1.Complicated Infection on Functional Outcome of Stroke

Objective To observe the influence of complicated infections on functional outcome of patients with stroke.Methods Two hundred and twenty-five stroke patiens confirmed by cranial CT scan or MR were prospectively divided into non-infection (145 cases) and infection (80 cases) groups. The body temperature of all the patients was recorded on admission and during hospitalization. the National Institutes of Health Stroke Scale(NIHSS), Barthel Index(BI) and modified Rankin Scale(mRS) were documented in all the cases. The difference in functioual outcome between the two groups was analyzed.Results The maximun body temperature of infection group was higher than that of the non-infection during hospitalization(P<0.01). The funtional outcome of the patients complicated with infections was significantly worse than that of the cases without infections assessed by BI(P<0.01), mRS(P<0.01) and NIHSS(P<0.01).Conclusion Infection of the stroke patients influenceed functional outcome and the prognosis.
2.Analysis of Doppler flow spectra of internal jugular vein in healthy adults

Objective To analyze the flow spectra of in ternal jugular vein (IJV) in healthy adults by pulsed Doppler ultrasonography. Methods IJV flow velocities in 120 healthy volunteers were recorded during cardiac and respiratory cycles respectively with the transducer placed in cervical region view. Results In healthy subjects, IJV waveforms were composed of one reverse wave A and two forward waves S and D during cardiac cycle. The flow velocities of wave S [( 27.17 ? 19.88 )cm/s vs ( 19.04 ? 14.26 )cm/s] and wave D [( 17.43 ? 12.78 )cm/s vs ( 12.70 ? 10.57 )cm/s] were quicker during inspiration than those during expiration (P 0.05 ]. 3.Study on regular patterns of superior vena cava Doppler flow spectra in patients with superior vena cava syndrome

Objective To consecutively investigate the changes of the superior vena cava flow Doppler patterns in patients with superior vena cava syndrome (SVCS) and predict SVCS development.Methods Twenty-two definative cases with SVCS due to tumor were enrolled in this study.At different stages of therapy the superior vena cava was detected by ultrasonography.The superior vena cava Doppler spectra during cardiac cycle and respiratory cycle were analyzed respectively.Results In 22 patients with SVCS the superior vena cava Doppler flow patterns presented a developing tendency,from turbulent,continuous and high velocity pattern to normalization.The S and D waves peak velocities were ( 154.78 ?52.15 ) cm/s,( 159.38 ? 46.56 ) cm/s respectively before therapy,while after therapy ( 58.78 ? 33.43 ) cm/s,( 34.96 ? 17.56 ) cm/s respectively.The S wave and D wave respective differences in respiratory cycle were ( 2.14 ? 2.08 ) cm/s,(2.73?2.68)cm/s before therapy,while those were ( 7.68 ? 6.22 ) cm/s,( 6.32 ? 4.98 ) cm/s after therapy.Conclusions The regular changes of the superior vena cava Doppler flow patterns in tumor-induced SVCS development course were embodied in dynamic diversity.These changes could suggest SVCS development and evaluate the therapy effect on SVCS.
4.Quality standard for Yinxie Tablets

AIM: To establish quality standard for Yinxie Tablets(Fructus Gardeniae, Radix Astragali, Radix Angelicae Sinensis, etc.). METHODS: Fructus Gardeniae, Radix Astragali and Radix Angelicae Sinensis were identified by TLC, and the content of geniposide was determined by HPLC. RESULTS: Fructus Gardeniae, Radix Astragali and Radix Angelicae Sinensis could be identified by TLC. The calibration curve of geniposide was linear (r=0.999 5). The precision was perfect (RSD=0.98%). The average recovery was 101.81% with RSD of ~0.54% . CONCLUSION: This method is simple, quick and specific, suitable for the quality control.
5.Percutaneous injection of hemostatic agents under the guidance of contrast enhanced ultrasonography for stopping hemorrhage from severe splenic rupture

Objective To evaluate percutaneous injection of hemostatic agents under the guidance of contrast-enhanced sonography(CEUS) in controlling hemorrhage from severe splenic trauma. Methods Eight patients with splenic traumas of grade Ⅲ, Ⅳ were consecutively enrolled in this study. Percutaneous injection of hemocoagulase atrox and α-cyanoacrylate was performed by CEUS-guidance in all patients. Ultrasound examination was performed at 1,3,7 and 14 days after focal injection. Vital signs and hemoglobin (Hb) level were recorded. Results After the injection,immediate CEUS demonstrated that active bleeding from the injury sites disappeared. From 30 minutes after the injection arterial blood pressure began to ascend ( P <0. 05), and heart rate decreased to the normal level after the treatment ( P <0.05). Hb decreased at first and then gradually increased to the normal level. Conclusions Pereutaneous injection of hemoeoagulase atrox and α-cyanoacrylate under the guidance of CEUS can effectively control bleeding from a severe splenic trauma. CEUS can be helpful to the diagnosis of the suspected splenic trauma and to ensure hemostatic materials to be injected exactly into injury sites.
6.Localization diagnosis of insulinoma by contrast-enhanced ultrasonography compared with laparoscopic ultrasonography

Objective To compare the value of contrast-enhanced ultrasonography (CEUS) with that of laparoscopic ultrasonography (LUS) in localization diagnosis of insulinoma. Methods Thirty-three patients with insulinoma,who received operation, were retrospectively reviewed. All cases were examined by both CEUS and LUS before the tumor removed. Both imaging modalities in diagnosis of insulinoma were analyzed. Results By this study the sensitivity and accuracy of CEUS were 92.31% and 90.0% respectively. Those of LUS were consistent with pathologic findings completely. The accuracy of CEUS was not different with that of LUS (χ2 = 4.21, P = 0. 04). Conclusions CEUS can be used for location diagnosis of insulinoma. For the patients expecting laparoscopic treatment, combination of CEUS with LUS will be suggested before insulinoma is removed.
7.Experiment research of contrast-enhanced ultrasonography in diagnosis of pancreatic laceration

Objective To approach the diagnostic value of contrast-enhanced ultrasonography(CEUS)for the detection of traumatic laceration of pancreas. Methods Sixty cases of pancreatic traumatic model were made in twelve healthy swines after the animals were anesthetized and laparotomized. Then the conventional ultrasonography(US) and CEUS were performed in each case to diagnose the traumatic region,immediately. The results were compared with surgical findings. Results Among sixty injuries,the detection rate of conventional ultrasonography was 66. 7%,the detection rate of CEUS was 88.3%. Conclusions CEUS shows higher detection rate than conventional US in diagnosing pancreatic laceration,and it also can improve the diagnostic value of ultrasound for the detection of pancreatic laceration.
8.Protection Provided by a Gabexate Mesylate Thermo-Sensitive In Situ Gel for Rats with Grade III Pancreatic Trauma.

BACKGROUND/AIMS: This study investigated the protection provided by gabexate mesylate thermo-sensitive in-situ gel (GMTI) against grade III pancreatic trauma in rats. METHODS: A grade III pancreatic trauma model with main pancreatic duct dividing was established, and the pancreas anatomical diagram, ascites, and serum biochemical indices, including amylase, lipase, C-reactive protein (CRP), interleukin 6 (IL-6), and tumor necrosis factor-α (TNF-α), were examined. The pancreas was sliced and stained with hematoxylin eosin and subjected to terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) staining. RESULTS: Ascites, serum amylase, lipase, CRP, IL-6, and TNF-α levels were significantly increased in the pancreas trauma (PT) groups with prolonged trauma time and were significantly decreased after GMTI treatment. The morphological structure of the pancreas was loose, the acinus was significantly damaged, the nuclei were irregular and hyperchromatic, and there was inflammatory cell invasion in the PT group compared to the control. After GMTI treatment, the morphological structure of the pancreas was restored, and the damaged acinus and inflammatory cell invasion were decreased compared to the PT group. Moreover, the cell apoptosis index was significantly increased in the PT group and restored to the same levels as the control group after GMTI treatment. CONCLUSIONS: GMTI, a novel formulation and drug delivery method, exhibited specific effective protection against PT with acute pancreatitis therapy and has potential value as a minimally invasive adjuvant therapy for PT with acute pancreatitis.

9.Impact of needle size and sonographic feature on accuracy of ultrasound-guided breast biopsy.

OBJECTIVETo assess the accuracy of ultrasound-guided 16G and 18G core needle biopsy for detecting ultrasound visible breast lesions with different sonographic features.
METHODSA total of 955 sonographically detected breast lesions examined with ultrasound-guided core needle biopsy (US-CNB) and subsequently surgically excised from July 2005 to July 2012 were retrospectively reviewed. Histological findings of US-CNB and the surgical specimens were analyzed for agreements, sensitivities, false negative rates, and underestimate rates according to different sonographic features.
RESULTSThe pathological results of the US-CNB showed malignant lesions in 84.1%, high-risk lesions in 8.4%, and benign lesions in 7.5% of the samples. The overall agreement rates were 92.4% for 16G CNB and 92.8% for 18G CNB; their complete sensitivities and false negative rates were both 98.6% and 1.4%, respectively; the high-risk underestimate rates and DCIS underestimate rates were 48.0% and 46.2% for 16G CNB vs 53.3% and 41.2% for 18G CNB, showing no significant difference between the two groups (P>0.01). For both 16G and 18G CNB, the agreements were better for mass lesions than for non-mass lesions (P<0.01). For the mass lesions with a diameter no greater than 10 mm, the agreement rates were lower than the overall data (P<0.01). Calcification in the lesions did not affect the agreement rates (P>0.01).
CONCLUSIONUltrasound-guided 16G and 18G CNB are both accurate methods for evaluating ultrasound visible breast mass lesions with a diameter larger than 10 mm.

10. Relationship between thyroid nodules and lifestyle characteristics in Chinese centenarians

Objective:
To study the correlation of lifestyle characteristics with thyroid nodules in a population-based sample of centenarians in Hainan.
Methods:
The study was based on China Hainan Centenarian Cohort Study (CHCCS) conducted in 18 cities and counties in Hainan province from 2014 to 2016. A group of multidisciplinary team interviewed and examined local centenarians with structured questionnaires and ultrasonography procedures. A total of 918 centenarians were analyzed after excluding those who refused ultrasonographic examinations or had relevant missing data. Thyroids of centenarians were examined by 3-year experienced sonographer, details on lifestyle characteristics and dietary habits were collected by standard procedure.
Results:
Of the 918 centenarians, 683 (74.4%) had thyroid nodules under the ultrasonography procedures. The prevalence of thyroid nodules in different group of areca nut consumption varied significantly (
