1.Nano copper/low-density polyethylene intrauterine device reduces post-implantation uterine bleeding and pain

BACKGROUND:Intrauterine device made of bare copper can release a great amount of oxides that are easy to cause bleeding and pain in women. Nano copper/low-density polyethylene intrauterine device can wel solve the above drawbacks, which is conducive to the maintenance of the reproductive health of women of childbearing age. OBJECTIVE:To explore the effect of nano copper/low-density polyethylene intrauterine device on post-implantation uterine bleeding and pain. METHODS:A total of 98 voluntary women asking for intrauterine device insertion were selected and randomized into control group (n=49) with 220C (TCu220C) T type copper intrauterine device and observation group (n=49) with nano copper/low-density polyethylene intrauterine device. Al the women were folowed up for 12 months, and incidence of adverse events, including uterine bleeding and pain, was observed and compared between the two groups. RESULTS AND CONCLUSION: During the folow-up, there were one case of uterine bleeding and two cases of pain in the observation group and the incidence of adverse events was 6%; in the control group, there were five cases of uterine bleeding and eight cases of pain, and the incidence of adverse events was 27%. There was a significant difference in the incidence of adverse events between the two groups (P< 0.05). These findings indicate that the nano copper/low-density polyethylene intrauterine device can effectively reduce the occurrence of uterine bleeding and pain after implantation.
2.Determination of L-Tryptophan by Using Ordered Mesoporous Silica Modified Carbon Paste Electrode Based on Enhancement Effects of Surfactant

An ordered mesoporous silica ( OMS ) was synthesized through hydrothermal process. The synthesized material was characterized by different techniques such as X-ray diffraction, nitrogen adsorption-desorption and transmission electron microscope. The electrochemical characteristics of the OMS modified carbon paste electrode ( OMS/CPE ) were investigated by using cyclic voltammetry and electrochemical impedance spectroscopy. Compared with bare carbon paste electrode ( CPE ) , the OMS/CPE exhibited a larger electrode surface and a faster electron-transfer rate. Electrochemical behaviors and kinetic properties of L-tryptophan ( L-Trp) at the OMS/CPE in the presence of sodium dodecylsulphate ( SDS) were studied. The results showed that the voltammetric response of L-Trp at OMS/CPE was improved due to the synergistic effect of OMS and SDS. The electrochemical oxidation of L-Trp at OMS/CPE in the presence of SDS was an irreversible process involved two electrons and two protons, and the electrode process was controlled by the adsorption step. Some parameters such as SDS concentration, accumulation time, accumulation potential and pH were optimized. Under optimal conditions, the oxidation peak current was linearly proportional to L-Trp concentration in the range of 8. 0×10-8 to 4. 0×10-6 mol/L, with a detection limit of 7. 0×10-8 mol/L (S/N=3 ) . The proposed method was applied for the determination of L-Trp in oral liquid with the recoveries of 99 . 6%-102 . 6%.
3.Application of modified three-step fluorescence in situ hybridization in prenatal diagnosis

Objective To investigate a simple and efficient fluorescence in situ hybridization method for prenatal diagnosis.Methods Thirty-six cases of chorionic villus samples were hybridized with probes 18/X/Y,13/21 by using traditional cuhure methods and modified three-step methods in controlled experimentation during 2012 to 2013.The hybridization rate and fluorescence signals were analyzed.Results A total of 72 hybrid zone were detected.Probe 21/13 hybridization rate and fluorescence signals of three-step modified methods were higher than that of traditional methods(99.72% ±0.42% vs 85.90% ±4.15%,t =20.4,P <0.01; 2.58 ±0.50 vs 1.52 ± 0.55,t =7.53,P <0.01).Probe 18/X/Y has the same hybridization rate and fluorescence signals between three-step modified methods and traditional methods (99.57% ±0.53% vs99.70% ±0.42%,t=1.30,P>0.05; 2.22±0.42 vs2.36±0.48,t=1.57,P>0.05).The coincidence rate of two methods was 100% (36/36).Conclusion The modified fluorescence in situ hybridization three-step methods in the study was simple,rapid,effective and environment-friendly.At the same time,it has some defects such as the signals of 18/X/Y was not concentrated enough.Further exploration is needed.
4.Analysis on set-up errors of position fixation with simple perforated foam pad in colorectal cancer radio-therapy

Objective To explore the set-up errors of position fixation with simple perforated foam pad in colorectal cancer radiotherapy and their causes as well as the improvement measures.Methods Sixty cases of color-ectal cancer patients undergoing radiotherapy were collected.Prone position was adopted with the position fixed by perforated foam pad and membrane.CBCT imaging was taken weekly before treatment,setup errors were corrected if necessary.Errors throughout the course of treatment for each patient were recorded and compared between patients, followed by analyzing the reasons of errors.Results Before calibration,the maximum errors on the direction of left and right (X-axis),front and back (Z),upside and downside (Y)in these 60 patients were 0.5cm,0.9cm and 0.7cm respectively with the average errors of (0.22 ±0.03)cm,(0.38 ±0.03)cm and (0.27 ±0.04)cm respec-tively.Conclusion The method of applying perforated foam pad and thermoplastic mask in colorectal cancer radio-therapy is currently the most commonly used in the clinical position fixation techniques,by which the small intestine, bladder and other pelvic tissues can be well protected.Due to poor comfort prone position,it is necessary to further improve perforated foam pad through improving the quality of the foam pad,thus improving patients comfort and posi-tioning repeatability and reducing position fixation errors.
5.Clinical application of new type individualized fixation device for radiotherapy of rectal cancer

Objective To explore the clinical effect of new type rectal cancer radiotherapy individualized fixation device in the radiotherapy of rectal cancer. Methods From June 2015 to December 2016,60 patients with rectal cancer who accepted the external irradiation in Zhejiang Tumor Hospital were divided into two groups by random number table method. A group(31 cases) received new type rectal cancer radiotherapy position fixation devices with thermoplastic film. B group(29 cases) received simple foam pad with thermoplastic film. Before each treatment,Cone beam CT(CBCT) scan was conducted. The applied CBCT image and the planned reconstruction image were compared in the direction of X(left and right),Y(upper and lower)and Z(front and rear) axis. The setup error was recorded, and the correlation between the two groups was analyzed. Results The average setup error of patients in A group in X (left and right),Y(upper and lower),Z(front and rear) axis were (1. 61 ± 0. 18)mm,(1. 82 ± 0. 13)mm,(1. 91 ± 0. 11)mm,respectively. The average setup error of patients in B group in X(left and right),Y(upper and lower),Z (front and rear) axis were (2. 22 ± 0. 13)mm,(2. 43 ± 0. 14)mm,(2. 36 ± 0. 13)mm,respectively. There were statistically significant differences between the two groups(t=14. 958,17. 501,11. 283,all P<0. 001). Conclusion The new type of rectal cancer radiotherapy position fixing device is more comfortable than the simple foam pad,and the setting error is smaller than the simple foam pad.
7.Genetic analysis of a 45,X male fetus.

OBJECTIVE:
To analyze the prenatal diagnosis procedure for a 45,X male fetus.
METHODS:
A 31-year-old women underwent amniocentesis due to a moderate risk of trisomy 21. The fetal cells were subjected to chromosomal karyotyping, BACs-on-Beads (BoBs) assay, chromosomal microarray analysis and fluorescence in situ hybridization.
RESULTS:
Combined analyses revealed that the whole of Yp has translocated to 21p, which yielded a fetal karyotype of 45,X,dic(Y;21)(q11;p11).ishdic(Y;21)(SRY+,CEPY+;CEP21+).
CONCLUSION
BoBs and modified N-banding method are helpful for the diagnosis of 45,X male fetus with Yp translocation.
8.Correlation between urinary exosomal miR-155 and the onset and severity of type 2 diabetic kidney disease

Objective:To explore the relationship between urinary exosomal microRNA (exo-miR) - 155 in patients with type 2 diabetes mellitus (T2DM) and the onset and severity of diabetic kidney disease (DKD).Methods:From January to May 2019, 5 patients with T2DM normoalbuminuria and 5 patients with type 2 DKD were recruited from the Department of Endocrinology and Metabolism of Chongming Hospital of Shanghai Health Medical College as a microRNA screening cohort. Urine samples were collected to extract urinary exosomes, and the urine exo-miR spectrum was detected and analyzed using the miRCURY LNA array. From June 2019 to October 2022, 351 patients with T2DM who met the enrollment criteria and were matched by age and sex were included in the validation cohort in the Department of Endocrinology and Metabolism of the hospital. Patients were divided into 3 groups according to urinary albumin/creatinine ratio (UACR): normoalbuminuria group (UACR<30 mg/g, n=143), microalbuminuria group (30 mg/g≤UACR≤300 mg/g, n=171) and macroalbuminuria group (UACR>300 mg/g, n=37). According to DKD diagnostic guidelines, microalbuminuria group and macroalbuminuria group were classified into DKD group. Real-time fluorescence quantitative PCR was used to detect the expression level of exo-miR-155 in urine. Results:The results of transmission electron microscopy, nanoparticle tracking analysis, and Western blotting showed that the extraction of exosome vesicles was successful. In the screening cohort, according to the screening criteria of P<0.05 and fold changes (FC)>1.5, 226 differentially expressed miRNAs were identified from the urinary exosomes of the DKD group compared to the T2DM group. Among them, miR-155 ranged highest (FC=32.75, P<0.001). In the validation cohort, compared with the normoalbuminuria group [0.76 (0.55, 0.95)], the macroalbuminuria group [1.84 (1.18, 2.42)] had the most significant increase in urinary exo-miR-155 level ( Z=-7.411, P<0.001), followed by the microalbuminuria group [0.86 (0.69, 1.25)] ( Z=-4.092, P<0.001), and the urinary exo-miR-155 level in the macroalbuminuria group was significantly higher than that in the microalbuminuria group ( Z=-5.841, P<0.001). The correlation analysis showed that urinary exo-miR-155 level was positively correlated with UACR ( r s=0.329, P<0.001) and negatively correlated with estimated glomerular filtration rate ( r s=-0.249, P=0.015). The results of receiver operating characteristic curve analysis showed that the area under the curve of urinary exo-miR-155 level predicted DKD progression in T2DM patients was 0.892 (95% CI 0.859-0.925), corresponding cutoff value was 0.982, and the sensitivity and specificity were 71.9% and 87.7%, respectively. Multivariate logistic regression analysis showed that urinary exo-miR-155≥0.982 was an independent risk factor for progression to DKD in T2DM patients ( OR=3.310, 95% CI 1.981-5.530, P<0.001). Conclusion:The expression level of urinary exo-miR-155 is increased in T2DM patients with microalbuminuria and macroalbuminuria, which is related to the degree of albuminuria, and can be used as a predictive marker to identify potential DKD.
9.Study of two cases of prenatally detected small supernumerary marker chromosomes.

OBJECTIVETo determine the origin of two prenatally detected small supernumerary marker chromosomes (sSMCs).
METHODSThe sSMCs were analyzed with combined G-banding, C-banding, fluorescence in situ hybridization (FISH), and single nucleotide polymorphisms array (SNP-array) techniques.
RESULTSIn case 1, G-banding analysis has identified a 47,XY,+mar karyotype. Affymetrix CytoScan 750K Array scan has suggested arr 15q11.2q12(22 770 421-26 604 587)?, while FISH analysis suggested 47,XN,+mar.ish i(15)(q12)(D15Z1+,SNRPN++,PML-). In case 2, G-banding analysis has suggested 46,X,+mar/46,XY, FISH analysis showed two SRY hybridization signals, indicating 46,X,i(Y)(p10)/46,XY.
CONCLUSIONMultiple techniques needed be applied for verification of the origin of sSMCs identified in prenatal diagnosis.

10.Abnormal changes of functional connectivity between anterior cingulate cortex and amygdala in ado-lescents patients with first-episode depression

Objective To study the resting-state functional connectivity( FC) changes of anterior cingulate cortex(ACC) and amygdala in adolescent patients with medication-naive first-episode major de-pressive disorder(MDD).Methods Medication-naive first-episode MDD adolescents( n=30) and pair-wise matched healthy controls(HCs,n=29) underwent resting-state functional magnetic resonance images(fM-RI).To study whether there were different in FC changes between amygdala and ACC and whole brain re-gions,and whether abnormal FC was related to clinical characteristics.Results Compared with HCs,the ado-lescent MDD patients showed decreased FC between the left amygdala and right thalamus( MNI x,y,z:15,-21,18)(t=-2.85)and increased FC between the left amygdala and the bilateral temporal middle gyrus,left occipital gyrus,right medial prefrontal cortex(MPFC)(MNI x,y,z:9,66,15) ( t=2.80).The right amygdale showed decreased FC with bilateral middle frontal gyrus,left caudate(MNI x,y,z:-15,-9,21)(t=-2.53), right thalamus(MNI x,y,z:12,-36,12)(t=-3.01),right cerebellum(MNI x,y,z:3,-84,-33)(t=-2.45) and increased FC with right temporal middle gyrus,right MPFC( MNI x,y,z:6,69,12) ( t=2.41).The left ACC showed decreased FC with left cerebellum( MNI x,y,z:12,-87,-24) ( t=-2.36),left frontal inferior triangle gyrus,right frontal inferior orbital gyrus and increased FC with right frontal middle orbital gyrus.The right ACC showed decreased FC with left caudate(MNI x,y,z:-15,21,6)(t=-2.29),right temporal middle gyrus,right MPFC(MNI x,y,z:9,54,36)(t=-2.29),right frontal inferior orbital gyrus,right angular(MNI x,y,z:45,-51,21)(t=-3.01),left dorsolateral prefrontal cortex(MNI x,y,z:-21,33,36) ( t=-2.61).The increased FC between the left amygdale and bilateral temporal middle gyrus showed positive correlation with the score of HAMD(r=0.50,P<0.05;r=0.45,P<0.05).And the FC between the right amygdale and right middle frontal gyrus was positive correlation with duration of disease( r=0.63,P<0.05).Conclusion There is widespread FC disorder in amygdala and ACC in adolescents patients with first-episode depression.Abnor-mal FC changes may be the neuropathological mechanism of depression.
