1.Drug-resistance of Pathogens in Cardiovascular Hospital

OBJECTIVE To investigate the pathogens and their drug-resistance in Fuwai Cardiovascular Hospital and provide antibiotics use suggestion for clinical treatment.METHODS The pathogens were identified by VITED 32 and analyzed by WHONET 5.4 RESULTS The pathogens mainly consisted of Gram-negative bacilli,which were highly sensitive to imipenem and meropenem except Pseudomonas aeruginosa;E.faecalis was much more sensitive to penicillin and gentamicin than E.faecium.Most coagulase-negative Staphylococcus(CNS) were resistant to oxacillin and showed low susceptibility rates to most antibiotics.No Gram-positive cooci were found to be resistant to vancomycin and teicoplanin.CONCLUSIONS To investigate the pathogens and their drug resistance is very important to prevent and control nosocomical infections.
2.Antibiotics Resistance in Surgery Wards of Fuwai Cardiovascular Hospital

OBJECTIVE To investigate the major pathogens and their antibiotics resistance in surgery wards in Fuwai Cardiovascular Hospital.METHODS The pathogens were classified and their antibiotics resistance was analyzed.RESULTS The pathogens mainly consisted of Gram-negative bacilli,which were sensitive to cefoperazone/sulbactam,pipercillin/tazobactam and amikacin with the sensitive rate of 62.7-97.6%.All Gram negative bacilli were sensitive to imipenem except Pseudomonas aeruginosa,with the sensitive rate of 95.2%~100%.In the Gram-positive bacteria,the susceptibility rates of Enterococcus faecium were lower than E.faecalis.Coagulase negative Staphylococcus and Staphylococcus aureus were mainly resistance to oxacillin and most other antibiotics,all Gram positive bacteria were sensitive to vancomycin.CONCLUSIONS To know the major pathogens and their susceptibility rates can prevent and control nosocomial infections effectively.
3.Evaluation of Gd-DTPA enhanced MR in the Diagnosis of The Degeneration Stage of Cerebral Cysticercosis

Objective To detect the value of qualitative and orientating diagnosis in the degeneration stage of cerebral cysticercosis with Gd-DTPA enhanced MR. Methods Sixty-nine cases of cerebral cysticercosis were diagnosed by enhanced MR as degeneration stage, and confirmed by immunological examination and/or by surgery. MR plain scanning was conducted for the same cases. Results The plain scanning showed single or multiple lesions with long T1 and long T2 signals, and the enhanced scanning showed nodular or annular lesions. The diameter of the lesion after enhanced scan was not more than 22mm with an average value of 8.1mm. Some cases showed single lesion on plain scanning but showed multiple lesions after enhanced scan. Conclusion The enhanced MR shows more typical features of the degeneration stage cerebral cysticercosis. It can define the number, position and range of the lesions, and can improve the accuracy of differential diagnosis, and therefore be of importance in formulating treatment scheme and prognosis.

5.Changes of serum inflammatory mediators in secondary septic shock after chemotherapy for acute leukemia

Objective To observe and analyze the change state of serum inflammatory mediators of patients with secondary septic shock after chemotherapy for acute leukemia .Methods 56 patients with secondary septic shock after chemotherapy for acute leukemiawere were selected as the group A,56 patients without secondary septic shock after chemotherapy for acute leukemia were selected as the group B ,56 healthy persons of the same age were selected as the group C,then the serum inflammatory mediators levels of three groups were detected and compared,and the serum indexes of group A with different severity degree of shock were compared, then the detection results of group A and group B with different types ( lymphocytic leukemia and nonlymphocytic leukemia)were compared.Results The serum inflammatory mediators levels, including TNF-α, IL-4, IL-6, IL-10 and other inflammatory factors, including PGE2 , sICAM-1, sVCAM-1 and sCD44, there were group A >group B >group C, severe shock >moderate shock>mild shock ( all P<0.05 ) .The above serum inflammatory mediators and other inflammatory factors levels of different types in group A were higher than those in group B, and the above mediators in lymphocytic leukemia of group A and group B were higher than those in nonlymphocytic leukemia (all P<0.05).Conclusion The serum inflammatory mediators of patients with secondary septic shock after chemotherapy for acute leukemia show higher expression state,and the influence of shock degree and leukemia types for the expression are great.
6.Effect of sixpetal clematis root injection on histomorphology and collagen expression in cartilage of osteoarthritis animal model

[Objective]To study the effect of sixpetal clematis root injection(WLX)on histology,collagen phenotype and ultramicrostructurc in cartilage of osteoarthritis animal model.[Method]OA model animals were randomly divided into 3 groups:group S,group D and group K.Animals in group S received WLX and animals in group D received sterilized normal saline intramuscularly,group K was control and received nothing.After the animals were killed,cartilage specimen were obtained and sections were made and stained with HE/SOFG for Mankin score;the expression level of collagen Ⅰ and Ⅱ in matrix was tested by immunohistochemistry;change of ultramicrostructure of cartilage was observed with transmission electron microscope.[Result]Mankin score in group S was better than that in group D and K at each test period;result of IHC shows that positive stain area of collagen Ⅰ was lower in group S than that in group D and K,and positive stain area of collagen Ⅱ was higher than that in group D and K at each test period;cartilage impairment degree in group S was lower than in group D and K,and impairment manifestation in group D and K was similar on the whole.[Conclusion]sixpetal clematis root injection may protect articular cartilage by maintaining and promoting the chondrocyte synthesizing proteoglycan and type Ⅱ collagen.
7.Nano copper/low-density polyethylene intrauterine device reduces post-implantation uterine bleeding and pain

BACKGROUND:Intrauterine device made of bare copper can release a great amount of oxides that are easy to cause bleeding and pain in women. Nano copper/low-density polyethylene intrauterine device can wel solve the above drawbacks, which is conducive to the maintenance of the reproductive health of women of childbearing age. OBJECTIVE:To explore the effect of nano copper/low-density polyethylene intrauterine device on post-implantation uterine bleeding and pain. METHODS:A total of 98 voluntary women asking for intrauterine device insertion were selected and randomized into control group (n=49) with 220C (TCu220C) T type copper intrauterine device and observation group (n=49) with nano copper/low-density polyethylene intrauterine device. Al the women were folowed up for 12 months, and incidence of adverse events, including uterine bleeding and pain, was observed and compared between the two groups. RESULTS AND CONCLUSION: During the folow-up, there were one case of uterine bleeding and two cases of pain in the observation group and the incidence of adverse events was 6%; in the control group, there were five cases of uterine bleeding and eight cases of pain, and the incidence of adverse events was 27%. There was a significant difference in the incidence of adverse events between the two groups (P< 0.05). These findings indicate that the nano copper/low-density polyethylene intrauterine device can effectively reduce the occurrence of uterine bleeding and pain after implantation.
8.Relation between IL-8 gene polymorphism and delayed Alzheimer's disease in Xinjiang Han people

Objective To study the relation between IL-8 gene-251A/T polymorphism and delayed Alzheimer's disease (AD)in Xinjiang Han people .Methods Eighty delayed AD patients served as a delayed AD group and 80 subjects undergoing physical examination served as a control group in this study .Distribution of IL-8 gene-251A/T polymorphism in two groups was detected by PCR-RFLP .Results The distribution of genotypes and alleles was different in two groups (P=0 .023 , P= 0 .010) .Further stratified analysis showed that the frequency of A allele was significantly higher in delayed AD group than in control group (OR= 1 .851 ,95% CI:1 .159 -2 .957 ,P=0.010) ,indicating that A allele is the risk factor for delayed AD .The frequency of AA genotype was significantly higher in delayed AD group than in control group (OR=3 .370 ,95% CI:1 .143-9.939 ,P=0 .023) ,indicating that the risk of delayed AD was 3 .370-fold higher in subjects with AA genotype than in those with TT genotype .The AT genotype was not related with the risk of delayed AD (OR=1 .944 ,95% CI:0 .994-3 .803 ,P=0 .051) .Conclusion IL-8 gene-251A/T pol-ymorphism is related with the risk of delayed AD .
9.Distribution and antibiotic resistance of the microorganisms isolated from blood samples during 2011-2015 in Fuwai Hospital

Objective To analyze the distribution and antibiotic resistance of the microorganisms isolated from blood samples in the Cardiovascular Institute,Beijing Fuwai Hospital,for clinicians to improve antimicrobial therapy.Methods Blood samples were cultured and bacterial isolates were identified and tested for their susceptibility to antimicrobial agents.Results A total of 2 017 (8.3%) strains of microorganisms were isolated from 24 348 blood samples.Finally,1 009 nonduplicate strains were analyzed,including gram positive cocci (n=574,56.9%),gram negative bacilli (381,37.8%),and Candida species (54,5.4%).The top gram positive cocci were coagulase-negative Staphylococcus (31.8%),Streptococcus (11.0%),Staphylococcus aureus (5.5%),Enterococcus faecalis (4.0%),and Enterococcus faecium (1.5%).The top gram negative bacilli were Acinetobacter baumannii (6.7%),Pseudomonas aeruginosa (6.0%),Escherichia coli (5.7%),Klebsiella pneumoniae (4.3%),Stenotrophornonas maltophilia (3.5%),and Enterobacter cloacae (3.3%).Candida albicans was the most frequently isolated Candida species.Staphylococcus isolates were sensitive to linezolid,vancomycin,tigecycline,and quinupristin-dalfopristin.Enterococcusfaecalis were sensitive to linezolid,tigecycline,high level streptomycin,high level gentamicin,penicillin,ampicillin,and vancomycin.Enterococcus faecium were less sensitive than Enterococcus faecalis.All Enterococcus strains were sensitive to linezolid,tigecycline,high level streptomycin,high level gentamicin,and vancomycin.Gram negative bacilli were relatively sensitive to cefoperazone-sulbactam,piperacillin-tazobactam,cefepime,amikacin and carbapenems.A.baumannii and P.aeruginosa isolates showed lower susceptibility to carbapenems than E.coli,K.pneumoniae and E.cloacae.Conclusions The distribution of the pathogens isolated from blood samples was relatively stable in the past five years in our hospital.Gram positive cocci are more prevalent than gram negative bacilli in blood samples.Clinicians should select antimicrobial agents according to the distribution of pathogens and the antimicrobial resistance profile.
10.Study on the Quality Standard of Jinbo Capsule

OBJECTIVE:To provide reference for the establishment of quality standard for Jinbo capsule. METHODS:TLC was conducted for the qualitative identification of Phellodendron chinense and Melia toosendan in preparation;HPLC was used for the content determination of berberine hydrochloride in preparation;the column was Diamonsil C18 with mobile phase of acetoni-trile-0.1% phosphoric acid (50∶50,V/V) at flow rate of 1 ml/min,detection wavelength was 265 nm,column temperature was 25 ℃ and volume injection was 10 μl. RESULTS:TLC spots of P. chinense and M. toosendan in preparation were clear and well-separated. The linear range of berberine hydrochloride was 0.044-0.154 mg/ml(r=0.999 9);RSDs of precision,reproducibili-ty and stability tests were no more than 2.0%,recovery was 98.36%-102.15%(RSD=1.11%,n=9). CONCLUSIONS:The meth-od is simple,accurate and reproducible,and can be used for the quality control of Jinbo capsule.
