OBJECTIVE:To provide reference for the individualized treatment of opioid drugs by investigating the effects of OPRM1 A118G gene polymorphism on opioids dosage in Chinese Han Population patients with cancer pain. METHODS:66 Chi-nese Han Population patients with medium and severe cancer pain were selected from a third grade class A hospital. Using fluores-cence in-situ hybridization,OPRM1 A118G genotypes were detected to compare the opioids dosage of different genotype in pa-tients with cancer pain. RESULTS:Among 66 patients,distribution frequency of AA,AG and GG genotype were 36.36%, 53.03% and 10.61%,respectively;those of A and G allele were 62.88% and 37.12%;the daily dosage of opioids in AA,AG, GG genotype patients respectively were(21.67±13.41),(42.00±32.18)and(87.14±73.65)mg,with statistical significance(P<0.001). Among 39 patients receiving opioids for the first time,the dosage of 6 AA genotype patients and 15 AG genotype patients had been adjusted,with statistical significance(P=0.026). After adjusting dosage,daily dosage of AA genotype and AG genotype patients were (16.11 ± 9.16) and(28.57 ± 18.52)mg,with statistical significance (P=0.011). CONCLUSIONS:OPRM1 A118G genotype can influence the dosage of opioid in Chinese Han Population patients with cancer pain,and gene polymorphism can be used as evidence for dosage guide of opioids.

Objective To investigate the correlation between the expression changes of brain-derived neurotrophic factor (BDNF) in colon mucosa and abdominal pain in irritable bowel syndrome (IBS). The density of nerve fiber in colon mucosa and ultrastructural alterations of nerve fiber in IBS were also observed. Methods From September 2008 to January 2010,the IBS patients who visited the department of gastroenterology of our hospital and met the Rome Ⅲ diagnosis criteria were selected and divided into IBS with diarrhea (D-IBS) and IBS with constipation (C-IBS) according to their clinical features. The patients with colon polyps detected by colonoscopy in our hospital were selected as control group. All subjects were asked to fill in Self-Rating abdominal pain or abdominal uncomfortable Scale according to abdominal symptom in the last 2 weeks before visit and underwent colonoscopy. Four biopsy specimens were taken from the colon mucosa of rectosigmoid junction. Ofwhich,two specimens were for protein isolation and detection of BDNF expression level,one specimen was used for PGP 9. 5 immunohistochemistry staining in paraffin slices. Another specimen was used to observe the ultrastructure changes of nerve fiber in colon mucosa under transmission electron microscopy. Results Total 40 IBS patients were enrolled in this study,of those 21 were D-IBS patients,19 were C-IBS patients,and 21 were controls. The abdominal pain severity score and frequency score of IBS patients were (2. 3±0. 8) and (2. 1±0. 7),which were significantly higher than those of control group (0. 4±0. 7 and 0. 3±0. 5,P＜0. 001). Compared with the control group,the BDNF expression in colon mucosa was significantly elevated in IBS patients (P= 0. 003 ),and which correlated with the severity and frequency of abdominal pain/discomfort (r=0. 57,P＜0. 001and r=0. 46,P= 0. 003,respectively). The immunohistochemistry result indicated that the nerve fiber density in colon mucosa of IBS patients was significantly higher than that of controls,and there were ultrastructural changes of colon mucosal nerve fibers in IBS patients. Conclusion Increased colon mucosal BDNF expression may be associated with abdominal pain symptom in IBS patients. The impaired ultrastructural of mucosal nerve fibers may cause the increased BDNF expression in colon mucosa,and result in the increased mucosal nerve fiber density in IBS patients.

Objective To compare drug therapy in patients suffering from chronic heart failure between Tianjin and Kashi region of Xinjiang autonomous region and to analyse their disparities in drug uses by guideline between the two regions.Methods All data were retrospectively taken from the hospitalized chronic heart failure cases from June 2005 to June 2006 in the 2nd Hospital of Tianjin Medical University and the 2nd People's Hospital of Kashi in Xinjiang respectively.The heart functions level NYHA Ⅱ-Ⅳ were recorded for analysis of drug use in the two regions.Results 749 cases were enrolled,491 from Tinajin and 258 from Kashi of Xinjiang.The average age of group Tianjin[(70.39±10.84)yrs]was older than that[(62.91±12.54)yrs]of group Kashi(t=8.488,P＜0.01).The distribution of≥165 cases in the group Tianjin(75.6%)was higher than that of group Kashi(51.2%)(X2=45.642,P＜0.01).The male cases in group Tianjinwere 264(53.8%)andthatwere 134(51.9%)in group Kashi with no significant differences in sex distribution between the two groups(P＞0.05).The first three etiological diseases were coronary heart disease(84.9%),rheumatic heart disease(5.9%)and dilated cardiomyopathy (3.3%)in group Tianjin,and that were coronary heart disease(63.2%),pulmonary heart disease(19.8%)and dilated eardiomyopathy(6.6%)in group Kashi respectively.The proportions of level NYHA Ⅱ,Ⅲ,Ⅳ on admission were 29.7%,39.1%and 31.2%in group Tianjin respectively,and that were 15.5%,39.5%and 45.0%in group Kashi respectively,with significant difference in heart function levels between the two groups(X2=22.770,P＜0.01).Theusages of nitrides and β-blockers in group Tianjin were more than that in group Kashi(both P＜0.01).The usages of diuretics,digitalis was more in group Kashi(all P＜0.01).There was no difference in ACEI usages between the two groups.The dosages of drugs in group Tianjin achieved the target dosages by the guideline and the dosages in group Kashi did not achieve the targets.The use frequency of β-blockers was more in male cases and＜65yrs respectively of group Tianjin.The usage frequency of uretics and aldosterone antagonist were more in＜65yrs cases of group Kashi and the usages of nitrides,ACEI,B-blockers,calcium antagonists,aldosterone antagonist of male cases were more than that of female cases(all P＜0.05)in this group.The usages of uretics,digitalis,aldostemne antagonist increased following the grading of heart function of the two groups(P＜0.05).Moreover the usage of ACEI decreased following the worsen heart function in group Tainjin and this was not the same in group Kashi.Conclusion There are differences in drug medications for chronic heart failure between Tianjin and Kashi.The majority of treatment drugs in Tianjin is approaching the guideline and there is a gap from guideline in Kashi of Xinjiang,especially on the dosage.

AIM: To understand whether reactive oxygen species promote the rupture of atherosclerotic plaques by regulating the balance of matrix metalloproteinase-1,3 (MMP-1,3) and tissue inhibitor of matrix metalloproteinase-1 (TIMP-1) in smooth muscle cells. METHODS: Aortic smooth muscle cells from 4-6months-healthy abortive fetuses were incubated for 24 hours with xanthine (100 ?mol/L) and xanthine oxidase (5 U/L) in vitro . MMP-1,3 and TIMP-1 in the concentrated culture media were measured by Western blotting ( n =3 independent experiments). RESULTS: Incubation with xanthine/xanthine oxdiase decreased the amount of MMP-1 in the aortic smooth muscle cells (21.2%?5.5% of the control group),and pro-MMP-1 was activated completely. Reactive oxygen species (ROS) also activated pro-MMP-3,and increased the production of MMP-3 in the aortic smooth muscle cells. On the other hand,ROS inhibited the production of TIMP-1 in the aortic smooth muscle cells. CONCLUSION: It is complicated that ROS regulates the balance of MMPs and TIMPs. ROS may contribute to matrix degradation and the rupture in the atherosclerotic plaques.

Objective To observe the role of brain-derived neurotrophic factor (BDNF) in the plasticity of enteric nervous-smooth muscle system,and to investigate the effects of BDNF induced plasticity on gastrointestinal motility in mice.Methods Male hybrid BDNF knockout (BDNF+/) mice and wild type (BDNF+/+) mice were selected,eight in each group.Gastrointestinal motility of BDNF+/+ mice and BDNF+/ mice were tested and compared.Longitudinal muscle strips of mice colon smooth muscle were prepared.The effects of carbachol (1 × 10 5 mol/L) and BDNF (1 × 10 7 mol/L) on contractile function of muscle strips were observed.And the effects of tetrodotoxin (TTX,1 × 10-6 mol/L) on BDNF induced contractile function of muscle strips were also studied.The changes of the density of mice intestinal myenteric plexus and the expression of smooth muscle α-actin (α-SMA) in colon smooth muscle were detected by immunohistochemical techniqne.The ultrastruetural alterations of myenteric plexus,neuromuscular junction (NMJ) and smooth muscle cells were detected by transmission electron microscope (TEM).T-test or Rank sum test was performed for comparison between groups.Results Number of feces particles and water content in feces of BDNF+/-mice ((3.80±0.75) and (39.60±1.47)％) were both lower than those of BDNF+/+ mice ((6.30± 1.03) and (51.00± 1.61) ％),and the differences were statistically significant (t=4.792,12.827;both P＜0.05).Carbachol (1 × 10-5 mol/L) could significantly increase contraction activity of smooth muscle of BDNF+/+ mice (R =3.26 ± 0.43) and BDNF+/-mice (R=2.15 ± 0.36),and the difference was statistically significant (t=15.754,9.632;both P＜0.05).The effects on contraction exciting of smooth muscle strips of BDNF+/+ mice were more significant than BDNF+/ mice,and the difference was statistically significant (t =5.972,P＜0.05).BDNF could significantly increase contraction of muscle strips of BDNF+/+ mice and R value increased from 1 to 1.41±0.09,and the differences were statistically significant (t=13.674,P＜0.05).TTX could obviously inhibit the excitatory effects of BDNF,R value decreased from 1.41 ± 0.09 to 1.03 ± 0.04 (t=11.692,P＜0.05).The density of myenteric plexus of BDNF+/ mice (median 5.8％,interquartile range 4.2％-7.0％) was significantly lower than that of BDNF+/+ mice (median 9.0％,interquartile range 7.1％-10.8％),and the difference was statistically significant (Z =3.730,P＜ 0.05).The expression of α-SMA of BDNF+/-mice (median 33.4％,interquartile range 28.8％-38.5％) was significantly lower than that of BDNF+/+ mice (median 44.6％,interquartile range 39.2％-48.8％),and the difference was statistically significant (Z=4.565,P＜0.05).The results of TEM indicated ultrastructural alterations of myenteric plexus,NMJ and smooth muscle in BDNF+/-mice.Conelusionss BDNF could induce the plasticity of morphology and function in enteric nervous-smooth muscle system,which may play an important role in mice gastrointestinal motility.

Objective To investigate the assessment methods and mechanisms of nonsteroidal antiinflammatory drugs (NSAID)-induced injury in rat small intestinal epithelial barrier,and to explore the protective effects of mucosal protective agents and antacids on it.Methods A total of 96 rats were evenly divided into the morphologic observation group and the mechanism research group,and 48 in each group.Then each group was evenly divided into eight subgroups:the healthy control group,the model group (model established with indomethacin),the teprenone prevention group,the rabeprazole prevention group,the treatment control group,the teprenone treatment group,the rabeprazole treatment group and the teprenone and rabeprazole combined group (combined group),six in each group.Exfoliated cells gap density of small intestine of each subgroup was determined by confocal laser endomicroscopy.Serum level of tumor necrosis factor-α (TNF-α)was measured with enzyme-linked immunosorbent assay (ELISA). The expression of nuclear factor-κB (NF-κB),caspase-3,zonula occludens-1 (ZO-1 )and occludin at protein level was detected by Western blotting.The LSD-t test or Hamhane′s T2 test was performed for statistical analysis.Results The exfoliated cells gap densities of the teprenone prevention group and the rabeprazole prevention group were (57.43 ± 24.55 )/1 000 and (59.80 ± 21 .14 )/1 000,respectively, which were both lower than that of the model group ((110.93±50.58)/1 000),and the differences were statistically significant (t= 53.50 and 54.13,both P < 0.01 ).The exfoliated cells gap density of the combined group was (40.53 ±15 .39)/1 000,which was lower than that of the treatment control group ((93.80±40.65 )/1 000 ),and the difference was statistically significant (t =44.27,P <0.01 ).The serum levels of TNF-α of the teprenone prevention group and the rabeprazole prevention group were (25 .80±8.97)ng/L and (22.74 ±7.15 )ng/L,repsectively,which were both lower than that of the model group ((44.48 ± 7.42 )ng/L),and the differences were statistically significant (t = 18.68 and 21 .74,both P <0.01 ).The serum level of TNF-αof the combined group ((13.66 ±4.98)ng/L)was lower than that of the treatment control group ((24.67±6.70)ng/L),and the difference was statistically significant (t = 9.02,P < 0.01 ).The caspase-3 levels of teprenone prevention group and rabeprazole prevention group were 1 .47 ±0.35 and 1 .58 ±0.34,and the NF-κB levels of these two groups were 1 .27±0.14 and 1 .21 ± 0.10,respectively.Compared with those of model group (2.44 ± 0.45 and 1 .69±0.13),the differences were statistically significant (t =0.97,0.86,0.42 and 0.48,respectively, all P <0.01 ).The levels of caspase-3 and NF-κB of the combined group were 0.66±0.06 and 0.44 ± 0.21 ,respectively,which were lower than those of the treatment control group,and the differences were statistically significant (t=0.34 and 0.56,both P <0.01).The expressions of occludin at protein level of the teprenone prevention group and the rabeprazole prevention group were 0.69 ±0.16 and 0.74 ±0.11 , and the levels of ZO-1 were 0.81 ± 0.08 and 0.84 ± 0.12.Compared with those of the model group (0.45 ±0.22 and 0.64±0.07 ),the differences were statistically significant (t =0.24,0.29,0.17 and 0.21 ,respectively,all P <0.01 ).The levels of occludin and ZO-1 of the combined group were 2.50 ± 0.46 and 1 .76±0.18,which were higher than those of the treatment control group,and the differences were statistically significant (t =1 .50 and 0.76,both P <0.01 ).Conclusions The exfoliated cells gap density may be a valuable indicator to predict the degree of inflammation response and permeability of epithelial barrier as well as to evaluate efficacy of medication.Teprenone and rabeprazole have prevention and treatment effects on NSAID-induced injury in rat small intestine.

ObjectiveTo explore the effect of simvastatin on the level of serum adiponectin and asymmetric dimethylarginine (ADMA) in patients with acute stroke.MethodsNinety patients with acute stroke were selected and divided by random digits table method into common treatment group and intervention treatment group with 45 cases each.The volume of cerebral infarction and the evaluation score of clinical neurological impairment degree were calculated.The levels of serum adiponectin and ADMA before and after treatment were detected by erzyme-linked immunosorbent assay.The changes of serum adiponectin and ADMA before and after treatment in two groups were compared and their correlations with the volume of cerebral infarction and the evaluation score of clinical neurological impairment degree were analyzed.Results The level of serum adiponectin and ADMA in two groups before treatment had no statistical differences (P ＞ 0.05 ).The levels of serum adiponectin in two groups after treatment increased and ADMA decreased significantly,which had significant differences compared with those before treatment ( P ＜ 0.05 ).The level ofserum adiponectin in intervention treatment group was significantly higher than that in common treatment group and ADMA was obviously lower than that in common treatment group after treatment [(5.92 ± 0.15)mg/L vs.( 4.51 ± 0.13 ) mg/L,( 0.96 ± 0.13 ) μ mol/L vs.( 1.08 ± 0.15 ) μ mol/L ] ( P ＜ 0.05 ).Rank correlation analysis showed that the level of serum adiponectin before treatment was negatively correlated with the volume of cerebral infarction and the evaluation score of clinical neurological impairment degree ( r ＝ -0.75,-0.59,P ＜ 0.05).The level of ADMA before treatment was positively correlated with the volume of cerebral infarction and the evaluation score of clinical neurological impairment degree ( r =0.68,0.71,P ＜ 0.05 ).Conclusion Simvastatin can increase the level of serurm adiponectin and decrease the level of ADMA efficiently in patients with acute stroke,and improve the prognosis of the patients.

OBJECTIVETo assess mental fatigue by noninvasive monitoring of the physiological signals.

METHODSThe changes in the physiological parameters including the electrodermal activity, heart rate and heart rate variability were analyzed in 14 subjects performing the reaction-time tasks when fatigue and changes in the reaction time occurred.

RESULTSThe average skin conductance level, average heart rate, and heart rate variability parameters including the total power density, percentage of the very low power density, percentage of high power density all differed significantly between the sober state and the mental fatigue state.

CONCLUSIONMonitoring the physiological parameters including the electrodermal activity, heart rate and heart rate variability is a noninvasive, effective and practical approach to mental fatigue assessment.

OBJECTIVETo propose a new method for acquiring pulmonary function parameters based on measurement of volume changes of thoracic impedance.

METHODSWe studied the relationship between the volume changes of thoracic impedance and pulmonary function parameters during forced breathing based on bioimpedance measurement, and developed an instrument for measuring thoracic impedance. Using this instrument and a MRI spirolab III lung function test instrument, both based on flowmeter measurement, we measured such pulmonary function parameters including forced vital capacity (FVC), forced expiratory volume in one second/FVC (FEV1/FVC), and peak expiratory flow in 10 healthy volunteers and compared the measurement results.

RESULTSThe differences in the parameters measured using the two instruments were all within two folds of the positive and negative standard deviations of the average values, demonstrating good consistency in the measurement between the two methods.

CONCLUSIONSThe measurement results of the bioimpedance-based instrument we developed show good consistency with those by the commercially available pulmonary function test instrument.

Objective: To explore the trend of blood pressure among children and adolescents and its correlation with pubertal development and nutritional status, so as to provide a scientific basis for developing prevention strategies targeting early stage hypertension. Methods: In September 2023, a multi stage random cluster sampling was used to select 20 241 primary and secondary school students aged 7-18 years from 57 schools in Jinzhong, whose height, weight, and blood pressure were measured. The Chi square test for trend was used to analyze the change trend in the detection rate of elevated blood pressure and hypertension in children and adolescents with the development of adolescence puberty, and the Logistic regression was used to analyze the correlation between elevated blood pressure and hypertension and nutritional status. Results: The prevalence of elevated blood pressure was 17.3% among children and adolescents. The middle and late pubertal groups had the highest prevalence (23.2% and 24.3%), followed by the early pubertal group (19.5%) and the prepuberty group (10.8%) ( χ 2 trend =372.86， P <0.01). The prevalence of hypertension was 14.8%, with the highest prevalence reported in the late pubertal group (22.4%), followed by the middle pubertal group (18.9%), and the early pubertal (13.0%)/prepuberty (12.2%) groups ( χ 2 trend =175.43， P <0.01). The prevalence of elevated blood pressure and hypertension increased with pubertal development, regardless of gender, region, or nutritional status ( χ 2 trend =9.21-771.90, P <0.01). Overweight and obesity were influencing factors of elevated blood pressure and hypertension among children and adolescents during all stages of pubertal development ( OR =1.2-2.8， P <0.01). Conclusion The prevalence of elevated blood pressure and hypertension among children and adolescents during pubertal development is high, showing an increasing trend with pubertal development.
