1.Profile of Clients Attending a Methadone Clinic

Background: Client characteristics provide useful information for designing programs that target individuals with risk factors for substance use and for determining client retention. Therefore, this study examined the profiles of clients attending a methadone clinic.
Methods: A cross-sectional analysis of clients of a methadone clinic was conducted through a survey to obtain a profile of methadone clients.
Results: Of the 51 patients who responded (response rate: 66.2%), the mean (SD) age at which they started substance use was 19.8 (5.1) years. Friends were cited as the most regular source of drugs (82.4%), and heroin was the most commonly used drug (98%). Daily substance use was reported by 72.5% of the respondents; 23.5% admitted to having stolen money to purchase drugs; 92.2% tried quitting substance use on their own and 98% stated that the main reason for registering at the clinic was that they wanted to stop their drug dependence. Approximately 60% of clients were receiving methadone doses of less than 60 mg/day.
Conclusion: Heroin is still the most popular drug of abuse and most clients still receive methadone doses below the recommended level, despite evidence of poor patient retention rates associated with these low doses.
2.Endoscopic retrograde cholangiopancreatography for acute gallstone pancreatitis: Implementation of an institution safety protocol during the COVID-19 outbreak in Malaysia

Coronavirus disease 2019 (COVID-19) is a highly
contagious, severe acute respiratory syndrome that poses
significant health risks to healthcare providers. A delicate
balance is needed between timely intervention for ill patients
without apparent COVID-19 infection and the safety of
healthcare personnel who provide essential treatment in the
midst of the pandemic. We report our experience managing
a 70-year-old man who presented with acute gallstone
pancreatitis at our hospital during the COVID-19 outbreak in
Malaysia. We also describe the safety protocol measures
that have been implemented in our institution to protect the
healthcare personnel from this disease during endoscopic
retrograde cholangiopancreatography. This case illustrates
the importance of meticulous planning, risk assessment,
effective team communication and strict adherence to
recommendations when providing treatment during an
unprecedented pandemic.
3.P16INK4a: A Potential Diagnostic Adjunct For Prediction Of Highgrade Cervical Lesions In Liquid-Based Cytology: With HPV Testing And Histological Correlation

Human papillomavirus (HPV) is a necessary cause of cervical cancer and its precursors. Increased
expression of high-risk hrHPV viral oncogenes in abnormal cells might increase the expression of
p16INK4a. We aimed to determine the role of p16INK4a in detecting hrHPV-transformed epithelial
cells in liquid-based cervical cytology, and compared the results with hrHPV DNA testing by realtime
polymerase chain reaction (RT-PCR). Fifty-seven cytological samples were tested for p16INK4a
immunomarker and hrHPV DNA. Test performance of both tests was determined by comparing
sensitivity, specificity and predictive values using available histological follow-up data as gold
standard. Of 57 samples, 36 (63.2%) showed immunoreactivity for p16INK4a and 43 (75.4%) were
hrHPV-infected. A fairly low concordance rate (k = 0.504) between p16INK4a immunolabelling
and hrHPV DNA status was noted. For prediction of cervical intraepithelial neoplasia (CIN) II and
worse lesions, p16INK4a had a sensitivity and specificity of 93.5% and 60%; whereas hrHPV DNA
testing had a sensitivity and specificity of 100% and 20%. Dual testing by combining p16INK4a
and hrHPV showed sensitivity and specificity of 100% and 33.3%. In conclusion, p16INK4a is
useful in predicting severity of the cytological abnormalities. Although p16INK4a is more specific
but less sensitive than hrHPV in detecting high-grade cervical lesions, a combination of both tests
failed to demonstrate significant improvement in diagnostic sensitivity, specificity and predictive
value. Larger-scale prospective studies are required to assess further whether this biomarker should
be routinely used as primary screening tool independently or in combination with hrHPV testing to
improve diagnostic accuracy in cervical cytology.
4.Successful Reconstructive Surgery for Facial Deformity Caused by Paederus fuscipes: An Illustrative Case Report

Full thickness skin graft is a simple and reliable method for closure of small facial wound defect. A thorough
understanding of how a skin graft heals and how to perform the procedure is essential for successful outcome.
We report the use of full thickness skin graft in a wound closure of a facial skin defect caused by Paederus
fuscipes, locally known as charlie. An 8-year old boy developed blister and painful swelling over his right cheek
following skin contact with charlie. This lesion gradually became extensive, eventually leading to tissue loss and
facial wound defect. A full thickness skin grafting was performed with satisfactory functional and excellent
aesthetic result.

Aims:
Wax apple is an important fruit crop in Malaysia and other tropical countries. However, the black spot disease caused by Lasiodoplodia theobromae can damage the wax apple plants, reducing fruit production and quality. Chemical fungicides are commonly used to overcome this disease. However, their overuse might increase fungal resistance to chemicals. Therefore, this study was aimed to evaluate the in vitro antifungal properties of an Aloe vera film incorporated with cinnamon oil on L. theobromae in wax apples.
Methodology and results:
In vitro antifungal tests were conducted using the poisoned food. This present study found that the inhibition of L. theobromae mycelia when treated with A. vera film with different percentages of cinnamon oil was significantly different compared to the control film at (P<0.05). Results suggest that A. vera film without cinnamon oil showed the highest percentage of inhibition (37.31%) than other films with cinnamon oil due to the antagonistic and less synergistic effect. However, A. vera film with 0.07% of cinnamon oil showed the highest percentage of mycelia inhibition (36.15%) compared to the film with 0.06%, 0.05% and 0.04% cinnamon oil, with mycelia inhibition of 30.55%, 24.25% and 18.82%, respectively, while the mancozeb (positive control) showed 100% inhibition.
Conclusion, significance and impact of study
In conclusion, A. vera film alone had 37.31% inhibition compared to 36.15% inhibition in A. vera film with 0.7% cinnamon oil. As a result, a tiny amount of cinnamon oil added to the mixture might aid in controlling the black spot disease in wax fruits.
