4.Clinical study of electrophysiological changes of optic nerves in early period of type 1 diabetes mellitus

AIM:To investigate the value of pattern visual evoked potential (PVEP) and flash electroretinogram (FERG) in early diagnosis and prevention of diabetic retinopathy (DR), analyzing the correlation of early stage DR with PVEP and FERG.
METHODS: Sixty patients, 30 males and 30 females, participated in observation group. Their average age was 19. 42 ± 7. 78years. The duration of DM was < 5a. Best corrected visual acuity was 5. 0. Fasting blood glucose was 7. 8± 3. 6mmol/ L. There were 60 subjects, 30 males and 30 females, in control group. Their average age was 17. 2 ± 6. 52years. Best corrected visual acuity was 5. 0. Every participator was tested with PVEP and FERG according to ISCVE standard. The amplitude of PVEP and P100 latency were recorded. And the b-wave latency, b-wave amplitude, a - wave latency, a - wave amplitude were showed down.
RESULTS: In observation group, P100 amplitude decreased and P100 latency increased, compared to those of control group ( P< 0. 01); b - wave latency, b -wave amplitude, a - wave latency, a - wave amplitude were different from those in control group(P<0. 01); the fasting blood glucose kept stable; P100 amplitude, b -wave amplitude and a-wave amplitude were not related to the DM duration; P100 latency, a-wave latency and b-wave latency were related to the DM duration.
CONCLUSION: PVEP are sensitive to optic neuron damage; FERG is desirable to detect the lesion of Müller cells and bipolar cells. P100 amplitude by PVEP, b-wave amplitude by FERG may be the most sensitive parameter for DR at early stage.

6.Clinical observation of early breast cancer after breastconserving surgery and radiotherapy

Objective To observe the oncololigcal and cosmetic outcomes and complication of breastconserving surgery and radiotherapy for early breast cancer. Methods A total of 78 early breast cancer patients, who were treated in the First People's Hospital of Pingdingshan between January 2001 and September 2008, were identified and followed. Among them, 36 were in stage Ⅰ and 42 in stage Ⅱ. Tangential irradiation was used with a dose of 50 Gy. Tumor bed received a boost dose of 10 Gy electron beam. For axillary lymph node-positive patients, ipsilateral supraclavicular field was hybrid irradiated with 50 Gy X and electron beam.Results The total follow-up ranged from 6 to 98 months. Local recurrence was observed among 4.9% of the patients. The 3-year and 5-year overall survival rate were 96.6% and 92.7% ,respectively. The breast cosmetic satisfaction rate was 94.9%. The main complications reported were acute skin reaction and upper limb edema.Conclusion Breast-conserving surgery and post-operative radiotherapy in early breast cancer patients can lead to satisfactory survival rates and cosmetic effects and hence improved life quality.
8.The rate of no-diagnostic procedures and operator experience in ultrasound-guided fine-needle aspiration biopsy of the thyroid nodules

Objective In clinical practice,ultrasound-guided fine-needle aspiration biopsy (US-G FNAB) is the gold standard in diagnosing the pathological nature of undetermined thyroid nodules before operation.Re---sults of FNAB may vary substantially among operators with different skills.The aim of this study is to evaluate whether operator experience in US-G FNAB influences the rate of diagnostic procedures.Methods A total of 600 consecutive US-guided FNAB done by a single radiologist with 22-or 27-gauge needles from Feb.2008 to Apr.2012 were retrospectively analyzed.All specimens were prepared and fixed without the cytologist on site and were subsequently analyzed by 2 expert cytologists.The procedures were chronologically divided into 6 groups and further stratified according to the overall (A),solid nodules (B),mixed nodules (C),pure cystic nodules (D),acellular sample(E),heavily blood-stained sample(F),exclusively colloid material(G),incorrect slide fixation(H) and they were classified as diagnostic or no-diagnostic.Results The rate of no-diagnostic procedures for each group in learning curves were reported as:① Among A,34% in group 1,15% in group 2,18%in group 3,10% in group 4,7% in group 5,and 8% in group 6.② Among B,4% in group 1,1% in group 2,0% in group 3,0% in group 4,0% in group 5,and 0% in group 6.② Among C,10% in group 1,1% in group 2,4% in group 3,0% in group 4,1% in group 5,and 0% in group 6.④ Among D,20% in group 1,13% in group 2,14% in group 3,10% in group 4,6% in group 5,and 8% in group 6.⑤ Among E,25% in group 1,15% in group 2,15% in group 3,10% in group 4,6% in group 5,and 8% in group 6.⑥ Among F,4% in group 1,0% in group 2,1% in group 3,0% in group 4,0% in group 5,and 0% in group 6.⑦ Among G,1% in group 1,0% in group 2,1% in group 3,0% in group 4,1% in group 5,and 0% in group 6.⑧ Among H,4% in group 1,0% in group 2,1% in group 3,0% in group 4,0% in group 5,and 0% in group 6.Among the 600 procedures,there were 2 cases of vertigo and 4 cases of moderate bleeding at the biopsy site,all of which resolved spontaneously.No major complications were recorded.Conclusions The rate of no-diagnostic US-guided FNAB is heavily dependent on the operator's experience.We estimate that at least 200 procedures are needed in order to achieve the levels of diagnostic accuracy reported in the literature.We therefore suggest operators to get specific training before routinely perform this procedure in clinical practice.
9.Advances in Isobaric Tags for Relative and Absolute Quantitation Techniques Research

Quantitative proteomics is a novel subject of proteomics research. There are several new techniques employed for the protein quantitative study. Isobaric tags for relative and absolute quantitation (iTRAQ) technology for protein quantitation using mass spectrometry is a recent powerful means of determining relative and absolute protein levels in up to four samples simultaneously. The iTRAQ reagent produced high quality, reproducible result in enriched complexes, organelles, and whole cell lysates. The status of the recent promising techniques and their possible future evolution were reviewed.
10.A clinical study of early continuous high-volume-hemofiltration in the treatment of severe acute pancreatitis

Objective To evaluate the efficacy of early continuous high-volume-hemofiltration in the treatment of patients with severe acute pancreatitis (SAP).Methods Based on the method of prospective,randomized and controlled clinical trial,60 patients with SAP between January 2005 and July 2011 from the First Affiliated Hospital of Nanchang University were divided into control group and hemofiltration group.The hemofiltration group was treated with early continuous high-volume-hemofiltration and not in the control group.The changes of vital signs,clinical symptoms and laboratory indicators were compared between the two groups before and after the treatment.Results After hemofiltration,the clinical symptoms such as abdominal pain,fever,tachycardia and respiratory distress in hemofiltration group were significantly remitted compared to those in the control group (P <0.05).The APACHE Ⅱ score (13.3 ± 1.0 vs 14.1 ± 1.2) and the level of TBil[(20.4±11.3) μmol/L vs (28.1 ±10.9) μmol/L],creatinine[(178.7 ±71.8)μmol/L vs (215.6 ± 51.3) μmol/L],blood urea nitrogen[(10.1 ± 5.6) mmol/L vs (13.2 ± 3.8) mmol/L] and ALT[(51.3 ± 13.2) U/L vs (62.5 ±14.3) U/L] were decreased compared to those in the control group (all P values <0.05).The level of PaO2/FiO2(197.3 ±32.4 vs 178.3 ±31.7) was increased (P < 0.05).After hemofiltration,heart rate was decreased gradually (P < 0.05) in the hemofiltration group than in the control group.Mean artery pressure (mAP) increased gradually (P < 0.05) in the hemofiltration group than in the control group.Conclusion Early continuous high-volume-hemofiltration has significant effects on the treatment of SAP including the improvement of clinic symptoms,the blockade of development from systemic inflammatory response syndrome (SIRS) to multiple organ dysfuction syndrome(MODS),improvement of organ function and prevention from the complications.It may become one of the important therapies for SAP.
