Objective To investigate the implications of ratio of the CD4+ and CD25+ positive regulatory T cells (CD4+CD25+Tregs) in peripheral blood mononuclear cells (PBMC) and its associated regulatory factors such as forkhead transcription factor 3 (Foxp3) mRNA transcriptional activity in PBMC,serum levels of transforming growth factor beta-1 (TGF-β1),and interleukin 10 (IL-10) in the immunopathology of patients with middle to late staged nasopharyngeal carcinoma (NPC) based on a clinical trial.Methods In this study,18 NPC cases at middle to late stage as observing group and 10 healthy persons as control group were included to detect their ratio of the CD4+CD25+Tregs in the PBMC with flow cytometry (FCM) technique,transcriptional activity of Foxp3 with RT-PCR procedure,and serum levels of TGF-β1 and IL-10 with enzyme-linked immunosorbent assay (ELISA) method.A comparative analysis was used to explore their implications in the immunopathological correlation of NPC cases with their lesion.Results The ratio of the CD4+CD25+Tregs to total CD4+T cells in PBMC was significantly increased [(4.23 ±0.53)% vs (2.65 ±0.31)%,t =8.60,P <0.01],accompanied with significantly elevated levels of Foxp3 transcription in PBMC (3.699 ± 0.309 vs 1.109 ± 0.146,t' =31.08,P < 0.05],and serum contents of TGF-β1 [(645.56 ± 39.61) pg/ml vs (488.82 ± 36.91) pg/ml,t =10.27,P < 0.01] and IL-10 [(1.27 ± 0.21) pg/ml vs (0.68 ± 0.08) pg/ml,t' =10.61,P < 0.05] in these patients,when compared with that of healthy controls.Conclusions It may be true that CD4 + CD25 + Tregs,transcriptional regulatory factor Foxp3,and cytokines TGF-β1 as well as IL-10 altogether were composed of a regulating system in a positive feedback way to promote the developing process of immunotolerance phenomena in the tumor microenvironment and the initiation of immunoescape among patients with middle to late staged nasopharyngeal carcinoma.
3.Construction of shRNA targeting AGR2 gene and effect of AGR2 on bio-logical function of nasopharyngeal carcinoma cells

AIM: To construct the shRNA targeting anterior gradient protein 2 (AGR2) gene for exploring the effect of AGR2 on the biological behavior of nasopharyngeal carcinoma (NPC) cells.METHODS: The expression of AGR2 at mRNA and protein levels in NPC cell lines 6-10B and 5-8F was detected by real-time PCR and Western blot.The pSR-GFP/Neo-AGR2-shRNA expression vector targeting AGR2 was constructed.Based on the interference targeting AGR2, the cell migration and motility were determined by Transwell migration and motility assays.RESULTS: The expression of AGR2 was increased in NPC cell line 5-8F compared with NPC cell line 6-10B (P <0.05).When the AGR2 expression in 5-8F cells was interfered, the cell migration, invasion and tumorigenicity were weakened.CONCLUSION: The expres-sion of AGR2 is up-regulated in NPC cell line 5-8F.pSR-GFP/Neo-CLU-shRNA successfully inhibits the expression of AGR2 in NPC cell line 5-8F.AGR2 inhibits the migration, invasion and tumorigenicity of 5-8F cells in vivo.
4.Anti-Helicobacter pylori treatment in patients with idiopathic thrombocytopenic purpura

Objective To observe the effect of anti-Helicobacter pylori (H.pylori) treatment on patients with idiopathic thrombocytopenic purpura (ITP) and its effect on blood platelet. Methods From April 2006 to April 2008, a total of 31 patients diagnosed as having ITP and H.pylori infection were collected. These patients were given standard antibiotic therapy for H.pylori eradication (omeprazole 20 mg, clarithromycin 0.5 g, and amoxicillin 1.0 g, twice per day for 1 week). The effect of anti-H.pylori treatment was analyzed, and the blood platelets were counted before the treatment and on day 7, 14, and 28 after the treatment. Results Of the 31 ITP patients with H.pylori infection, 21 were cured and 10 were not effective, with the effective rate 67.74 %. Blood platelet increased in the cured group but did not change in the no-effect group. Conclusion Anti-H.pylori treatment could increase the blood platelet, and H.pylori infection may be related to the reduction of blood platelet in ITP patients.
5.Protective mechanism of metallothionein on cultivated rat cardiomyocytes in hypoxic preconditioning

AIM: Studying the mechanism of protective role of metallothionein (MT) in hypoxic preconditioning(HPC) of cultivated rat cardiomyocytes. METHODS: Using the model of hypoxia/reoxygenation of cultivated rat cardiomyocytes. Determing the contents of MT, malonyldialdehyde (MDA) - metabolism product of lipid peroxidation and the activities of Na+ - K+ ATPase, Ca2+ - Mg2+ ATPase of cardiomyocytes 24 h after HPC, the determining the relevant changes after using MT antibody. RESULTS: After 24 h in HPC, the contents of MT and activities of Na+ - K+ ATPase, Ca2+ - Mg2+ ATPase were obviously higher than those in the control and hypoxia/reoxygenation(P< 0. 05 ), and the contents of MDA were decreased remarkedly (P < 0.01 ). Then after using MT antibody, the activities of two enzyme were progressively decreased and the contents of MDA were significanily higher than those in the control and MT antibody - free groups(P < 0.01 ). CONCLUSION: HPC may induce excessive synthesis of MT, and MT can protect myocardial reoxygenation injury by eliminating lipid peroxidation and rising the activities of Na+ - K+ ATPase and Ca2+ - Mg2+ ATPase.
6.Expression of ezrin in squamous cell eardnoma,basal cell carcinoma and seborrheic keratosis

Objective To investigate the expression of ezrin in seborrheie keratosis(SK),basal cell carcinoma (BCC).squamous cell carcinoma (SCC)and its relation to elinical pathology parameters.Methods Skin samples were collected from 36 patients with SCC,27 patients with BCC,20 patients with SK and 10 normal human controls.Immunohistochemistry was performed to detect the expression of ezrin in these samples.The relationship between ezrin expression and prognosis of SCC was assessed using COX regression analysis.Results The positivity rate of ezrin in SK,BCC and SCC samples was 25.0%,66.7%,91.3%,respectively,compared to 20.0%in normal controls.Compared with the controls,a significant increase was observed in the expression of ezrin in patients with BCC and SCC(both P<0.05).but not in those with SK(P>0.05).Moreover.increased expression level of ezrin was correlated with a high degree of tumor malignancy,advanced pathological stage,and occurrence of lymph node metastasis of SCC (r=0.87,0.80,0.89,respectively,all P<0.01).COX regression analysis revealed that the expression level of ezrin was an independent factor for the prognosis of SCC.Conclusion The expression level of ezrin is closely related to the malignancy of skin tumors,pathological grading and lymph node metastasis of SCC.
7.Molecular mechanism of SH2-Bβ in regulating JAK2/STAT3 during development of obesity

The molecular techniques were used to analyse tyrosine phosphorylation of JAK2 and STAT3 in leptin receptor overpression cell lines and SH2-Bβ knockout (SH2-Bβ-/-) mice. The serum level of leptin in SH2-Bβ mice was measured by ELISA. The results showed that SH2-Bβ dramatically enhanced the leptin-stimulated tyrosine phosphorylation of JAK2 and STAT3 in vitro. Leptin-stimulated activation of JAK2 and phosphorylation of STAT3 were significantly impaired in hypothalamus of SH2-Bβ-/- mice. The fasting and postprandial serum levels of leptin and body weight were markedly increased in SH2-Bβ-/- mice. Therefore, SH2-Bβ is an endogenous enhancer of leptin sensitivity and regulates body weight via leptin/ JAK2/STAT3 pathway.
8.Evaluation of self-management treatment effect and analysis of influencing factors in patients with chronic Keshan disease in Gansu Province

Objective:To evaluate the effect of self-management treatment for patients with chronic Keshan disease in Gansu Province.Methods:From March to June 2018, 243 patients with chronic Keshan disease who were treated by self-management treatment outside the hospital in 7 Keshan disease counties in Gansu Province were selected as the survey subjects, and their general demographic and clinical data were collected. At the same time, the survey subjects were given a 6-month self-management treatment, and the self-management scale was used to evaluate the patients' self-management behavior before treatment and 6 months after treatment. The higher the score, the better the patients' self-management behavior. The influencing factors of self-management scale were analyzed by multiple linear regression analysis.Results:Excluding incomplete data, a total of 158 patients with chronic Keshan disease were included in this survey, including 96 males and 62 females. Compared with patients before self-management treatment, the patients' cardiac function classification and work ability improved significantly after 6 months of self-management treatment ( Z = - 4.685, - 5.934, P < 0.05); cardiothoracic ratio decreased (0.61 ± 0.08 vs 0.63 ± 0.09, t = 5.175, P < 0.05); in the cardiac color Doppler ultrasound indexes, left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were increased (0.41 ± 0.11 vs 0.36 ± 0.07, 0.21 ± 0.07 vs 0.18 ± 0.05, t = - 6.504, - 5.391, P < 0.05); the total score of the self-management scale was increased [(53.86 ± 9.29) scores vs (51.46 ± 10.50) scores, t = - 3.696, P < 0.05]. Further analysis of the efficacy of the patients' cardiac function classification showed that the patients' medication compliance was positively correlated with the efficacy of the cardiac function classification ( r = 0.243, P < 0.05). The result of multiple linear regression analysis showed that education level and efficacy of the cardiac function classification were influencing factors of the self-management scale for patients with chronic Keshan disease ( t = 2.466, 2.635, P < 0.05). Conclusion:The self-management treatment for patients with chronic Keshan disease can effectively improve heart function and improve self-management behavior.
9.Prevalence of overweight and obesity and their associations with sleep duration among primary and secondary school students in Gansu Province

Objective:
To investigate the prevalence of overweight/obesity and its associations with sleep duration among children and adolescents aged 6-17 in Gansu Province, so as to formulate tailored prevention and control strategies and intervention measures.
Methods:
Using a multi stage stratified cluster random sampling method, a questionnaire survey and physical examination were conducted among 2 172 students aged 6-17 from 2 monitoring points in Gansu Province from May to July 2022. Using Chi square test to analyze the inter group differences in the detection rates of overweight and obesity and the composition of sleep duration, and using Logistic regression model to analyze the correlation between overweight and obesity and sleep duration.
Results:
The prevalence of overweight and obesity were 10.45% and 6.86%, respectively. The average sleep duration was ( 8.28 ±1.66)h/d. The detection rate of overweight was high among high school students aged 16-17 and those with sleep duration<8 h/d ( χ 2=12.51, 19.32, 14.96), while the detection rate of obesity was high among male and non residential students ( χ 2= 5.89 , 9.59)( P <0.05). For both boys and girls, the rates of overweight and obesity among primary school students increased with the decrease in sleep duration ( χ 2=38.84, 9.80, 19.61, 41.60, P <0.05). The rates of obesity in boys were higher than girls across varying sleep durations among the general population (sleep duration <8 h/d: 6.07%, 11.11%, 8~<9 h/d: 5.76%, 6.09 %, 9~<10 h/d: 6.02%, 8.19 %, ≥10 h/d: 4.04%, 6.90%). Multivariate Logistic regression analysis showed that insufficient sleep duration among primary school students was positively associated with the risk of overweight and obesity, and adequate sleep duration among high school students was negatively associated with the risk of overweight and obesity (sleep duration 8~<9 h/d: OR primary school students = 1.89 , 9~< 10 h/d: OR primary school students =1.54, 8~<9 h/d: OR high school students =0.30, P <0.05).
Conclusions
Insufficient sleep is a risk factor for overweight and obesity among primary and secondary school students at monitoring sites in Gansu Province. Prevention strategies and interventions should be developed for different populations to ensure adequate sleep duration and reduce the risk of overweight and obesity among primary and secondary school students.
10.Arthroscopy-assisted reduction and internal fixation via traditional anterolateral approach for posterolateral tibial plateau fractures

Objective:To evaluate the arthroscopy-assisted reduction and internal fixation (ARIF) via the traditional anterolateral approach for the treatment of posterolateral tibial plateau fractures (PTPF).Methods:A retrospective study was conducted of the 79 patients with PTPF who had been treated from March 2014 to February 2020 at Department of Articular Surgery, Linyi Central Hospital. They were 37 males and 42 females, aged from 17 to 71 years (average, 46.0 years). According to treatment methods, they were assigned into an ARIF group (37 cases) and a control group (42 cases). The ARIF group was treated by ARIF via the traditional anterolateral approach and the ARIF varied according to the extents of articular collapse and split and displacement of fracture fragments. The control group was treated by traditional reduction and internal plate fixation of the proximal tibia. The 2 groups were compared in terms of operation time, blood loss, iliac bone grafting, hospitalization time, follow-up time, Hospital for Special Surgery (HSS) score, maximum flexion and extension, fracture healing time, Rassmussen functional and anatomical scores, visual analogue scale (VAS) pain score and complications.Results:There was no statistically significant difference in the general data between the 2 groups, showing comparability ( P>0.05). All patients were followed up for 6 to 18 months (average, 11.4 months). The operation time [(48.9±8.2) min], blood loss [(73.1±20.8) mL], hospitalization time [(9.3±2.5) d], and fracture healing time [(12.5±1.8) weeks] in the ARIF group were all significantly less than those in the control group [(55.2±9.9) min, (100.8±30.3) mL, (11.8±2.8) d and (15.1±2.1) weeks] while the HSS score [(93.5±4.6) points], maximum flexion angle (107.8°±10.4°) and Rassmussen functional score [(27.1±1.8) points] in the ARIF group were significantly higher than those in the control group [(88.4±7.4) points, 100.4°±10.0° and (26.1±2.4) points] (all P<0.05). There was no significant difference between the 2 groups in iliac bone grafting, follow-up time, maximum extension angle, Rassmussen anatomical score, VAS score, or rate of complications (all P>0.05). Conclusions:Compared with traditional surgery, ARIF which varies with the extents of articular collapse and split and displacement of fracture fragments may lead to shorter operation time, reduced surgical trauma and more accurate reduction of the articular surface. Therefore, it can be an additional choice in the treatment of PTPF.
