Objective Acute esophageal variceal bleeding is one of the most dreadful complications of cirrhotic portal hypertension.This study evaluated a modified gastro-esophageal decongestion (MGED) procedure in the management of acute variceal bleeding that could not be controlled by drugs and/or endoscopic therapy.Methods A MGED procedure was performed in 136 patients with uncontrollable acute variceal bleeding by drugs and/or endoscopic therapy.The perioperative and long-term outcome of these patients were retrospectively studied.Resluts Immediate control of variceal bleeding was achived in 97.8％ (133/136) patients.Perioperative mortality occurred in 5.1％ (7/136) patients.The 3-year and 5-year rebleed rate was 7.0％ (9/129) and 13.2％,respectively.The 3-year and 5-year overall survival rate was 91.5％ (118/129) and 79.8％ (103/129),respectively.Conclusions In this study,the MGED procedure was an effective life-saving procedure in controlling acute esophageal variceal bleeding for its safe,simple and less time-consuming advantages.

Objective To investigate the characteristics of everyday memory of adults with subjective memory complaints (SMC),and probe into the correlation between everyday memory function and life quality.Methods Through convenience sampling,150 adults with SMC were face-to-face interviewed by an investigator,with demographic characteristics collected,and the cognition function,everyday memory function and life quality were also assessed during the interview.Results Among the 150 adults with SMC,the impairment rate of everyday memory function reached 96％,and the median score of RBMT-II was 16.00 (interquartile range 6.00),indicating a low level of everyday memory function,with the Name andDelayed recall Story items having lower scores.All participants were divided into two groups according to cognitive status,with 67 (44.70％) adults having normal cognitive function and 83 (55.30％) impaired cognitive function.After comparison,significant differences were found in the total score of RBMT-II and scores ofFaces recognition andDelayed recall Story between the two groups (P＜0.05).In all dimensions of SF-36,the median score of general health dimension for the 150 adults was 55.00 (inter-quartile range 31.25).The Spearman rank correlation analysis showed that the scores of several dimensions in RBMT-II were positively correlated with the scores of several dimensions in SF-36 (P＜0.05).Conclusion Everyday memory function of people with SMC was positively CorTelated with their life quality.

Objective To introduce the illness perception questionnaire-memory (IPQ-M) and translate it into Chinese and to evaluate its reliability and validity.Methods The Chinese version of IPQ-M was translated from the original,back translated and adjusted for culture adaptation.A total of 152 community-dwelling people with subjective memory complaints (SMCs),who were aged 50 years or above,were investigated with the Chinese version of IPQ-M.40 participants among them were re-evaluated 2 weeks later.Results Exploratory factor analysis got 9 factors which could explain 70.757％ of the total variance.The factor loading of each item on the corresponding factors was higher than 0.4.The correlation between Chinese version of IPQ-M and GDS were-0.208 to 0.763 (P＜0.01).The Cronbach's α coefficient was 0.872,the testretest reliability was 0.894,and the Spearman-Brown split coefficient was 0.890.Conclusion The Chinese version of IPQ-M has good reliability and validity,and it can be used in Chinese elderly people with SMCs.

ObjectiveTo investigate the effects of granuloeyte colony stimulating factor (G-CSF) on myocardial apoptosis and left ventricular function following coronary mieroembolization (CME).MethodsA total of 68 male Sprague Dawley rats were randomized into control group(n=24), G-CSF-treated group(n= 24) and sham-operated group(n= 20). The control group and G-CSF -treated group underwent injection of homologous microthrombotic particle suspension into left ventricle when clamping the ascending aorta. The sham-operated group were treated with saline.rhG-CSF(100 yg · kg<'-1> · d<'-1>) were injected subcutaneously for 5 days since 2 h after operation in C-CSF group, meanwhile, the control group and sham-operated group were treated with saline respectively. The rats were sacrificed at day 3,7,14 and 28 after operation. The mRNA expressions of Bcl-2, Bax, Fas, FasL and glyceraldehyde phosphate dehydrogenase (GAPDH) were evaluated by real time PCR. The ratio of Bcl-2/Bax was calculated. The protein expressions of easpase-3 and cleaved poly ADP-ribose polymerase (PARP) were detected by Western blot. Myocardial apoptosis and leftventricular function were checked byterminal-deoxynucleotidyl transferase mediated nick end labeling (TUNEL)staining and hemodynamics respectively. ResultsCompared with sham-operated group,the mRNA expression of Bcl-2, Bax, Fas and FasL were increased whereas the ratio of Bcl-2/Bax markedly decreased in control group(0.28±0.04 vs. 2.98±0.49), P<0.05.Simultaneously, the protein expressions of caspase-3 and cleaved PARP enhanced (0.762±0.129 vs. 0.133±0.027;0.992±0.146 vs. 0.386+0.074) ,all P<0.05. The myocardial apoptosis index was elevated(17.2±1.9 vs. 1.2±0.6)and the left ventricular function decreased significantly (P<0.05). Compared with control group, the mRNA expression of Bct-2 increased and that of Bax, Fas and FasL decreased in G-CSF-treated group. Meanwhile, the ratio of Bcl-2/Bax markedly increased(2.07±0.29 vs. 0.28±0.04)and the protein expression of caspase-3 and cleaved PARP decreased(0.371±0.041 vs. 0.762±0.129;0.548±0.093 vs. 0.992±0.146, all P<0.05). The myocardial apoptosis index was reduced (6.1±1.0 vs. 17.2±1.9)and the left ventricular function increased significantly (P<0.05).ConclusionsG-CSF could lighten the myocardial apoptosis and improve the left ventrieular functionafter CME.

Objective To screen the hierarchical,mutually supplementary Appropriate Technology in Health suitable for the promotion and application in the county and township two-level medical institutions in Chongqing for gradually establishing and per-fecting the grading and classification of Appropriate Technology in Health database,and expanding Appropriate Technology in Health reserves.Methods Using Lv Yalan[1] assessment index system of appropriate health technology,then the experts′scoring method in the field was adopted for evaluating and screening the Chongqing Municipal Science and Technology Proj ectbased on stratification of diagnosis and treatment of disease complementary appropriate technology research,evaluation and integration dem-onstrationAppropriate Technology in Health group[2].Results The comprehensive scores of various techniques were high,in which the lowest score was 94.61 points,including:the drug seretide inhalation technique 98.24 points,the drug salbutamol inhala-tion technique 98.56 points,inhaled medications to control asthma technique 97.08 points,the drug spiriva(tiotropium)inhalation technique 97.97 points,6-min walk test 96.31 points,pulmonary ventilation function inspection technique 95.61,summary pulmo-nary function testing technique 94.59,children bronchial get up early screening technique 94.59,child bronchial asthma detection technique-C-AT(children′s asthma control test)evaluation form 96.94.Conclusion According to the inclusion criteria for Appro-priate Technology in Health and the experts′scores,9 items of Appropriate Technology in Health included by preliminary screening are selected for putting in storage,combined with the health resources and the prevalence rate in the demonstration counties and townships,these 9 items of technology may be promoted and applied.

OBJECTIVE:To provide reference for rational drug use in pediatric department. METHODS:The utilization of pe-diatric prescription drugs in inpatient department of our hospital during 2008 to 2014 was analyzed in respects of number,consump-tion sum,DDDs,etc. RESULTS:The consumption sum of antimicrobial drugs increased from 657 000 yuan in 2008 to 1 453 000 yuan in 2014. The consumption sum of creatine phosphate sodium increased from 384 000 yuan in 2012 to 889 000 yuan in 2014. The consumption sum of Lysine hydrochloride and sodium chloride injection entered the top 5 in 2014,reaching 205 000 yuan. The consumption sum of essential medicines changed greatly due to varieties. Top 5 antimicrobial drugs in the list of DDDs mainly wereβ-lactam,showing a descreasing trend on the whole. CONCLUSIONS:There are some problems in use of part drugs. It is need to strengthen propaganda and intervention of rational drug use.

Objective:To investigate Intraflagellar Transport 80 (IFT80) protein expression in bone, lung, pancreatic, stomach, in-testinal, prostate, breast, and ovarian cancers to explore its mechanism in cancer cell proliferation and to diagnose and identify new tar-gets in cancer treatment. Methods:Immunohistochemistry was used to investigate the expression of IFT80 in gastric cancer tissue of different stages and in eight other kinds of human cancer tissues. We studied the relationship between cancer cell proliferation and inhi-bition of IFT80. Immunofluorescence method and cell culture were used to study the cilia and IFT80. Results:Results showed the fol-lowing:a) the expression of IFT80 was high in gastric and lung carcinoma tissues, moderate in breast and colorectal cancers, low in bone and ovarian cancers, and nearly absent in prostate and pancreatic cancers;b) inhibition of IFT80 in the A549 cancer cell line accel-erated cell proliferation and resulted in shorter, lower quality cilia;and c) IFT80 was abundantly expressed in cancer tissues of well-dif-ferentiated stage-IIA gastric cancer and normal gastric tissues, but was hardly expressed in late-stage, poorly differentiated gastric can-cer. IFT80 could have various degrees of expression in gastric carcinoma of other stages and differentiation. Conclusions:Different can-cer organs showed variation in IFT80 expression. IFT80 can be distributed in the organs with mechanical motion function, such as lungs and stomach. IFT80 is distributed on the cell cilia and can adjust the number and length of the cilia by reducing IFT80 protein ex-pression. Through a variety of ways, IFT80 directly or indirectly participates in the proliferation of cancer cells. Thus, the lowest or nearly zero expression of IFT80 can be seen in cancer tissues of high-grade malignancy, such as advanced cancers with poor differentia-tion.

Objective To evaluate a modified Suguira procedure for the treatment of variceal bleeding.Methods A modified Suguira procedure was performed in 62 patients with acute variceal bleeding (11 cases) that could not be controlled by endoscopic therapy or with a history of massive bleeding (51 cases) after endoscopic therapy.Results Perioperative mortality occurred in 2％ (1/62) patients.Esophageal anastomotic leak occurred in 2％ (1/62) patients,and anastomotic stenosis developed in 5％ (3/62) patients.Twelve months after operation,esophageal varices disappeared in 79％ (48/61) patients,diminished in size in 18％ (11/61),remained unchanged in 3％ (2/61) ; Fundal gastric varices disappeared in 98％ (60/61) patients,diminished in size in 2％ (1/62).The rebleeding rate was 3％ (2/61) and 8％ (5/61) in 3 years and 5 years,respectively.Conclusions The modified Suguira procedure is safe and effective for long-term control of variceal bleeding after a failed endoscopic therapy.

Objective To explore the impact of patients with esophageal cancer knowing diagnosis on the mental health of their children and investigate the factors.Methods 107 family carriers whose parent diagnosed esophageal cancer in General Hospital of Jinan Command were interviewed.Respondents were categorized into two groups:one group was those whose parents knew their diagnosis(Group A,n=45)and the other group was those whose parents did not know(Group B,n=62).The Chinese version of Self-reporting Inventory(SCL-90) was used.Results The scores of 107 children of esophageal cancer patients in depression(1.62±0.30),anxiety(1.78±0.34) and hostility (1.93±0.47) were higher than national norms (P＜0.01).Compared with group A,group B showed a significant higher degree of anxiety((1.89±0.33) vs (1.62±0.33),P＜0.01).In multiple regression analysis,age and the knowledge of esophageal cancer diagnosis had statistically significant difference (P＜ 0.05).Conclusion Patients' knowledge of the esophageal cancer diagnosis contribute to relieving family carers' symptoms of anxiety.

Objective To introduce the surgical strategy of enlarged laminectomy (with partial facet joint dissection to expose the nerve root), and to discuss its benefit for cervical ossification of the posterior longitudinal ligament(OPLL) with myelopathy. Methods Totally 82 patients with cervical OPLL were treated by enlarged laminectomy from January 1998 to December 2005. There were 47 males and 35 females, with an average age of 57 years (ranged, 39-84 years). Among them, there were 31 cases of the solitary type, 40 cases of the continuous type, and 11 cases of the mixed type. JOA scoring system and the visual analogue scale (VAS) scoring were applied to evaluate the neurological function and neck/shoulder pain respectively.Ishihara method was employed to measure cervical curvature index(CCI). The degree of spinal cord backward expanding and displacement were calculated in MR1. Results The mean decompression length was 5.2 (4-6) segments. The mean follow-up duration was 41 months (ranged, 13-58 months). JOA score has improved from 10.9(7-15) preoperatively to 13.9(11-17) postoperatively (t=14.65, P＜0.01). The excellent and good rate was 98.7%. The palsy of C5 nerve root occurred in only 2 patients, both recovered after surgery. Zhe mean postoperative VAS score was 1.4(1-3), comparing with the preoperative score of 5.3 (4-6). The pain in neck/shoulder was alleviated obviously (t=15.46, P＜0.01 ). CCI decreased from 18.8% to 10.5%(t=5.03, P＜0.01 ),but did not follow by neuron function deterioration. MRI indicated that the cross-sectional area at the level of maximum compression of the dural sac increased from 85.4 mm2 preoperatively to 153.8 mm2 postoperatively (t=16.33, P＜0.01), and the mean spinal cord posterior shift was 6.2 mm (t=15.35, P＜0.01). Conclusion The enlarged laminectomy is proved to be effective in treating cervical OPLL, in terms of significant posterior shift of the spinal cord, relief of cervical/shoulder pain, lower rate of the palsy of C5 nerve root, with no recurrence of spinal cord compression symptom.
