction and remyelination of neural reflex pathwaysare the essentialprocess, during which oligodendrocytes play an important role in spinal cord injury repair.
OBJECTIVE:To observe theeffect ofoligodendrocyte transplantation for acute spinal cord injury in rats.
METHODS:Insulin-like growthfactor 1 induced bone marrow mesenchymal stem cels differentiating into oligodendrocytes, and those oligodendrocytes were transplanted into rats with acute spinal cord injury as induced cel transplantation group. Simple normal salineandnatural oligodendrocytes were transplanted into the rat injured spinal cord as control group and oligodendrocyte group, respectively. Rat behavioral changes were observed by inclined planetest and Basso-Beattie-Bresnahan (BBB) scores. Neurological recovery and survivalof the transplanted cels was detected and observed using spinal evoked potential and immunohistochemical staining, respectively.
RESULTS AND CONCLUSION:Compared with the control group, BBB scores and the criticalelevation angle oftheincline planetestsignificantly increased, latencies of spinal motor and sensory evoked potential were on the decline (P< 0.05), and there were no significant differencesin above indicators between the two groups at 4 and 8 weeks after transplantation. Moreover, survivedoligodendrocytes after transplantation could be found in the lesions of spinal cord in both two groups. In conclusion, insulin-like growth factor 1-induced bone marrow mesenchymal stem cels can differentiate into oligodendrocytesthat exact an excelentrole in acute spinal cord injury repair after transplantation, which achieve the equal clinical efficacy tothenatural oligodendrocytes.
4.Relationship between Onset of Malnutrition and Growth Hormone-Insulin-Like Growth Factor Axis in Children with Congenital Heart Disease

Objective To investigate the relationship between onset of malnutrition and growth hormone-insulin-like growth factor(GH-IGF) axis in children with congenital heart disease(CHD).Methods Fifty cases with CHD were divided into two groups,13 cases of cyanotic group and 37 cases of acyanotic group.And the acyanotic group was then subdivided into two groups,25 cases of heart failure group and 12 cases of without heart failure group,and 20 normal children were selected as control group.The nutritional status of these children were evaluated by detecting the serum total protein,serum protein,body height and weight.IGF-Ⅰ and insulin-like growth factor binding protein-3(IGFBP-3) and growth hormone binding protein(GHBP) were detected by immunoradiometric assay.Results The acute malnutrition ratio and chronic malnutrition ratio in the children with CHD were 62% and 28%,respectively.The acute malnutrition ratio was similar between cyanotic group and acyanotic group(P=0.105);the chronic malnutrition ratio significantly increased in cyanotic group than that in cyanotic group(P=0.006).The levels of serum IGF-Ⅰ and IGFBP-3 decreased in the patients with CHD,which significantly decreased in cyanotic group and heart failure group.The level of serum GHBP decreased in cyanotic group and heart failure group.Conclusions The levels of serum IGF-Ⅰ and IGFBP-3 in patients with CHD decrease,which are related to growth hormone receptor down regulated,that is one of the molecular mechanisms of growth hormone resistant or insensitivity,growth hormone counteract or insensitivity are one of the reasons of acute and chronic malnutrition in children with CHD.

8.Clinical analysis of orbital implantation and postoperative complications

Objective To discuss the cause, managment and prevention of the postoperative complications of orbital implantation by clinical case analysis, and to provide reference for the future clinical work. Methods 175 cases of orbital implantation from Aug. 2011 to Aug. 2013 were retrospectively analyzed in our department. The postoperative complications occurrence, related symptomatic treatment, and the reasons of complications were analyzed. Results Complications occurred in 175 cases of 9 cases(5. 14%),including 4 cases of the con-junctival cyst (2.29%),3 cases of orbital implants exposure(1.71%),1 case of orbital chronic infection(0.57%)and 1 case of subcon-junctival tissue atrophy(0. 57%). All of them had obtained good curative through cyst excision or orbital implanting again after the active treatment. There were no complication occurred again. Conclusion The postoperative complications should be positively analyzed, and early positive precaution could reduce most of the complications.
9.The study on the effect of lovastatin on the expression and activity of β-site APP cleaving enzyme

Objective To study the effect of lovastatin on the expression and activity of β-site APP cleaving enzyme (BACE1). Methods The SH-SY5Y cells were treated by lovastatin. Cell viability was assessed by MTT assay, and the levels of cholesterol were assayed spectrophotometrically, and the activity of BACE1 was detected by fluorometric assay, and the expression level of BACE1 was detected by Western Blot. Results Compared with the control group, the cholesterol levels were decreased by 33.0% (3.33 vs. 4.97; F=5.13, P =0.020) and the activity of BACE1 was reduced by 13.8% (343.14 vs. 398.22; F=3.773, P =0.035) after 24h treatment of 5μmol/L lovastatin; the cholesterol levels were decreased by 49.2% (2.65 vs. 5.22; F=12.239, P =0.001) and the activity of BACE1 was reduced by 38.0% (274.75 vs. 443.14; F=13.610, P <0.01). Treatment with 5μmol/L lovastatin,for 48 h did affect the expression level of BACE1. Conclusions Lovastatin do not affect the expression level of BACE1, but may inhibit its activity and decrease the cholesterol level, thus providing a new approach to prevent or treat Alzheimer's disease (AD).
10.High dose volume and low dose volume of the total and ipsilateral lung in prediction of acute radiation pneumonitis

Objective To observe the incidence of acute radiation pneumonitis (ARP) in patients with nonsmall cell lung cancer treated by three-dimensional conformation radiotherapy or intersity-modulated radiation therapy,and investigate the predictive value of high dose volume and low dose volume of the total and ipsilateral lung on ARP.Methods 174 patients with non-small-cell lung cancer verified by pathology who received radical radiotherapy in our hospital from Aug 2013 to Nov 2015 were screened.Radioactive dose was 60 Gy-70 Gy,with 97 patients' dose of 60-64 Gy and 77 patients' dose of 65-70 Gy.Chemotherapy was done by paclitaxel + pentium first line treatment.155 patients were treated with radiotherapy alone and 19 with radiotherapy combined with chemotherapy.The V5~ V30,MLD (mean lung dose),V5ipis ~ V30ipsi,MLDipsi were calculated from the dose volume histogram system.The ARP was evaluated according to Common Toxicity Criteria 4.0 (CTC4.0).Univafiate and nultivariate analysis and receiver operating characteristic (ROC) curves were performed to assess the correlated factors of ARP.Results Of 174 patients,26 patients (14.94%) developed radioactive pneumonia at grade 2,11 patients (6.32%) at grade 3,2 patients (1.15%) at grade 4.The correlative factors included the radiochemotherapy regimen (sequential chemotherapy to radiotherapy or concurrent radiochemotherapy),V5,V10,V20,V30,MLD of the total lung and ipsilateral lung according to univariate analysis.Multivariate analysis of all the factors mentioned above showed the V5,V20ipsi and MLD were the independent predictive factors associated with ARP.ROC analysis indicated that the cut-off point of V5,V20ipsi,MLD were 62.38%,47.14%and 16.85Gy respectively.According to the V5,V20ipsi value,174 patients were divided into three groups:B group V5<62%+V20ipsi≥47%,C group V5≥62%+V20ipsi<47%,D group V5≥62%+V20ipsi≥47%.The chisquare test between the C group and D group has statistical significance.Conclusion V5,V20ipsi,and MLD are the independent risk factors of ARP.Compared with V5,limiting V20ipsi threshold is more likely to result in ARP.
