International Journal of Surgery 2014;41(7):454-457
Objective Acute esophageal variceal bleeding is one of the most dreadful complications of cirrhotic portal hypertension. This study evaluated a modified gastro-esophageal decongestion (MGED) procedure in the management of acute variceal bleeding that could not be controlled by drugs and/or endoscopic therapy. Methods A MGED procedure was performed in 136 patients with uncontrollable acute variceal bleeding by drugs and/or endoscopic therapy. The perioperative and long-term outcome of these patients were retrospectively studied.Resluts Immediate control of variceal bleeding was achived in 97.8% (133/136) patients.Perioperative mortality occurred in 5.1% (7/136) patients. The 3-year and 5-year rebleed rate was 7.0% (9/129) and 13.2%,respectively.The 3-year and 5-year overall survival rate was 91.5% (118/129) and 79.8% (103/129),respectively.Conclusions In this study, the MGED procedure was an effective life-saving procedure in controlling acute esophageal variceal bleeding for its safe, simple and less time-consuming advantages.

Objective To evaluate the effects of esophagogastroplasty of esophagogastrostomy in preventing anastomotic leakage, stricture and reflux esophagitis after resection of lower esophageal or cardiac carcinoma. Methods 273 cases of lower esophageal cardiac carcinoma were randomly divided into 2 groups:The treatment group(145 cases) was treated by esophagogastrostomy encircled by sero muscular gastric wall flap while the control group(128 cases) was performed with esophagogastrostomy only. Results No anastomotic leakage or stricture was observed in treatment group, with 10 cases of reflux esophgitis only.In control group, anastomotic leakage, stricture and reflux esophagitis were 9(7 0%),5(3 9%),and 21(17 1%) respectively. Conclusions The esophagogastroplasty of esophagogastrostomy can completely avoid anastomotic leakage, stricture and significantly reduce reflux esophagitis after resection of lower esophageal or cardiac carcinoma.

Objective To evaluate the therapeutic effects of EN,PN support on postoperative recovery of (patients) with portal hypertension. Methods 135 patients with portal hypertension who underwent (esophagogastric) devasularization were randomly divided into three groups:EN,PN and control group. (Postoperatively), EN and PN nutritional support and routine fluid replacement therapy was respectively given to the 3 groups. Several clinical and laboratory parameters were studied in order to determine the therapeutic effect of the two different forms of nutrition. Results Nutritional status and immunological function of patients were improved in both EN and PN study groups but with no significant difference, while alterations in liver function and incidence of abdominal infection were higher in PN group. Conclusions EN is an optimal form of nutritional support for patients with portal hypertension after operation.

The transcript assembly is essential for transcriptome studies trom next-generation sequencing data. However, there are still many faults of algorithms in the present assemblers, which should be largely improved in the future. According to the requirement of reference genome or not, the transcript assembly could be classified into the genome-guided and de novo methods. The two methods have different algorithms and implementation processes. The quality of assembled transcripts depends on a large number of factors, such as the PCR amplification, sequencing techniques, assembly algorithm and genome character. Here, we reviewed the present tools of transcript assembly and various indexes for assessing the quality of assembled transcripts, which would help biologists to determine which assembler should be used in their studies.

Objective To research the effect of PEGylation on rBla g 2 from Blattella germanica. Methods rBla g 2 allergen expressed in E.coli was purified by Ni+affinity chromatographY,then was PEGylated by mPEG2-NHS(Mr,10×103).The PEG-rBla g 2 was purified by CM-Sepharose,a cation exchange chromatography. SDS-PAGE,Western blot and ELISA were used to characterize its biologicat actovoty. Results The relative molecular mass of the purified rBla g 2 was about 39×1023.PEG-rBla g 2 was analyzed by SDS-PAGE.Five bands were visualized by staining with Coomassie Brilliant Blue R-250,while seven bands by staining with I2-KI.Cation exchange chromatography could separate rBla g 2 and PEG-rBla g 2.The 100×103(Mr)and 130×103(Mr)of PEG-rBla g 2 could combined with the special IgE from sera of one cockroach-allergic patient by Western blot. The immunological activities of PEG-rBla g 2 in vitro decreased remarkablv bv ELISA,which was only 42%of rBla g 2.Conclusion PEGylated allergen can retain the ability of combining with special IgE from sera, while its immunological activities reduce enormously, which establishes the basic work of researching recombinant low-sensitive allergens.

Objective To evaluate a modified Suguira procedure for the treatment of variceal bleeding. Methods A modified Suguira procedure was performed in 62 patients with acute variceal bleeding (11 cases) that could not be controlled by endoscopic therapy or with a history of massive bleeding (51 cases) after endoscopic therapy. Results Perioperative mortality occurred in 2% (1/62) patients.Esophageal anastomotic leak occurred in 2% (1/62) patients, and anastomotic stenosis developed in 5% (3/62) patients.Twelve months after operation, esophageal varices disappeared in 79% (48/61) patients, diminished in size in 18% (11/61),remained unchanged in 3% (2/61) ; Fundal gastric varices disappeared in 98% (60/61) patients, diminished in size in 2% (1/62).The rebleeding rate was 3% (2/61) and 8% (5/61) in 3 years and 5 years, respectively. Conclusions The modified Suguira procedure is safe and effective for long-term control of variceal bleeding after a failed endoscopic therapy.

Objective To explore the application value of carpal shoot through view in the palmar plating of distal radius fractures. Methods From March 2015 to May 2016,there were 62 acute distal radius fractures received various operation in our department,in which 48 patients were only performed volar locked plating.Among these 48 cases, there were 21 males and 27 females, with ages ranging from 17 to 75 years (mean 43.6±10.6 years). There were 15 cases of type A,11 cases of type B and 22 cases of type C distal radial fractures according to AO/OTA classification. All the operations were performed through modified Henry's approach between the radial artery and flexor carpi radialis.Once achieved satisfactory reduction,we performed rigid fixation of distal radius using anatomical locking plates. In all cases, bicortical drilling was performed.Screw lengths selected measured 1 to 2 mm less than the depth gauge number avoiding dorsal cortex penetration. After plate fixation of these 48 cases, standard posteroanterior and lateral radiographs were taken followed by the carpal shoot through view.To obtain carpal shoot through view, the patient's forearm was maximally supinated, the elbow flexed about 60°-70°and the wrist maximally dorsiflexed. The image intensifier beam is directed over the base of the thenar eminence.Compare the ratio of dorsal protrusion between standard view and carpal shoot through view, then statistical analysis was performed. After operation,CT scan was routinely taken to further test the effectiveness of carpal shoot through view. Results Five cases were detected dorsal cortex screw protrusion using standard posteroanterior and lateral views (10.4%,5/48).There were totally 16 cases (including the former 5 cases) of dorsal screw penetration uncovered by the carpal shoot through view(33.3%,16/48.Statistically significant difference was existed between the two fluoroscopy methods.Additionally,in 2 cases,a screw had penetrated the distal radioulnar joint (DRUJ),which was only apparent on the shoot through view. All the improper screws were exchanged and again were verified by the carpal shoot through view. The overall screw exchange rate was 37.5% (18/48).Conclusion Compared with standard AP and lateral fluoroscopy, the carpal shoot through view can reliably reveals dorsal screw penetration.Meanwhile,it provides excellent visualization of DRUJ.
